[Malignant melanoma: epidemiological data from the Düsseldorf area].

Malignant melanoma represents only 1.5-2.5% of malignant neoplasms in Europe. Since 1990 the clinical data of melanoma patients are registered at the clinic for dermatology university Düsseldorf. 925 patients were treated between 1990-1996 (not including in situ melanomas, extracutaneous melanomas and multiple melanomas). In 1996 the incidence was 15 cases/100,000 habitants. A worldwide doubling of the morbidity is expected every ten years especially in the caucasian population.

[Epidemiologic data in renal lithiasis in adults].

The morpho-costitutional analysis of 574 urinary lithiasis emitted by tunisean adults permitted to define an épidemiology's profile. This resemble to the épidemiology's profile of under-developed conry: Amore raised frequency of the renal lithiasis at the man than at the woman with a sec ratio of 2.4. An average age of +14 years with a peak to 4th decade in 2 sexes. The upper localitation of the calculi is founded in 94% cases. The fréquency of the relapses, the mode of expulsion and the size of calculi are différent of those published in the litérature. Probably because the time of study which last 4 years is too short, so it don't enable us to find a result like the literature. The surgery is the mode of most fréquent élimination (51%). This s dû to the présence great size calculi in our popûlation and to the récent introduction of the lithotritie in our country.

Decision-making in otitis media in children, Part I: epidemiologic data and definitions for a reliable cost-effectiveness analysis.

There is a gap between the medical and economic weight of acute otitis media and the imprecise knowledge and irrational attitude towards diagnosis and treatment. The wide variations among prescribers need to be analyzed. Precise definitions are proposed for acute otitis media and endpoints for outcomes being estimated. An empirical antibiotic treatment gives a greater cure rate than a conservative approach with antipyretic and analgesic drugs. The incremental efficacy expected from more extended-spectrum antibacterials is not clinically significant.

[Insulin treatment in type 2 diabetes: epidemiological data].

After several years of disease duration, blood glucose control is difficult without using insulin in type 2 diabetic patients. However, large differences are observed regarding the use of insulin from the south to the north of Europe. A positive effect of insulin therapy on metabolic control compared to oral antidiabetic agents is shown only in clinical trials performed on selected patients. As to degenerative complications, a great excess of retinopathy is observed in insulin-treated patients in several surveys, without possibility to determine the specific effect of treatment from that of disease severity. The UKPDS is the only randomised long-term clinical trial with a large number of patients. The aim was to compare conventional to intensive antidiabetic treatment, whatever the type or drug, insulin or sulfonyureas. After a ten-year follow-up, glycemic control was better with the intensive treatment, the microvascular complications less frequent by 25%, but the benefit on cardiovascular complications or mortality was modest. Another clinical trial from US shows that a too strict insulin regimen could perhaps have no effect or even opposite to the objectives regarding cardiovascular morbidity. Considering these clinical uncertainties and the need for more definite long-term clinical data, it is justified to conduct additional long-term clinical studies in this field. Furthermore after secondary failure of oral anti-diabetic agents in type 2 diabetic patients, it appears reasonable to discuss the rational for insulin treatment based on an individual assessment, particularly in older diabetics, weighing the whole benefits and risks for a given patient of such an important therapeutic change.

[Recent epidemiologic data on illicit drugs in France: prevalence and health impact of consumption, availability, and product quality].

[Suicide in Abitibi-Temiscamingue region: epidemiological data for the period of 1992-96].

A permanent register of suicide cases has been established and has made it possible to produce a quantitative description of the suicide phenomenon in Abitibi-Témiscamingue for the period 1992-1996. This study reviews 211 suicide cases reported by the Chief Coroner Office of Quebec. A content analysis grid was applied to the coroners' files and has facilitated the study of 87 variables potentially related to suicide. The results show the evolution of suicide rates in this region in relation to those in other parts of Quebec, and the differences among various parts of the region. Several variables are shown to be significantly related to suicide (sex, age, method used, suicide site, etc.). A review of recent literature on suicide throughout the world indicates that patterns of suicide in Abitibi-Témiscamingue follow present trends in North America and Europe, but with aspects particular to the region.

[Domestic violence. Epidemiologic data].

Domestic violence has been little studied in France. However a national survey (Enquête Nationale sur les Violences Envers les Femmes en France, ENVEFF) shows that 9% of the women had experienced a domestic violence, including 2.3% very severe. Even if these violences are related to several characteristics or situation of the couples, they are observed in every social or cultural group. Health professionals have a role to play in the identification of the problem and the related care.
