Effect of various antibacterial preparations on the pathogenic oral flora in elderly patients fed via nasogastric tube.

Oropharyngeal colonization by pathogenic gram-negative bacilli (GNB) and Staphylococcus aureus is associated with aspiration pneumonia. Decolonization in high-risk populations may be important. We prospectively evaluated six antiseptic compounds in nasogastric tube-fed frail elderly patients; only polymixine reduced oropharyngeal colonization with GNB. None had an effect on S. aureus colonization.

Periorbital abscess due to ethmoiditis in a neonate.

Orbital complications of ethmoiditis are not uncommon in children. They are, however, much rarer in the very young. A case is presented in which a neonate of gestational age 39 weeks developed a periorbital abscess. The relevant literature is reviewed with respect to embryology, aetiology and treatment.

[Influence of a diet enriched with polyunsaturated fatty acids on some indices of the metabolism and course of acute gastro-intestinal diseases in children in the 1st year of life].

The influence of new products "Vitalakt" and "Malysh", enriched with polyunsaturated fatty acids, on the evolution of acute gastro-intestinal diseases and the lipids metabolism characteristics (total lipids, cholesterol and its ethers, phospholipids, nonetherified fatty acids in the blood serum), as well as on blood serum, protein fractions was studied. A total of 137 children were examined and the diet of 87 of them included the new dairy products "Vitalakt" and Malysh". These new dairy mixtures have been found to exert a beneficial effect on the course of the disease, tended to normalize the blood serum lipids characteristics and to reduce dysproteinemia. All this warrants recommending the mixtures "Vitalakt" and "Malysh" to be included in the diet of infants in the first year of life, suffering from acute gastro-intestinal diseases.

Mixed bacterial infection of the pericardium.

Polymicrobic bacterial infection of the pericardium was detected in a 59-year-old patient ultimately proven to have esophageal carcinoma. Such infections are becoming more frequent, often combine gram-negative bacilli and anerobes, and may be deceptively mild clinically. Origin from an occult head and neck or gastrointestinal focus rather than from pulmonary (pneumococcal) or distant (staphylococcal) sources distinguishes these infections. Newer diagnostic techniques (computerized tomography and echocardiography) coupled with surgical drainage and appropriate antibiotic therapy may improve the current 67% mortality.

Unveiling novel antimicrobial peptides from the ruminant gastrointestinal microbiomes: A deep learning-driven approach yields an anti-MRSA candidate.

INTRODUCTION: Antimicrobial peptides (AMPs) present a promising avenue to combat the growing threat of antibiotic resistance. The ruminant gastrointestinal microbiome serves as a unique ecosystem that offers untapped potential for AMP discovery. OBJECTIVES: The aims of this study are to develop an effective methodology for the identification of novel AMPs from ruminant gastrointestinal microbiomes, followed by evaluating their antimicrobial efficacy and elucidating the mechanisms underlying their activity. METHODS: We developed a deep learning-based model to identify AMP candidates from a dataset comprising 120 metagenomes and 10,373 metagenome-assembled genomes derived from the ruminant gastrointestinal tract. Both in vivo and in vitro experiments were performed to examine and validate the antimicrobial activities of the AMP candidates that were selected through bioinformatic analysis and subsequently synthesized chemically. Additionally, molecular dynamics simulations were conducted to explore the action mechanism of the most potent AMP candidate. RESULTS: The deep learning model identified 27,192 potential secretory AMP candidates. Following bioinformatic analysis, 39 candidates were synthesized and tested. Remarkably, all synthesized peptides demonstrated antimicrobial activity against Staphylococcus aureus, with 79.5% showing effectiveness against multiple pathogens. Notably, Peptide 4, which exhibited the highest antimicrobial activity against methicillin-resistant Staphylococcus aureus (MRSA), confirmed this effect in a mouse model with wound infection, exhibiting a low propensity for resistance development and minimal cytotoxicity and hemolysis towards mammalian cells. Molecular dynamics simulations provided insights into the mechanism of Peptide 4, primarily its ability to disrupt bacterial cell membranes, leading to cell death. CONCLUSION: This study highlights the power of combining deep learning with microbiome research to uncover novel therapeutic candidates, paving the way for the development of next-generation antimicrobials like Peptide 4 to combat the growing threat of MRSA would infections. It also underscores the value of utilizing ruminant microbial resources.

Possible activation of an intramyocardial inflammatory process ( Staphylococcus aureus)after treatment with infliximab in a boy with Crohn disease.

UNLABELLED: In an 11-year old boy with Crohn disease, long-term therapy with prednisolone was decided to be augmented by infliximab, a monoclonal antibody to tumour necrosis factor alpha. The patient, who was in a stable clinical condition with low signs of inflammation at that time, deteriorated 3 days after the first dose of 200 mg infliximab with fever, nausea, myalgia and signs of cardiac failure. Echocardiography demonstrated destruction of the aortic valve with third grade insufficiency. In the blood culture, a susceptible strain of Staphylococcus aureus was isolated. Antibiotic therapy resulted in rapid improvement, but the aortic valve had to be replaced by a homograft due to massive insufficiency. At surgery, there were no signs of endocarditic vegetations, the aortic ring and the right coronary aortic cusp had been destroyed by a massive intramyocardial abscess, leading from close beneath the aortic ring in the direction of the right atrium. The size of the process suggested chronic infection or reinfection after several weeks. As clinical and laboratory signs as well as echocardiographic changes pointed to a dramatic flare-up shortly after the administration of infliximab, we suggest that this chronic inflammatory process was activated by infliximab. CONCLUSION: when using infliximab, special awareness of potential risks is necessary in patients with chronic infections or in a state of immunodeficiency.
