[A dart and a microscope for the King--imaginary memoirs of a current microsurgery specialist addressing the Sun King about diseases of the Great Century].

Based on the facts described in King's Louis XIV medical journal, this lecture makes the amazing inventory of the illnesses and the poor state of the medical art in a period when medical empiricism was prevalent. It is, however, the courage of patients and their resistance to utterly absurd and painful treatments that foremost arouse our admiration. By the way, this historical journey also provides the author with a pretext for valorising current developments--in comparison with treatments of the past--such as in the field of microsurgery where some pathologies that were up till now considered to be inaccessible to any possibility of surgical anatomical or functional restoration, are now controlled.

[Remov ing the tumor of the cerebellopontile angle with microsurgery to treat the secondary trigeminal neuralgia].

OBJECTIVE: In order to explore the relationship between the tumor of cerebellopontile angle(CPA) and trigeminal neuralgia and to select the surgical treatment. METHOD: With retrosigmoidal approach, the tumors of CPA were removed in 56 patients with the tumor of CPA and coexisting trigeminal neuralgia. The trigeminal nerves were decompressed with microsurgery. RESULT: All tumors were totally removed except one meningcoma which was removed by only 2/3 parts. Post-operatively, trigeninal neuralgia disappeared with no cases of severe complications or death. CONCLUSION: The pressure to the root of trigeminal nerve which comes from the tumor of CPA may be one of the etiologies of trigeminal neuralgia, which could be treated with the removal of the tumor of CPA. The micrurgy of removing the tumor with retrosigmoidal approach is an ideal functional operation. The application of endoscope could increase the rate of total removement and reduce the complications.

Delayed resolution of negative pressure pulmonary edema associated with low-molecular-weight dextran after microsurgery--a case report.

Negative pressure pulmonary edema (NPPE) is an uncommon perioperative complication after total or partial upper airway obstruction during anesthesia or respiratory distress. The cardinal event causative of the pathophysiology is believed to be the generation of a sudden and marked negative intrapleural pressure against a closed glottis, resulting in a disruption of the normal intravascular Starling mechanism. Ultimately, excessive intravascular fluid which transudes into the interstitial space eventuates in pulmonary edema. In most reports, patients suffering from NPPE could recover within 24 h after appropriate treatment. Here, we report a case of highly suspected NPPE with delayed resolution associated with the perioperative use of dextran, following microsurgery for digital anastomosis of the nearly amputated right thumb. We have reviewed the possible pathomechanisms of NPPE, and discuss herein the possible relationship between the delayed resolution of NPPE and perioperative use of dextran.

Transanal endoscopic microsurgery (TEM): a new technique and development during a time period of 20 years.

The technique of transanal endoscopic microsurgery (TEM) was made available for clinical use in 1983. To our knowledge, this technique is currently the only one-port system in endoscopic surgery by which a direct endoluminal approach to the target organ by using a natural opening of the body. Use of the stereoscopic view also is unique in procedures performed routinely. The first indication for use of this new technique was excision of rectal adenomas. It provided the potential of low complications and low recurrence rates compared with the conventional surgical procedures. With the development of endorectal ultrasound for staging, and because many adenomatous polyps contained early rectal cancer, the question arose as to how to deal with these patients after successful local excision. Studies showed that the indication for use of the TEM-technique was extended to early, good differentiated rectal cancer-so-called, low-risk rectal cancer. Many studies showed that the TEM-technique is the optimal operation to avoid complications for patients with rectal polyps and low-risk pT1 tumours of the rectum. The main problem remains regarding how to identify these patients preoperatively and separate them from those who already have a high risk of local recurrence and lymph node metastasis at the time of operation. The effectiveness of any preoperative or postoperative combined treatment to reduce the risk of local recurrence and lymph node metastasis remains unclear. Combined treatment could be a future option to also cure more advanced cancer. Currently, the TEM-technique is the only endoscopic technique that uses a natural opening to reach the target organ, and is a valuable surgical technique with a low complication rate for patients with adenomatous rectal tumours and early rectal cancer.

[The first case of management of the rectovaginal fistule using transanal endocsopic microsurgery].

The authors present an unusual and, according to the available literature data, so far unused method--management of the rectovaginal fistule, using a T.E.M. technique (transanal endoscopic microsurgery). In their case-review of a female patient, they present advantages of this technique in a case of a rare disorder - a benign rectovaginal fistule. All of the following employment of a harmonic scalpel in the T.E.M. method, as well as of a tissue adhesive Tissucol and of a surgical rectoscope in the vaginal phase of the procedure, deserve attention of surgeons and gynaecologists.

[Organization of experimental microsurgery research at the General Hospital in Maribor--principles and personal experience].

In this report, the authors discuss the organization of experimental microsurgical activities from the standpoint of their own experience. Their opinion is that elementary microsurgical technique and routine could be obtained only in the experimental laboratory. Each institution dealing with microsurgery should organize such a functional unit. The importance of an experimental laboratory is not only for education, but also for basic microsurgical research. To attain adequate results, specially in nonclinical institutions, it is necessary to follow consequently clearly defined principles of organization and provide adequate conditions. The competence of such a concept is illustrated with the results of the authors' own researches during 1987. They completed 44 microvascular anastomoses in rats, on vessels of approximately 1 mm in diameter and obtained initial and early patency rates of 88.6 and 93.2% respectively. These patency rates could be sufficient for the safe clinical application of the microvascular technique.

[History of endonasal microsurgery of lacrimal system].

The history and development of surgical treatment of lacrimal system from rhinological point of view was presented. The lacrimal drainage system has provoked great deal of investigation throughout history, because ophtalmologists couldn't attain persistent and permeable drainage between the lacrimal and the nose. The change came, when first Italian rhinologist Toti described external dacryocystorhinostomy (DCR), and later West performed endonasal DCR. The use of microscope and endoscope in rhinosurgery caused the significant progress also in lacrimal surgery. The development of endonasal microsurgery enabled to introduce other non invasive procedure, such as endoscopic laser dacryocystorhinostomy or transcanalicular dacryo-cystorhinostomy (endo-canalicular DCR). One of the newest method in the treatment of complete and partial obstruction of lacrimal drainage system is also baloon dacryocystoplasty.

Prevention of complications and correction of postoperative problems in microsurgery of the lower extremity.

Prevention of complications in microsurgery of the lower extremities begins with proper patient selection and patient preparation. Complications are highest in elderly patients who have had multiple trauma, smokers, patients with arterial or venous insufficiency, and those with hypercoagulability. Some patients may be best served by primary amputation. If multiple flaps are required, a higher success rate can be achieved with simultaneous rather than sequential transfers. However, for simultaneous transfers, a higher re-exploration rate must be anticipated. The key to avoiding long-term problems with infection is adequate preparation of the wound. Multiple debridements may be necessary. If early coverage can be obtained in trauma cases, a lower rate of chronic osteomyelitis has been observed. Attention to details in the operating room, such as patient warmth and perfusion, will increase the success rate. Recipient vessels must be out of the "zone of injury" and normal in appearance and pulsatility. It is safer to use normal recipient vessels and vein grafts than to attempt anastomoses to vessels affected by "post-traumatic vascular disease." If thrombosis occurs in the early postoperative period, prompt re-exploration can result in flap salvage, with a high degree of predictability if the cause of the thrombosis can be determined and corrected.

Microsurgery and in vitro fertilization/embryo transfer for infertility resulting from distal tubal lesions.

The live birth rates were analyzed after 688 microsurgical operations for infertility resulting from distal tubal lesions and after 885 in vitro fertilization cycles for tubal factor infertility at the Ljubljana University Department of Obstetrics and Gynecology. The cumulative live birth rate five years after surgery was 31% (17% cases lost to follow-up). The cumulative live birth rate after four in vitro fertilization cycles for tubal infertility was 40% (8% per treated cycle). With two options for treatment, operable cases had a better prognosis than did the inoperable ones. If pregnancy did not occur two to three years after surgery, the patients underwent in vitro fertilization. The complementary use of microsurgery and in vitro fertilization has substantially improved the prognosis for infertility resulting from distal tubal lesions.

Microsurgery in hand and forearm reconstructive surgery.

Recent developments in microsurgery have provided new approaches to the management of severe injuries of the upper extremities. Free vascularized autotransplants used in the treatment of severe hand and forearm injuries have provided significant advantages over conventional methods, in terms of both treatment and functional outcomes. These advances have reduced the treatment time and provided more effective treatment for patients.

[Usefulness and results of microsurgery in the treatment of female sterility].

The purpose of this study is to determine the advantages of surgical procedures performed on tubal sterility. The results of tubal reconstructive microsurgery are reported on the basis of the type of surgical procedure: lysis of adhesion (13 cases); terminal salpingoneostomy and combined procedures (23 cases); unilateral tubo-tubal anastomosis (1 case). Twenty out of 36 patients had adequate follow up: uterine pregnancy was achieved in 50% of patients who had a pure microsurgical lysis of adhesions and in 41.6% of patients who had a microsurgical salpingostomy. The ectopic pregnancy rate was 0%. The percentage of success in terms of uterine pregnancies obtained agrees with that of many other authors.

Absorbable versus non-absorbable sutures in ovarian microsurgery: experimental results in the rabbit model.

Microsurgery has been shown to reduce the risk of adhesion formation when compared to conventional surgery in cases of benign ovarian pathology. However, the microsurgeons have contrasting opinions concerning the use of absorbable versus non-absorbable suture materials. In the present study we compared nylon versus vicryl sutures for ovarian surgery using the rabbit as an experimental model. No differences were found in terms not only of post-operative adhesions but also in terms of post-operative reproductive outcome for the sutures analyzed.
