Reintegration success of osseotite implants after intentional countertorque liberation in the endentulous human mandible.

PURPOSE: Implants that rotate during abutment screw tightening may not necessarily constitute implant failure. This prospective study was designed to determine to what degree integrated implants could gain a state of reintegration after intentional countertorque to liberation from the bone. MATERIALS AND METHODS: There were 11 patients with fully edentulous mandibles restored with 2 "clinical" implants (3.75 x 10 mm) placed in the canine sites and an additional nonused "test" implant (3.75 x 10 mm) placed at the symphysis of the anterior mandible. At second-stage surgery, the test implant was countertorqued (T1) using a torque-metering wrench until it detached from the bone. The implant was returned to its original position and allowed to heal for a period until a second countertorque test (T2) was performed. In group A, patients (n = 5) received the countertorque T1 at 3 months and T2 at 5 months. In group B (n = 6) T1 was at 1.5 months, and T2 was at 3.5 months. After completion of T2, the group A test implants were removed with a 5-mm trephine for histologic examination. RESULTS: For both groups, peak forces, all higher than 37.4 Ncm at T1, indicate that all implants were integrated. At T2, torque values were higher than T1 values for most of the test implants. The clinical implants were restored with "patrix" abutments and an overdenture. Histology staining showed distinction between old and new bone, which formed during the remodeling process. DISCUSSION: The increase in peak forces needed to detach implants at T2 may be explained by bone-to-bone healing or the additional bone contact promoted by bleeding that occurred during the T1 procedure. CONCLUSION: The results suggest that all implants were reintegrated after being intentionally countertorqued to liberation and resumed healing in the original osteotomy position.

Societal reintegration after liver transplantation: findings in alcohol-related and non-alcohol-related transplant recipients.

OBJECTIVE: To compare the degree of societal reintegration between alcohol-related and non-alcohol-related liver transplant recipients. SUMMARY BACKGROUND DATA: Orthotopic liver transplantation (OLTX) is the treatment of choice for end-stage liver disease of various etiologies. Returning patients to society to lead active and productive lives is a key goal of OLTX. METHODS: A questionnaire assessing societal reintegration was administered by phone to 84 alcoholic liver disease (ALD) OLTX recipients (ALDs) and 68 non-ALD OLTX recipients having undergone OLTX at a single-center urban not-for-profit teaching hospital. Sixty-eight non-ALD OLTX recipients, serving as the control group (Controls), were matched to the ALDs by age, sex, and length of follow-up. Participation levels were assessed in the following areas: employment, homemaking, academic study, support of others through financial and/or care-giving efforts, and involvement in social or community groups and activities. RESULTS: Seventy-nine percent of ALDs and 81% of Controls were male. Median age was 53 years for ALDs and 54 years for Controls. Median length of follow-up for both groups separately was 52 months. No significant differences were noted between ALDs and Controls in the proportion of employed individuals, homemakers, students, and supporters of others. Controls were significantly more likely than ALDs to be involved in structured social activities and routine volunteer work. CONCLUSIONS: Alcohol-related and non-alcohol-related OLTX recipients appear to return to society to lead similarly active and productive lives. ALD OLTX recipients appear less likely to be involved in structured social activities and routine volunteer work than non-ALD OLTX recipients.

Ectopic expression of URA3 can influence the virulence phenotypes and proteome of Candida albicans but can be overcome by targeted reintegration of URA3 at the RPS10 locus.

Uridine auxotrophy, based on disruption of both URA3 alleles in diploid Candida albicans strain SC5314, has been widely used to select gene deletion mutants created in this fungus by "Ura-blasting" and PCR-mediated disruption. We compared wild-type URA3 expression with levels in mutant strains where URA3 was positioned either within deleted genes or at the highly expressed RPS10 locus. URA3 expression levels differed significantly and correlated with the specific activity of Ura3p, orotidine 5'-monophosphate decarboxylase. Reduced URA3 expression following integration at the GCN4 locus was associated with an attenuation of virulence. Furthermore, a comparison of the SC5314 (URA3) and CAI-4 (ura3) proteomes revealed that inactivation of URA3 caused significant changes in the levels of 14 other proteins. The protein levels of all except one were partially or fully restored by the reintegration of a single copy of URA3 at the RPS10 locus. Transcript levels of genes expressed ectopically at this locus in reconstituted heterozygous mutants also matched the levels found when the genes were expressed at their native loci. Therefore, phenotypic changes in C. albicans can be associated with the selectable marker rather than the target gene. Reintegration of URA3 at an appropriate expression locus such as RPS10 can offset most problems related to the phenotypic changes associated with gene knockout methodologies.

Virulence and karyotype analyses of rad52 mutants of Candida albicans: regeneration of a truncated chromosome of a reintegrant strain (rad52/RAD52) in the host.

The virulence of Candida albicans mutants lacking one or both copies of RAD52, a gene involved in homologous recombination (HR), was evaluated in a murine model of hematogenously disseminated candidiasis. In this study, the virulence of the rad52Delta mutant was dependent upon the inoculum concentration. Mice survived at a cell inoculum of 1 x 10(6), but there was a decrease in survival time at dosages of 1.5 x 10(6) and especially at 3 x 10(6) cells per animal. The heterozygote RAD52/rad52 behaved like wild type, whereas a reintegrant strain was intermediate in its ability to cause death compared to these strains and to the avirulent rad52/rad52 null at inocula of 1 x 10(6) and 1.5 x 10(6) cells. A double mutant, lig4/lig4/rad52/rad52, was avirulent at all inocula used. PCR analysis of the RAD52 and/or LIG4 loci showed that all strains recovered from animals matched the genotype of the inoculated strains. Analysis of the electrophoretical karyotypes indicated that the inoculated, reintegrant strain carried a large deletion in one copy of chromosome 6 (the shortest homologue, or Chr6b). Interestingly, truncated Chr6b was regenerated in all the strains recovered from moribund animals using the homologue as a template. Further, regeneration of Chr6b was paralleled by an increase in virulence that was still lower than that of wild type, likely because of the persistent loss of heterozygosity in the regenerated region. Overall, our results indicate that systemic candidiasis can develop in the absence of HR, but simultaneous elimination of both recombination pathways, HR and nonhomologous end-joining, suppresses virulence even at very high inocula.

Micro-enterprise development schemes as effective reintegration assistance programs for voluntarily-repatriated Vietnamese asylum seekers.

"From the creation of the Comprehensive Plan of Action (CPA) in 1989 through December 1994, about sixty-eight thousand asylum seekers have voluntarily repatriated to Vietnam. In response to this reverse migration, the international community has funded numerous reintegration assistance programs to facilitate the asylum seekers' return. This article examines reintegration assistance programs currently operating in Vietnam and presents an argument that small scale income generation projects constitute the most effective and efficient use of scarce repatriation funding."

Social control in China: applications of the labeling theory and the reintegrative shaming theory.

This article delineates the underlying philosophy and functions of social control in the Chinese society. This topic is particularly interesting because specific control functions are grounded in a unique macro-control system, which is totally different from that typical of Western countries. The article also scrutinizes the implications of labeling theory and reintegrative shaming theory, as they are elaborated in the West, and tests their sensitivity to cross-cultural differences. Although some caveats are in order, the evidence presented here tends to support the reintegrative shaming theory rather than labeling theory.

Reliability and validity of a postal version of the Reintegration to Normal Living Index, modified for use with stroke patients.

OBJECTIVE: To adapt the Reintegration to Normal Living Index (RNLI) for postal use with stroke patients. DESIGN: Reliability was examined using test-retest on 26 stroke patients. Construct validity was tested on 76 patients by examining correlations between the modified RNLI and related scales. SUBJECTS: Patients at three months to one year post stroke. RESULTS: All items demonstrated better than chance agreement between test and retest and seven items substantial agreement (kappa = >0.61). The modified RNLI correlated positively with related scales. Patients with stronger reintegration to normal living had better outcomes in anxiety, depression, daily activity and quality of life. CONCLUSION: This postal instrument appears reliable and valid and may be a useful outcome measure in stroke studies.

[Reintegration of victimized children and adolescents in their parents' view].

This study aims at learning, from some aggressor families' point of view, the way reintegration of child and adolescent victims into their own families happen, in the city of Ribeirão Preto-SP in 2002. The methodology used is descriptive and qualitative; data were collected through interviews and observation of participants. Nine families whose children were institutionalized were surveyed. Results showed that the institutionalization of children is a way for some families to rethink the kind of education parents are providing their childrenwith/their own education. For the other ones, it was perceived as a support for behavioral problems. As noticed, some parents stressed a negative influence falling upon their children, which made them more disobedient on their return home, and causing disciplinary procedures harder to be established. The conclusion is that reintegration of the target children/adolescents is harnessed by violence happening inside families.

The U.S. study of work incapacity and reintegration.

In many countries, including the United States, the number of persons being awarded long-term or permanent disability benefits has risen dramatically in recent years. Government agencies, advocates for the disabled, and others are looking for ways to help persons with disabilities return to the labor force. The Work Incapacity and Reintegration (WIR) Study was developed to address that issue. The United States and five other countries--Germany, Denmark, Sweden, Israel, and the Netherlands--have participated in a cross-national study of work incapacity under the auspices of the International Social Security Association. The study had two objectives: to examine the factors that influence the pattern of work resumption among persons disabled by a back condition and to identify the medical and nonmedical interventions that are most effective in helping such persons reenter the labor force. Samples for the U.S. national study were drawn from four cohorts: Social Security Disability Insurance (DI) beneficiaries, Supplemental Security Income (SSI) beneficiaries, and recipients of temporary disability insurance (TDI) benefits from the states of California and New Jersey. Only the TDI recipients were included in the comparative study. This article discusses the study design and methodology and summarizes the findings of the U.S. national study. Findings from the U.S. study show significant differences between the two cohorts in terms of work resumption and other characteristics. The proportions of respondents from the TDI cohorts who were working at the third and final study contact ranged from 53 percent to 65 percent, compared with less than 5 percent of the DI and SSI respondents. Respondents from the DI and SSI cohorts were on average about 10 years older than the TDI respondents, were less well educated, and reported more physical demands in their usual work. They also reported lower levels of functional capacity, higher levels of pain, and a much greater tendency to have other chronic illnesses. The types of medical treatments provided were remarkably uniform across cohorts and, within cohorts, between those who did and did not resume working. Thus, no medical intervention was identified that showed a significantly higher success rate in terms of facilitating a return to work. However, changes made in the work environment by the employer were an important factor in work reintegration; about 80 percent of respondents who resumed working did so with the help of workplace accommodations. In addition, since respondents with fewer physical demands in their job were more likely to return to work, there appears to be some potential for job retraining as a means of promoting a return to work. The Social Security Administration should consider these findings in developing strategies to help disabled workers reenter the labor force.

[Prognosis, long-term sequelae and occupational reintegration 2-4 years after severe craniocerebral trauma].

122 patients with severe head injuries were assessed in terms of their global outcome and social and vocational reintegration 2 to 4 years after trauma. Some patients were studied with regard to physical, psychological, psychosocial, and cognitive long-term consequences. Less than half of the patients (43%) were found to be without employment handicap, while the remainder showed various degrees of occupational difficulty. Most of the patients had significant long-term problems related to cognitive and psychosocial dysfunction. The cognitive disorder, physical handicap, age, and duration of unconsciousness are all important prognostic factors for vocational reintegration, neuropyschological impairment being the single most important factor.

[From deintegration to professional reintegration following a sick leave from work for a mental health problem].

The aim of this study is to better understand the factors involved in the process of professional reintegration following an absence because of a mental health problems. The methodology is based on data collected from individual interviews with workers who were on a sick leave for reasons of mental health problems as certified by a medical diagnosis. The analysis of the path of people allows to better seize the articulation between events preceding the work interruption, the process of restoration of capacities and conditions favoring or hindering the return to work. This study allows to conceive the interruption of work and the process of restoration of capacities as a turning point between the process of deintegration and professional reintegration and build a model that encompasses the dynamic of the whole factors involved.

Knowledge base version reintegration.

Given two versions of a knowledge base (KB), independently modified, we investigated the problem of incorporating changes made to one KB version into the other. We have implemented a system that will perform such a reintegration, autonomously, using predetermined user preferences. This effort has lead to a greater insight into the version reintegration problem and has highlighted those areas where user intervention would be the most beneficial in a semi-autonomous system.

Memory retraining to support educational reintegration.

A memory retraining package specifically designed to facilitate reintegration of head injured patients into an educational environment is described. Two adolescent patients who had severe head injuries were administered the memory retraining package approximately three months postinjury. A single case study and multiple baseline design was used to evaluate the efficacy of the memory retraining program. The results suggested that this is a promising avenue for improving memory functioning and facilitating educational reintegration, but only where moderate rather than severe memory deficits are involved. Studies involving groups of patients and the collection of data on generalization are required to confirm usefulness.

School programmes in drug rehabilitation and social reintegration in the Philippines.

In recognition of the role of education in the process of the social reintegration of drug-dependent persons, the Dangerous Drugs Board, the Philippines' highest policy-making and co-ordinating body on all matters pertaining to drug abuse control, the Department of Education, Culture and Sports, and other drug-rehabilitation centres have jointly initiated and are implementing a formal school programme at the secondary level, within the confines of rehabilitation centres. Thus far, a built-in school programme has been established in the three rehabilitation centres existing in the country, from which 500 individuals have graduated. A follow-up study is currently underway to determine the effect that these programmes have had with respect to the successful reintegration of those individuals into community life.

Drugs and youth: an international perspective on vocational and social reintegration.

Despite the negative attitudes of the general public and even of professionals concerning the potential of drug-dependent youth to reintegrate socially and to be employed, there is ample and growing evidence for a more positive outlook. This paper describes the vocational rehabilitation process which can serve as a basic tool for most drug rehabilitation programmes. The author illustrates a number of interesting and innovative approaches, both public and private, which have all demonstrated some measure of positive outcome. Mention is made of the need for and great value of direct involvement of the community in the rehabilitation and reintegration process. Programmes for employed drug-dependent persons are also cited as a preventive and highly effective approach.

[Abdomino-perineal amputation: socio-occupational reintegration and late postoperative complications].

The major social, professional and organic sequelae of abdomino-peritoneal amputation are underestimated. A study covering 274 abdomino-perineal amputations performed between 1965 and 1981 served to determine the degree of social and professional reintegration in 64 patients who underwent surgery at age below 60. No patient was without sequelae. The leading complications are genital (87%) and urinary (30%). 65% of patients who had undergone surgery resumed work, but 26% were unable to work and are on national insurance. Specific "stoma care consultation" can help to reintegrate the patients who have undergone surgery and lessen their handicap by means, for example, of stoma irrigation.

[Social reintegration: a dynamic program to help psychiatric patients acquire functional and interpersonal skills].

Since new mental health policies have been developed, government and community-based organizations appear to be working closer toward facilitating the social reintegration process regarding persons experiencing psychiatric problems. This article describes a project which fosters the development of a patient's functional and interpersonal skills in a para-hospital environment: supervised apartments. The program's objective is to encourage awareness of an individual's situation and especially to allow him or her to acquire and experiment a certain number of skills for the ultimate purpose of reintegrating the community. This project is carried out by the psychiatry department of Notre-Dame Hospital in collaboration with the Maison l'Echelon Inc., a non-profit organization that manages a group of supervised apartments for psychiatric patients.

[Occupational reintegration and subjective quality of life after inpatient rehabilitation in myocardial infarct].

The present study was carried out on an unselected population of 107 myocardial infarction patients admitted for immediate post-acute rehabilitation. The patients were assessed (a) at discharge from multidimensional in-patient treatment, and (b) after a period of 12 months back home. Our aim had been to trace those psychological and general cardiological parameters that contribute to a patient's reintegration into work life as well as improved quality of life. It was found that patients with good physical coping did not suffer a reinfarction in the subsequent twelve months period. Younger or middle-age, male patients significantly more often returned to work. Remarkably, no correlation was found between good psychological coping with the infarction and return to work. As had been expected, a higher quality of life was achieved by those patients whose psychological status is only slightly impaired, who do not attach too great importance to their job, and who reported good physical coping as well as social reintegration and successful adjustment in their lifestyle.
