Inflamed adenolymphoma with cholesterol granuloma.

Twelve cases of adenolymphoma with inflammation are described. Pre-operatively, three of these patients presented clinical signs of inflammation in the parotid gland. In ten cases purulent exudate could be aspirated from the tumour. Rupture of the cyst may cause an aseptic inflammation in the surrounding tissue. It is concluded that adult patients with clinical or cytological signs of inflammation in the parotid gland must be supervised until the gland is normal in order not to miss an underlying tumour.

Adenolymphoma↗

Acute facial-nerve paralysis with parotid adenolymphoma.

An 82-year-old woman presented with acute facial paralysis in association with parotid swelling following fine-needle aspiration cytology of a parotid adenolymphoma. Evacuation of a tense haematoma decompressed the nerve and led to complete recovery within 6 months. Additionally, the tumour is no longer evident, either clinically or radiologically, and fine-needle aspiration cytology appears to have been both diagnostic and, inadvertently, therapeutic in this case.

Adenolymphoma↗

Corpora amylacea in adenolymphoma (Warthin's tumor).

Round or polyhedral, acellular, lamellated or amorphous eosinophilic structures were observed in the cystic spaces of adenolymphomas. These eosinophilic bodies ahd several of the histochemical characteristics of amyloid, viz., pink color with green specific birefringence and bright red fluorescence with alkaline Congo red; positive DMAB-nitrite and DC reactions, and spontaneous autofluorescence. It is suggested that these bodies are derived from spontaneous assembly or polymerization of microfibrils of desquamated epithelial cells lining the cystic spaces of the tumor, as has been described to occur in prostatic corpora amylacea.

Adenolymphoma↗

Immunohistology of adenolymphoma (Warthin's tumour): evidence for a role of vascularization in the organization of the lympho-epithelial structure.

The epithelial, vascular and lymphoid components present in two cases of adenolymphoma (Warthin's tumour) were characterized by immunohistochemistry on frozen sections. Epithelial cells were positive for cytokeratin and keratin and were in close contact with numerous factor VIII related antigen-positive capillary vessels, running parallel to the epithelial basement membrane. The capillary vessels were in communication with high endothelial post-capillary venules which were often lined by HLA-DR-positive endothelial cells. The lymphoid tissue was organized in B-cell and T-cell areas. The B-cell areas consisted of B4+/To15+ B-lymphocytes admixed with DRC-1-positive follicular dendritic reticulum cells. The T-cell areas were mainly populated by T11+/T3+/T4+ lymphocytes admixed with some S-100+/HLA-DR+ interdigitating reticulum cells. Plasma cells and T8+ lymphocytes were more numerous in the proximity of the epithelium. Leu-M3+/PAM-1+ macrophages were scattered singly in the T-cell areas or were arranged in sinus-like structures around the epithelial cysts. The epithelial basement membrane, the vascular membrane and the fibrillar reticular stroma of the lymphoid tissue were immunoreactive for laminin and type IV collagen. The possibility is discussed that the accumulation of lymphoid tissue in Warthin's tumour is modulated by the epithelial cells, perhaps through their ability to organize a peculiar pattern of vascularization.

Adenolymphoma↗

Adenolymphoma with massive necrosis and squamous metaplasia.

A 61-year-old female presented with a longstanding parotid mass with recent change in size. Histologically, the mass was composed of extensively necrotic tissue with a peripheral rim of ribbons of epithelium. In one focus, a double layer of oxyphilic epithelium overlying a lymphoid stroma in a papillary configuration was present, establishing the diagnosis of adenolymphoma (AL). The possibility of infarction and/or infection complicating AL is discussed. In any parotid gland lesion clinically presenting as a longstanding tumor with recent change clinically suggestive of infarction and/or infection, the diagnosis of AL should be considered. In the pathologic specimen with extensive necrosis, areas of preserved epithelium and stroma should be searched for carefully to confirm the diagnosis.

Adenolymphoma↗

Pigmented cells in adenolymphoma (Warthin's tumor).

A study of a series of adenolymphomas showed the presence of pigmented cells in the lymphoid stroma in 85% of the cases. The pigmented cells contained light brown cytoplasmic pigment and manifested sudanophila, acid-fastness, silver reduction capacity, PAS positivity and yellow autofluorescence in ultraviolet light. The morphology, histochemical reactions and autofluorescence suggest that the pigmented cells are macrophages containing ceroid derived from breakdown of damaged organelles of the epithelial tumor cells. It is possible that these cells play a role in the elimination of undigestable pigment material present in the cystic spaces of the tumor.

Adenolymphoma↗

Poorly differentiated carcinoma arising from adenolymphoma of the parotid gland.

BACKGROUND: There is only one previous case report of a poorly differentiated carcinoma arising from an adenolymphoma of the parotid gland (Warthin's tumour). The absence of clinical symptoms, and the aspecificity of the radiological pattern make the diagnosis very difficult. CASE PRESENTATION: We here report the case of a 73-year-old man with Warthin's tumour who was brought to our attention because of a swelling in the parotid region. CONCLUSIONS: In this case with an atypical clinical presentation, the intra-operative examination of a frozen section of the parotid mass allowed us to diagnose the malignant tumour correctly and consequently undertake its radical excision.

Adenolymphoma↗

Adenolymphomas of the parotid and neck with a coexistent pleomorphic adenoma. Case report.

An 82-year-old man presented with multiple adenolymphomas of the parotid and neck (Warthin's tumours) with a coexistent pleomorphic adenoma. Two Warthin's tumours were within the parotid gland as was the adenoma, and four were in the neck region. The patient made an uneventful recovery from excision of the neck tumours and superficial parotidectomy. We know of no similar previous case, and emphasise the importance of confirming the histological diagnosis so that the correct treatment may be planned.

Adenolymphoma↗

[A rare case of multiple neoplasms of parotid gland: adenolymphoma and squamous cell carcinoma].

The authors have presented a very rare case of multiple primary neoplasms of parotid gland: adenolymphoma and squamous cell carcinoma. Primary multiple neoplasms and multifocal neoplasms are a very complex problem and therefore have been a topic of many articles. The authors describe mechanism of origin, frequency of occurrence, possibilities of diagnostics. The main reason of these neoplasms origin are simultaneously exposure of the tissue to damaging factors and special tissue sensitivity.

Adenolymphoma↗

[Clinical picture, diagnosis, and therapy of adenolymphoma of the parotid gland].

Clinical data on 33 patients with adenolymphoma (AL; Warthin's tumor) of the parotid gland are presented, including a case with development of mucoepidermoid carcinoma in the presence of AL. Specific features in the clinical course of AL are described, which depend on the tumor location, probable suppuration of the contents of the cystic cavities, etc. The diagnostic significance of some accessory methods of examination is evaluated: ultrasonic method, sialoscintigraphy, single-photon emission computer-aided tomography, cytological method, etc. High accumulation of (99m)Tc pertechnetate in AL was observed in some cases. The ratio of the glandular and lymphoid components in AL, which can vary, is clinically significant. The treatment strategy in AL is the same as in surgery for pleiomorphic adenoma of the parotid gland.

Adenolymphoma↗

[Ultrastructural study of adenolymphoma].

Ultrastructure of 10 adenolymphomas of parotid gland were studied. The neoplastic epithelium consists of "oncocytes" which are arranged in pseudostratified pattern. Their cytoplasm is almost totally packed with a large numbers of hypertrophic mitochondria showing a variety of peculiar forms. The amount and alteration of the mitochondria gradually increase in the order of pyramidal cell, common columnar cell and degenerated columnar cell. This might be a process of transposition representing degeneration due to metabolic disorder in the tumor. The obvious alterations of mitochondria are found in the cases with not only "hot" nodule but also "cold" nodule shown in the studies of 99mTc scintigraphy. There seems no relationship between "hot" nodule and alteration of mitochondria.

Adenolymphoma↗

[2 cases of adenolymphoma of the parotid gland].

Starting from two cases of adenolymphoma of the parotid gland they had under observation, the Authors underline their doubts regarding the histogenesis, discuss the clinical approach and show their attitude towards the therapy. It consists of an enucleation or nucleoresection of the tumour in case only the superficial part of the gland is affected; a total parotidectomy with facial nerve preservation and without any radiotherapy, in case of a radical operation; with complementary radiotherapy if the operation has not been radical for certain.

Adenolymphoma↗

[Adenolymphoma of the parotid gland].

Three personal cases of adenolymphoma of the parotid are described and attention is drawn to the rarity and benign nature of this form. Its anatomical and pathological features are discussed, along with the various theories concerning its histogenesis. Of these, the salivary theory is the most widely accepted. Lastly, an account is given of the symptoms (primarily consisting of cosmetic damage), differential diagnosis, and treatment (simple enucleation). The long-term prognosis is always good. Recurrences (also benign) are very rare.

Adenolymphoma↗

[Adenolymphoma of the salivary glands. The anatomicoclinical, epidemiological and therapeutic aspects of 76 cases].

A series of 76 cases of adenolymphoma (Warthin's tumor) of the salivary glands is presented with emphasis on its epidemiological, clinico-pathological and therapeutic features; together with the analysis of the clinical records and of the epidemiological data, this study has reevaluated the histopathological features of the 76 cases of Warthin's tumor on hematoxylin-eosin stained slides. On these bases, the authors underline the frequent multifocality and/or bilaterality of Warthin's tumor (11.3% of cases), its frequent association with tumors of other sites (11.3% of cases), the prevalence in males (M:F ratio = 24:1) and in adults (mean age: 57 years) of this type of tumor. Furthermore, this study confirms the parotid gland as the elective site for Warthin's tumor and the possibility to categorize this entity into four different histological types. The latter finding should be kept in mind especially when fine needle aspiration (FNA) is used as a preoperative diagnostic procedure for Warthin's tumor to avoid misleading diagnosis and overtreatment. From the analysis of the results the authors underline the need for a complex and multidisciplinary approach to the diagnosis of Warthin's tumor in order to allow conservative surgery with preservation of the facial nerve.

Adenolymphoma↗

[Malignant adenolymphoma-oncocytic adenocarcinoma of the parotid gland].

The authors present an extremely rare case of a malignant form of parotid gland adenolymphoma treated by surgical intervention. Further, diagnostic problems and methods of treatment are discussed as well as unsuccessful course of the disease, due to extensive, distant metastases.
