Idiopathic acute recurrent pancreatitis.

Acute recurrent pancreatitis (ARP) results most commonly from alcohol abuse or gallstone disease. Initial evaluation fails to detect the cause of ARP in 10-30% of patients, and as a result the diagnosis of "idiopathic" ARP is given. In these patients, a more extensive evaluation including specialized labs, ERCP, endoscopic ultrasound, or magnetic resonance cholangiopancreatography typically leads to a diagnosis of microlithiasis, sphincter of Oddi dysfunction, or pancreas divisum. Less commonly, hereditary pancreatitis, cystic fibrosis, a choledochocele, annular pancreas, an anomalous pancreatobiliary junction, pancreatobiliary tumors, or chronic pancreatitis are diagnosed. Determining the etiology is important, as it helps to direct therapy, limits further unnecessary evaluation, and may improve a patient's long term prognosis.

Performance characteristics of a model secreting acinus.

An acinus is the principal secretion unit of an exocrine gland. This paper sets up a mathematical model of an acinus and examines the effect on the output of varying the operating parameters, both of the secretion mechanism and the fluid properties. Particular reference is made to acini of the pancreas. The question of whether the effects of cystic fibrosis on the pancreas can be attributed to the increased viscosity of the exudate is also considered. The work identifies key parameters, and their quantitative ranges, which would need to be examined experimentally in order to resolve this question.

Serum trypsinlike immunoreactivity in cystic fibrosis. An aid in assessing progressive involvement of the pancreas.

Serum trypsinlike immunoreactivity (TLI) was measured in 42 children with cystic fibrosis (CF) and related to age and steatorrhea. The mean TLI value in 106 age- and sex-matched control subjects was 22 +/- 7.2 ng/mL. In patients with CF, TLI showed a widespread distribution (range, 0.3 to 214 ng/mL), and an exponential inverse correlation between TLI and age was found. The same trend in the decline in TLI values with increasing age was evident in patients who were followed up. Reduced TLI concentrations were associated with fat malabsorption, but in younger patients with CF, normal or elevated TLI values did not exclude steatorrhea. Elevated serum TLI levels in patients with CF at an early age might reflect pancreatic ductal obstruction. Tendency for TLI to decrease with advancing age and in patients who are followed up can reflect ongoing degenerative destruction within the pancreatic gland. Serum TLI estimation in patients with CF should be used to monitor the progression of pancreatic impairment.

[Multilobular biliary liver cirrhosis as a purely hepatic form of mucoviscidosis].

Report about a patient with intraoperative and histological secured multilobular biliary cirrhosis of the liver and local formed purulent bile-duct and gall inflammation as an isolated hepatical form of the cystic fibrosis of the pancreas.--The character was complicated by the development of a portal vein high pressure and esophagus varicose vein haemorrhage, who at once needed a Shunt operation. Fromout the point of our view Sulprim is a good way to the therapie of purulent bile-duct inflammation in the case of cystic fibrosis of the pancreas.

[Myocardial fibrosis in cystic fibrosis. A new case report (author's transl)].

We report the case of a 9 month-old infant in whom acute heart failure was the presenting symptom of cystic fibrosis. Post-mortem examination showed lesions of myocardial fibrosis associated with typical lesions of cystic fibrosis in the pancreas. Hypoproteinemia and anemia were present and earlier could have suggested the correct diagnosis before the episode of acute heart failure. The analogy of the histologic myocardial changes in cystic fibrosis and in congenital lipomatosis of the pancreas suggests a common pathogeny related to the deficiency of the exocrine secretion of the pancreas.

[Exocrine pancreas diseases in children].

Except cystic fibrosis, disorders of the exocrine pancreas are uncommon in the child; however they are numerous and varied in terms of their pathogenesis and clinical manifestations. The author surveys the various diseases of this organ in pediatrics. More recent theories concerning pancreatic involvement in cystic fibrosis are discussed.
