[Annular elastolytic giant cell granuloma--differential diagnosis of cutaneous granulomatosis].

Annular elastolytic giant cell granuloma (AEGCG) is a rare granulomatous skin disease characterized by phagocytosis of elastic fibres by multinucleated giant cells. A 46-year-old female patient developed multiple annular lesions with elevated borders and central atrophy on the limbs. Histological examination revealed the absence of elastic material in the granulomatous infiltrate and fragments of elastic fibrils in the cytoplasm of the multinucleated giant cells. Immunohistological investigation revealed that the ratio of CD4+ to CD8+ T-lymphocytes was 3:1. Our patient was treated with clofazimine orally, 200 mg daily for the first 10 days, then 100 mg daily. After 12 weeks a definite improvement was achieved. Our case demonstrates that AEGCG, which is mostly localized on head and neck, has to be taken into account in the differential diagnosis of generalized cutaneous granulomatous disease.

[Significance of skin changes in diabetes mellitus].

500 patients of a dispensaire of diabetes were inspected dermatologically. 335 patients had a duration of the glucose metabolism intoleration less than 10 years and 165 patients more than 10 years. 215 patients stated a striking family case history according diabetes. 54 per cent (273) of the cases showed an adiposity, 418 patients (83.6 percent) had pathological changes of the skin and mucosal changes. It is clear that dermatomycoses, eczema and pyodermias occur more often with acute metabolic disorder and adiposity. Neurovascular changes of the skin occur with long persistent glucose metabolism intoleration. Bad healing tendencles of wounds, pruritus, intensive dermatomycoses, balanitis, vulvitis, furunculosis and pyodermias are important references to a glucose metabolism intoleration. They should give rise to look for a diabetes irrespective of the age of the patient.

[Histological simulators of Kaposi's sarcoma in the initial phase].

Early Kaposi's sarcoma might be simulated by many different conditions, especially when the vascular component is prominent. the criteria suggested to differentiate early Kaposi's simulators are discussed. We found that these criteria are high in sensibility but low in specificity, therefore, we suggest using these criteria only when they are found in clusters.

[Dermatosis with the phenomenon of transepidermal elimination].

A revision was carried out on all cases of dermatosis involving transepidermal elimination seen in the last 25 years in the Department of Medical Surgical Dermatology and Venereology, School of Medicine, University of Granada. The study included clinical-statistical, therapeutic, histopathologic and evolutional aspects. In the discussion the low incidence of perforating dermatoses in the Eastern Andalusian area is emphasized.
