Early ultrastructural adaptive changes of ileal enterocytes after proximal small bowel resection as determined morphometrically.

The aim of the present study was to evaluate in terms of quantitative measurements whether the well-known histomorphological and functional adaptive changes in the intestinal mucosa after small bowel resection are accompanied by alterations on the ultrastructural level. Therefore, samples of the ileal remnants after a 60% proximal resection were processed for ultrastructural evaluation and analyzed employing point counting planimetry and direct measurements. Microvillus surface area increased from the bottom of the crypts to the villus tips in both resected and sham-operated animals. This increase in microvillus surface area from the crypt to the villus was significantly less pronounced after proximal resection, while there were no changes in the crypt compartment. No significant differences of the relative areas of the nuclei, mitochondria, and the rough endoplasmic reticulum were observed when comparing the different positions along the villus crypt axis in normal and hyperplastic mucosa. In agreement with functional and enzyme histochemical results, these ultrastructural findings provide further evidence for an altered pattern of enterocyte maturation after proximal resection, which is most probably due to an increase in the migration rate of the enterocytes.

Immunohistochemical study of the expression of MUC6 mucin and co-expression of other secreted mucins (MUC5AC and MUC2) in human gastric carcinomas.

To investigate the expression of MUC6 mucin in gastric carcinomas, we generated a novel monoclonal antibody (MAb CLH5) using an MUC6 synthetic peptide. MAb CLH5 reacted exclusively with the MUC6 peptide and with native and deglycosylated mucin extracts from gastric tissues. MAb CLH5 immunoreactivity was observed in normal gastric mucosa restricted to pyloric glands of the antrum and mucopeptic cells of the neck zone of the body region. In a series of 104 gastric carcinomas, 31 (29.8%) were immunoreactive for MUC6. The expression of MUC6 was not associated with histomorphological type or with clinicopathological features of the carcinomas. Analysis of the co-expression of MUC6 with other secreted mucins (MUC5AC and MUC2) in 20 gastric carcinomas revealed that different mucin core proteins are co-expressed in 55% of the cases. MUC6 was co-expressed and co-localized with MUC5AC in 45% and with MUC2 in 5% of the cases. Expression of MUC2 alone was observed in 25% of the cases. All carcinomas expressing MUC2 mucin in more than 50% of the cells were of the mucinous type according to the WHO classification. The co-expression of mucins was independent of the histomorphological type and stage of the tumors. In conclusion, we observed, using a novel well-characterized MAb, that MUC6 is a good marker of mucopeptic cell differentiation and is expressed in 30% of gastric carcinomas, independent of the clinicopathological features of the cases. Furthermore, we found that co-expression and co-localization of mucins in gastric carcinomas is independent of histomorphology and staging. Finally, we observed that intestinal mucin MUC2 is expressed as the most prominent mucin of the mucins tested in mucinous-type gastric carcinomas.

Intestinal granuloma formation in normal and SCID BALB/c mice infected with Angiostrongylus costaricensis.

We studied the histomorphology of granuloma formation and cytokine production in Angiostrongylus costaricensis-infected immunocompetent and immunodeficient BALB/c mice. Histological examination of the infected intestine showed well developed granulomas in BALB/c mice. In contrast, in SCID mice, clusters of eggs and larvae and a progression of infiltration of inflammatory cells were also observed, but little fibroblastic activity was seen. Analysis of plasma of the infected mice demonstrated a dramatic contrast in the cytokine profile between normal and SCID mice. Normal mice infected with A. costaricensis showed highly elevated plasma IFN-gamma levels at week 3 of infection, but plasma IL-4 remained close to the background levels obtained from non-infected mice. In contrast, the amount of those cytokines in the plasma of SCID animals was little affected by this parasitic infection. These results suggest that Th1-mediated immunity is required for granuloma formation in response to A. costaricensis infection in mice.

[Parallel morphological, enzyme and immunological research on the jejunal mucosa in patients with gluten enteropathy, chronic nonspecific enteritis and ulcerative colitis].

The results are reported from the study on 120 patients, 30 of them with gluten enteropathy, 50--with chronic unspecified enteritis and 40--with chronic ulcerous colitis. Small intestinal aspiration biopsy was performed to all patients with histomorphological, electron microscopic, enzymatic and immune-fluorescent study. The secretory immunoglobulins in the jejunal juice were studied in 40 patients. Morphological changes, various degrees, were established, that correlate with the increased number of IgG and IgM secretory cells. The exclusion of gluten from the diet in case of gluten enteropathy was followed by normalization of the number and percentage ratio of IgSC and of secretory immunoglobulins. The enzymatic and immune disorders, in chronic unspecific enteritis correlate with the electron microscopic and precede the histologic ones. The role of immune system of small intestine in the etiology and pathogenesis of the various intestinal diseases is discussed.

[Snow white small intestinal villi in hypobetalipoproteinemia].

In contrast to the severe clinical picture of abetalipoproteinemia patients with hypobetalipoproteinemia are often asymptomatic. We demonstrate a 52-years-old female patient with a white mucosa of the small intestine casually observed by endoscopy. The white appearance of the mucosa was limited to the villi. As demonstrated by light and transmission electron microscopy this was caused by fat loaded enterocytes similar to the picture of abetalipoproteinemia. Fasting serum lipids and apolipoproteins were only if the lower norm level for some parameters, but no increase of the serum lipids was observed after an oral fat load. Because of the missing symptoms, the typical histomorphology and laboratory findings the snow white mucosa of the small intestine is due by the hetocygote form of the autosomal dominant hypobetalipoproteinemia with fat loaded enterocytes.

[Intestinal leiomyoma. Case report and discussion of the difficult assessment of grade].

The case of a male patient operated for a leiomyoma of the jejunum is presented. The clinical symptoms, the morphological differential diagnosis and the problems evaluating aggressiveness of this tumour are discussed. We intend to demonstrate that histopathological diagnostic implicate a certain grade of uncertainty too. With the example of leiomyoma we like to show that diagnosis means a tight cooperation of the clinician and his pathologist by collecting clinical and histomorphological data.

Clinical long-term follow-up results in intestinal neuronal dysplasia (IND).

Commonly available information on intestinal neuronal dysplasia (IND) is sparse. Especially well documented long-term courses are lacking. The aim of this study was to correlate defecation as a clinical parameter of the long-term course in malformations of the enteric nervous system with the morphological diagnosis. 57 children with intestinal neuronal dysplasia (IND) or aganglionosis with cranial intestinal dysganglionosis (agIND), diagnosed between 1983 and 1992, were analysed including histomorphological classification, collection of clinical data and evaluation of the defecation mode by questionnaire as a parameter of the long-term course. Of 29 dysganglionic (IND) patients, 9/29 cases (31 %) had been treated conservatively, 18/29 cases surgically (62.1%), in two children (6.9%) no therapy had been necessary. All 28 patients with Hirschsprung's disease and cranial IND (agIND) underwent resection. 46 of the children could be followed up 3.64 years after the end of the main therapeutic period and with a mean age of 6.7 years at the time of follow-up; 43.5% of the analysed children still showed severe constipation. 23.9% only were really cured; 15.2% had normal defecation still using conservative treatment and 17.4% had diarrhea. No significant difference was found between both groups, IND and agIND, and the results were independent of treatment modality. The results were much worse than in idiopathic constipation as reported in the literature and even worse in comparison to unselected Hirschsprung collectives. It has to be concluded that in IND with chronic constipation intensive long-term care is necessary and it is crucial that treatment algorithms should be outlined urgently together by pediatric gastroenterologists and pediatric surgeons. AgIND seems to need more extended resection following an exact histomorphological mapping by biopsies taken during enterostomy procedure.

On the progenitor cell migration velocity.

An attempt is presented to extract cell kinetic information from histomorphological features. It is applicable to rapidly proliferating tissues like the intestinal epithelium. Each replicating tissue has an origin where cells are formed and a periphery toward which cells migrate. The migration path along which they move is denominated as tissue radius on which all cell positions are mapped. Cell migration on the radius is associated with cell proliferation at tissue origin. Each mitosis there is associated with the displacement of all cells distal to it by one cell position. The more mitoses positioned between a cell and tissue origin, the greater its migration velocity. It is possible therefore to derive the cell migration velocity v(x) from the cumulative mitotic distribution on the radius, N(x). v(x) = N(x)/tm (tm = mitotic time). In this form v(x) represents also cell production at any point on the radius and may serve for the computation of other cell kinetic parameters like generation time. These arguments are illustrated on the rat incisor tooth inner enamel epithelium which has been studied in the normal and rapidly erupting tooth.

Absorption and excretion of iodixanol after intragastric administration to rats.

Absorption and excretion of iodixanol 320 mg I/ml were investigated in rats after intragastric administration of 2.5 g I/kg b.w. Animals were observed for up to 96 hours after treatment, and blood, urine and feces taken at several time-points throughout the experiment. Concentrations of iodixanol in serum and urine were measured by means of reversed-phase high-performance liquid chromatography. Fecal concentrations of iodixanol, based on iodine measurements, were determined by X-ray fluorescence spectrometry. Serial radiographs were obtained and histopathological examination was performed on selected tissues. The results indicate that less than 1% of the intragastric dose of iodixanol is absorbed from the intestine into the blood stream. No adverse clinical signs were observed, and there were no treatment-related histomorphological findings.

[Gastrointesinal stromal tumors--clinical and morphological aspects].

Gastrointestinal Stromal Tumors (GISTs) are uncommon mesenchimal tumors that usually arise in the wall of the gastrointestinal tract. Although heterogenous in morphology, GISTs represent a group of tumors probably originating from the intestinal pacemaker cell, interstitial cell of Cajal. The tumour cell phenotype can be established only when histomorphology is coupled with immunostaining. They account for approximately 0.1% to 3% of all gastrointestinal neoplasms. GISTs arise anywhere along the GI tract, from the oesophagus to the rectum. Most tumors (70%) occur in the stomach and about 20-30% occur in the small intestine. A small percentage of GISTs arise elsewhere in the GI tract, omentum, or retroperitoneum. We describe four GISTs arising in stomach, small intestine, omentum and colon with their clinical and morphological presentation.

Comparison of endoscopic brush cytology with biopsy for detection of Helicobacter pylori in patients with gastroduodenal diseases.

Upper GI endoscopy was performed in 50 adult patients attending the gastroenterology OPD with gastroduodenal diseases. Gastric antral mucosal brushings were taken for cytological evaluation and urea broth test followed by antral biopsy. The slides prepared were stained with Wright Giemsa (WG), Hema toxylin & Eosin (H&E) and Papanicolaou (Pap). Brush Cytology Smears were compared with biopsy sections for detection of Helicobacter pylori and the staining techniques were evaluated to see which stain was easiest to perform and interpret on routine basis. H&E stained sections were examined for histomorphological parameters associated with H. pylori infection. Brush cytology was rapid and simple with observed sensitivity of 90% and specificity of 100%. On evaluating the various stains, M.B. and W.G. were found to the rapid and easy to perform and the bacteria stood out very well against the background. Of the histomorphological parameters studied, presence of actively and lymphoid aggregates and absence of intestinal metaplasia correlated positively with the presence of the bacteria. Cytological brushing urea broth test was not very sensitive (55.2%) and specific (51%) for detection of the bacteria.

[Pathogenesis and histomorphology of ampullary carcinomas and their precursor lesions. Review and individual findings].

Most adenomas and carcinomas of small intestine and extrahepatic bile ducts arise in the region of Vater's papilla. In FAP it is the main location for carcinomas after proctocolectomy. In many cases symptoms due to stenosis lead to diagnosis in an early tumor stage. In about 80% curative intended resection is possible. Operability is the most relevant prognostic factor. Inflammatory changes, fibrosis, regeneratory changes after endoscopic manipulation, hyperplasia, preneoplastic lesions close to carcinoma, deeply sited carcinomas under protruded, non-neoplastic duodenal mucosa make the diagnosis difficult on biopsy material. Histologically, intestinal type adenocarcinoma, pancreatobiliary type adenocarcinoma, undifferentiated carcinomas and unusual types can be differentiated. In our own series comprising 45 resected ampullary carcinomas 6 from 10 intestinal type adenocarcinomas, and 4 carcinomas of unusual types expressed the immunohistochemical marker profile of intestinal mucosa (keratin 7-, keratin 20+, MUC2+). 17 from 21 pancreatobiliary type adenocarcinomas, 4 undifferentiated carcinomas, as well as 3 papillary carcinomas showed the immunohistochemical profile of pancreaticobiliary duct mucosa (keratin 7+, keratin 20-, MUC2-). 3 invasive carcinomas which were negative for these markers, showed one of these characteristic marker-combinations in non-invasive adenomatous parts. These findings support the concept of histogenetically different ampullary carcinomas which are developing from the intestinal or from the pancreaticobiliary type mucosa of Vater's papilla. Molecular alterations in ampullary carcinomas are similar to those of colorectal as well as pancreatic carcinomas, although they appear in different frequencies. In future studies molecular alterations in ampullary carcinomas should be correlated closely with the different histologic tumor types. The histologic classification should reflect consequently the histogenesis of ampullary tumors from the two different types of papillary mucosa.

[Analysis of prognostic factors in gastrointestinal stromal tumors of the small intestine].

OBJECTIVE: To explore the prognostic factors in patients with gastrointestinal stromal tumors of the small intestine. METHODS: Tumor slides stained with hematoxylin and eosin from these patients were reviewed. Two histomorphologically representative areas were identified and arrayed on a tissue microarray. Immunohistochemistry staining were performed using antibodies to detect the expression of c-kit protein (CD117), CD34, smooth muscle actin, desmin, S-100, Ki-67, P53 and bcl-2 protein. The relationship between clinicopathologic features and prognosis was analyzed by univariate analysis. RESULTS: The 1-, 3-, 5-year survival rate of 58 such patients were 98.3%, 69.7%, and 50.9% respectively. The prognosis was related with tumor size and gender by univariate analysis (P< 0.05). CONCLUSION: More attention should be paid to the male patients with small intestine stromal tumors, especially those with tumors size> 5 cm, because those tumors are more likely to metastasize than smaller tumors (< or = 5 cm).

Resistance of gnotobiotic nude mice to Listeria infection.

Unlike the acute and short-term course of experimental listeriosis in phenotypically normal (nu/+) mice, Listeria monocytogenes infection in conventionally raised athymic Nude mice results in chronic inflammation with persistence of bacteria and lesions in the internal organs such as liver and spleen. Although it could be shown that germfree Nude mice had no intestinal microbial flora and lacked measurable serum immunoglobulins, neither the course nor the histomorphology of experimental listeriosis differed significantly from that of their conventionally raised littermates. The results indicate that the typical response of Nude mice to Listeria infection is primarily determined by inborn factors and not by prior antigenic stimulation.

[Pathomorphological and histochemical changes in the intestines of white rats due to low-frequency vibration].

The authors carried out experiments on animals (40 white male rats) which were subjected to daily a two-hour action of the common vertical sinusoidal vibrations with a frequency of 20 Hr (I group) and 50 Hr (II group) with acceleration of 22,0 m/sec2 for a period of ten days (I group--A and second group--A) and 30 days (I group--B and II group--B). Histomorphological and histochemical methods were used and the authors found changes in microcirculation and structure of the intestinal wall manifested in the animals of the II group. The obtained results were in support of the clinical data on the disturbances in intestinal function in patients with vibration disease and corresponded to the experimental data on the occurrence of vascular and dystrophic changes in the internal organs under vibration action.

Preterm as compared with full-term neonatal calves are characterized by morphological and functional immaturity of the small intestine.

Intestinal diseases in neonatal calves may be due to morphological and functional immaturity. We have studied histomorphology, crypt cell proliferation rates (based on incorporation of 5-bromo-2'-deoxyuridine into DNA), presence of apoptotic cells (based on terminal deoxynucleotidyl transferase-mediated X-dUTP nick end labeling), and brush border enzyme activities in preterm calves (277 d of gestation), euthanized on d 1 (P0) or 8 (P8), and in full-term calves (290 d of gestation), euthanized on d 1 (F0) or 8 (F8). Vacuolated epithelial cells were present in ileum of P0 and F0 but not in P8 and F8. During the first 8 d, villus sizes, crypt depths, and proliferation rates of crypt cells in the small intestine of preterm calves did not significantly change. In contrast, in full-term calves during the first 8 d, villus sizes in jejunum decreased, crypt depths increased in small intestine and colon, and crypt cell proliferation increased in duodenum and jejunum. Submucosal thickness in jejunum was highest in P0, but in ileum it increased with gestational age and feeding. Gestational age x feeding interactions indicated increased activities of aminopeptidase N and reduced lactase activities only in F8 and reduced dipeptidylpeptidase IV activities only in P8. In conclusion, in preterm calves the small intestinal epithelium was immature and brush border enzyme activities differed in part from those in full-term calves.

[Pathogenic action of the duodenal contents in acute and chronic exacerbated human pancreatitis].

The authors carried out studies on 16 rabbits - 10 experimental and 6 control with implanted duodenal fistula. They poured for a period of 4 days through the fistula the duodenal content of 6 sick persons with acute and chronic relapsing pancreatitis into the duodenum of the experimental animals and the content of 2 healthy persons-to the animals of the control group. After each pouring the activity of pancreatic and intestinal enzymes was determined in sera and duodenal contents of the rabbits. The duodenum, pancreas, liver and kidneys were examined histomorphologically after killing the experimental animals. The obtained results showed that there were no changes in the enzyme activity and morphologic picture of the organs after pouring duodenal content of healthy persons. The enzyme activity was reduced in the duodenal content of the experimental animals, but in the sera was increased. Pancreatitis, duodenitis, fatty dystrophy of liver and parenchymatous dystrophy of kidneys was established histomorphologically. The authors suggest the appearance of some toxic substances in the duodenal content of sick persons with acute and chronic relapsing pancreatitis.
