Improved skin blood flow and cutaneous temperature in the foot of a patient with arteriosclerosis obliterans by vasopressin V1 antagonist (OPC21268). A case report.

A seventy-four-year-old woman with arteriosclerosis obliterans, diabetes mellitus, and hypertension was admitted for the treatment of intermittent claudication and coldness and pain in the right lower extremity. After the administration of a vasopressin V1 antagonist, OPC21268, the symptoms were markedly improved. Furthermore, blood flow in the dorsalis pedis artery and the cutaneous temperature in the right foot increased in response to acute and chronic administration of OPC21268. OPC21268 may be a new useful therapeutic tool for the treatment of arteriosclerosis obliterans.

The effect of HR (O-Beta-Hydroxyethylo-Rutoside, Venoruton) on the deformability of erythrocytes in patients with arteriosclerosis obliterans of lower limbs.

Deformability of red cells and plasma levels of cAMP and cGMP were studied in patients with arteriosclerosis obliterans of lower limbs. Deformability of red cells and plasma levels of cAMP were found to be decreased in these patients. Venous injection of HR (Venoruton) in one dose (1,000 mg) to patients, led to an increase of red cell deformability and normalisation of cAMP plasma level. The plasma level of cGMP was not changed. The beneficial effect of HR in arteriosclerosis obliterans depends, first of all, on improving the deformability of red cells. The decreased deformability observed in arteriosclerosis seems to be a result of disturbances in the rate of spectrin phosphorylation of the erythrocyte membrane. By normalising the plasma level of cAMP HR had a considerable influence on the appropriate state of elasticity of the red cell membrane.

Estimating the stenosis probabilities in arteriosclerosis obliterans using generalized estimating equations.

For each of 211 arteriosclerosis obliterans patients, the degree of stenosis of arteries at four sites were examined at Hiroshima University Hospital to analyse the relationship between the degree of stenosis and age, sex and site. The generalized estimating equations using a proportional odds model for the stenosis probability with exchangeable correlation was applied to this clustered, ordered polytomous data. Since the correlations between the responses were quite small, we could apply the independence estimating equations. The best mean model selected by Akaike information criterion suggested that age dependency of the stenosis probability was not observed in the central sites but was observed in the peripheral sites, and symmetry of the risk of stenosis was seen in the left and right sites. Details of the analysis will be described.

[Thrombosis test in vitro of 107 cases with arteriosclerosis obliterans].

Thrombosis test in vitro of 107 cases with arteriosclerosis obliterans were measured and controlled by 36 normal persons. Of 107 cases, 13 cases belonged to ischemic phase, 47 cases nutritional disturbance phase, 47 necrosis (24 cases were first degree, 9 cases second degree, 14 third degree). Types distribution were: simple blood stasis 65 cases, blood and damp stasis 34, and 8 others. 46 patients were treated systematically by TCM-WM. The result proved that the indexes of thrombosis test in vitro of patients were much higher than that of normal persons (P less than 0.001). The relevant indexes were different in varied periods, degrees and types (P less than 0.05-0.01) and between before and after treatment (P less than 0.001). Of treated 46 cases, total effective rate was 86.96%, amputation rate was 13.04%. Formation of thrombus in vitro lowered 88.03%. The authors suggested that measurement of thrombosis test might be a accurate way to reflect the course of the disease and a basis for treatment and differentiation of symptoms.

The exercise pressor reflex and changes in radial arterial pressure and heart rate during walking in patients with arteriosclerosis obliterans.

Little is known about the time course and extent of the changes in radial arterial pressure and heart rate taking place in patients with arteriosclerosis obliterans during walking, as well their subtending mechanisms. For this reason the authors measured these variables in 23 patients with arteriosclerosis obliterans and in nine normal subjects (control group), during treadmill walking and several tests. In the patients a rapid and marked increase in radial arterial pressure was recorded during walking, whereas the same parameter either fell abruptly or persisted at elevated levels during recovery. This pattern markedly differed from that recorded in normal subjects, and it was mainly brought about by the activation of the exercise pressor reflex. The following findings suggested that the exercise pressor reflex was activated: the conditions required for activation of the reflex were present in our patients; the pressure changes observed during walking tightly paralleled the changes due to this reflex activation; the hypertensive response to walking was enhanced by increases in severity of disease and in walking speed and duration; the reflex activity persisted during recovery; and the pressure pattern during walking was reproduced by walking with arrested blood circulation to a lower limb. On the contrary, the behavior of heart rate was similar in patients and normal subjects both during walking and recovery because it was not influenced by the exercise pressor reflex.

Blood zinc levels in patients with arteriosclerosis obliterans, thromboangiitis obliterans and Takayasu's disease.

Serum zinc concentration was measured by atomic absorption spectrophotometry in 30 normal males, 17 normal females, 20 patients with arteriosclerosis obliterans, 26 patients with thromboangiitis obliterans, 40 patients with Takayasu's disease. The mean serum zinc concentration was 93.9 +/- S.E.4.0 mug/100 ml in the normal male controls, 75.1 +/- 3.3 mug/100 ml in patients with arteriosclerosis obliterans without ulcers (P less than 0.05) and 79.2 +/- 6.5 mug/100 ml in patients with thromboangiitis obliterans without ulcers. Serum zinc concentration showed to be more decreased in patients with these diseases who had ulcers. The mean serum zinc concentration was 77.9 +/- 4.0 mug/100 ml in the normal female controls, 71.8 +/- 5.1 mug/100 ml in patients with Takayasu's disease who had never been treated with steroids and 59.1 +/- 2.7 mug/100 ml in patients with this disease who had been treated with this drug (P less than 0.01). The zinc level was significantly lower in CRP positive patients with Takayasu's disease than in CRP negative patients (P less than 0.05).

Platelet-derived microparticles in patients with arteriosclerosis obliterans: enhancement of high shear-induced microparticle generation by cytokines.

We evaluated the plasma concentrations of cytokines and platelet-derived microparticles in patients with arteriosclerosis obliterans and studied the effect of cytokines on platelet-derived microparticle generation under high shear stress. Interleukin-6 levels peaked at 48 hours after vascular surgery, while thrombopoietin started to increase at 24 to 48 hours postoperatively and peaked on the seventh day. Platelet activation markers were increased in the arteriosclerosis obliterans patients preoperatively. Levels of P-selectin and CD63 both increased further, peaking at 6 to 24 hours postoperatively. Platelet-derived microparticle levels were also increased preoperatively. At 6 hours postoperatively, the plasma level of platelet-derived microparticles was significantly increased. Plasma platelet-derived microparticle level was lower at 12 hours but only returned to the preoperative value at 7 days after grafting. There was a difference in the platelet-derived microparticle level at 7 days between patients with or without antiplatelet therapy (cilostazol). The effect of cytokines on platelet activation under high shear stress was also studied. Interleukin-6 and thrombopoietin enhanced both P-selectin expression and platelet-derived microparticle generation under high shear stress. These results suggest that platelet-derived microparticles are released by platelet activation after vascular grafting when certain cytokines increase under high shear stress and that antiplatelet therapy may reduce platelet-derived microparticle levels postoperatively.

Effect of the serotonin blocker sarpogrelate on circulating interleukin-18 levels in patients with diabetes and arteriosclerosis obliterans.

We aimed to evaluate the effect of treatment with sarpogrelate, a serotonin 2A receptor antagonist, on circulating interleukin (IL)-18 levels in patients with diabetes and arteriosclerosis obliterans. Patients received sarpogrelate (100 mg 3 times daily) for 2 months. We evaluated the degree of cryaesthesia (a feeling of cold in the foot and toes) as the clinical outcome, and measured circulating IL-18, IL-6 and lipid protein concentrations. An improvement in clinical outcome occurred after initiation of sarpogrelate therapy; a significant decrease in IL-18 levels was observed after 2 months of therapy. Levels of IL-6 and lipid proteins, including triglyceride, total cholesterol and high-density lipoprotein cholesterol, were not significantly altered by treatment. Our data suggest that by reducing circulating IL-18 levels, sarpogrelate treatment may contribute to the inhibition of arteriosclerosis obliterans progression in patients with diabetes.

[Arteriosclerosis obliterans(ASO) in diabetic patients].

The major risk factors of arteriosclerosis obliterans(ASO) in diabetic patients are age, male gender, smoking, poor glycemic control, hypertension, and dyslipidemia. Thus, to prevent the development and progression of ASO, intensive intervention in the risk factors should be required. HOPE study reported that treatment of ACE-inhibitor in the patients with ASO for 5 years clearly decreased the relative risk for incidence of cardiovascular events comparing with the placebo group. Furthermore, UKPDS Group demonstrated that the adjusted odds ratios to the development of ASO for each 1% HbA1c increase and each 10 mmHg systolic blood pressure increase were respectively 1.28 and 1.25 from multivariate analysis. However, optimal levels of HbA1c and blood pressure for prevention of ASO still have not been suggested. Therefore, large-scale intervention trial in Japanese diabetic subjects should be needed.

[Local changes in prostanoids in arteriosclerosis obliterans].

In order to identify local changes in prostanoids in arteriosclerosis obliterans (ASO), plasma levels of thromboxane B2 (TXB2) and 6-keto-prostaglandin F1 alpha(6-Keto-PGF1 alpha) in cubital venous blood (CV blood), in femoral arterial blood (FA blood) and in femoral venous blood (FV blood) of 27 patients with ASO of lower limbs and of 10 healthy controls were measured by RIA technique. The following results were obtained. 1) In the ASO patients, levels of TXB2 and the ratio between TXB2 and 6-Keto-PGF1 alpha (TXB2/6-Keto-PGF1 alpha) in FV blood of ischemic legs were elevated in comparison to those in CV blood or FA blood of the same group or those in FV blood of the controls. 2) This change was more clearly observed in the femoro-popliteal group, but in the aorto-iliac+ combined group, they were elevated not only in FV blood but also in FA blood. 3) No significant difference was observed between the Fontaine I + II group and the Fontaine III + IV group in these local changes in prostanoids. 4) The severer the angiographic manifestation between the abdominal aorta and the common femoral artery, the higher the plasma TXB2 levels in FA blood. Also, the poorer the peripheral run-off, the higher the plasma TXB2 production in the legs. These results suggest that the imbalance between TXA2 and PGI2 is obvious in ischemic legs of ASO patients, TXA2 is elevated downstream from damaged vessels, and the local plasma TXB2 levels reflect the severity of the vascular damage in ASO patients.

[Correction of dyslipidemia in patients with arteriosclerosis obliterans as a method for improving the results of endovascular angioplasty].

The authors analyse the results of endovascular angioplasty of the pelvic and lower limb arteries in 147 patients of various age suffering from arteriosclerosis obliterans. It was found that the results of endovascular angioplasty depended on the state of the lipid balance. The necessity for correcting the shifts in the lipid metabolism is shown. The article evaluates the results of dyslipidemia correction by dietotherapy, pharmacotherapy, and operation for partial ilio-shunting in patients with arteriosclerosis obliterans of the pelvic and lower limb arteries. The efficacy of the listed methods of treatment are analysed according to the severity of dyslipidemia.

[Effect of mild gravitational overload on the progression of lower limb arteriosclerosis obliterans].

The paper analyses the effect of mild gravitational overload (MGO) produced by patient's centrifugation in short-radius centrifuge in cranio-sacral orientation (+Gz) as a part of combined therapy for lower limb arteriosclerosis obliterans. Total 211 patients with I-II grade disease were divided into 3 groups: the first group (59 patients) was treated only by MGO, the second (100 patients) received both MGO and conventional conservative therapy, the third control group (52 patients) underwent conventional therapy only. The best clinical outcome was achieved in the 1st and 2nd groups. Pain-free walking distance increased 2-5-fold (vs 1.5 in control), mean volume blood flow velocity in different arterial beds increased 21-33% (vs 7.5% in control), index of regional perfusion - 25-29% (vs 10% in control), ankle-brachial index - 0.09-0.17 (vs 0.03 in control). Computer termography revealed recovery of leg and foot termographic picture in 86.67%, 92.59%, and 42.86% of cases in the 1st, 2nd and 3rd groups, respectively. Similarly, exercise tolerance measured by bicycle ergometry increased in the 1st and 2nd groups 1.5-1.9-fold (mean 60-85 s) and 1.3-fold (mean 37 s) in control. Thus mild gravitational overload has demonstrated clinical effectiveness in complex therapy of lower limb arteriosclerosis obliterans.

The analgesic effect of racemic ketamine in patients with chronic ischemic pain due to lower extremity arteriosclerosis obliterans.

BACKGROUND: Ketamine in sub-dissociative doses has been shown to have analgesic effects in various pain conditions, including neuropathic and phantom-limb pain, where conventional treatment has often failed. Chronic ischemic pain due to lower extremity arteriosclerosis obliterans often responds poorly to analgesics, and the pain-generating mechanisms are not well understood. METHODS: Eight patients with rest pain in the lower extremity due to arteriosclerosis obliterans were given sub-dissociative doses of 0.15, 0.30, or 0.45 mg/kg racemic ketamine and morphine 10 mg as a 5-min infusion on four separate days in a cross-over, double-blind, randomised protocol. Plasma levels of (S)- and (R)-ketamine and their nor-metabolites were analysed with an enantioselective high-performance liquid chromatography (HPLC) method. Pain levels were evaluated with a visual analogue scale (VAS). RESULTS: Individual pain levels were highly variable during and after all the infusions but the pooled pain levels showed a dose-dependent analgesic effect of ketamine with a transient but complete pain relief in all the patients at the highest dose (0.45 mg/kg). Side-effects, mainly disturbed cognition and perception, were pronounced and dose-dependent. Morphine 10 mg had an analgesic peak at 20 min and 5/8 patients had complete pain relief. These 3 patients also had high baseline pain scores, indicating a higher analgesic potency for the 0.30 and 0.45 mg/kg ketamine doses than for morphine 10 mg. CONCLUSION: We have demonstrated a potent dose-dependent analgesic effect of racemic ketamine in clinical ischemic pain. Due to a narrow therapeutic window, this analgesic effect is probably best utilised in combination with other analgesics.

[The current status of arteriosclerosis obliterans in Japan].

In Japan, the number of patients suffering from arteriosclerosis obliterans has increased remarkably in recent years, especially among the aged population. This paper presents an overivew of the current status of vascular surgery for atherosclerotic occlusive disease and the outlook for the future. It is important that endovascular surgery be further evaluated as an alternative to conventional arterial reconstruction, however, its precise indications and limitations can only be discussed with the accumulation of experience. The history of vascular surgery is extensive and not limited to the development of new techniques and methods of surgery alone. Further development in many fields will surely lead to great possibility in the future.

[Therapeutic angiogenesis by autologous transplantation of bone-marrow cells in a patient with progressive limb ischemia due to arteriosclerosis obliterans: a case report].

A 78-year-old woman with arteriosclerosis obliterans was admitted with complaints of progressive ischemic change and resting pain of the left leg. She had severe ischemic limb in stage IV of the Fontaine classification. Angiography showed total occlusion of the three main arteries with poor collateral flow at the left crus. Autologous transplantation of bone-marrow mononuclear cell to her left leg was performed to achieve therapeutic angiogenesis. One month later, follow-up angiography showed excellent angiogenesis of collateral vessels at the left crus. Her pain was relieved and leg amputation was not necessary. Recently, the TACT study in Japan showed the efficacy of angiogenesis by autologous bone-marrow transplantation for peripheral artery disease. This case illustrates the efficacy of this therapy.
