Estimation of the antral and duodenal gastrin cell population removed by gastrectomy from patients with peptic ulcer.

The total number of gastric and duodenal gastrin cells was determined in the gastrectomy specimens from eight patients with peptic ulcer. Planimetry was used to determine the antral and duodenal surface. The immunoperoxydase method with specific antigastrin antibodies was used for staining gastrin cells, and the mean concentration of nucleated gastrin cells per square millimeter of antral and duodenal surface was determined by light microscopy. The mean number of duodenal gastrin cells in the resected duodenum was 4.8 per cent of the total gastric gastrin cell mass. The concentration of gastrin cells in the antrum was quite variable from one mucosal site to another. The degree of extension of the antral gastritic areas was a major factor influencing the mean concentration value. The total number of gastrin cells in the stomach varied from approximately 7 to 74 million cells. Our data indicate that hyperplasia of gastrin cells cannot be demonstrated by studies performed in small specimens taken for biopsy.

[Surgery of aneurysm of the thoracic aorta. Operative results in 47 cases (author's transl)].

The authors report personal experience of surgical treatment of 47 aneurysms of the thoracic aorta, showing that the symptoms, the etiology, the treatment and the prognosis are variable depending on the site of the aneurysm. One may note regression of syphilis as a cause and an increase in the number of degenerative and traumatic aneurysms. In aneurysms of the ascending aorta, total replacement of the ascending aorta with reimplantation of the coronary arteries, according to a simplified technic, seems to give the best immediate and long term results.

Restriction endonuclease fingerprinting of herpes simplex virus DNA: a novel epidemiological tool applied to a nosocomial outbreak.

In a blind study, 14 isolates of herpes simplex virus type 1 (HSV-1) that included nine isolates from a temporal cluster of HSV infections in a hospital Pediatric Intensive Care Unit and five unrelated isolates were analyzed by digestion of their DNA with four restriction endonucleases. These enzymes (HsuI, BglII, EcoRI, and HpaI) cleave the DNA in about 52 sites. To date, at least 16 sites have been found to be variable in the sense that they may be present or absent independently of any other cleavage site. This characteristic is stable, and no change was observed on serial propagation of the strains in culture or following repeated isolation, as long as 12 years apart, from humans. Analyses of the isolates readily discriminated between those belonging to the temporal cluster of hospital infections and the unrelated strains. They also showed that there were two independent introductions of HSV-1 into the Pediatric Intensive Care Unit resulting in two clusters of epidemiologically related infections. This type of analysis has the potential of becoming a powerful tool for tracing the spread of HSV-1 and very likely of other herpesviruses in the human population.

Immunological heterogeneity of haemophilia B: a multicentre study of 98 kindreds.

An electroimmunoassay with a precipitating rabbit anti-human factor IX antiserum and an inhibitor neutralization assay with a non-precipitating homologous antibody were used to measure factor IX antigen (IX:Ag) in 117 patients from 98 kindreds with haemophilia B; and to investigate in a mixed population the incidence of different immunological types of the disease. Although the two assays showed an excellent correlation, the electroimmunoassay was selected for its simplicity as a criterion for classification. 52 kindreds, referred to as haemophilia B-, were characterized by severe deficiency of factor IX coagulant activity (less than 0.01--0.03 u/ml) and unmeasurable IX:Ag (less than 0.12 u/ml): this genetic variant of the disease appears to be related to a complete or marked suppression of factor IX synthesis. In 16 kindreds, a severe or moderately severe IX:C deficiency was associated with normal or increased levels of IX:Ag (haemophilia B+): among them, a subgroup of five kindreds could be identified by the additional abnormality of a prolonged Thrombotest clotting time (haemophilia BM). These patients are likely to be the expression of normal or increased synthesis of a factor IX molecule markedly defective in the site(s) responsible for coagulant activity. Reduced levels of IX:Ag (0.12--0.65 u/ml, characterized the remaining 30 kindreds, presenting with IX:C levels ranging from less than 0.01 to 0.21 mu/ml. In 28 there was a significant excess of IX:Ag over IX:C, suggesting a reduced capacity to synthesize the factor IX molecule accompanied by a variable defect in the coagulant site; the remaining two kindreds, which showed a concomitant reduction of IX:C and IX:Ag, are likely to be examples of a true reduction of factor IX synthesis.

Urban soil multifunctionality and seasonal variability of carbon-linked soil traits.

Urban soils can play a significant role in climate change mitigation due to their capacity to store carbon (C) and support microbial biodiversity. In this context, this study evaluated the effects of different fertilization strategies on soil quality, greenhouse gas emissions, and microbial communities in two urban green areas located in the Campania region (Southern Italy) over a three-year period. Mineral fertilization (MIN), micronized vermicompost (CMP), micronized biochar (BCH), vermicompost plus biochar (CMP + BCH) were compared to an unfertilized control (CNT). The results showed that soil physicochemical properties were mainly influenced by site-specific conditions and temporal variability, whereas cation exchange capacity was the soil parameter most responsive to fertilization treatments. The QBS-ar index, used as an indicator of soil biological quality based on soil arthropods, was primarily affected by seasonality, with higher values recorded during spring-summer and no significant effects attributable to fertilization treatments. Vermicompost, BCH, and their combination were associated with lower net soil-vegetation CO2 fluxes and smaller temporal increases in the measured flux compared with the control and mineral fertilization treatments. All treatments exhibited a negative estimated annualized net C balance, indicating that, under the adopted temporal upscaling procedure, the estimated gaseous exchanges exceeded the annual increase in soil organic carbon stocks. Nevertheless, BCH showed the least negative estimated annualized balance. Analyses of microbial diversity revealed that bacterial and fungal communities were mainly shaped by temporal and seasonal factors, while fertilization treatments had limited effects on microbial diversity and community composition. Overall, the findings indicate that biochar showed the most favorable estimated carbon balance and the lowest measured net CO2 fluxes under the conditions investigated. However, its effects on soil biological and microbial properties were limited in the short term, and none of the tested treatments achieved net carbon sequestration. These results suggest that biochar may contribute to climate change mitigation as part of long-term, site-specific management strategies rather than as a standalone solution for improving soil multifunctionality.

Factors influencing survival of children with nonmetastatic neuroblastoma.

A randomized study was designed to determine the effect of intermittent oral cyclophosphamide on the rate of metastases in children with localized and regional neuroblastoma. Secondary objectives of the study were to determine the value of the proposed staging system, to measure the influence of the peripheral lymphocyte count on prognosis, and to study the effect of the primary tumor site and regional node involvement. Since there was no difference in survival in the two groups of patients, whether or not they received chemotherapy, all 113 children were treated as a single group for the analysis of the secondary objectives. The proposed staging served as a good indicator of prognosis with a significant difference in survival seen between each of three stages. It was not possible to detect a significant influence by the three other prognostic variables studied, peripheral lymphocytes, primary site and regional node involvement. Small numbers in the various subgroups studied may account for the failure of differences to achieve statistical significance.

Inhibition of electron transport on the oxygen-evolving side of photosystem II by an antiserum to a polypeptide isolated from the thylakoid membrane.

A polypeptide fraction with the apparent molecular weight 11 000 was isolated from stroma-freed chloroplasts from Anthirrhinum majus. An antiserum to this polypeptide fraction inhibits photosynthetic electron transport in chloroplasts from Nicotiana tabacum. The relative degree of inhibition is pH dependent and has its maximum at pH 7.4. The maximal inhibition observed was 93%. The dependence of the inhibition on the amount of antiserum yields a sigmoidal curve which hints at a cooperative effect. A calculation of the Hill interaction coefficient gave the value of 10. The inhibition occurs on the water splitting side of photosystem II between the sites of electron donation of tetramethyl benzidine and diphenylcarbazide. Tetramethyl benzidine donates its electrons before the site where diphenylcarbazide feeds in its electrons. Analysis of the steady state level of the variable fluorescence also indicates that the inhibition site is on the water splitting side of photosystem II. Tris-washed chloroplasts are equally inhibited by the antiserum and the inhibition is also observed in the presence of an inhibitor of photophosphorylation like dicyclohexyl carbodiimide and in the presence of the uncoupler carbonylcyanide m-chlorophenyl hydrazone (CCCP) which means that the inhibitory action is directed towards the electron transport chain. Valinomycin which is supposed to affect the cation permeability of the thylakoid membrane has no influence on the inhibitory action of the antiserum. The same is valid for gramicidin. Methylamine on the other hand can induce a state in the thylakoids in which the antiserum is not effective. If the antibodies are already adsorbed prior to the methylamine addition then the high inhibitory effect by the antiserum remains unchanged upon addition of methylamine. From the experiments it follows that a component from the vicinity of photosystem II is accessible to antibodies that is, the component is located in the outer surface of the thylakoid membrane. It appears that the inhibitory effect is produced in the course of the light reaction.

Characterization of self-stimulation elicited from rat dorsolateral pariaqueductal gray.

Rats were stereotaxically implanted with chronic bipolar electrodes aimed at the periaqueductal central gray (PCG) and were subsequently tested for intracranial self-stimulation (ICSS) behavior. Consistent and reliable ICSS behavior was elicited from both the midventral and dorsolateral portions of the PCG, the latter result occurring in loci previously reported as predominantly aversive. Midventral PCG ICSS rates were significantly higher than dorsolateral PCG ICSS rates. The higher rates were not accounted for by either current intensity used or motor artifacts. Moreover, both sites displayed reliable ICSS behavior characterized by regular low-variability rates and no seizure activity, seen with other brainstem ICSS sites, but unlike diencephalic and telencephalic ICSS behavior which is characterized by irregular bursts of responding which are highly variable and sometimes accompanied by seizures. The differential ICSS responsivity between PCG structures suggests that they may be mediated by different anatomically-localizable substrates.

[Splitting of the DNA of new lambdoid bacteriophages by the specific Eco RI endonuclease].

The number of EcoRI restriction sites in DNAs of several lombdoid phages varies from 2 to 7 and the size of fragments ranges from 0.6 . 10(6) to 25 . 10(6) daltons. Effectiveness of restriction determined in vivo weakly correlates with a number of restriction sites, suggesting different sensitivity of different sites to EcoRI action. No correlation between molecular weights of DNA of different phages and the rate of heat inactivation of the phage has been found. The variability of number of the restriction sites and of the molecular weights of the fragments permits to conclude that: 1) closely-related organisms can be possibly used as convenient sources of certain genes (if a construction of new vector lambdoid phages is essential); 2) there is an opportunity to find phages with different distribution of the restriction sites to other restrictases within a group of phages with non-identical EcoRI restriction sites.

Optic nerve colobomas of autosomal-dominant heredity.

Fifteen members of a family had congenital colobomas of the optic nerves, all of which were bilateral. The bolobomas appeared as enlargement of the physiologic cup with severely affected eyes having huge cavities in the site of the disc. A variable amount of glial tissue was present within the colobomas. Either macular or extramacular serous detachment or sequelae were present in 21 of the 30 affected eyes, and often led to total blindness. Radioisotope cisternography in one patient failed to implicate cerebrospinal fluid as the source of the subretinal fluid. The disorder is apparently inherited as an autosomal dominant.

Cervical carcinoma with adenoid cystic pattern: a light and electron microscopic study.

Three cases of cervical carcinoma with an adenoid cystic pattern were studied by light and electron microscopy. All were found to be compatible with the histologic picture of adenoid cystic carcinoma in other body sites. Although there was some variability in the pattern among the tumors, areas of each exhibited a cribriform appearance characterized by small cells with sparse cytoplasm arranged around cystic gland-like lumina. Only one tumor displayed cylindromatous formations similar to those described by Billroth. Two of the tumors were associated with foci of squamous cell carcinoma while the third contained areas suggestive of such a component. Electron microscopic examination showed varied morphology of the cysts, manifested by true glandlike lumina, extracellular spaces containing replicated basement membrane or enclosed stroma. In view of the observed diversity in the light and electron microscopic morphology of adenoid cystic carcinomas of the cervix, it is apparent that this tumor is not a distinct histologic entity. Accordingly, it is recommended that the terminology "cervical carcinoma with adenoid cystic pattern" be utilized.

Two-to-one A-V block with four-to-three A-V nodal wenckebach, a form of spontaneous multilevel block.

Electrophysiologic studies in one patient with spontaneous 2:1 atrioventricular block and variable PR intervals revealed two sites of conduction block. Typical 4:3 A-V nodal Wenckebach was present in addition to Mobitz type II infra-His block. The HV of conducted complexes was 120 msec and the shortest recorded AH interval was 250 msec. When 2:1 A-V block with fixed PR interval was present, only 2:1 Mobitz type II infra His-block was present. The mechanism of this example of spontaneous multilevel A-V block is discussed and compared to other previously reported cases.

Changes in event related potentials during processing of Stroop stimuli.

In a simplified Stroop test, subjects responded with a button press to either the color or word dimension of Stroop stimuli. Response times were analyzed as a function of condition (color or word), stimulus type (congruent or noncongruent color-word pairing) and visual field of presentation (left or right). Changes in the stimulus evoked potential (Experiment I) and the preresponse (premotor) potential (Experiment II) were measured as a function of the independent variables and hemisphere of recording site. Stimulus type had the expected Stroop effect on response time, with noncongruent stimuli associated with longer RTs. The pattern of changes in the event related potentials suggested that the typical Stroop interference effect (color condition) is localized in the response selection stage of processing. However, the less common reverse Stroop effect (word condition) probably results from more complex processing interactions during encoding of the color and word dimensions.

[Subfractional composition and structural characteristics of G immunoglobulins in pathological states].

Physico-chemical properties of gamma-globulin fraction and immunoglobulins G (IgG) were studied, using temperature-perturbational differential spectrophotometry and ion exchange chromatography on DEAE cellulose, in blood serum of 118 patients with impairments of thyroid gland and of 28 patients with chronic pneumonia. Amount of tyrosine residues, perturbated by temperature, was decreased and an alkaline subfraction was increased in IgG of patients with toxic goiter and chronic pneumonia; the increase in the alkaline subfraction was more distinct in impairments of thyroid gland. The data obtained suggest that the alkaline subfraction is characterized by an increased rigidity and apparently by altered structural properties in toxic goiter. The tyrosine residues in IgG enable to evaluate effectively the availability of a cavity between variable domains (antigen-antibody site) of the immunoglobulin.

[Topographic anatomical data on the a.testicularis, a.ductus deferentis and a.cremasterica in ram].

130 Ram testes have been used in the study, and the following methods were applied to fill and study the testical arteries: röntgenography, stereoröntgenography, corosive method, Indian ink and gel injections. Variabilities occur as to the site where the a.testicularis arises in rams. Individual differences can also be observed in the arrangement of the epididymal arteries, in most cases, however, they arise from the first loops of the convolution. Both epididymal arteries are considerably thinner than the testicular artery, forming 2 independent vessel convolutions, located at both sides of the convolution of the a.testicularis, and not interfering with the loops of the latter. A.accessoria testis has been found in one ram only. 2 to 3 large waved loops can be observed in the pars marginalis and the a.testicularis of the ram. Bifurcation mostly occurs in the second third of the margo epididymidis. Individual variations have been found at further ramification of the rr.testiculares. The shape of the lobulus testis is indicated by the centripetal branch with its centrifugal twigs. In rams the a.ductus deferentis forms anastomoses both with the branches running from the a.epididymidis caudalis and the a.cremasterica.

[Tick-bite meningoradiculitis (author's transl)].

Meningoradiculitis due to tick-bite is rare and poorly documented. Its etiology remains hypothetical, but is thought to be caused by a viral infection carried by the tick. It is characterized by a striking clinical picture, especially symptoms of a subjective nature. The involved area extends asymmetrically from the site of the bite with variable latency. The CSF is characteristic, showing definite lymphocytosis and a moderate protein reaction. The prognosis is excellent although recovery may take several months. In the event of facial paralysis, there is a risk of postparalytic spasms. Treatment is symptomatic; no drug has proved effective.

The pathogenesis and long-term end results of pigmented villonodular synovitis.

Pigmented villonodular synovitis, bursitis and tenosynovitis may present a variable picture depended upon the site and whether they are localized or diffuse. In the localized form of the disease occurring in association with tendon sheaths, complete surgical excision should result in a cure. Local recurrences probably result from incomplete excision. The localized disease within the knee joint may represent a picture of internal derangement. Simple excision should result in complete cure. Diffuse pigmented villonodular synovitis (DPVS) poses a diganostic and therapeutic problem. Where possible, complete synovectomy is indicated but the recurrence rate after such surgery is unfortunately quite high. Radiation therapy should probably be considered only in patients in whom lesions recur after surgical synovectomy. Despite three decades of study, the etiology and pathogenesis of the lesion remains obscure. It is apparent, however, that the lesion is not a neoplasm and probably represents an inflammatory process.

Sexual dimorphism and mechanics of the human hip: a multivariate assessment.

The present research was undertaken to determine the effects of sexual dimorphism in the human pelvis and femur on the mechanics of human locomotion. The analysis was based on six biomechanical variables determined from 25 male and 32 female skeletal remains from the Dickson Mound site. Discriminant function analysis indicates that the mechanical variables which primarily contribute to dimorphism are the moment arm of the gluteus medius and the torque produced by the abductors at the hip. These mechanical aspects of hip function produce greater pressure on the femoral head in females.
