Cytodiagnostic problems in uterine sarcoma. Analysis according to a novel classification of tumor growth types.
OBJECTIVE: To clarify the cytologic diagnostic problems of uterine sarcomas and the differential properties between pure sarcomas and carcinosarcomas. STUDY DESIGN: Four leiomyosarcomas and 21 carcinosarcomas (homologous and heterologous) treated at the Saitama Cancer Center from 1991 to 2000 were analyzed macroscopically and microscopically. RESULTS: Of 4 leiomyosarcomas, 3 were intramuscular, localized type, with a negative diagnosis for sarcoma. Of 21 carcinosarcomas, 7 were exophytic type with little necrosis (B-1), 5 were exophytic type with marked necrosis (B-2), 6 were exophytic type with a small sarcomatous component (B-3), and 3 were endophytic type (B-4). All endometrial smears were positive for sarcoma in B-1, whereas 5 of 14 (36%) were positive in the latter 3 types (B-2, 3 and 4). CONCLUSION: In pure leiomyosarcomas, the sarcomatous portions are usually covered with normal endometrium. In carcinosarcomas, sarcomatous component is relatively limited in some cases and frequently covered with marked necrosis or carcinomatous tissue. These pathologically specific findings should make cytologic diagnosis difficult in uterine sarcomas.
[Cysts and tumors of the jaw bone].
The new entities of cysts and odontogenic tumors of the second edition of WHO "Histological Typing of Odontogenic Tumors" (Kramer et al., 1992) are presented. The classification is based on tumor behavior. The benign epithelial odontogenic tumors are classified in relation to tooth development. The malignant odontogenic tumors are subdivided in carcinomas, sarcomas and carcinosarcomas, which are very rare. This international classification has the purpose to make rare cases comparable, so that more detailed knowledge of their characteristics can be achieved. Types of bone tumor that also occur in other parts of the skeleton are not discussed here.

Carcinosarcoma (pure endocrine cell carcinoma with sarcoma components) of the stomach.
A case of gastric carcinosarcoma with a rare histology is reported. Grossly, a huge ulcerative tumor (Borrmann type 3 tumor, measuring 10 x 6 cm) was observed on the lesser curvature side of the stomach. Microscopically, the tumor consisted of carcinoma and sarcoma components. The main tumor component consisted of an endocrine cell carcinoma exhibiting positive reactions for cytokeratins, chromogranin A, synaptophysin, and CD56. The sarcoma component occupied less than 10% of the entire tumor area and consisted of spindle cells, some of which showed a rhabdomyosarcoma differentiation. Some of the spindle cells also showed an atypical cartilage or osseous differentiation. This sarcoma component exhibited positive staining not only for vimentin and desmin, but also for cytokeratins. Transitions between these two components were occasionally observed. These findings suggest that the sarcoma component in our case most likely originates from the endocrine cell carcinoma component.

In vitro growth characteristics and chemosensitivities of endometrial cancer using a soft agar clonogenic assay.
A soft agar clonogenic assay was used to analyze 91 fresh human endometrial tumor samples for in vitro growth and chemosensitivities. Overall cloning success was 81%. Of the 75 samples adequate for drug testing (30 or more colonies per plate), histologic analysis demonstrated 36 adenocarcinomas, seven adenoacanthomas, ten adenosquamous carcinomas, nine mixed mesodermal sarcomas, seven carcinosarcomas, three papillary adenocarcinomas, and three clear-cell carcinomas. Mixed mesodermal sarcoma specimens demonstrated the most efficient growth, followed by the adenosquamous adenocarcinoma and adenoacanthoma samples. In vitro chemosensitivities were determined by colony inhibition resulting from continuous exposure of tumor cells to known achievable peak plasma and one-tenth peak plasma concentrations of a particular agent. The plating efficiency reflected the clinical virulence by cell type. Thus, we found that malignant endometrial tumors can be cloned in vitro, and the observed chemosensitivities reflect the relative clinical resistance of the tumors to these agents. Based on the high cloning success, this tumor type may be well suited as a model using in vitro soft agar cloning for new drug screening, evaluation of novel drug combinations, and studies of human tumor biology.
A case of pulmonary carcinosarcoma with persistent mild fever.
Carcinosarcoma is defined as a malignant tumor with an admixture of carcinoma and sarcoma. Pulmonary carcinosarcoma accounts for about 0.27 percent of all lung neoplasms. It occurs frequently in males, particularly in smokers between 50 and 80 years of age. Preoperative diagnostic tests, such as sputum cytology, percutaneous fine needle biopsy and bronchoscopy, have a low yield in detection of pulmonary carcinosarcoma. The diagnosis is verified by postoperative pathologic findings and by immunohistochemical investigations in many cases. Surgical resection is the treatment of choice. As the metastasis to regional lymph nodes and distant organ is common at diagnosed time, the prognosis is quite poor. We report a case of pulmonary carcinosarcoma presented with persistent mild fever and blood-tinged sputum in a 66-year-old male.
Rare uterine cancers.
The most common malignant tumour of the uterus is endometrioid endometrial cancer. However, many less common malignant diseases also develop in the uterus, including both carcinomas and sarcomas. Most notable of these tumours are papillary serous carcinomas, clear-cell carcinomas, carcinosarcomas, stromal sarcomas, and leiomyosarcomas. These less common cancers can be aggressive, and account for a greatly disproportionate amount of deaths from uterine cancers. Because they are uncommon, physicians will usually have seen only a few cases, and randomised data to guide treatment often do not exist. This review summarises the epidemiology, clinical characteristics, and prognoses of the less common malignant diseases of the uterus, and presents the information available to guide the clinician about treatment options.
Carcinosarcoma of the bladder diverticulum and a review of the literature.
We report a carcinosarcoma within a bladder diverticulum together with a review of 18 previously reported cases of intradiverticular sarcomas and carcinosarcomas with respect to their clinical features, treatments and outcomes. Frequent deaths with intra-abdominal recurrences suggest the need for total cystectomy, rather than diverticulectomy.
