[Combined surgical operations in the abdominal cavity and retroperitoneal space].

The authors discuss their experience in combined operations on the organs of the abdominal cavity and retroperitoneal space in 366 patients whose ages ranged from 16 to 81 years. In 96 patients (26.2%) the operations were carried out for emergency indications and in 270 (73.8%) in a planned order. The authors substantiate the expediency of performing combined operations by a low mortality rate (1.9%) (in carrying out the operations separately the mortality rate comes up to 15%) and by an economical effect.

[A comparison of computer tomography and ultrasound for the demonstration of vessels in the abdomen and retroperitoneal space (author's transl)].

The value of computer tomography and ultrasound for demonstrating vessels in the abdomen and retroperitoneal space was compared. The methods were equal in their ability to demonstrate the major vessels situated centrally, but computer tomography proved better at differentiating arteries and veins situated peripherally. On the other hand, sonography proved a simple means of differentiating intrahepatic vascular structures.

[Ultrasonographic diagnosis of the retroperitoneal space. A case history series and some selected clinical cases].

A personal series and some clinical cases of pathology of the retroperitoneal space observed by ultrasonography during routine and emergency investigations are reported. Although the limitations of this diagnosis compared to the more recent image techniques (CT and NMR) are recognised, it is still considered an indispensable first level diagnosis in this extensive pathology (in terms of site, type and extent) of retroperitoneally located organs, tissues and systems.

[Aspiration biopsy of pathological formations of the abdominal cavity and retroperitoneal space].

Aspiration diagnostic puncture of a three-dimensional structure in the retroperitoneal space and abdominal cavity palpated through the anterior abdominal wall was performed in 58 patients. The puncture was carried out in cases of pathological processes arising from the stomach, large intestine, pancreas, kidneys, adrenal gland, ovaries, retroperitoneal lymph nodes, mesenchyma, and smooth-muscle tissue, and in appendicular and paravesical abscesses. The findings of cytological examination correctly pointed out the nature of the pathological process in 78% of cases. There were no false-positive results, false-negative results were encountered in 9 out 46 patients. The cytological data allowed the clinical diagnosis to be made precisely in cases in which the organic nature of the three-dimensional structure was doubtful. The expected complications due to possible damage of hollow organs and blood vessels by the needle were not encountered. Metastatic spreading of the tumor which could have been caused by the diagnostic manipulations also did not occur.

Lymphatic cysts of the retroperitoneal space. Could epiploplasty be the method of choice?

Lymphatic cysts of the retroperitoneal space among the rarest abdominal tumors. A very small number, usually single case reports, has been reported in the international literature. Symptoms vary according to the tumor size but usually are minimal so that they are discovered by chance, either during an ordinary abdominal examination or because the patient complains of discomfort due to the pressure of the cyst on adjacent organs and tissues. Surgical intervention is the only treatment. Total cystectomy with the treatment of choice is seldom feasible, because of technical difficulty. A case of a huge retroperitoneal lymphatic cyst is reported. Epiploplasty with drainage is proposed as a new method of surgical treatment.

[Diagnosis and treatment of pseudotumors of the abdominal cavity and retroperitoneal space].

The diagnosis and treatment of pseudotumors of the abdominal cavity and retroperitoneal space are discussed on the basis of 23 cases. It is pointed out that intraoperative morphological study is necessary for making a precise diagnosis. The use of the MK-14 antimicrobial glue improves the results of management of pseudotumors due to the creation of a depo of the antiseptic chlorhexidine in the abdominal cavity with simultaneous air-tightness of the wounds and hemostasis.

[Liposarcoma of the retroperitoneal space].

Date of an examination of 60 patients with liposarcomas of the retroperitoneal space aged from 40 to 60 are presented, women comprised 62%, men 38%. Operative treatment was undertaken in 48 patients (96%), radical operation--in 82%, palliative operations--in 14%. Recurrent tumors were revealed in 33 patients (66%), metastases--in 8%. Most frequent recidivations were noted in myxoid liposarcomas. A complete removal of the recurrent tumor was made in 72% of the cases. Out of 50 patients with liposarcomas 54.2% of the patients survived 5 or more years after the operation, in the myxoid variant--72.3%, in polymorphocellular form--23.1%.

[Ultrasound guided fine- and coarse-needle puncture of the abdominal and retroperitoneal space].

Within a period of 21 months, 167 fine-needle and 86 cannula (1.67 mm external diameter) punctures of the abdominal or retroperitoneal space were performed under sonographic control. Sufficient material was obtained with 93.4% of fine-needle punctures. With regard to differentiation between malignant and benign lesions, fine-needle punctures had a sensitivity of 86% and specificity of 100%, if the material was examined by an experienced cytologist. Sensitivity dropped to 55,1% if the same material was interpreted by a less experienced cytologist, but specificity was still 98.1%. Sufficient material for histological examination was always obtained with the cannula, already at the first puncture. With cannula puncture there were two complications: bleeding requiring transfusion and peritonitis.

Discharge properties of mechanosensitive afferents supplying the retroperitoneal space.

Functional properties of lumbar mechanosensitive afferent fibres in the white rami L3 to L4 and the lumbar splanchnic nerves, which supply the retroperitoneal space were investigated. The receptive fields of these afferents, consisting of 1 to 3 mechanosensitive sites, were found on large vessels (aorta, inferior mesenteric artery, segmental arteries), on nerves, on the peritoneum, in fat lobules, on lymph nodes and on the vertebral column. Most afferent units were associated with small vessels. About 50% of the afferents had ongoing activity (about 0.1 to 2 imp/s). Two thirds of the afferents conducted at less than 2 m/s, the rest at 2 to 16 m/s. About 60% of the units were excited by local application of bradykinin and 75 to 90% by local application of hypertonic NaCl solution (4.5 and 9%) or KCl solution (60 and 155 mmol/l). About 50% of the afferent units with receptive fields on large arteries were activated by an increase in the arterial blood pressure. The function of these afferent fibres is unclear. However, it is conceivable that they are involved in nociception.

[Endoscopy of the retroperitoneal space. Technics, results and current indications].

Retroperitoneoscopy (RPS), a form of direct vision endoscopy derived from mediastinoscopy, allows exploration of the majority of the lumbar and pelvic retroperitoneal space. Over a period of three years since May 1985, we have performed 135 RPS for 107 indications in 106 patients. Twenty-two examinations were performed for the diagnosis of retroperitoneal masses or adenopathy and a precise histological result was obtained in 20 cases. In 2 cases, the hypothesis of malignant lymphadenopathy was formally excluded. Lymph node extension from 78 pelvic cancers (bladder : 52, prostate : 22, cervix : 3, endometrium : 1) were studied by unilateral or bilateral pelviscopy; RPS corrected the imaging in 24 cases and there were 4 false negatives. Seven patients with suspected lymph node recurrence from malignant diseases (3 lymphomas, 4 miscellaneous tumours) were examined : 3 recurrences were confirmed and 2 were excluded. There were 2 failures responsible for 2 false negatives. The diagnostic accuracy of RPS (94%) is superior to that of other diagnostic investigations for retroperitoneal masses and evaluation of retroperitoneal lymph nodes. In many cases, it eliminates the need for exploratory laparotomy.

[Cysts and cyst-like formations of the abdominal cavity and retroperitoneal space in children].

The results of surgical treatment of 29 children with cysts, and 10--with cyst-like formations of the abdominal cavity and retroperitoneal space are described. In majority of the patients, a cyst at the terminal stage was revealed. In presence of complications, they were operated on with tentative diagnosis of acute appendicitis, or ileus. The operation consisted of cyst enucleation, its elimination with resection, or removal of the organ, creation of cystodigestive anastomosis. Postoperative lethality was 12.8%.

[Ultrasonically guided fine-needle puncture. The limits of the method in the abdomen and retroperitoneal space].

The sensitivity and specificity of ultrasound-directed fine-needle biopsy in the differentiation of malignancy or otherwise of structures in the abdomen or retroperitoneal space which look suspicious on ultrasound was evaluated from results in 558 patients (306 men, 252 women). Data from two different centres were used to characterize further the influence of different techniques of puncture, cytopathologists and groups of patients on the validity of the examination. Sensitivity (in the two centres) was 88 and 80%, respectively, specificity 100 and 99%. The validity of the examination was the same at both centres. In both centres the sensitivity was worst for pancreatic tissue, at only 72 and 61% respectively, compared with other organs (e.g. liver, lymph nodes, kidney) (P less than 0.05). Fine-needle biopsy of abdominal and retroperitoneal organs will thus be subject to a not insignificant number of false-negative results of cytological examination.

[Metastatic tumors of the retroperitoneal space].

In 22 patients retroperitoneal nonorganic tumors, considered to be primary, proved to be metastatic. Prior to surgery the character of neoplasms was recognized only in 4 patients. Due to unwarranted efforts of "radical" removal of tumors 2 of 4 patients died. In 3 cases neoplasms were urgently removed due to internal hemorrhage and obstruction. In 7 cases a palliative effect of radiotherapy. 5-fluoruracil thiophosphamide employed prior to detection of the primary focus, was obtained. The latter was found following 3-12 months in the stomach (1), small intestine (1) and rectum (4), kidneys (4) and genitalia (9) in further follow-up of patients.
