Hospital's attention to patient care gains it significant recognition.

Glendale Adventist Medical Center, Glendale, Calif., has received numerous awards for its Hearts N' Health program and "Healthy Families" Armenian cable access television program. Hearts N' Health is a health outreach program promoting cardiac fitness and other health issues in the Armenian community.

Utility of tuberculosis directly observed therapy programs as sites for access to and provision of antiretroviral therapy in resource-limited countries.

The overwhelming share of the global human immunodeficiency virus (HIV) infection and disease burden is borne by resource-limited countries. The explosive spread of HIV infection and growing burden of disease in these countries has intensified the need to find solutions to improved access to treatment for HIV infection. The epidemic of HIV infection and acquired immune deficiency syndrome (AIDS) has been accompanied by a severe epidemic of tuberculosis. Tuberculosis has become the major cause of morbidity and mortality in patients with HIV disease worldwide. Among the various models of provision of HIV/AIDS care, one logical but unexplored strategy is to integrate HIV/AIDS and tuberculosis care and treatment, including highly active antiretroviral therapy, through existing tuberculosis directly observed therapy programs. This strategy could address the related issues of inadequate access and infrastructure and need for enhanced adherence to medication and thereby potentially improve the outcome for both diseases.

[Vascular access in Spain: analysis of its distribution, morbidity, and monitoring systems].

Vascular access disfunction causes a therapeutic emergency with different implications in patients and care givers. The aim of this study was to know the distribution of different kinds of vascular access between prevalent hemodialysis patients, the proportion of incident patients that holds a permanent vascular access, the monthly hospital ratio for access repair and the use of surveillance systems of vascular access adequacy in the different Centers. This is a National survey that shows results of a questionnaire sent to all hemodialysis units in Spain on september 1999. Eighty-eight units answered the questionnaire (42%) providing information about 5,476 prevalent patients. Of these patients, 81% receives treatment through an arteriovenous fistula, 10% uses a central catheter and 9% a graft. Only 56% of incident patients have a permanent vascular access. Reasons for catheter use between prevalent patients were exhaustion of vascular sites in 42%, maduration of permanent access in 24%, thrombosis of the access in 14% and another reasons in 19%. Patients monthly hospitalization ratio for vascular repair was 3%, that might represent more than 5,600 hospital ingress by year. More than 80% of the Units releases different surveillance programs of access adequacy, 69% by fiscal detection problems, 47% by dynamic alterations and 18% by dilution and imaging procediments. The conclusions of this survey are that arteriovenous fistula is the most used vascular access in Spain. Almost half of the patients do not have vascular access in use for the beginning of hemodialysis. Exhausted vascular sites is the primary reason for central catheter using. A great proportion of Units are employing programs for access monitoring.

Correlates of urban mortality: a social area analysis.

A method of social area analysis developed by Shevky and Bell is used to analyze mortality among the census tracts of Des Moines, Iowa. "Social rank, urbanization, and segregation indices as well as age-standardized death rates were calculated for each census tract. All variables were treated as continuous, and correlation and regression procedures were used to analyze the data. The findings were consistent with those of previous studies and all relationships were as expected. Regression analysis revealed that segregation contributed little to the explanation of variation in age-standardized death rates, suggesting that segregation is not an important determinant of life styles affecting mortality independent of social rank. The results were interpreted in terms of social class differences in accessibility to medical assistance and assumption of the sick role."

An assessment of the effectiveness of lead pollution reduction strategies in North Lake Macquarie, NSW, Australia.

The North Lake Macquarie area of NSW, Australia, principally the suburbs of Boolaroo, Argenton and Speers Point, has been significantly polluted by lead emissions emanating from the Pasminco Metals Smelter (Cockle Creek). A lead accessibility reduction program has been carried out in North Lake Macquarie since 1991. The primary measure of success for the program is reduced blood lead levels, particularly in children. From 1991 to 2000 average child blood lead levels have decreased from 11 to 7.5 microg/dl. However, the lead accessibility reduction programs had failed to eliminate child blood lead levels that exceed the National Health and Medical Research Council maximum goal of 10 microg/dl, and a number of children still retain blood lead levels >25 microg/dl. Many factors have contributed to this failure, notably the continued presence of airborne high lead pollutants in ambient air, dusts and soil in residential areas. Significant reductions in lead mass emissions from 92 tonnes per annum (p.a.) in 1988 to approximately 15 tonnes p.a. to September 2000 have been achieved by the smelter operator. However, the reductions have been insufficient to ensure that ambient air quality consent conditions of 1 microg/m(3) are achieved in the residential areas surrounding the smelter. Sampling by the smelter operator also provides confirmation of continued deposition of high lead dusts in residential areas. The continued dust deposition places a high burden on the local community which must maintain obsessive levels of household cleanliness in order to minimise lead uptake from dusts deposited within the home and community. Evidence exists of institutional failure that may also have limited the success of the program. These failures include the use of inappropriate standards and non-approved techniques in remediation, lack of coordination between various arms of government and an unwillingness to enforce consent conditions. The smelter operator has also failed to proactively seek changes which would minimise community harm. It is suggested that for child blood lead levels to be further reduced it may be necessary to impose added restrictions on smelter emissions. Firstly, however, current emission requirements should be mandated and methodologies for remediation of historical lead depositions should also follow accepted standards and practices to maximise the benefits of the work to the affected community.
