[Lesions of the distal segment of the large intestine in Reiter's disease].

Altogether 23 male patients with Reiter's disease (RD) were followed-up in an arid zone. An interstitial syndrome was diagnosed in addition to the urethro-oculo-synovial syndrome. In one group (14 patients) RD followed acute enterocolitis (dysentery in 3 cases and 9 cases of unknown etiology). Halprovias were found in 5 patients, colon lesion in 12 patients. In the other group (9 patients) halprovias were found in 4 patients. The signs of enterocolitis, pathological microbial flora in the stool and changes in the colon mucosa were absent. An assumption was made of pathogenetic relationship of the urethro-oculo-synovial and intestinal syndromes in RD. A more serious RD course was noted in this combination of syndromes.

Reiter's Disease in Children. Characteristic of Cellular Compartment of Immunity.

Reiter's disease in children was shown to be not such a rare disorder and to occur as the polysystemic process. The inflammation is induced by trigger agent (mostly by Chlamydia trachomatis), but then afterwards the key role has inadequate activation of cell-mediated and humoral immunity that may be manifested by substantial enhancement of leuko- and lymphopoiesis. There is an elevation of total number of leukocytes, T cells and lymphocytes, expressing CD4, CD8 and CD25 receptors with sharp increase of CD4/CD8 ratio. The numbers of natural killer cells (CD16) and cells, expressing activation markers, are 2 times higher than in healthy children. Besides, levels of IL-4 and IFN-gamma were increased in great numbers. Severity and duration of the disease course are genetically predetermined. Children - carriers of HLA B27 antigen - had 10-fold elevation of CD71 cells and IFN-gamma in comparison with control group and patients with Reiter's disease, but without this antigen.

Bilateral retinal detachment in a case of Reiter's disease.

A patient is described who developed co-existent Reiter's disease and bilateral non-traumatic retinal detachment. Although numerous ophthalmological disorders are associated with Reiter's disease and HLA-B27, to our knowledge this is the first report of co-existent retinal detachment. Although this relationship may be fortuitous, there are a number of immunological mechanisms common to both entities. Retinal detachment, unlike bilateral conjunctivitis and acute anterior uveitis, may be clinically asymptomatic and its sequelae prevented by appropriate ophthalmological assessment.

Joint fluid cytology in Reiter's disease.

The diagnostic value of the finding of cytophagocytic macrophages (CPM) in the joint fluid of patients with Reiter's disease has been re-examined. CPM were found in 46% of Reiter's disease fluids and in 45% of other inflammatory knee joint fluids. Higher CPM scores, on a 4-point grading, were commoner in Reiter's disease but the difference was not statistically significant. Further, although the graded polymorphonuclear leucocyte phagocytosis shown by CPM was greater in Reiter's disease this also was not significant. It is concluded that the presence of CPM in joint fluid is of little discriminating value.

[The clinical characteristics of patients with Reiter's disease].

The authors analyze 46 case histories of patients with Reiter's disease. They describe the clinical, bacteriological, serological, and x-ray examinations and the features of the disease course. The majority (97.7%) of patients have developed the disease after casual sexual intercourse. Fifteen patients suffered from an acute disease, in the rest it coursed without manifest general symptoms. The principal and permanent symptom of Reiter's disease is urethritis complicated by prostatitis and vasculitis. The drugs for this disease are listed. The authors necessitate examinations and sanitation of the patients' sexual partners.

Reiter's disease after Salmonella typhimurium enteritis.

We describe a case of Reiter's disease in an HLA-B27 positive black woman after infection with Salmonella typhimurium. Although reactive arthropathy following Salmonella infections is not unusual, Reiter's disease is rare. This may be the second such case in the English literature, and the first reported in North America.

[Possible relations between Reiter's disease and psoriasis (author's transl)].

Description of a characteristic venereal type of Reiter's disease with a pustular exanthem and a balanitis circinata. In the course of the disease the pustular exanthem probably transformed into a typical Psoriasis vulgaris. In search of genetical markers it was not possible in this case to show the HLA-antigens, which are characteristic of the Reiter's disease and the Psoriasis vulgaris; the HLA-antigens A 10, B7 and B12 however could be proved. The question is up for discussion, wether a HLA-B 27-negative Reiter's disease might include a disposition to the appearance of a phenotypical Psoriasis vulgaris, the genotype of which is represented by rather "unusual" histocompatibility-gens.

[Characteristics of the clinical course of Reiter's disease in relation to the causative agent].

Reiter's disease patients (RD, 83 per cent) were divided into five groups according to the causative agent. The clinical variants were compared. For the clinical picture of Chlamydia-associated RD follicular lesions of the cervical, rectal, and conjunctival mucosa are characteristic; latent forms prevail with initial joint syndrome and enteropathy. RD of Chlamydia-ureaplasma aetiology is characterised by extensive clinical manifestation with obligatory involvement of the sacro-iliac joint. Enterocolitis RD is epidemic, highly active, with pronounced fever syndrome and a more favourable prognosis.

A renal lesion in a woman with Reiter's disease.

A 17 year old girl with Reiter's disease had extensive involvement of the entire urinary tract in addition to arthritis, vaginitis, and ocular involvement. In addition to a severe urethritis, cystitis and ureteritis there was evidence of glomerular and tubular dysfunction and abnormalities were seen on renal biopsy. The patient was treated with a five week course of doxycycline and made a slow but complete recovery.

Evaluation of arthritis in Reiter's disease by bone scintigraphy and radiography.

Tc-99m MDP bone scans were used to evaluate the articular inflammation in 38 patients with Reiter's disease and compared with clinical examination and radiologic findings. Our data showed that Reiter's disease predominantly involves the lower limbs, especially the heels, which may be a characteristic feature of Reiter's disease. Bone scans revealed a high diagnostic sensitivity in the detection of clinical arthritis in all peripheral joints, especially in the small joints of the four limbs. The diagnostic sensitivity of radiography was generally lower than bone scintigraphy. In the presence of positive radionuclide findings, clinical arthritis was found in most joints. The scintigram, however, detects a greater number of abnormalities than does clinical assessment in the sternoclavicular joints, shoulders, metacarpophalangeal joints, and tarsals. Because of its high sensitivity, bone scintigraphy is capable of detecting subclinical arthritis, and might provide more objective evidence of early inflammatory joint disease and additional information regarding the pattern of joint involvement. In view of the advantages of low patient radiation exposure, high sensitivity, and the ability to survey the whole body, we consider bone scintigraphy as useful and better than radiography in the detection of early articular inflammation and in establishing the extent and pattern of arthritis in Reiter's disease.

[Ophthalmological aspects of Reiter's disease].

Clinical picture and course of eye involvement were studied in 254 patients with Reiter's disease. Altogether 130 patients suffered from conjunctivitis of varying severity, 23 from keratitis, 21 from episcleritis, 20 from uveitis, 2 from detachment of the retina, and 5 from secondary glaucoma. Chlamydia were detected in one third of conjunctivitis patients in scrapings off the conjunctiva. The eyes may be accidentally infected by agents from the urogenital organs if the patient does not observe the hygienic rules. Uveitis developed as a rule after a prolonged course of Reiter's disease. They were resistant to antichlamydial therapy because of autoimmune factors that underlay the condition pathogenesis. Deposits of immune complexes on antigens of ocular vascular coating were detected with the use of monospecific antiglobulin sera in 7 of 10 patients with active uveitis symptoms. The risk of Reiter's disease development in HLA B27 carriers is 27.17 times higher than the mean incidence of this disease in the population. HLA A9 and B40 antigen combinations occurred 7 times more often in uveitis patients than in reference subjects, A1 and B27 combinations were 4-5 times more incident.

[Reiter's disease--a review of an interdisciplinary disease].

Over the past 20 years the term 'Reiter's disease' has been expanded from the classical triad of sterile urethritis, conjunctivitis and arthritis to include a broader range of reactive arthritides. Indeed, the classical syndrome as described by Reiter is rarely seen in its complete form, and the presence or absence of active infection in the joint is being redefined in the light of recent techniques of molecular biology. The terminology of Reiter's syndrome, reactive arthritis and seronegative spondylarthropathy are defined here using case examples.

[Reasons for the late diagnosis of Reiter's disease].

The main causes of the delay in reaching the appropriate diagnosis of Reiter's disease were analysed. It was concluded that even if the only sign from the disease symptomatic triad (mainly slowly progressing nonspecific urethritis or prostatitis) was observed, Reiter's disease should be suspected and its verification include immunological techniques. Tetracycline antibiotics were found to be mandatory indicated as an etiotropic treatment.

[Late cardiac manifestations of Reiter's disease (author's transl)].

Serious cardiac late manifestations were observed in two patients with Reiter's disease 15 and 26 years after the onset of the disease. There is a striking similarity with cardiac involvement in Bechterew's disease. Including cases reported in the literature there is an interval of 4-21 years, average 15 years, between the first conclusive symptoms of Reiter's disease and cardiac late manifestations. These present as isolated aortic valve insufficiency. Valves are not affected due to inflammatory changes but due to dilatation of the valve ring and of the left ventricular outflow channel. In addition, severe disturbances of the atrioventricular conduction are seen localised both in the AV node area and in the distal His-Purkinje fibres. In both observed cases surgical aortic valve replacement became necessary 20-31 years after disease onset. One patient had to have a permanent cardiac pacemaker.

[Cardiac involvement in Reiter's disease].

The present article discusses modern concepts on possible instances of cardiovascular involvement in Reiter's disease during the early and late stages of this medical condition. A case is reported in a clinical setting of left ventricular dilatation development and complicated life-threatening arrhythmia in a patient presenting with Reiter's disease.

[The urogenital inflammatory focus and its treatment in Reiter's disease].

The complement fixation test, microprecipitation, leukocyte migration inhibition, leukocyte agglomeration phenomenon, direct and indirect immunofluorescence were used to demonstrate that chronic prostatovesiculitis associated with Reiter's disease may be of immune-induced nature. This requires a differentiated approach to the choice of the treatment policy. The methods of the treatment for the urogenital inflammatory focus in men suffering from Reiter's disease are provided. It is indicated that complete cure of Reiter's disease can be attained only after elimination of inflammation in the urogenital organs.

A case of Reiter's disease complicated by fulminating colitis.

We describe a patient who, 6 months after the onset of Reiter's disease associated with a destructive peripheral arthritis and keratodermia blenorrhagica, developed fulminating colitis. The possible relationship between Reiter's disease and ulcerative colitis is discussed, and the need for further family studies to assess its validity is stressed.

Paravertebral ossification in psoriasis and Reiter's disease.

In a retrospective study of psoriatic arthropathy and Reiter's disease, it is shown that paravertebral ossification (PVO), of the type described by Bywaters and Dixon (1965) and other types of non-marginal syndesmophytes (McEwan et al., 1971) may be the sole or major radiological abnormality. This sign is of particular value where the appearances of the sacro-iliac or other joints are normal or equivocal and is at least as significant as the peripheral I.P. joint involvement in the hands, where these conditions are suspected. This radiological sign may precede the typical psoriatic rash.
