More than 90% of the 2 million teeth avulsed each year in the United States can be saved if the proper treatment is instituted. Storage in a tooth-preserving medium can sustain the life of the tooth for 12 hours. An avulsed tooth that has dried out should be soaked in Hank's solution before replantation to reconstitute degenerated cells.
It has been suggested that removal of the damaged nonvital periodontal ligament (PDL) may result in slower destruction of tooth substance following replantation of teeth. It has also been suggested that citric acid conditioning of surgically exposed dentin may enhance connective tissue attachment on root surfaces. The aim of this study was to examine the interrelationship between the acid etched root surface of the replanted tooth and the alveolar bone. Forty-five available anterior teeth from six monkeys were divided into 3 groups in this experiment. All teeth were extracted and dried for 2 hrs. In the first group the teeth were replanted immediately after endodontic therapy. The teeth in the second group were replanted after immersion in 10% sodium hypochlorite and subsequently irrigation with normal saline. In the third group, the cementum and necrotic PDL were removed by grinding the root surfaces which were then treated with citric acid (PH = 1) for 3 minutes followed by normal saline irrigation. All teeth were replanted without splinting and were subjected to histological examination at 12 week post-replantation. Close apposition of collagen fibrils of the new connective tissue to the exposed root surface was observed in most cases in the second and third group, although bony ankylosis and inflammatory root resorptions were also found in some areas. Teeth in the first group, the control experiment, had much more replacement and inflammatory resorptions. Then the result suggested that demineralization with citric acid on the denuded avulsed tooth prior to replantation may decrease the chance of replacement resorption and result in fibrous adhesion to the alveolar bone.

Long-term loss of avulsed and replanted teeth is a frequent clinical problem. Bone morphogenetic protein-7 (BMP 7) induces cementogenesis in periodontitis-associated periodontal ligament (PDL) defects. This study's aim was to assess the utility of rhBMP 7 in a tooth avulsion trauma model in growing individuals. Immature primary incisors of 12 minipigs were extracted. PDL and cementum were removed either partially (group 1: 4 mm2 [n=28 teeth]; group 2: 16 mm2 [n=26 teeth]) or totally (group 3 [n=26 teeth]). 500 microg rhBMP 7/g collagen matrix was applied to the teeth from one side while the corresponding teeth on the contralateral side served as controls (split mouth model). After an experimental period of 4 months, microradiography, fluorescence and light microscopy of nondecalcified sections were performed. All teeth of group 1 survived and all teeth of group 3 were lost, whether rhBMP-7 was applied or not. In group 2, nine out of ten teeth survived when rhBMP-7 was applied and four out of ten teeth were lost when rhBMP-7 was not applied. In the presence of rhBMP-7, eruption of teeth in group 2 was significantly improved (difference [median]: 5 mm, P<0.05, n=6). Even though there was a tendency towards increased deposition rates of cementum under rhBMP-7, this difference was not significant (Wilcoxon: P>0.05, ANOVA: P=0.002; n=6/group). In conclusion, rhBMP-7 improved survival rates and eruption of replanted teeth in growing individuals. No adverse effects were seen. Based on the present results, future clinical trials appear to be warranted.

Teeth that have been accidentally avulsed can be preserved, reconstituted, and replanted with an excellent long-term retention prognosis. The mechanisms for this process are now well understood. The most biologic methods for preserving and storing avulsed teeth are presented so that the emergency physician can choose appropriate modalities for treating patients who present with this problem.
A 42-year-old man had been previously hit in the mouth when he was 12 years old, and the upper left central incisor was completely avulsed. The patient immediately replanted the tooth and did not seek any dental treatment. After 28 years he noticed discoloration and abscess formation. Root canal treatment was performed, and the tooth has remained in a stable, functional position.
