Percutaneous ethanol injection therapy for neoplasms located on the surface of the liver.

There has been a reluctance to perform percutaneous ethanol injection therapy on lesions located on the surface of the liver because of the possibility of complications. We treated 16 lesions located on the surface of the liver in 14 patients by percutaneous ethanol injection and evaluated the complications and efficacy. Bleeding and seeding of malignant cells did not occur in any case. However, transient pain after injection of ethanol was more intense in these patients than in those whose lesions were not on the surface of the liver. Histopathologic examinations, imaging findings, and decrease in the serum alpha-fetoprotein levels showed that percutaneous ethanol injection was effective for tumors located on the surface of the liver. Our experience suggests that percutaneous ethanol injection can be performed safely even when lesions are located on the surface of the liver.

[Diagnosis and therapy of neoplasms as seen by atomic pathology].

The structure and function of an organism are only the two aspects of the same reality. Totally identical tissue pictures which we find under a microscope can also be obtained directly from the inside of an organism by the use of magnetic resonance (nmr) of atoms making up these tissues. The histopathological picture shows the structure in vitro only whereas an nmr picture shows it in vivo, in addition to the function as the atoms making it up are in constant motion changing their behaviour depending on the state of the environment. Thus this technique can be used to examine both the structure of organs with the flow of fluids in them and the concentration of components of their metabolism. Medical thermodynamics generalizes and unifies at the same time the world of atoms with psychic and emotional life of a man. Based on cellular pathology, three (used independently or together) classical treatment procedures, surgery, irradiation and chemotherapy, are only symptomatic treatment, not casual treatment. Each of them may, however, modify the neural, hormonal and immunological activity of an organism. In the view of more and more evident identity on the molecular level of these morphologically isolated systems, hormonotherapy and immunotherapy have finally come into use. Since neoplasms are a natural phenomenon, this requires from people, and not only doctors--a radical change of opinion on their nature, thusit requires directing intellectual and material activity towards also natural protection against them. Historical experience shows that neoplasms cause a complete specific, different from all others, kind of diseases. That is why also therapy must be different, based on atomic pathology and consistent with laws of Nature.

[Internal radiation therapy for malignant neoplasm].

Internal radiation therapy selectively targets beta- or alpha-emitting radionuclides to the area of the tumor tissue, and is therefore capable of treating disease regardless of the location and number of foci. The biological effect of internal radiation therapy is thought to be different from that of conventional external beam radiation. Thyroid cancer: The local recurrence and metastatic lesions from differentiated thyroid cancers can be controlled with 131I administration. Even though the patient does not have macroscopic disease, 131I is also utilized for thyroid remnant ablation in locally advanced cases. Recently, the maximum tolerable dose can be calculated based on the dosimetry of each patient, and safely administered. The therapeutic effect of this method is superior to the fixed dose method. 131I-MIBG: 131I-MIBG is taken up by sympathetic neurons as well as a group of tumors originating in the neural crest, especially phecromocytomas and neuroblastomas. The various symptoms caused by the hypersecretions of hormone-producing tumors can be improved. Pain palliation of bone metastases: Pain palliation using 89Sr is a very promising option in treating patients with painful bone metastases. The pain palliation mechanism of 89Sr is different from other drugs; therefore, complimentary usage is reasonable. The symptomatical improvement can last for several months, thus helping to maintain the quality of life of the patient.

[Optimization of methodology and extension of indications for beta therapy of neoplasms of the eyelids, conjunctiva and cornea].

The paper offers a theoretical foundation and results after clinical approbation of the method developed by the authors for beta therapy by enlarged single doses using, if necessary, a speeded-up fractionating and split courses of radiation. Beta therapy was used in 220 patients with tumors of the eye lids, conjunctiva and cornea, single doses of radiation, 40 Gr daily or each other day. By their morphologic type the tumors were mostly malignant epithelial and pigmented new-formations (sarcoma, melanoma). In the follow-up period, from 1 to 6 years, the following results were recorded: the treatment proved effective for epithelial tumors in 97%, melanocytic--in 73%; recurrences appeared in 3%; the number of postradiation complications made up 6.6%, of them disturbances in visual functions--2%: death from metastases in melanoma--8.7% of cases. The proposed method of radiation allowed to increase indications for beta therapy of this pathology to 5-6 mm of the tumor protrusion, to decrease the number of complications and terms of treatment. As a result of computer analysis of clinical results, a new method for calculation of optimal summary doses of beta radiation was worked out, that can be used by practitioners in conducting beta therapy for tumors of the eye lids, conjunctiva and cornea.

[Photodynamic diagnosis and therapy of neoplasms of the facial skin after topical administration of 5-aminolevulinic acid].

Topical application of 5-aminolevulinic acid (5-ALA) is a useful instrument for photodynamic diagnosis and therapy of skin tumours. Diagnostic fluorescence imaging after laser light irradiation (410 nm) revealed a high, tumour-specific fluorescence even in tumour areas not apparent prior to this examination technique. This demonstrates the possibility of photodynamic diagnosis to detect skin tumours. In the therapeutic group 8 patients with 6 solar keratoses and 12 basal cell carcinomas underwent laser light irradiation using a wavelength of 635 nm (dosage 100 J/cm2) 6 hours after topical application of 5-ALA in W/O emulsion. 2-12 hours after laser application we observed reddened tumour tissue with mild oedema, subsequently followed by a crust and epithelised within 4-6 weeks. 2 months after PDT a complete response was observed for all solar keratoses and for 10 of 12 basal cell carcinomas. Photodynamic therapy following topical application of 5-ALA may be an alternative treatment modality for skin tumours.

Multimodal therapy for neoplasms arising from a vesical diverticulum.

Both radical cystectomy and diverticulectomy for the treatment of neoplasms arising in a vesical diverticulum have resulted in poor survival, mainly secondary to early metastatic spread. In this study, nine patients were treated with a multispecialty approach in hopes of eradicating both local and distant disease. All patients underwent a combination of therapeutic modalities with three patients undergoing pre-operative radiotherapy (RT) and diverticulectomy; diverticulectomy and chemotherapy (three patients); diverticulectomy with chemotherapy and pre-operative RT (two patients); and definitive RT with cisplatin (one patient). With a mean follow-up of 4.0 years (median 3.2), four patients are free of disease, two are dead of other causes, one patient has developed an invasive recurrence, one patient is alive with metastatic disease, and one patient is dead of disease. Five patients (55%) developed local recurrences. Disease-specific survival for the group was 89%. Surgical monotherapy has been ineffective in controlling both local and metastatic disease in patients with diverticular tumors. This study suggests a significant benefit from systemic chemotherapy and RT when combined with surgery for these neoplasms.
