Cross-arch bar splint.

This article describes the use of a round bar connector that will aid the preservation of bone support for two isolated mandibular canines when a fixed partial denture replacing the anterior teeth is contraindicated. If the removable partial denture is constructed with only the two lone-standing canines as abutments, the removable partial denture will move occlusally and gingivally during function, thereby generating forces that may cause bone loss. By the addition of a straight round bar connecting the two canine crowns, the abutments are splinted together and masticatory forces are directed along the long axes of the teeth.

Indications for angled implants.

The development of dental implants has led to an expansion of indications and to perfection of implantation methods and implant systems. Angulation problems due to anatomic limitations, however, still remain. The indications for the use of angled Bonefit I.T.I. implants are described.

Incidence of various classes of removable partial dentures.

The purpose of a classification for removable partial dentures (RPDs) is to simplify identification and enhance its teaching. A classification also allows a longitudinal comparison of various classes of RPDs to determine whether the teaching of RPD design is consistent with the relative frequencies of RPD use. This study surveyed the types of removable partial dentures being fabricated in a regional dental laboratory and compared these findings with data from previous studies. Results indicate that mandibular RPDs are more common than maxillary RPDs and the class I mandibular RPD is the most common type of RPD for either dental arch. A palatal strap was the most frequently used maxillary major connector and a lingual bar was used three times more often than a lingual plate in the mandibular arch. Cast circumferential clasps were used twice as often as RPI clasp designs. The percentage of Kennedy class I RPDs was 40%, class II 33%, class III 18%, and class IV 9%. Comparisons with a previous study indicate the percentage of Kennedy class II has increased, whereas class I, class III and class IV RPDs have not changed significantly. Findings of the study indicate that the frequency of use of the various types of RPDs have changed in the past 30 years.

Alternative restorative approach: a clinical report.

This clinical report describes a technique for restoring a maxillary right posterior segment using a combination of a conventional full-coverage retainer and adhesive resin-bonded retainer with a nonrigid connection, periodontal ridge augmentation, and a coronal-radicular amalgam alloy core. A technique has been outlined that allows for the preservation of the maximum amount of natural tooth structure while restoring a maxillary right posterior segment.

An improved technique for processing an overdenture on implants.

This article shows an alternate procedure for making an overdenture on implants. This method helps to eliminate a chairside reline for the dentist by directly incorporating the clips into the heat-cured acrylic resin. The procedure is divided into three main sections. The first section deals with the impressions, casts, and records. The second section relates to the making of the bar, and the third section shows the assembling of the clips.

Bar-retained overdentures for implants--technical aspects.

The principles of the laboratory procedures for making overdentures retained by bar abutments on ITI implant fixtures are described, and details of producing the master casts, making a soldered bar, and processing a prosthesis incorporating a cobalt-chromium alloy strengthener are given.

A six-year prosthodontic study of 509 consecutively inserted implants for the treatment of partial edentulism.

A total of 509 consecutive Brånemark TM implants (Nobelpharma AB, Gothenburg, Sweden) were inserted in 146 patients between December 1982 and May 1989, on which 217 fixed partial dentures were planned. The cumulative failure rates after abutment connection were 3.9% and 4.1% for the maxillae and mandible, respectively. The lack of continuous prosthesis stability was limited to 4.1% for the maxillae and 5.4% for the mandible. The average annual marginal bone loss was 0.77 mm (SD = 1.0) and 0.96 mm (SD = 0.9) for the maxillae and mandible, respectively, during the first year and averaged 0.1 mm for the following years. The mode of connection between teeth and implants or the use of porcelain instead of composite resin as occlusal material did not influence the marginal bone height around the implants. Technical complications were most often related to the materials used. The results of a medium-term follow-up encourage the use of the Brånemark osseointegration system in the treatment of partial edentulism.

Odontogenic myxoma of the maxillary sinus: a clinical report.

This report describes the transoral maxillary resection of an odontogenic tumor and prosthetic reconstruction with titanium implants placed horizontally in the residual palate. The treatment provided enabled the removal of the lesion and prosthetic restoration of the missing structures, while maintaining optimal facial esthetics and obtaining support, retention, and stability for the prosthesis (Fig. 5).

Osseointegrated implant rehabilitation of the previously irradiated mandible: results of a limited trial at 3 to 7 years.

The use of osseointegrated dental implants in patients with defects of the head and neck acquired as a result of ablative surgery for malignancy is an accepted and valuable extension of the well-documented use of these implants for the treatment of edentulism. In patients who have received ionizing radiation as part of tumor therapy, the use of dental implants is poorly documented. A limited trial of 21 Brånemark implants placed in the previously irradiated mandibles of four patients is reported here. The implants have been in place and functioning for periods of 3 to 7 years. No implants have been lost. Patient selection, treatment, and complications encountered are discussed. Guidelines for further use of osseointegrated implants in patients with previously irradiated mandibles are suggested.

Mandibular staple bone plate implant and prosthesis. Report of a case.

A case treated by the mandibular staple implant fastener system and prosthesis is presented. The phases of therapy, involving preprosthetic surgical preparation, construction of conventional dentures, implantation of the orthopedic appliance, preparation of an interim postsurgical prosthesis, and construction of the definitive prosthesis, are described and illustrated.

The influence of dental restorations on salivary gland scintigraphy with 99mTc-pertechnetate.

During sequential salivary gland scintigraphy with 99mTc-pertechnetate, circumscript "cold" areas with less than background activity are sometimes seen in the region of the oral cavity. This phenomenon is due to the presence of extensive dental restorations and should not be misinterpreted in terms of deficient 99mTc-pertechnetate uptake in the minor salivary glands of the oral cavity.
