[Effect of immunosuppressants FK506 and rapamycin on the function of progesterone receptor: protein "p59-HBI", intersection between immunology and endocrinology?].

In the absence of hormonal ligand, inactive, heterooligomeric, 8-10S steroid receptor complexes include a p59 protein (apparent M(r) approximately 59 kDa) bound to th heat shock protein hsp90 (apparent M(r) approximately 90 kDa), which itself binds to the ligand binding domain LBD of the receptor molecule, p59 is thus an hsp binding immunophilin HBI, which, through its interaction with a chaperone, may intervene in several cellular functions. We report that, in cell-free experiments at 0 degrees C, FK506 and rapamycin do not release p59 nor hsp90 from the 9.5S rabbit uterus progesterone receptor, suggesting that the binding of p59 to hsp90 does not interfere with the rotamase site of HBI. There is no "transformation/activation" of the receptor, but an up to 2 fold increase in progesterone agonist and antagonist binding to the receptor is observed. It is suggested that a functional interaction between HBI and receptor activity may be mediated by hsp90.

Dentists appear to plan the treatment of one and the same patient very differently. Each dentist uses both professional and relational concepts, but the differences in treatment plans are in this article just partially explained.

Attention, the intersection of cost and quality in health care.

A method for assessing health care technology that models the demand on the clinician's attention exerted by patients' data (diagnostic and therapeutic) can provide a means for simultaneously reducing the cost and improving the quality of health care. The attentional demand exerted by patients' data can be measured by the amount of uncertainty in the data. Uncertainty can be expressed mathematically by the concept of entropy in information theory.

[Fatal accidents at train intersections in Denmark].

Material was collected concerning fatal level-crossing accidents in Denmark during the period 1.1.1980 to 31.12.1987. The epidemiological and traumatic circumstances are illustrated. A total of 51 fatal level-crossing accidents occurred which corresponds to 1% of all the annual traffic deaths in Denmark. The majority of the accidents occurred in good weather conditions and between 12 noon and 3.59 pm. Single-person accidents constituted 75%. The majority of the persons killed belonged to the local population and were familiar with the local conditions. The majority of the accidents were the local conditions. The majority of the accidents were due to poor visual conditions and lack of attention. In level-crossings with a keeper, the commonest causes were blinding by the sun and fog. The majority of the accidents occurred in level-crossings marked by light and sound signals. The risk for accidents in these crossings is six times as great as in crossings with half barriers. Private cars were most frequently involved and the persons killed in these died of head and cerebral injuries because the train buffers hit them at head height. Derailing of the trains was only seen on three occasions where the obstructing vehicles were heavy lorries. The fact that more derailings did not occur must be due to the low rate of the trains in the railway sidings where the majority of accidents occurred. A few proposals are made for prophylactic measures.

[Diagnostic usefulness of Ca 19-9 antigen in cancer of the pancreas and neoplasms of the pancreato-biliary intersection].

Antigen CA 19-9 was measured in the serum of a group of 16 patients with pancreatic cancer, in 19 patients with non-pancreatic gastrointestinal malignancies, and in 19 others with benign pancreatobiliary disease. The test showed a high sensitivity with a low specificity and a positive predictive value for diagnosis of pancreatic cancer when confronted with a group of controls that included both benign and malignant gastrointestinal disease. The low specificity was due to high CA 19-9 values in other gastrointestinal cancers, especially in carcinoma of the ampulla and the biliary tract. When the latter were added to the group of patients with pancreatic cancer, and the diagnostic accuracy of the test was re-evaluated, sensitivity, specificity and both positive and negative predictive values increased. Values of CA 19-9 over 100 U/ml are highly predictive of malignancy in patients with pancreato-biliary disease.

[Clinico-angiographic indications for surgical intersection of the superficial temporal artery in patients with central chorioretinal dystrophy].

Clinical and angiographic assessment of the status of 28 patients with sclerotic central chorioretinal dystrophy, carried out before and after surgery, has revealed a discrepancy between the improved hemodynamics in the orbital artery and vascular coating and the clinical results of the operation. Analysis of the fluorescent angiograms of the fundus oculi has helped detect the patients with the 'dry' and pseudotumorous forms of the disease, for whom crossing the superficial temporal artery is indicated.

Intersecting languages in psychoanalysis and philosophy.

The introduction establishes a general view of the literature in which philosophers have profited from their encounters with analysis. It provides a frame within which to present more specific ideas about the method and language of psychoanalysis as viewed by philosophers. The method of interpretive reconstruction is unfolded from its original context of Freud's archaeological analogy. Further, the vocabulary of reconstruction, which is an intimate part of this analogy, is employed by Anna Freud is her discussion of defense mechanisms. Texts are cited and explicated. Meanwhile, the method of reconstruction is given independent, though related, application in the work of R. G. Collingwood, an archaeologist-philosopher-historian. The juxtaposition of Freud and Collingwood suggests that the methods of philosophy and analysis are more alike than the particular problems they try to solve. Both methods are oriented toward solving the problem of discovering meaning amid absurdity. The introduction of two specific examples lends substances to this claim. In the final section on the practice of interpretation, the question is raised as to how the introduction of the method of reconstruction affects the debate about the epistemological status of psychoanalysis as a science. Psychoanalytic knowledge shows itself to be more like that available to the historian than that accessible through physical theories. Still, physics and analysis can be compared. One must look to the interpretation of symbols. In psychoanalysis, giving an interpretation--in which nonsense becomes understandable--is a form of explanation. This methodological result suggests a conclusion about the relation between metapsychology and clinical practice.

Susceptibility of hepatic microcirculation to reperfusion injury: a comparison of adult and suckling rats.

Primary graft failure and vascular thromboses are frequent complications of liver transplantation, yet the mechanisms responsible remain unclear. Previous work from our laboratory has shown that hepatic reperfusion injury results in damage at the microvessel level. The present study was performed to determine whether an increased susceptibility of immature animals to microvascular injury during reperfusion might be a contributing factor in these complications. Suckling (35 to 50 g) or adult (250 to 400 g) rats were subjected to 30 or 60 minutes of hepatic ischemia to the left and median lobes followed by 90 minutes of reperfusion. Control animals were sham-operated, time-matched rats. At the end of reperfusion, fluorescein-labeled albumin was injected systemically to mark perfused sinusoids. Frozen sections of liver biopsies were viewed under fluorescence microscopy. The perfused sinusoid density was determined by point count analysis and expressed as the number of intersections of perfused sinusoids with 25 randomly oriented points superimposed on the sinusoid field. In sham-operated rats, at both 30 and 60 minutes, there were no differences between sucklings and adults. After 30 minutes of ischemia and 90 minutes of reperfusion, adults showed a significantly decreased density of perfused sinusoids (4.5 +/- 0.1 intersections per field) when compared with suckling rats (6.0 +/- 0.3 intersections per field, P less than .001). However, in rats subjected to 60 minutes of ischemia followed by 90 minutes of reperfusion, the microvascular injury was more severe in suckling rats (2.7 +/- 0.2 intersections per field) than in adults (4.7 +/- 0.2 intersections per field, P less than .001).(ABSTRACT TRUNCATED AT 250 WORDS)
