Malignant melanoma in association with phototherapy.

A 54-year-old Caucasian male suffering from psoriasis had been treated in 1978 and 1980 with PUVA (cumulative UVA dose 492.5 J/cm2) and from 1980 to 1989 with UVB (cumulative UVB dose 264.5 J/cm2). Other occasional treatment had comprised the topical administration of glucocorticosteroids, dithranol or tar. In 1989, on the upper part of the patient's back a newly developed pigmented skin lesion was excised under the clinical diagnosis of a pigmented, dysplastic nevus. Histological examination revealed a superficial spreading malignant melanoma (0.4 mm thick, Clark level II). In this case, exposure to high cumulative doses of therapeutic UV irradiation might have played an etiological role for the development of malignant melanoma.

Comparative efficacy of psoralen - UVA photochemotherapy versus narrow band UVB phototherapy in the treatment of psoriasis.

OBJECTIVE: To compare the clinical improvement in psoriasis with psoralen - UVA photochemotherapy versus narrow band UVB phototherapy alone. DESIGN: An interventional quasi- experimental study. PLACE AND DURATION OF STUDY: Dermatology Department, Nishtar Hospital, Multan, from May 2002 to June 2003. PATIENTS AND METHODS: Forty patients with chronic plaque type psoriasis were included in the study. These were divided into two equal groups each having 20 patients. PUVA or UVB therapy was given thrice weekly. Clinical improvement in lesions was observed by decrease in the severity of erythema, scaling and plaque elevation. RESULTS: Clearance of psoriasis was achieved in significantly greater proportion of patients treated with PUVA (85%) than with UVB phototherapy (60%) (p=0.03814). Significantly fewer number of exposures were required for clearance with PUVA (median number of treatments=17) as compared to UVB (median number of treatments=25.5) (p=0.0002). Median cumulative dose for PUVA was 69.1 J/cm2 and for UVB was 34.15 J/cm2, respectively (p<0.0001). CONCLUSION: When given thrice weekly, PUVA was more effective treatment for psoriasis than narrow band UVB phototherapy.

Treatment options for localized scleroderma.

Localized scleroderma, or morphea, is a chronic disease that causes a thickening and induration of the skin. For plaque type morphea, the treatments of choice include super-potent corticosteroids and calcipotriol. For the more generalized forms, as well as the linear forms, UVA is currently the best therapeutic modality. Patients with localized scleroderma are managed by both rheumatologists and dermatologists. There is still much therapeutic uncertainty in this disease.

Vitiligo management update.

Vitiligo is an acquired skin disorder caused by the disappearance of pigment cells from the epidermis, and results in well defined white patches that are often symmetrically distributed. The lack of melanin pigment makes the lesional skin more sensitive to sunburn. Vitiligo can be cosmetically disfiguring and is a stigmatizing condition, leading to serious psychological problems in daily life. It occurs worldwide in about 1% of the population, mostly between the ages of 10-30 years, and as often in males as in females. The cause is unknown, but might involve genetic factors, autoimmunity, toxic metabolites, and/or a higher vulnerability of melanocytes. Some new treatments for this condition include corticosteroid + UVA treatment, UVB narrow wave band (311 nm) irradiation, and transplantation of autologous pigment cells. In widespread vitiligo, residual pigment can be removed by depigmentation agents. Sunscreens, camouflage products and good guidance may help the patient to better cope with this disease.

[Features of diagnosis and treatment of endotoxicosis syndrome in acute chemical poisoning].

Experience gained in the treatment of more than 750 patients with acute poisoning complicated by endotoxicosis showed that the entire complex of typical changes in endotoxicosis markers and homeostasis parameters (hematological, immunological, hemorheological, LPO/AOD) should be taken into consideration during the diagnostic studies. These shifts should be timely corrected by the basic efferent methods of artificial detoxication (hemosorption, hemodiafiltration, hemofiltration) with obligatory physio- and chemohemotherapy (laser-UV hemotherapy, infusion of sodium hypochlorite).
