Prevention of PONV by acustimulation with capsicum plaster is comparable to ondansetron after middle ear surgery.

PURPOSE: To compare the efficacy of stimulation of P6 acupoint with capsicum plaster in comparison with iv ondansetron for the prevention of postoperative nausea and vomiting (PONV). METHODS: 120 patients of either sex, ASA I-II, undergoing elective middle ear surgeries under general anesthesia were included in this randomized, prospective, double-blinded and placebo-controlled study. The anesthetic technique was standardized. Patients were divided into three groups. Group I was the control group. Capsicum plaster (1 x 1 cm) was affixed at the P6 acupoint on both forearms 30 min before induction of anesthesia in patients of Group II. Patients of Groups I and III received an inactive adhesive plaster at the same site. Ondansetron 4 mg iv was given to patients of Group III at the end of surgery and the rest of the patients received a placebo. The plasters were removed six hours after transferring the patients to the postoperative unit. Criteria were fixed for the administration of rescue antiemetics (ondansetron 4 mg iv). PONV and the requirement for rescue antiemetics were recorded by a blinded observer. RESULTS: The incidence of PONV and the requirement for rescue antiemetics were significantly lower in both the acustimulation and ondansetron groups at six hours. At 24 hr there was a reduction in the requirement for rescue medication in the ondansetron group. CONCLUSION: Stimulation of the P6 acupoint with capsicum plaster is an effective method for prevention of PONV after middle ear surgery and its efficacy is comparable to ondansetron for the first six hours after surgery.

[Radiolysis of acrinol in radio-sterilization of acrinol pharmaceuticals].

Acrinol in dry solid state after gamma irradiation with 80 kGy (8 Mrad) undergoes 8% decomposition, with G(-M)=30, and in wet solid state (10% moisture content) undergoes 15% decomposition, with G(-M)=56, as shown by absorbed spectrometric measurement. From these results, it is estimated that the irradiation dose of 25 kGy (2.5 Mrad) prefered by many countries as sterilization dose will achieve the radiolysis rate of 2.8% or 4.7% for dry solid state or wet solid state acrinol, respectively. The stickiness power of plaster tape made of raw rubber and polyterpene resin don't decrease with irradiation dose of less than 40 kGy (4 Mrad). Then, the radio-sterilization on the dose of 25 kGy (2.5 Mrad) may be applied to solid pharmaceuticals such as commercial rubber adhesive plaster with acrinol pad. Besides, acrinol in 0.1% aqueous solution after irradiation of 10 kGy (1 Mrad) at room temperature undergoes 20% decomposition, with G(-M)=0.5. The radiolysis rate in this state is estimated 45% with the dose of 25 kGy (2.5 Mrad). Then, the radio-sterilization with this dose must not be applied to liquid pharmaceuticals such as 0.1% acrinol aqueous solution. While, it was reported by Hosobuchi and Sato that the antimicrobial effect of irradiated acrinol to Staphlococcus aureus increased with irradiation dose. Then, it is expectable that some materials with antimicrobial activity are prodused by gamma irradiation.

Effects of topical diclofenac (DHEP plaster) on skin, subcutis and muscle pain thresholds in subjects without spontaneous pain.

The aim of this study was to determine whether topical application of diclofenac hydroxyethylpyrrolidine (DHEP) modifies somatic pain sensitivity in subjects without spontaneous pain. Twenty male subjects (aged 19-40 years), who had not reported any pain for at least 1 month, underwent measurement of pain thresholds to bilateral electrical stimulation in the quadriceps muscle and overlying subcutis and skin. Following the double-blind study design, one diclofenac adhesive plaster (13 x 10 cm; 180 mg DHEP) was then applied over one quadriceps while a matched placebo plaster was placed contralaterally. Each subject was given two other plasters (diclofenac and placebo) and instructed to substitute those over the quadriceps after 12 h and to wear them for a further 12 h. Thirty minutes after removal of the second plasters, thresholds. were remeasured in all subjects as on the previous day. Thresholds at the first evaluation were within normal range in nine subjects (group 1) and below normal in muscle (hyperalgesia) in the remaining 11 (group 2). No significant changes were observed in skin or subcutis thresholds after diclofenac or placebo treatment in either group. In contrast, muscle thresholds significantly increased after diclofenac compared with placebo treatment (group 1: p < 0.05; group 2: p < 0.007); the increase was significantly higher in group 2 than in group 1 (p < 0.002). Topical application of diclofenac had a selective hypoalgesic effect on muscles, which was more pronounced in the case of hyperalgesia. These results suggest that the preparation is particularly effective in the treatment of algogenic conditions of deep somatic tissues.

Two simple methods for the evaluation of topically active anti-inflammatory steroidal ointments.

Simple laboratory methods for quantitating the topical anti-inflammatory activity of steroidal ointments are described. One is of croton oil ear edema in rats and the other is a new method using homologous passive cutaneous anaphylaxis (PCA) in rats. In order to avoid problems such as the animals' licking and/or rubbing the ointment at the applied sites, which might result in oral uptake, each rat was housed individually and fitted with a plastic collar in the croton oil experiment. The sites of ointment application in the PCA experiment were covered with adhesive plaster. Optimal experimental conditions were as follows. In the former method, ointments were applied to the inside surface of the ear 5 min after the irritant treatment and anti-edematous activity was determined after 6 h. In the latter, ointments were applied 3 h before the antigenic challenge to the dorsal area of animals which had been passively sensitized by anti-serum, and inhibition of the increased permeability was determined 45 min after the challenge. These methods were found to be reliable with respect to sensitivity and reproducibility of data. Ointments of halcinonide, betamethasone-17-valerate, hydrocortisone-17-butyrate, fluocinonide, flumethasone-21-pivalate and beclomethasone-17,21-dipropionate were evaluated by these methods.

Recovery of some common solvents from an adhesive commercial skin adsorption pad by thermal desorption-gas chromatography.

A thermal desorption-gas chromatography (GC) system was developed for use with commercial adhesive plasters used for monitoring exposure of hands to common solvents. The efficiency of solvent adsorption on the activated carbon pads located on the plasters was determined for acetone, trichloroethylene, D-limonene, methanol, ethyl methyl ketone, toluene, tetrachloroethylene and m-xylene. The degree of solvent recovery for the system was also investigated for each solvent, as was its sensitivity and reproducibility. All solvents exhibited > 90% adsorption on the pads at spiking levels of 100-200 ng for each solvent, except for m-xylene and d-limonene. Solvent recovery was dependent on the volatility of the solvent at spiking volumes of about 1 microliter per pad with solvents with boiling points above 110 degrees C showing recoveries of < 75%. Increasing primary desorption times and temperatures increased these values. The precision was good with RSD < 5% for all solvents over the range 0.5-5.0 microliters of applied solvent. It was possible to detect 15-60 ng of each solvent component within solvent mixtures on the pads with the exception of D-limonene. It is concluded that all solvents tested except D-limonene can be determined on the pads under the conditions for thermal desorption-GC analysis described. The pads were used under protective gloves with six workers using xylene isomers as solvent in the workplace, when apparent solvent breakthrough through their gloves was observed.

Removing eyebrows impairs recognition of famous faces, or doesn't, depending on how the eyebrows are removed.

Matching photos of famous people's faces with their names was as fast with photos that had eyebrows masked by adhesive plaster as with unretouched original photos. But matching was slower with photos that had eyebrows erased and replaced with adjacent skin texture and colour than with original photos. The conjecture that erasure impairs recognition because it alters a face's configuration was examined by repeating the experiment with the photos shown upside down, the rationale being that because configural information is difficult to encode from inverted faces the erasure effect should diminish. With inverted faces, matching was no different for original, eyebrows-masked, and eyebrows-erased photos. Eyebrows appear to be less important for face recognition as informative parts and features than as sources of information about a face's configuration.

Differentiation of malignant lymphoma and non-lymphoma by an occlusive dressing method employing bromodeoxyuridine.

Although lymphoma cells can proliferate in skin tissue, lymphocytes which have infiltrated the skin due to inflammatory changes are generally unable to do so. It may be possible to differentiate malignant lymphomas from benign lymphocytic infiltrations in skin tissue by detection of cell cycle-related antigens. We developed a novel in vivo method for studying the cell kinetics of human skin using bromodeoxyuridine (BrdU) with an occlusive dressing. Following the application of BrdU, BrdU-labeled lymphoid cells were counted in the dermis. The patients studied included 22 with various types of lymphomas (lymphoma group) and 22 with different forms of inflammatory skin disease (non-lymphoma group). Skin specimens were obtained after applying occlusive adhesive plasters containing 2% BrdU dissolved in 0.9% sodium chloride solution and were then immunostained with anti-BrdU monoclonal antibody. The average percentages of labeled cells were 15.9 +/- 7.6% for the lymphoma group and 4.7 +/- 2.6% for the non-lymphoma group. This difference was statistically significant (p < 0.0001). Using this method, malignant lymphomas and benign lymphocytic infiltrations of the skin could be differentiated by counting BrdU-labeled lymphoid cells, and this method could help in determining a prognosis.

Oxygen tension and consumption measured by a tc-PO2 electrode on heated skin before and after epidermal stripping.

Oxygen tensions, cutaneous blood flow rate, and skin oxygen consumption rate were determined by tc-PO2 measurements at an electrode temperature of 45 degrees C. The epidermal surface was stripped by 50 applications of adhesive plaster to the surface. Ten healthy, normotensive adults were examined. Cutaneous blood flow rate was 41.2 +/- 8.6 ml X (100 g)-1 X min-1 before and 42.8 +/- 5.9 ml X (100 g)-1 X min-1 after epidermal stripping. Oxygen consumption before stripping was 0.327 +/- 0.065 ml O2 X (100 g)-1 X min-1, and after stripping it was determined at two different saturation levels to be 0.208 +/- 0.072 ml O2 X (100 g)-1 X min-1 and 0.251 +/- 0.096 ml O2 X (100 g)-1 X min-1. Capillary temperature was estimated to be approximately 43 degrees C before and after stripping. At this temperature mean arterial PO2 was estimated to be 18.1 kPa (136 mmHg), which would be reduced by the computed local metabolism to a mean capillary PO2 of 14.4 kPa (108 mmHg) before stripping and 15.2 kPa (114 mmHg) after. Stripping increased mean skin PO2 from 10.9 +/- 0.6 kPa (82.3 +/- 4.7 mmHg) to 14.6 +/- 1.0 kPa (109.4 +/- 7.7 mmHg). Thus, stripping eliminated 82% of the gradient between the capillaries and electrode while reducing the computed oxygen consumption by 23-36%. It is concluded that the epidermal membrane is a significant barrier to oxygen diffusion and that the transcutaneous oxygen electrode has a significant effect on skin PO2 owing to its own even low oxygen consumption. This will reduce the observed skin PO2 significantly.

Contact dermatitis from radiata pine.

Sawdust from radiata pine trees which are grown extensively throughout South Australia causes various forms of contact dermatitis. The various forms are described in nine cases which illustrate that the early diagnosis of industrial dermatitis is necessary to prevent the condition from becoming chronic. A chance association between colophony in radiata pine sawdust and colophony in adhesive plaster, which caused contact dermatitis when used in patch testing, led to the discovery of these cases.

Cross-reactivity with Tagetes in Arnica contact eczema.

A 69-year-old patient, with known mercury and adhesive plaster allergy, developed facial dermatitis within 24 h of contact with arnica (Arnica). Skin testing showed positive reactions to arnica and, among various other plants of the Compositae, also to Tagetes sp. hybr. (marigold). Cross-reactivity between Tagetes and arnica has not previously been described.

Active uptake of copper and zinc during haemodialysis.

The uptake of copper and zinc by patients undergoing regular haemodialysis has been assessed by measuring the dialysis fluid for copper and zinc concentration, and the blood entering and leaving the dialysis coil for red cell copper, plasma free copper, and plasma zinc levels during priming of the coil and subsequent haemodialysis, and by in-vitro studies.Red cells avidly removed copper from dialysis fluid when mixed with saline during priming, but did not take up copper during the haemodialysis. At both these stages plasma actively took up both copper and zinc from dialysis fluid, even against a concentration gradient, so that loss of these metals from plasma to dialysis fluid did not occur. In the dialysis systems investigated the sources of the copper in the dialysis fluid were the copper plumbing of the tap-water and the dialysis coil, and the major source of zinc was the zinc oxide of the adhesive plaster around the dialysis coil.

Forensic casework of personal identification using a mixture of body fluids from more than one person by Y-STRs analysis.

We applied Y-STRs (DYS385/DYS19/YCAII) to an adhesive plaster left at a crime scene. This plaster may have included body fluids from more than one person. Firstly, we performed preliminary examinations, ABO-blood type examinations, and commonly used DNA examinations (D1S80, HLADQalpha, TH01, and PM) on these specimens. As a result of these examinations, we could evidence that suspect A did not contacted with the plaster, but could not confirm the presence of perspiration from suspect B. As the next step, we applied Y-STR examination to the plaster. Using this examination, we detected alleles that coincided to those of suspect B. We also concluded that the fluid from an unidentified person was vaginal fluid based on crime scene investigation. Y-STRs examination data obtained from 124 persons in Tokushima prefecture showed that 1.613% of individuals demonstrated haplotypes 10-18/15/19-23, which was detected from the plaster and from suspect B. Therefore, we considered that there was a high probability that the persiration detected in the plaster was that of suspect B. Based on these studies, we concluded that Y-STR examination of trace evidence was very useful to screen suspects using materials that contained body fluid from more than one person.

Paw preferences in dogs.

The distribution of paw preferences were studied in 28 dogs. The paw preference was assessed by counting the right and left paw movements performed to remove an adhesive plaster from the eyes. The significance of the right minus left paw reaches in percentages was evaluated statistically in each animal. There were three distinct groups in respect to paw preferences in dogs: right-preferent (57.1%), left-preferent (17.9%), and ambidextrous (25.0%). Statistical analysis showed that the observed frequencies for each group were not merely chance variations which would be expected in a random sample. It was concluded that the population bias can be expressed in a distribution skewed toward a right-hand bias as seen in man.

Reduction of displaced supracondylar fracture of the humerus in children by manipulation in flexion.

Sixty-one children with posteriourly displaced supracondylar fractures of the humerus were treated by minipulation with the elbow flexed 40 degrees-60 degrees. This method proved to be safe, easy to apply and was followed by a high rate of success. The elbow was immobilized using a collar-and-cuff or a broad sling and binding of the arm to the forearm with zinc oxide adhesive plaster. In difficult children, the limb was put under the child's clothing to avoid its being used. Redisplacement occurred in only 8.2 per cent of the patients and in none of these was it severe enough to justify remanipulation.

[An observation of the percutaneous absorption of total alkaloids and terpenoid lactones of Tripterygium wilfordii].

The main ingredients of Tripterygium wilfordii are total alkaloids and terpenoid lactones. The authors studied the percutaneous absorption of these compounds on mice by means of ultravillet spectrophotometry. The results showed that these alksloids and lactones could pass through the skin of the mice. The rate of 12-hour percutaneous absorption was 13.40% for the former and 17.60% for the latter. After the application of Tripterygium wilfordii adhesive plaster to the human skin for 12 hours, the two ingredients were absorbed 17.34% and 22.13% respectively. This suggests that Tripterygium wilfordii can really be administered through the skin.

[Development of self-generating lower body negative pressure device].

OBJECTIVE: To develop a kind of self-generating lower body negative pressure device. METHOD: The device consists of a flexible bellows, reinforced by several steel rings and sealed by gasproof adhesive plaster. An adjustable valve, a one-way flap valve, shoulder straps and handles are incorporated on the top of the bellows. The user's lower body was contained in the bellows. As the user's legs were extended, the bellows was elongated and the air pressure in it decreased. RESULT: Negative pressures of -58, -46, -38 and -26 mm-Hg respectively was created with the adjustable inlet valve completely closed, one-quarter open, half open, or fully open. CONCLUSION: This self-generating lower body negative pressure device combines exercise and LBNP into one thing and is safe and easy to use. It might be used in preventing cardiovascular deconditioning during spaceflight or for anti-G training of pilots.

[Preternatural anus and its care (author's transl)].

The most careful surgical technique, the guiding principles of which are outlined, is a prerequisite for the subsequent possibilities for correct care of preternatural anus and for preventing otherwise unavoidable complications. Four different programs are available to us for care; truss pad, adhesive bag, irrigation therapy and the Erlanger magnetic closure. For prophylaxis of frequent hernias and prolapses, wearing a made-to-measure body binder is indispensable. Skin lesions, formerly feared concomitant symptoms, can now be prevented or eliminated as a result of the development of non-irritant adhesives, combinations with sterculia rings or adhesive plasters and differentiated skin care products. A more comprehensive organization of those with preternatural anus within the framework of the German Ilco and the establishment of preternatural anus clinics and therapists would be desirable.

[Clinical and experimental studies on the treatment of severe facioplegia with compressing drug, acupuncture and infrared rays].

In 160 patients with severe facioplegia detected by the strength-duration curve, partial or complete reaction of degeneration of the facial nerve and its related muscles occurred in 158 patients and no reaction of degeneration in 2 patients. All the patients were arbitrarily divided into 2 groups. One group of 62 patients was treated with the regimen 1: Mianmasan adding acupuncture and the other of 98 patients with the regimen 2: modified Mianmasan adding acupuncture and red lighting. The treatment concretely was an application method by dusting the Mianmasan, over the scarified skin corresponding to the selected acupoints and motor points, and finally stick a piece of adhesive plaster on them. The clinical experiment was carried out by using double contrast method, i.e., autogenous and allogenic contrasts with ear pulse wave and skin temperature records. It was confirmed that the affected side of the face had chronic tissue ischemia and returned to normal after treatment. The results showed that among 160 treated patients, 70(43.8%) were cured, 46 (28.7%) markedly improved, 43 (26.9%) improved and 1 (0.6%) remained ineffective. By the statistical analysis, it was demonstrated that the curative effect of regimen 2 was superior to that of regimen 1 (P less than 0.01), and the effective rate in the patients with partial reaction of degeneration was higher than that in those with complete reactcon of degeneration (P less than 0.05).
