Unwanted pregnancy: it's existing solutions from the perspective of young adults in Bangkok and the Abortion Law.

727 students of 15 to 24 years old in Bangkok were randomly selected to study their opinion towards existing solutions for unwanted pregnancy and the conformity or controversy of their suggested solutions to Thai Abortion Law. The SPSS/PC package program was used for data analysis. The results show that more than 80 percent of the youth in this study do not prefer the abortion method, despite it's being vastly used as a mean of solving the problem and that the abortion must be done under the Thai Criminal Law section 305 (1) and (2). Otherwise, it might be reasonable for some critical medical conditions such as high risk pregnancy or anti HIV+ve. Hence, it is confirmed that the youth opinion towards abortion agree with the law almost completely except for the aspects of the baby's health and genetic diseases. With regard to better solutions of unwanted pregnancy, Thai young adults strongly recommend resolutions of allowing progress of pregnancy on the basis of kindliness and understanding of human-being in crisis with warm support from within and outside the families. Besides, strengthening of virtue of Thai culture along with sufficient sex education for young generation are also needed.

Early spontaneous abortions and fetal thymic abnormalities in maternal-to-fetal HIV infection.

The thymus is thought to play a major role in the immunopathogenesis of human immunodeficiency virus (HIV) infection, particularly in maternal-to-fetal HIV transmission. Characteristic lesions of the HIV-infected thymus include a prominent CD4+ CD8+ T lymphocyte depletion at the corticomedullary junction, the region of the thymus where immune selection occurs. At least threefold excess early spontaneous abortions were noted in a cohort of 124 HIV-infected pregnant women. In these 13 abortuses a very high rate (54%) of HIV vertical transmission was documented, with the thymus gland particularly affected. It is possible that the thymic insult in HIV-infected fetuses contributes to immune rejection of the fetus, possibly by an imbalance of maternal and fetal T1- and T2-type cytokines, known to be important in HIV disease progression. We propose, therefore, that the early spontaneous abortions occurring in HIV-infected pregnant women are due, at least in part, to abnormal immune forces created by HIV infection of the thymus.

Increased risk of abortion following Neospora caninum abortion outbreaks: a retrospective and prospective cohort study in four dairy herds.

Explosive abortion outbreaks in 4 Dutch dairy herds during 1992 to 1994 are reported. In 50 of 51 fetuses submitted during the first 3 wk of the outbreaks characteristic histological lesions compatible for infection with Neospora caninum were seen. Diagnosis of infection was confirmed by immunohistochemistry in 40 fetuses (78%). No evidence for other abortifacients was found. The abortion risk of the herds was investigated in a prospective and retrospective cohort study. The prospective study showed that cows aborting during the outbreaks and N. caninum seropositive nonaborting cows had a two- to three-fold increased risk of abortion compared with N. caninum seronegative cows. Retrospective examination showed that seropositive nonaborting cows had an increased risk of abortion before the outbreaks, which may indicate that these animals were infected with N. caninum prior to the outbreaks. It is concluded that serostatus can be used for selective culling of cows to decrease future risk of abortion in dairy herds.

[Anti-brucella agglutinins in aborted women].

52 (6.46 per cent) out of 805 women who aborted spontaneously in Gujarat state, India, were found to have positive Brucella agglutin. Infection with brucella was further confirmed by various serological tests (Rapid plate test, standard agglutination test, Rose Bengal test, Coombs' test, complement fixation test, indirect bacterial haemagglutination test, gel diffusion test and determination of C-reactive protein). Culturing 650 blood clots, 300 placentae and 800 samples of urine of women who aborted gave totally negative results. Women who were selected for this study immediately after admission to the Civil Hospital, Ahmedabad, India, came from rural areas and belonged to the farming community. They had a history of contact with animals and were of the lower socio-economic group. The relationship between brucellosis and spontaneous abortion is discussed. The aborted women would most probably have contracted the infection from their domesticated animals, since brucellosis is prevalent amongst the animals in the State of Gujerat.

Selective pregnancy reduction: medical attitudes, legal implications, and a viable alternative.

"Selective pregnancy reduction" is a medical procedure used to reduce a multiple pregnancy, often a multiple pregnancy induced by in vitro fertilization or drug therapy. In such instances, healthy embryos are sacrificed in order to maximize the chances of survival of the remaining embryos or to allow the mother to choose the number of babies she wishes to deliver. Physicians appear to rely on Roe v. Wade in assuming the legality of the procedure, although such an assumption may be erroneous. Rather than continue to implant an excessive number of pre-embryos, the American Medical Association and the Association of Obstetrics and Gynecology should adopt guidelines similar to those established by the Voluntary Licensing Authority in London, which limit the number to be inserted to a maximum of three. Careful ultrasound monitoring could ensure that no more than three embryos implant when fertility drugs are used. Such practices would help physicians avoid the many moral, ethical, legal, and philosophical problems caused by selective pregnancy reduction.

An oligomer of the major outer membrane protein of Chlamydia psittaci is recognized by monoclonal antibodies which protect mice from abortion.

Monoclonal antibodies (MAbs) were generated against an ovine abortive strain of Chlamydia psittaci. A plaque reduction assay was used to select 19 neutralizing antibodies which appeared to be heterogeneous in isotype, specificity, and recognized proteins. Different neutralizing MAbs were tested for their protective abilities against abortion in a pregnant-mouse model. All of the protective MAbs selected had the same isotype, were serotype 1 specific, and recognized a protein of about 110 kDa by immunoblotting. The recognized epitopes were resistant to sodium dodecyl sulfate and reducing agents, but all of them were heat sensitive. The protein was able to form disulfide-linked polymers. Immunological cross-reaction studies with rabbit sera showed a link between the 110-kDa protein and the major outer membrane protein (MOMP). The 110-kDa protein was purified by immunoaffinity and shown to be dissociated after heating into MOMP by silver staining and immunoblotting. These results show homogeneity among protective MAbs directed to heat-sensitive epitopes located on an oligomer of the MOMP of C. psittaci.

[Clinical use of selective reduction in multifetal pregnancies].

Selective reduction under ultrasound guidance was carried out in 12 multifetal pregnancies with over 3 fetuses during 8 to 14 gestational weeks. It was performed transabdominally in 9 women but in other 3, tansvaginal approach was used. Ten women have already delivered 19 viable infants, but the remaining 2 aborted after amniotic rupture during the second trimester. Selective reduction is a safe, feasible method for management of multifetal pregnancies with over 3 fetuses.

Clot-selective coronary thrombolysis with tissue-type plasminogen activator.

Coronary thrombolysis, an intervention that can abort the sequelae of acute myocardial infarction, was accomplished within 10 minutes in dogs by intravenous administration of clot-selective, tissue-type plasminogen activator. In addition to inducing clot lysis, this promising fibrinolytic agent restored intermediary metabolism and nutritional myocardial blood flow, detectable noninvasively with positron tomography, without inducing a systemic fibrinolytic state.

Semen parameters and sperm morphology in men in unexplained recurrent spontaneous abortion, before and during a 3 year follow-up period.

To investigate the role of the 'male factor' in the pathogenesis of recurrent spontaneous abortion (RSA), especially sperm morphology abnormalities, 120 previously selected couples with unexplained RSA were studied for sperm parameters retrospectively and prospectively. The patients were subdivided into three subgroups, depending on their reproductive outcome during the 3 years of follow-up study: (i) 48 RSA couples who achieved a successful pregnancy; (ii) 39 RSA couples who experienced further abortions, and (iii) 33 RSA couples who experienced infertility during the follow-up period. A semen analysis was performed twice at the time of inclusion in this study, and twice again during the 3 year follow-up period. No significant differences in semen parameters were observed between RSA males and fertile controls. Instead, significant differences were observed between the group of RSA couples who experienced infertility during the follow-up and the other two groups (RSA couples who achieved successful pregnancy and RSA couples who experienced miscarriages and no live birth during the follow-up) for sperm concentration (P < 0.01 and P < 0.01 respectively), sperm motility (P < 0.01 and P < 0.01 respectively) and sperm morphology abnormalities (P < 0.01 and P < 0.01 respectively). Sperm morphology abnormalities do not seem to be involved in determining RSA; instead, they are an aetiological factor in determining infertility in patients, along with the other semen parameters, in the RSA couple's subsequent reproductive life. Semen analysis is an important test in the clinical management of RSA couples.

The psychological sequelae of therapeutic abortion--denied and completed.

OBJECTIVE: The purpose of this article is to review the available literature on the psychological sequelae of therapeutic abortion, addressing both the issue of the effects of the abortion on the woman involved and the effects on the woman and on the child born when abortion is denied. METHOD: Papers reviewed were initially selected by using a Medline search. This procedure resulted in 225 papers being reviewed, which were further selected by limiting the papers to those reporting original research. Finally, studies were assessed as to whether or not they used control groups or objective, validated symptom measures. RESULTS: Adverse sequelae occur in a minority of women, and when such symptoms occur, they usually seem to be the continuation of symptoms that appeared before the abortion and are on the wane immediately after the abortion. Many women denied abortion show ongoing resentment that may last for years, while children born when the abortion is denied have numerous, broadly based difficulties in social, interpersonal, and occupational functions that last at least into early adulthood. CONCLUSIONS: With increasing pressure on access to abortion services in North America, nonpsychiatrist physicians and mental health professionals need to keep in mind the effects of both performing and denying therapeutic abortion. Increased research into these areas, focusing in particular on why some women are adversely affected by the procedure and clarifying the relationship issues involved, continues to be important.

Mutagenic interaction between near-(365 nm) and far-(254 nm)ultraviolet radiation in repair-proficient and excision-deficient strains of Escherichia coli.

The mutational interaction between radiation at 365 and 254 nm was studied in various strains of E. coli by a mutant assay based on reversion to amino-acid independence in full nutrient conditions. In the two repair-proficient strains (K12 AB 1157 and B/r), pre-treatment with radiation at 365 nm strongly suppressed the induction of mutations by far-UV, a phenomenon accompanied by a strong lethal interaction. The frequency of mutations induced by far-UV progressively declined with increasing dose of near-UV. Far-UV-induced mutagenesis to T5 resistance was almost unaltered by pre-treatment with near-UV. In AB 1886 uvrA there was no lethal interaction between the two wavelengths but the mutagenic interaction was synergistic. This synergism was maximal at a 365-nm dose of 8 X 10(5) J m-2. It is proposed that in the wild-type strain, cells containing potentially mutagenic lesions are selectively eliminated from the population because of abortive excision of an error-prone repair-inducing signal. In excisionless strains, 365-nm radiation may be less damaging to the error-prone than to the error-free post-replication repair system. Alternatively, mutation may be enhanced because of the occurrence of error-prone repair of 365-nm lesions by a system that is not induced in the absence of 254-nm radiation.

5-HT(1)-like receptor agonists and the pathophysiology of migraine.

Recently, AG-25086 and GR-43175 have been reported to be highly effective in aborting acute migraine attacks. These compounds seem selectively to stimulate a sub-population of 5-HT(1)-like receptors to inhibit noradrenaline release from certain sympathetic neurovascular terminals, to contract dog saphenous vein and dog, monkey and human basilar arteries, and to decrease the arteriovenous anastomotic component of carotid blood flow in the cat. GR-43175 neither has any antinociceptive effect nor crosses the blood-brain barrier. Pramod Saxena and Michel Ferrari review the clinical effectiveness of these 5-HT(1)-like receptor agonists and their selective pharmacology, both of which strongly suggest that excessive dilatation in the extracerebral cranial (scalp and/or dural) vasculature is an integral part of the pathophysiology of migraine.

Meiosis in Neurospora crassa. I. The isolation of recessive mutants defective in the production of viable ascospores.

A scheme has been devised for efficient isolation of recessive meiotic mutants of Neurospora crassa. These mutants were detected by their reduced fertility or by the abortion of ascospores. Their isolation involved the selection and screening of the strains arising from ascospores disomic (n + 1) for linkage group I (LG I), which bears the mating-type locus. These strains are self-fertile heterokaryons that contain two types of haploid nuclei of opposite mating types (A + a). Selfings of these strains are homozygous for genes on all linkage groups except LGI and therefore allow the expression of recessive mutants with an altered sexual cycle. Using this selection procedure, three classes of mutants were detected. In one class, mutants had an early block in perithecial development (class I), and in another mutants had altered perithecia, but apparently unaltered fertility (class III). No recessive mutants were observed and all mutants tested (eight of class I and two of class III) were expressed only when used as the maternal parent. A third mutant class displayed normal production of perithecia, but defective formation of asci (class IIA), or black ascospores (class IIB). Four of 13 class IIA mutants were analyzed, and two of them [asc(DL131) and asc (DL400)] were definitely recessive analysis of 10 of 13 class IIB mutants disclosed six recessive, mutually complementing mutants: ase(DL95), asc(DL243), asc(DL711), asc(DL879), asc(DL917m) and asc(DL961). Mutants asc(DL95), asc(DL243) and the previously studied mei-1 mutant (Smith 1975) complemented one another in crosses, but did not recombine. These may be alleles of the same gene, or they may comprise a gene cluster.

Antenatal diagnosis and prognosis of conjoined twins--a case report.

In this report, two conjoined twin cases that were diagnosed at the 19th and 25th week of gestational age are reported. In the first case the pregnancy was terminated because of the very poor prognosis. In the second case the decision was made to continue the pregnancy after counseling explaining the possibility of a separation procedure with good prognosis being carried out after birth. The babies were delivered at 38 weeks of gestation and the separation procedure was carried out at ten months of age without any complications. In selected cases, there is no need to abort, because of the possibility of a separation procedure after birth with good prognosis. Color Doppler ultrasound examination in an early stage of pregnancy in cases of conjoined twin can make it possible to decide which cases are candidates for a separation procedure after birth.

[Contraceptive counseling following pregnancy].

20 patients selected at random were asked after an abortion if they could indicate an optimal point at which they ought to have received advice on contraceptive measures. 9 patients decided they would have preferred the advice before the operation, 10 patients opted for post operative instructions and one patient had no opinion. However, all the women felt in general that it was important to receive instructions. It is clear that there is no single point in time that is optimal for all women in general.

Spontaneous and induced pregnancies in hyperprolactinemic women.

Sixty-nine pregnancies were observed in 57 hyperprolactinemic women (5 with pituitary macroadenoma, 20 with microadenoma, and 32 with normal tomography of the sella turcica). Ten of these pregnancies took place spontaneously in women with mild to moderate hyperprolactinemia (up to 70 ng/ml); 2 were induced by exogenous gonadotropins, 2 by clomiphene, 42 by bromocriptine, and 9 by metergoline; and 4 occurred after pituitary selective adenomectomy. The observed complications included spontaneous abortion (10 cases); headache (7 cases); sellar enlargement (5 cases); and bitemporal hemianopsia (1 subject with macroadenoma). Among 24 women in whom prolactin levels were reevaluated at least 1 month after parturition and/or lactation, 8 showed a decrease in prolactin concentration (less than 50% of pregestational levels), with actual prolactin normalization in 3 and resumption of cyclic menses in 2 previously amenorrheic women. In contrast, no changes in prolactin levels occurred after pregnancies that ended in abortion. These data suggest the following: 1) conception is not uncommon in women with moderate hyperprolactinemia; and 2) pregnancy may be safely induced without prior surgery and/or radiotherapy in hyperprolactinemic women, except those with large pituitary adenomas, and a considerable number of these patients even show a clinical and biochemical improvement after pregnancy.

Cost analysis of ceiling tile replacement for noise abatement.

OBJECTIVE: Our purpose was to evaluate the cost and benefit of noise reduction after replacement of acoustic ceiling tile in an open ward neonatal intensive care unit (NICU). STUDY DESIGN: An observational, prospective study evaluated noise in an open ward NICU before and after suspended ceiling tiles were replaced. Noise was rated by a 10-point visual analog scale and measured with a decibel meter. RESULTS: The mean decibel level was 55 before and 53 after renovation (p < 0.05). The median visual analog score was 5 before and 4 after renovation (p = 0.09). The cost of ceiling tile replacement was $2400. CONCLUSIONS: The renovation resulted in a small decrease in the decibel level in the NICU, although this measured decrease was not perceived by observers. With a formal prerenovation noise evaluation, a more efficient ceiling tile might have been selected or the project may have been aborted or modified because of the low levels of noise already present.

Human teratogens, prenatal mortality, and selection bias.

Etiologic inferences on human teratogens are usually derived from case-control studies conducted either at birth or in spontaneous abortion series. Because both teratogens and defects may be associated with an increased risk of prenatal mortality, the possibility exists that selection bias may affect etiologic inferences. The authors derive relations between the true odds ratio (OR) relating a teratogen and a defect at the time of the occurrence of the defect and the apparent odds ratios observed in spontaneous abortion series and at birth, as functions of prenatal mortality. Depending on the pattern of interaction between the teratogen and the defect in affecting the rate of prenatal mortality, selection bias may lead to overestimation or underestimation of the true odds ratio. With increasing multiplicative effects on prenatal mortality, changes in selection bias tend to increase the observed odds ratio in spontaneous abortion series but to decrease the observed odds ratio at birth. For a range of rates of prenatal mortality, weak associations between exposures and defects (OR = 0.3-3) may well be due to selection bias; conversely, weak teratogens (OR less than 3) may be missed in case-control studies of defects conducted at birth. Selection bias due to prenatal mortality must be considered in the interpretation of etiologic studies of birth defects.
