A comparison of DSM-II and DSM-III in the diagnosis of childhood psychiatric disorders. I. Agreement with expected diagnosis.

This study was conducted to compare DSM-II and DSM-III in the diagnosis of childhood and adolescent psychiatric disorders. Twenty psychiatrist-raters completed standardized diagnostic questionnaires for 24 actual case histories. This report, the first of four, presents the rater agreement with the "expected diagnosis," ie, the diagnosis that we considered most appropriate for each case. The average rater agreement with the expected diagnosis was less than 50%. It was highest in cases of mental retardation, psychosis, hyperactivity, and conduct disorder. In only five cases did the most common diagnosis of the raters differ from the expected diagnosis. Analyses of these cases and those we selected to present specific diagnostic problems to the raters have produced suggestions to improve the reliability of DSM-III.

Prenatal diagnosis of genodermatoses by ultrastructural diagnostic markers in extra-embryonic tissues: defective hemidesmosomes in amnion epithelium of fetuses affected with epidermolysis bullosa Herlitz type (an alternative prenatal diagnosis in certain cases).

We report the first use of amnion epithelium for prenatal diagnosis. Prenatal diagnosis of recessive epidermolysis bullosa atrophicans generalisata gravis Herlitz type can at present be achieved with safety by detailed ultrastructural analysis of fetal skin. Because of the close developmental origin of amnion and skin, which has been elucidated by the recent development of anti-amnion antibodies against dermo-epidermal junction antigens and by their abnormal binding in epidermolysis bullosa skin, there is presumably some morphological relationship between amnion epithelium and skin. In a comparative study of extra-embryonic tissues, we found ultrastructurally complete hemidesmosomes in all 24 investigated normal amnion samples from gestational weeks 15-27, but not in 7 reflected chorion samples from weeks 16-22. The results of placental chorion samples were not reliable. Amnion of 5 fetuses affected with epidermolysis bullosa atrophicans generalisata gravis revealed only hypoplastic hemidesmosomes, the same defect as in the respective skin. In a recent case where unfortunately only non-skin material was available, a positive prenatal diagnosis of epidermolysis bullosa atrophicans gravis Herlitz was performed from the amnion material. The diagnosis was confirmed by investigation of the fetal skin after termination. Investigation of amnion membranes is therefore an alternative for prenatal diagnosis of epidermolysis bullosa atrophicans gravis Herlitz in certain cases. The possibility and limitations of the general use of amnion for prenatal diagnosis are discussed.

Versailles minimal dataset for diagnosis of ALS: a distillate of the 2nd Consensus Conference on accelerating the diagnosis of ALS. Versailles 2nd Consensus Conference participants.

The 2nd Consensus Conference (Versailles) recommended that an ALS knowledge-base for initial healthcare providers, diagnosing neurologists and confirming neurologists should be defined to include a simplified version of diagnostic criteria less formal than the World Federation of Neurology El Escorial Revisted Criteria ('ALS diagnosis - An algorithm'), a set of rules concerning red flags which should increase the suspicion of ALS as the diagnosis and minimize the time between suspicion and referral for confirmation of diagnosis ('ALS axioms of referral'), as well as a site of symptom onset-specific checklist of minimal clinical examination, neuroimaging, electrodiagnostic, pulmonary function and laboratory test information required to confirm the diagnosis of ALS ('Versailles minimal dataset'). Although introductory discussions addressed the advantages and disadvantages of earlier diagnosis, false-positive or false-negative diagnosis, the frequency of follow-up and what potential biological markers to be followed, these issues will have to be further evaluated at future consensus conferences.

Aortic dissection: effect of prospective chest radiographic diagnosis on delay to definitive diagnosis.

PURPOSE: To determine the effect of the interpretation of plain chest radiographs on the time to definitive diagnosis of aortic dissection. MATERIALS AND METHODS: The authors evaluated chest radiographs from 75 patients in whom chest radiography was performed before aortic dissection was diagnosed. The radiographs and available comparison images were retrospectively reviewed to identify cases in which aortic dissection should have been suggested before the diagnosis was made. RESULTS: Radiographic reports suggested that only 19 patients (25%) had an aortic dissection or thoracic aortic aneurysm or needed additional imaging of the aorta. No statistically significant correlation existed between interpretation of the chest radiographs and delay to diagnosis, type of dissection, availability of previously obtained images, or presence of characteristic clinical symptoms. Retrospective analysis showed that the chest radiographs of 36 patients (48%) contained sufficient findings to suggest the diagnosis. In five patients, failure to prospectively suggest dissection was associated with a delay to diagnosis of more than 24 hours. CONCLUSION: Although prospective interpretation of the chest radiograph shortened the delay to definitive diagnosis for only a limited number of patients, retrospective analysis suggests that the delay can be shortened for additional patients.

[Neurotic depression--a disease without a diagnosis or a diagnosis without a glossary? ].

With the transition from ICD-9 to ICD-10 the diagnosis of neurotic depression was omitted. Freyberger showed that this diagnosis in the ICD-10 was replaced mainly by the diagnoses of dysthymia, recurrent depression and depressive episode (with this ranking of frequency). A renowned German psychiatrist criticized this change as replacing an unsubstantiated dichotomic with an unsubstantiated dimensional model. The same was the case with the change from DSM-II to DSM-III: Torgersen criticized here that the heterogeneous diagnosis of neurotic depression was basically replaced by the similarly heterogeneous diagnosis of major depression. The underlying rationale behind the omission of the traditional diagnosis of neurotic depression in the new glossaries must be seen in the critical contributions of such renowned researchers as Winokur, Klerman and Akiskal during the 60's and 70's of the last century. The present work compares these exclusively phenomenological approaches to classification with a psychodynamic approach, and the author defends the possibility of operationalizing such an attempt as validly as a mere descriptive one. This view is supported by the successful work of a group of German scientists in promoting the OPD (operationalized psychodynamic diagnostics) system. Operationalizable elements are not only to be found in the critics' own contributions, but in recent empirical studies on dysthymia and the other subsequent diagnoses as well. An operationalized and verifiable diagnosis of neurotic depression would have to go far beyond the two main criteria of the ICD-9 (psychoreactive genesis, exclusion of psychosis) and include new insights and perspectives; this, however, is considered feasible. It must be recognized that there is no place in the prevailing diagnostic scene for such a diagnostic construct. Perhaps with time diagnostic modes will change, as has often been the case in the past.

[Study on genetic diagnosis and prenatal diagnosis of alpha-thalassemia].

OBJECTIVE: To develop a single-tube multiplex polymerase chain reaction (mPCR) technique to detect three common deletional alpha-thalassemias (alpha-Thal) in Chinese, and to perform genetic diagnosis and prenatal diagnosis for an alpha-Thal family from Hebei province, China. METHODS: Fourty-two blood samples including samples from one alpha-Thal family from Hebei province were assayed. The mPCR containing 7 primers, gel electrophoresis and DNA sequencing were used for the genetic diagnosis and prenatal diagnosis. RESULTS: The gene types of the fourty-two DNA samples analyzed by the mPCR-gel electrophoresis technique were in accordance with the results by Southern blot and three separate PCR techniques. A HbH child and a fetus of the alpha-Thal family were diagnosed as--(SEA)/alpha(cs)alpha and alpha alpha/alpha alpha respectively by using the mPCR and DNA sequencing. The result of postnatal analysis of the cord blood was consistent with the prenatal result (alpha alpha/alpha alpha). CONCLUSION: The developed mPCR technique can be used for genetic diagnosis and prenatal diagnosis of the 3 deletional alpha-Thal in Chinese.

[Diagnosis and differential diagnosis of psoriatic arthritis on basis of follow up of 215 cases].

The interval between the appearance of the symptoms of psoriasis and/or arthritis and the setting up the diagnosis of the psoriatic arthritis was studied in 215 patients suffering from definite psoriatic arthritis. About 2.3 years were over until the setting-up of the diagnosis in these 30 patients whose psoriasis and arthritis began simultaneously. The interval to set up the diagnosis of psoriatic arthritis was 5.4 years in average if the psoriasis itself was the first sign and it was 8.6 years in case the arthritis preceded psoriasis. The symptoms promoting to set up the right diagnosis were in order to frequency as follows: appearance of psoriasis, sausage digits, distal interphalangeal involvements, nail changes and transformation the monoarthritis into asymmetrical oligoarticular or polyarticular form. The difficulty of the differential diagnosis was studied. 15 different previous false diagnoses were enumerated the rate of which was the highest (67.3%) in the group starting with arthritis. The authors call the attention to the importance of looking for psoriatic skin and nail changes in every nonclassified arthritic patient in the interest of an early diagnosis and right therapy in course of the follow-up.

[Radiological diagnosis (and differential diagnosis) of lethal type (congenital type) of hypophosphatasia (author's transl)].

Diagnosis of Hypophosphatasia can be proved by typical radiological findings. The paper presents a case report of a newborn living only a few hours. Diagnosis according to clinical aspect was "Osteogenesis imperfecta" although a postmortal x-ray-babygram was available but which had not been shown to a radiologist. Anatomical diagnosis with the histological findings was then altered to "Metaphyseal chondrodysplasia, Jansen-type". Finally the diagnosis could be corrected to congenital, lethal type of Hypophosphatasia after the babygram had been shown to a radiologist. The typical radiological features of congenital Hypophosphatasia are described subsequently and differential diagnosis of Congenital Osteochondrodysplasias is discussed. The importance of early or postmortal diagnosis for genetic counselling is stressed as well as a close cooperation between obstetricians and pediatric radiologists. Typical biochemical findings are mentioned.

[Nuclear magnetic resonance tomography of the breast: diagnosis, differential diagnosis, problems and possible solutions. II: Diagnosis].

Eighty-five MR examinations were carried out on both breasts of 72 patients. A special surface coil was used. The results were compared with the findings of mammography and with the post-operative findings. There were 16 fibrocystic mastopathies, seven isolated cysts, 22 fibro-adenomas and 25 carcinomas. Differentiation between fibro-adenomas and carcinomas is only possible on the basis of morphological criteria. Cysts and fibrocystic mastopathies can be identified with certainty. Typical patterns of the various pathological conditions are described, but these will have to be confirmed by the study of a larger number of patients, in order to make them statistically significant. The advantage of MR consists in the simultaneous demonstration of the thoracic wall and the axilla.
