[The endoscopic polypectomy of colonic polyps with invasive adenocarcinoma. A series of 19 cases].

We review the endoscopic polypectomies done at the "Juan Canalejo" Hospital in the last 6 years (1984-1989). Out of a 509 adenomatous polyps, 19 showed invasive adenocarcinoma seen in 18 patients (there were two carcinomatous polyps in one single patient), 11 men and 7 women. The median age was 67 years and all of them were located in the left side of the colon or the rectum. The average size was 3 cm and the most frequent histologic type was villoglandular (58%) followed by the villous (26%) and adenomatous (16%). The endoscopic resection was curative in all but one patient that presented vascular invasion in the resected polyp.

Diet and chemoprevention of colon polyps and colorectal cancer.

Clinical trials in colorectal cancer prevention are based on carcinogenesis research and epidemiologic observations that relate certain macronutrients, micronutrients, non-nutritive factors in foods, and synthetic chemicals to colon neoplasms. These factors appear to act at various points along the putative normal mucosa --> adenomatous polyp --> cancer sequence. This gives an opportunity to develop innovative clinical trails, both in agent selection and in the use of intermediate markers. Clinical colorectal cancer prevention trials are not in progress that employ dietary fiber, a variety of vitamins and minerals and several synthetic drugs, especially the NSAIDs. Most of these trials use the adenomatous polyp as a surrogate marker for colon cancer, thereby reducing trial duration, number of subjects required and overall expense. This paper is a brief review of the basic and epidemiologic background for colorectal cancer prevention trials and their current status.

[Electron microscopy of colonic polyps. I. Comparative study of epithelial cells from adenomatous and adenopapillary polyps].

Ultrastructural characteristics of premalignant epithelial dysplasia were studied electronmicroscopically by comparing epithelial cells of adenomatous- and adenopapillary polyps. In adenomatous polyps the rate of absorptive and goblet cells, their electron microscopic appearance did not differ from those of the normal colonic mucosa. In dysplastic areas of adenopapillary polyps severe proliferation of undifferentiated epithelial cells and disturbance of the maturation of absorptive and goblet cells was found. Severe damage to the cell membrane, atypical vacuola and dense bodies in the apical areas of absorptive cells were also seen. An abundance of Leuchtenberger's inclusion bodies as an important sign of the malignant transformation have occurred.
