[Prevalence and certain characteristics of chronic hyperplastic laryngitis].

Screening for chronic hyperplastic laryngitis (CHL) has been performed among 70,000 workers and peasants of the Moscow Region. CHL was detected in 355 examinees (0.47%). The data were analysed for 166 CHL patients (age 31-50, 87 males, 29 females). 8 patients had laryngostenosis but tracheostoma was indicated only one of them. Malignant transformation occurred in 3 patients. Laryngeal lesions were symmetric in 36 patients, unilateral with separate inflammatory infiltrates clinically similar to tumors were seen in 45 patients. Local forms of CHL may look as separate small nodules, contact lesions in the region of vocal processes of the arytenoid cartilages, and diffuse inflammatory edema of the vocal cords. In rare cases inflammatory tumor-like laryngeal lesions in CHL depend on local flock of lymphoid elements--lymphoid hyperplasias. A cytological study in CHL is effective in identification of the signs of dystrophy, keratinization and epithelial cell atypia as well as the presence of microflora.

[Gas composition of capillary blood of patients with laryngeal cancer in the course of conservative surgical treatment with polymeric endoprosthesis].

Gas composition of capillary blood was studied in 120 patients with laryngeal cancer, primarily of stage IIIa (T3N0M0). Hypoxia was present in all the examinees whether or not laryngostenosis was clinically apparent. The analysis has shown that conservative surgery of laryngeal cancer with application of tubal polymeric endoprostheses corrects ventilatory disorders and improves blood gas composition. The degree of correction of ventilatory disorders depends on the kind of respiratory intraoperative maintenance. Gas composition appeared significantly better in the group of patients exposed to transtracheal high-frequency artificial pulmonary ventilation than in the group exposed to volumic artificial pulmonary ventilation.

[Tracheostomy in patients with locally advanced laryngeal cancer].

The authors present the results of treatment of 62 patients with locally advanced laryngeal cancer (LALC) stage III-IV (T3-4NOMO) treated in 1986-2000. The study was made of tracheostomy effects in laryngostenosis following radical operation (not tracheostomy as a stage of laryngectomy) on regional and distant metastasizing as well as on further growth, tumor recurrence and survival in treated patients with LALC. The authors conclude that tracheostomy conducted before radical surgery has a negative effect on heeling the postoperative wound, promotes regional metastasizing and tumor recurrence in patients with LALC, lowers 5-year survival.

Laryngeal trauma.

Laryngeal trauma may be obscured by associated injuries of the face and chest. An early assessment of these injuries should be made. Many cases may require observation only. In patients with severe laryngeal fractures, preservation of the airway and control of hemorrhage must first be assured. Following this, splinting by external or internal means should be carried out. Three severe laryngeal injuries were splinted with open and closed plastic moulds, with satisfactory results. It is proposed that early recognition and proper management of severe cases of this nature would shorten the treatment and achieve the best results.
