Bottom sediments of the Arabian Gulf--II. TPH and TOC contents as indicators of oil pollution and implications for the effect and fate of the Kuwait oil slick.

Measurements of total petroleum hydrocarbon (TPH) concentrations in 77 core samples collected in 1992 from the bottom sediments of the Arabian Gulf were used to delineate oil pollution levels and their distribution in the region. Seven chronic moderately (TPH 50-89 microg g(-1)) and heavily (TPH 266-1448 microg g(-1)) polluted areas were identified; three in the northern part of the region and four in the southern part. Oil pollution in these areas was attributed to natural oil seepage, accidental damage to pipelines, accidental spillage from tankers, the Nowruz oil slick, and tanker deballasting. Present-day intermediate (TPH 50-114 microg g(-1)) and high (TPH 200-1122 microg g(-1)) pollution levels were identified in 10 areas. Of these, three polluted areas in the northeastern corner, offshore Saudi Arabia and offshore Bahrain, Qatar and United Arab Emirates are probably directly affected by the Kuwait oil slick. A new scenario is suggested for the movement and fate of the oil slick, in which additional large oil discharges from northern sources, as well as substantial quantities of eroded oiled sediments and oil floating from heavily impacted tidal flats along the Saudi Arabian coastline, serve as sources of oil pollution. A definite relationship exists between the grain-size distribution and the TPH content of bottom sediments, with the highest TPH concentrations in the muddy sediments, suggesting that adsorption onto muds is the primary mechanism of oil pollutant accumulation in the Arabian Gulf. Total organic carbon measurements do not correlate positively with the grain-size distribution and TPH contents of the sediments, and hence cannot be used as indicators for petroleum hydrocarbon pollution in the Arabian Gulf.

Correlation between CMV genotypes, multiple infections with herpesviruses (HHV-6, 7) and development of CMV disease in kidney recipients in Kuwait.

The possible correlation between cytomegalovirus, human herpesvirus types 6, 7 and cytomegalovirus-related clinical symptoms was studied in kidney transplant patients in Kuwait. Cytomegalovirus infection was diagnosed using the pp65 antigenemia assay. DNA of cytomegalovirus was detected by nested polymerase chain reaction (nested-PCR). PCR was also used to amplify the genes coding for structural proteins of human herpesvirus-6 (240 bp) and human herpesvirus-7 (186 bp). Glycoprotein B genotypes of cytomegalovirus were determined by restriction fragment length polymorphism. The average number of cells positive for cytomegalovirus pp65 antigen showed a steady increase with the severity of the cytomegalovirus-related symptoms. Furthermore, cytomegalovirus pp65 antigen positivity was significantly more frequent among recipients of cadaver kidney (45.5%) than among those who received live related kidneys (22.6%). Cytomegalovirus gB genotype 1 was detected more frequently (P<0.036) in recipients with live related donor kidney (38%) than in patients of cadaver kidney (13%). The genome of human herpesvirus-6 was detected at the same rate in patients with or without cytomegalovirus-related symptoms. However, the genome of human herpesvirus-7 was detected significantly more frequently (P<0.0001) in asymptomatic patients (41.7%) than in recipients with symptomatic cytomegalovirus infection (17%). We conclude that cytomegalovirus gB genotypes are not associated with the outcome of a cytomegalovirus infection in kidney transplant patients, that human herpesvirus-6 does not play a role in cytomegalovirus pathogenesis and that the role of human herpesvirus-7 in cytomegalovirus-related morbidity in kidney recipients remains unclear.

Medical risk factors in perinatal mortality in Kuwait: a case-controlled study from Al-Jahra district.

This study is the first of its kind in Kuwait. It evaluates various risk factors associated with perinatal mortality (PNM) in the Al-Jahra district hospital during the period January 1995-December 1997. Risks associated with PNM were age above 30, parity above 8, history of abortion or perinatal death, lack of antenatal care, presence of chronic maternal disease, hydramnios, antepartum haemorrhage, breech presentation and low Apgar scores. The effect of the appropriate preventative programmes involving highlighting women at risk of perinatal mortality and probable intervention are discussed.

The antistreptococcal immune responses in children with acute rheumatic fever in Kuwait.

The antistreptococcal immune responses in 130 children with Acute Rheumatic Fever (ARF), seen in Kuwait, over a period of three years, are presented. The ages of children ranged between three to 13 years. One hundred and three presented in initial attacks while 27 presented in recurrences. The geometric mean of antistreptolysin 'O' (ASO) and antihyalurodinase (AH) titres for all patients were 488 Todd Units and 595 units, respectively. In children under the age of eight years, but not above this age, the geometric mean of ASO titres was significantly higher in children with carditis than in those without carditis (P less than 0.01); there was no significant difference in AH titres between the two groups. Analysis of the Geometric Mean Titres (GMT) of both antibodies in relation to other variables is also presented.

Acute diarrhoea and rotavirus infections in young children in Kuwait.

The role of rotavirus infections in acute diarrhoea in young children was studied over a period of one year. Rotavirus was detected by electron microscopy and enzyme immunoassay methods in 40.2% of faecal specimens from 343 children with acute diarrhoea and in 4.7% of 86 controls. The infections were most common in children aged 2-12 months (42.3%). Twelve per cent of the rotavirus infected children were also infected with bacterial enteropathogens (Salmonellae, Shigellae and enteropathogenic Escherichia coli). The incidence of rotavirus infections was not related to sex, socio-economic or nutritional status of children. Rotavirus-associated diarrhoea differed in several clinical parameters from bacterial associated and nonspecific diarrhoea. Rotavirus was detected throughout the year but was most frequent during months with little rainfall and low humidity (March-May). Our results suggest that in Kuwait, rotavirus infection is a major cause of childhood diarrhoea.

Hypernatraemic dehydration in infants in Kuwait with special reference to therapy of associated metabolic acidosis.

Over a period of 16 months 510 children with diarrhoea were admitted to the Al-Adan Hospital, Kuwait, of whom 26 (5.1%) developed hypernatraemic dehydration. Prominent clinical features included vomiting (92.3%), fever (84.6%) and convulsions (19.2%). The majority were below six months of age with a mean age of 3.1 months. The sex distribution was equal. Twenty infants (77%) had severe metabolic acidosis and were treated with a combination of sodium bicarbonate and 5% glucose in water until the acidosis was corrected after which a solution of sodium chloride replaced the use of sodium bicarbonate. The sodium concentration of the intravenous fluid varied from 15 to 30 mmol/l and was given at a rate of 100 to 120 ml/kg/day. One infant died. The 25 survivors, (96.15%), which included three who developed convulsions during treatment, recovered without any neurologic sequelae.

Eye injuries among children in Kuwait: pattern and outcome.

One hundred and ninety four children with ocular injuries were admitted to the Eye Department at Ibn Sina Hospital, Kuwait during a 1-year period (1983). They represented 8.7% of total admissions and 56% of all ocular injuries admitted in the same period. These figures are higher than any previously reported figures in comparable series. The risk of severe ocular injury to children was found to be 0.35% per 1000 per year. There were 47 (24%) perforating injuries and 111 (57%) concussion injuries. Catapulted missiles were the commonest cause of injury (26%). Early presentation to hospital accounted for a low rate of secondary infection. Visual outcome of 6/12 or better was achieved in 78.5% of concussion injuries and 27.3% of perforating injuries. Education in organized school programmes has been recommended to reduce the incidence of ocular injuries in children.

Helicobacter pylori in dyspeptic patients in Kuwait.

Two hundred and four patients, mainly Arabs, attending for upper gastrointestinal endoscopy at the gastroenterology clinic in Mubarak Al-Kabeer Hospital, Kuwait, were examined for evidence of infection with Helicobacter pylori and associated inflammation. Biopsy specimens of antrum, body, and duodenum; gastric juice; and antral mucosal brushings were investigated by microbiological, cytological, and histopathological methods. Clinical conditions diagnosed at endoscopy included gastritis, gastric ulcer, duodenitis and duodenal ulcer, but half the patients had endoscopically normal gastric and duodenal mucosae. H pylori was detected by one or more of the procedures in at least one specimen from 197 (96.6%) of the patients. Histological and cytological analysis showed equal sensitivity, but bacteriological culture was less reliable. The proportion of positive cases was high, compared with other reported series, which may have been accounted for by the variety of diagnostic techniques used in this study, the selected population (all with gastrointestinal symptoms) or genetic or environmental predisposing factors peculiar to the sample population.

Single-tube, nested PCR for the diagnosis of human brucellosis in Kuwait.

The polymerase chain reaction (PCR) offers a sensitive and specific way of detecting microbial DNA in clinical samples. The aims of the present study were to develop an assay, based on a single-tube, nested PCR, for identifying Brucella in samples of human blood and then to explore the use of this test in diagnosis. The primers chosen were derived from IS711, the insertion sequence gene found in all species of Brucella. The assay amplified a 52-bp final product which was detected colorimetrically. The PCR was sensitive and specific, giving positive reactions with 14 strains of Brucella from five species. The lower limit of detection in vitro was 30 organisms. There were no false-positive reactions either with a range of bacteria known to evoke serological cross-reactions with Brucella (Vibrio cholerae, Yersinia enterocolitica, Serratia marcescens, Haemophilus influenzae, Pseudomonas aeruginosa and Escherichia coli K12) or with organisms producing similar clinical syndromes (Mycobacterium tuberculosis and Salmonella typhi). The results of a preliminary field trial of the assay in Kuwait indicate that the assay may be a valuable technique in the diagnosis of human brucellosis, meriting further study with larger numbers of cases. All 28 subjects with brucellosis (diagnosed on the basis of typical clinical features and confirmed by positive serology and, in three cases, by positive blood cultures) were PCR-positive whereas 28 healthy controls and 28 patients with febrile illness attributable to infections other than brucellosis were PCR-negative.

Antibacterial resistance and their genetic location in MRSA isolated in Kuwait hospitals, 1994-2004.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) continues to be a major cause of serious infections in hospitals and in the community worldwide. In this study, MRSA isolated from patients in Kuwait hospitals were analyzed for resistance trends and the genetic location of their resistance determinants. METHODS: Between April 1994 and December 2004, 5644 MRSA isolates obtained from different clinical samples were studied for resistance to antibacterial agents according to guidelines from the National Committee for Clinical Laboratory Standards and the British Society for Antimicrobial Chemotherapy. The genetic location of their resistance determinants was determined by curing and transfer experiments. RESULTS: They were resistant to aminoglycosides, erythromycin, tetracycline, trimethoprim, fusidic acid, ciprofloxacin, chloramphenicol, rifampicin, mupirocin, cadmium acetate, mercuric chloride, propamidine isethionate and ethidium bromide but susceptible to vancomycin, teicoplanin and linezolid. The proportion of the isolates resistant to erythromycin, ciprofloxacin and fusidic acid increased during the study period. In contrast, the proportion of isolates resistant to gentamicin, tetracycline, chloramphenicol and trimethoprim declined. High-level mupirocin resistance increased rapidly from 1996 to 1999 and then declined. They contained plasmids of 1.9, 2.8, 3.0, 4.4, 27 and 38 kilobases. Genetic studies revealed that they carried plasmid-borne resistance to high-level mupirocin resistance (38 kb), chloramphenicol (2.8-4.4 kb), erythromycin (2.8-3.0 kb) and cadmium acetate, mercuric chloride, propamidine isethionate and ethidium bromide (27 kb) and chromosomal location for methicillin, the aminoglycosides, tetracycline, fusidic acid, ciprofloxacin and trimethoprim resistance. Thus, the 27 kb plasmids had resistance phenotypes similar to plasmids reported in MRSA isolates in South East Asia. CONCLUSION: The prevalence of resistance to erythromycin, ciprofloxacin, high-level mupirocin and fusidic acid increased whereas the proportion of isolates resistant to gentamicin, tetracycline, chloramphenicol and trimethoprim declined during the study period. They contained 27-kb plasmids encoding resistance to cadmium acetate, mercuric chloride, propamidine isethionate and ethidium bromide similar to plasmids isolated in MRSA from South East Asia. Molecular typing of these isolates will clarify their relationship to MRSA from South East Asia.

Oral health knowledge and behavior among male health sciences college students in Kuwait.

BACKGROUND: Health auxiliary personnel have an important role in oral health promotion when they graduate and start working in the health care system. This study aims to find out oral health knowledge and oral health behavior of male Health Sciences College students. METHODS: A questionnaire was distributed to all students at the male Health Sciences College in Kuwait (N = 153) during the academic year 2001/2002. The students filled the anonymous questionnaire in the class after the lecture. The response rate was 84% (n = 128). The questions consisted information on the general background, oral health behavior and oral health knowledge. RESULTS: Oral health knowledge seemed to be limited and very few background factors were associated with it. More than half of the students had visited a dentist during the previous 12 months, but only one third of students were brushing twice a day or more often. CONCLUSIONS: It may be concluded that the male Health Sciences College students seemed to have appropriate knowledge on some oral health topics, but limited knowledge on the others. Their toothbrushing practices are still far behind the international recommendation (twice a day) and also the knowledge, why it should be done so frequently also very limited.

The Kuwait University Anxiety Scale: psychometric properties.

This study was undertaken to construct the Kuwait University Anxiety Scale in two comparable Arabic and English versions. The intent was to provide a research tool as well as a measure of predisposition to develop anxiety in general among adults and adolescents. The scale is comprised by 20 brief statements answered on a 4-point intensity scale, anchored by 1: Rarely and 4: Always. Factor analysis yielded three high-loaded factors of Cognitive/Affective, Behavioral/Subjective, and Somatic Anxiety, with moderate interfactor correlations. Item-remainder correlations ranged from .27 to .74. Reliabilities ranged from .88 to .92 (alpha) and between .70 and .93 (test-retest), denoting good internal consistency and stability. Criterion-related validity of the scale ranged between .70 and .88 (5 criteria), while the loadings of the scale on a general factor of anxiety were .93 and .95 in two factor analyses, demonstrating the scale's criterion-related and factorial validity. Discriminant validity of the scale was demonstrated. The scale correlated .65 with scores on the Beck Depression scale. Male adolescents attained a higher mean score than male undergraduates. Females have significantly higher mean scores than their male counterparts. Kuwaiti norms (N = 4,660) were reported. An English version of the scale is available.

Adequacy of an English version of the Kuwait University Anxiety Scale based on back translation and testing bilingual subjects.

An English version, a translation of the Kuwait University Anxiety Scale written originally in Arabic, was prepared. Several cycles of translation and back translation were carried out. Bilingual Arab college students, studying English language and literature (N=74) responded to the two versions of this scale in counterbalanced order. The correlation between the two forms was .96 denoting high cross-language equivalence. The nonsignificant mean difference for total scores between the two versions was an indication that they functioned as equivalent stimuli. Thus, the English version of the scale is highly recommended for use with English-speaking subjects.

CTG repeat number at the myotonic dystrophy locus in healthy Kuwaiti individuals: possible explanation of why myotonic dystrophy is rare in Kuwait.

BACKGROUND: Myotonic dystrophy is caused by unstable (CTG)(n) repeat expansion. On normal chromosomes, this repeat is highly polymorphic, with a copy number ranging from 4 to 38. Myotonic dystrophy is considered more prevalent in Western European and Japanese populations but less prevalent, rare, or even absent in others. It has been proposed that the expanded (CTG)(n) alleles originated from the group of the large normal alleles. OBJECTIVE: To determine whether there is a lower prevalence of the large alleles in the Kuwaiti population. DESIGN AND PARTICIPANTS: We determined the size distribution of the CTG repeats by means of polymerase chain reaction in blood DNA derived from 185 healthy Kuwaiti individuals representing the 5 Kuwaiti provinces. RESULTS: We found a total of 17 (CTG)(n) alleles, with a range of 5 to 37 repeats. The (CTG)(5) allele was the most frequent single allele (100/370 [27.0%]), whereas the (CTG)(10-13) was the most frequent class of alleles (161/370 [43.5%]). Using 18 repeats as the cutoff point, chi(2) analysis showed a statistically significant lower frequency of greater than 18 alleles in the Kuwaiti population compared with the European population (chi(2) = 12.7; P<.001). CONCLUSIONS: These data may explain the rare occurrence of myotonic dystrophy in the Kuwaiti population. Further study of healthy families within the high-normal repeat range is in progress to investigate the possible instability of the (CTG)(>18) alleles in our area.

Common origin of human T-lymphotropic virus type-I from Iran, Kuwait, Israel, and La Réunion Island.

We found previously that Kuwaiti HTLV-I isolates had two nucleotide substitutions in the most frequently sequenced regions of HTLV-I genome, namely: T-->C 4783 in the pol and T-->C 6569 the env genes. These substitutions were observed rarely in other HTLV-I isolates and seemed to be good markers of the HTLV-I lineage, the "epicentre" of which was located in Mashhad, Iran. To test this hypothesis we sequenced the fragments of HTLV-I genome including sites 4783 and 6569 from seven isolates obtained from the Iranians either living in (five isolates) or originating from (two isolates) Mashhad. RFLP-based tests were also designed and used for typing of the substitutions. All seven isolates were positive for T-->C 4783 and six, from which env fragment was amplifiable, were also positive for T-->C 6569. It is highly probable that all the isolates from Mashhadi Jews belong to the same HTLV-I lineage, although they were not typed yet for the presence of T-->C 6569 substitution. Only 2 "non-Middle Eastern" HTLV-1, both from La Réunion Island were positive for both of the substitutions. Another possible member of Mashhadi lineage of HTLV-I is one isolate from southern India and two isolates from the American Indians, British Columbia, Canada. The determination of the T-->C 4783 and T-->C 6569 markers in HTLV-I isolates of different geographical/ethnic origin may be useful for the reconstruction of the routes of HTLV-I spread from the Middle East and/or Indian subcontinent to other regions of the world and, possibly, for gaining insights into the origin of HTLV-I in Asia.
