A patient mix model for hospital financial planning.

A linear programming model was formulated to examine the impact on hospital finances and resource use of changes in patient mix under current and proposed government reimbursement regulations. The model specifically incorporated the costs, revenues, and resource consumption patterns associated with patients drawn from different intensity level and payer class combinations. The necessary data were obtained from financial reports and other records, and the model was used in policy analysis at a major university teaching hospital.

[Calculation of the minimal substrate expenditures for energy metabolism in microorganisms].

The paper presents a method for calculating the minimal expenses of substrates from the medium necessary to compensate energy donor disbalances in an intracellular process under study (for instance, in the production of a unit biomass or in the synthesis of one mole of monomer) between the number of regeneration events and the number of events in which each energy donor is utilized. The method employs the performance tables of energetic processes and is based on linear programming method. The paper includes the performance tables RE, RU and RS describing the stoichiometry of energetic processes in Escherichia coli, and the results of calculation for the cases of E. coli growth on glucose and acetate as sources of carbon.

Breast cytology diagnosis with digital image analysis.

An interactive computer system has been developed for evaluating cytologic features derived directly from a digital scan of breast fine needle aspirate slides. The system uses computer vision techniques to analyze cell nuclei and classifies them using an inductive method based on linear programming. A digital scan of selected areas of the aspirate slide is done by a trained observer, while the analysis of the digitized image is done by an untrained observer. When trained and tested on 119 breast fine needle aspirates (68 benign and 51 malignant) using leave-one-out testing, 90% correctness was achieved. These results indicate that the method is accurate (good intraobserver and interobserver reproducibility) and that an untrained operator can obtain diagnostic results comparable to those achieved visually by experienced observers.

[The features of using intraosseous implants with intramobile elements].

Functional features of type Brånemark implants are compared with those of type IMZ implants. The problem of determining optimal stiffness characteristics of type IMZ implants has been solved by means of linear programming. The conclusion was made on considerable improvement in distribution of loads when type IMZ osseointegrated implants are used as supporting elements in the denture.

Use of a spreadsheet program for Deming's linear regression analysis.

A spreadsheet program for Deming's linear regression analysis suitable for use with popular software packages such as Lotus 1-2-3 or Quattro is described. The program is controlled by an autoexecuting simple menu of operations allowing use by those with only limited experience of spreadsheet operation. Regression coefficients, identification of suspect outlying data values and confidence limits and graphical displays of fitted regression lines are provided.

A BASIC program for deriving linear regression formulas for intraocular lens power prediction.

We present an original BASIC program written specifically for the ophthalmologist that allows calculation of linear regression formulas for prediction of emmetropic lens power from the surgeon's own data. The program features ongoing data storage, options for analyzing specific ranges (such as longer axial lengths), and calculation of residuals. The program is user-friendly and may be used with minimal computer knowledge. The authors will copy the program at no charge on a user's initialized diskette.

A general purpose non-linear curve fitting program for the British Broadcasting Corporation Microcomputer.

Software for non-linear curve fitting has been written in BASIC to execute on the British Broadcasting Corporation Microcomputer. The program uses the direct search algorithm Pattern-search, a robust algorithm that has the additional advantage of needing specification of the function without inclusion of the partial derivatives. Although less efficient than gradient methods, the program can be readily configured to solve low-dimensional optimization problems that are normally encountered in life sciences. In writing the software, emphasis has been placed upon the 'user interface' and making the most efficient use of the facilities provided by the minimal configuration of this system.

PALSSE: a program to delineate linear secondary structural elements from protein structures.

BACKGROUND: The majority of residues in protein structures are involved in the formation of alpha-helices and beta-strands. These distinctive secondary structure patterns can be used to represent a protein for visual inspection and in vector-based protein structure comparison. Success of such structural comparison methods depends crucially on the accurate identification and delineation of secondary structure elements. RESULTS: We have developed a method PALSSE (Predictive Assignment of Linear Secondary Structure Elements) that delineates secondary structure elements (SSEs) from protein Calpha coordinates and specifically addresses the requirements of vector-based protein similarity searches. Our program identifies two types of secondary structures: helix and beta-strand, typically those that can be well approximated by vectors. In contrast to traditional secondary structure algorithms, which identify a secondary structure state for every residue in a protein chain, our program attributes residues to linear SSEs. Consecutive elements may overlap, thus allowing residues located at the overlapping region to have more than one secondary structure type. CONCLUSION: PALSSE is predictive in nature and can assign about 80% of the protein chain to SSEs as compared to 53% by DSSP and 57% by P-SEA. Such a generous assignment ensures almost every residue is part of an element and is used in structural comparisons. Our results are in agreement with human judgment and DSSP. The method is robust to coordinate errors and can be used to define SSEs even in poorly refined and low-resolution structures. Health behaviors and their relation to metabolic control and periodontal status in type 2 diabetic patients: a model tested using a linear structural relations program.

BACKGROUND: Although it has been reported that diabetic control is beneficial in managing periodontal disease, there are few studies in the literature where the relationship between singular behavioral factors and periodontal status has been examined. The aim of this study was to examine the simultaneous interrelationships among selected medical/dental variables and 5 behavior components with general health behavior (GHB), perceived fatigue (PF), diet control (DC), regular diet (RD), and dental behavioral inventory (HU-DBI) scores, using data from a study of 102 type 2 diabetic patients. METHODS: A hypothesized model was tested by a linear structural relations program (LISREL). RESULTS: There was a significant correlation between oral health behavior and calculus accumulation (r = 0.399; P<0.001). Correlations were found between calculus and periodontal status measured by gingivitis index and probing depth (r = 0.232; P<0.05, r = 0.279; P<0.01, respectively). The final model was found to be consistent with the data (chi2 = 55.0, df = 47, P = 0.197; GFI = 0.922). Oral health behavior affected periodontal status through its effect on calculus but not plaque accumulation. General health behavior had significant effects on oral health behavior and diet behavior (P<0.05 and P<0.001, respectively). Diet behavior affected both plaque accumulation and metabolic control (P<0.05 and P<0.01, respectively). However, the result was the virtual absence of a significant pass coefficient between metabolic control and periodontal status. CONCLUSIONS: The severity of the 2 diseases seemed to be connected indirectly through health behaviors such as diet behavior and oral health behavior.

Navy Asbestos Medical Surveillance Program (1984-1990): linear regression analysis for effect of asbestos exposure on pulmonary function testing.

This study examined records from the Navy Asbestos Medical Surveillance Program for 1984 through 1990 for Caucasian men (N = 129,598) using a population-based, cross-sectional, linear regression model. Continuous dependent variables were forced expiratory volume in 1 s and forced vital capacity (FVC), and continuous independent variables were age, height, weight, and tobacco use. A mid-period estimate of asbestos exposure was used because those values were reported as categorical variables. With asbestos exposure, forced expiratory volume in 1 s changed -3.2 cm3/year (t = -8.6, p = 0.000), and forced vital capacity changed -5.1 cm3/year (t = -11.8, p = 0.00). Those with more than 5 years of asbestos exposure demonstrated impairment over those with less exposure, and those with more than 15 years of exposure demonstrated even more impairment. These findings support the association of pulmonary function impairment with asbestos exposure for workers studied during this period.

Navy Asbestos Medical Surveillance Program (1991-1999): linear regression analysis for the effect of asbestos exposure on pulmonary function testing.

The effect of asbestos exposure on pulmonary function was studied using data from the Navy Asbestos Medical Surveillance Program. Records were selected for Caucasian men from 1991 to 1999 (N = 89,318) and were analyzed using a cross-sectional, linear regression model. Dependent variables were forced expiratory volume in 1 s (FEV1) and forced vital capacity (FVC), with independent continuous variables of age, height, weight, smoking, and asbestos history. Overall, the continuous variable for asbestos exposure demonstrated significant protection of +1.1 cm3/year (t = 3.278, p = 0.001) for FEV1 and +1.6 cm3/year (t = 4.225, p = 0.000) for FVC. There was significant interaction between asbestos exposure and smoking history (FEV1, -0.09 cm3/year2, t = -6.467, p = 0.000; FVC, -0.097 cm3/year2, t = -5.663, p = 0.000). This study suggests that workers within the program demonstrated minimal additional pulmonary function changes during the period, particularly if they do not smoke tobacco. The study also supports continuing smoking cessation efforts for all asbestos-exposed workers.

A non-linear multi-criteria programming approach for determining county emergency medical service ambulance allocations.

In this paper an integer, non-linear mathematical programming model is developed to allocate emergency medical service (EMS) ambulances to sectors within a county in order to meet a government-mandated response-time criterion. However, in addition to the response-time criterion, the model also reflects criteria for budget and work-load, and, since ambulance response is best described within the context of a queueing system, several of the model system constraints are based on queueing formulations adapted to a mathematical programming format. The model is developed and demonstrated within the context of an example of a county encompassing rural, urban and mixed sectors which exhibit different demand and geographic characteristics. The example model is solved using an integer, non-linear goal-programming technique. The solution results provide ambulance allocations to sectors within the county, the probability of an ambulance exceeding a prespecified response time, and the utilization factor for ambulances per sector.

Use of a linear gradient flow program for liquid chromatography-mass spectrometry protein-binding studies.

A rapid screening method to measure drug-protein binding using an immobilized human serum albumin (HSA) column was developed. This method utilizes a linear gradient flow-rate to accelerate the elution of strong binders to the HSA column. Post-column addition of a pressure relief valve enables mass spectrometric detection at relatively high mobile phase flow-rates (i.e., 2 ml/min).

An improved method for determination of acid dissociation constants of peptides.

PURPOSE: The method described here enables the proton dissociation constants of several amino acid residues of a peptide to be determined simultaneously in aqueous solution without prior knowledge of the exact concentration of the peptide. METHODS: The method used here employs a non-linear fitting program, the BEST program, or a linear least-squares method in combination with the BEST program. These methods are discussed in detail with an emphasis on the quality of the potentiometric titration data that are obtained. Two representative peptides, one with two proton dissociation constants (Ka1, Ka2) and the other with four proton dissociation constants (Ka1-Ka4) were used to illustrate the advantages and the limitations of these two complementary methods. RESULTS: The pKa values of TVL, a schizophrenia-related tripeptide, were found to be 3.62 (+/- 0.02) and 7.17 (+/- 0.05); the pKa values of ELTLQE, a hexapeptide, were found to be 2.32, 3.77, 4.58 and 7.74. CONCLUSIONS: The methods reported here are generally applicable to a variety of peptides. The possibility of integrating these procedures into a preparative chromatographic system for the "on-line" assessment of the pKa values of peptides during the purification stage is an attractive and novel feature of this method.

A non-linear regression analysis program for describing electrophysiological data with multiple functions using Microsoft Excel.

The objective of this present study was to demonstrate a method for fitting complex electrophysiological data with multiple functions using the SOLVER add-in of the ubiquitous spreadsheet Microsoft Excel. SOLVER minimizes the difference between the sum of the squares of the data to be fit and the function(s) describing the data using an iterative generalized reduced gradient method. While it is a straightforward procedure to fit data with linear functions, and we have previously demonstrated a method of non-linear regression analysis of experimental data based upon a single function, it is more complex to fit data with multiple functions, usually requiring specialized expensive computer software. In this paper we describe an easily understood program for fitting experimentally acquired data, in this case the stimulus-evoked compound action potential from the mouse optic nerve, with multiple Gaussian functions. The program is flexible and can be applied to describe data with a wide variety of user-input functions.

BJ: an S-Plus program to fit linear regression models to censored data using the Buckley-James method.

Most researchers are familiar with ordinary multiple regression models, most commonly fitted using the method of least squares. The method of Buckley and James (J. Buckley, I. James, Linear regression with censored data, Biometrika 66 (1979) 429-436.) is an extension of least squares for fitting multiple regression models when the response variable is right-censored as in the analysis of survival time data. The Buckley-James method has been shown to have good statistical properties under usual regularity conditions (T.L. Lai, Z. Ying, Large sample theory of a modified Buckley-James estimator for regression analysis with censored data, Ann. Stat. 19 (1991) 1370-1402.). Nevertheless, even after 20 years of its existence, it is almost never used in practice. We believe that this is mainly due to lack of software and we describe here an S-Plus program that through its inclusion in a public domain function library fully exploits the power of the S-Plus programming environment. This environment provides multiple facilities for model specification, diagnostics, statistical inference, and graphical depiction of the model fit.

ISMOD: an all-subsets regression program for generalized linear models. I. Statistical and computational background.

This paper describes a system written to carry out regression analyses under certain generalized linear models that are widely used in biomedical research. These include continuous response models such as the Weibull, log-logistic, log-normal and Cox proportional hazards models used in survival analysis, and also discrete Poisson, binomial and multinomial response regression models. The system fits models, generates residuals and other diagnostic output, and has an all-subsets regression feature. This paper describes the models implemented and gives statistical background; Part II describes the ISMOD system and presents examples of its application.

Modular DNA-programmed assembly of linear and branched conjugated nanostructures.

A new strategy for self-assembly and covalent coupling of encoded molecular modules into nanostructures with predetermined connectivity has been developed. The method uses DNA-functionalized oligo(phenylene ethynylene)-derived organic modules for controlling the assembly and covalent coupling of multiple modules. Rigid linear modules (LM) and tripoidal modules (TM) were functionalized with short oligonucleotides at each terminus. They can hybridize and thereby link up modules containing complementary sequences. Each terminus of the oligo(phenylene ethynylene) modules also consists of a salicylaldehyde moiety, which can form metal-salen complexes with other modules. The salicylaldehyde groups of two modules are brought in proximity when their adjoining DNA sequences are complementary, and they selectively form a manganese-salen complex in the presence of ethylenediamine and manganese acetate. The resulting structures consist of a matrix of linear and branched oligo(phenylene ethynylene)s which are linked by conjugated and rigid manganese-salen complexes. These nanostructures are potential conductors for applications in molecular electronics.
