Detecting possible vaccination reactions in clinical notes.

The Vaccine Safety Datalink is a collaboration between the CDC and eight large HMO's to investigate adverse events following immunization through analysis of medical care databases and patients' medical charts. We modified an existing system called MediClass that uses natural language processing (NLP) and knowledge-based methods to classify clinical encounters recorded in electronic medical records (EMRs). We developed the knowledge necessary for MediClass to detect possible vaccine reactions in the outpatient, ED, and telephone encounters recorded in the EMR of a large HMO. We first trained the system using a manually coded gold standard training set, and achieved high sensitivity and specificity. We then ran a large set of post-immunization encounter records through MediClass to see if our method would generalize. Compared to methods that use administrative and clinical codes assigned to the EMR by clinicians, the system significantly improves the positive predictive value for detecting possible vaccine reactions.

Psychosomatic aspects of skin diseases from the standpoint of immunology.

The psychosomatic aspects of skin disease were studied both clinically and experimentally, from the standpoint of immunology. We found that emotional stress has a great influence on the immune system, as was manifested in skin disease. Skin test in allergic patients significantly improved with autogenic training and relaxation. For clarification of the effects of autogenic training and relaxation, various parameters were simultaneously assessed during the treatment. The serum levels of histamine and dopamine-beta-hydroxylase fluctuated, as determined by the microvibration test. The levels of IgE and findings on the PK test varied only slightly. Before the onset of urticaria, there were changes in the life-style and considerable stress in daily life as well as exposure to an allergen. Using mice subjected to stress, the functions of T cells and macrophages were evaluated. Stress appeared to have a definite influence on the functions of these cells, as related to the important role of the immune system and skin. Thus, the role of stress in clinical disease must always be given consideration.

Immune function in female elite rowers and non-athletes.

OBJECTIVE: To compare immune function in female rowers and controls in the resting state, and then correlate the results with a two month history of upper respiratory tract infection (URTI). METHODS: Subjects included 20 elite female rowers located at the ARCO Olympic Training Centre in Chula Vista, California, and 19 non-athletic female controls. These two groups were compared cross sectionally for immune function and infection rates. RESULTS: Granulocyte/monocyte phagocytosis, oxidative burst activity, and plasma cytokine concentrations (interleukin-6, tumour necrosis factor-alpha, and interleukin-1 receptor antagonist) did not differ significantly between groups. Phytohaemagglutinin induced lymphocyte proliferative response (adjusted whole blood method) was significantly higher (31% and 36% for optimal and suboptimal concentrations respectively) in rowers than in controls. Natural killer cell activity was substantially higher (1.6-fold for total lytic units) in the female rowers than in controls. Two month health logs disclosed 5.2 (1.2) and 3.3 (1.1) days with URTI symptoms for the rowers and controls respectively (p = 0.268). For all 39 subjects combined, and for the 20 rowers separately, none of the immune parameters correlated significantly with number of days with URTI symptoms. CONCLUSIONS: In this cross sectional comparison of elite female rowers and non-athletes, a group difference was found for natural killer cell activity and phytohaemagglutinin induced proliferative response (whole blood technique), but not other measures of immune function. The number of days with URTI symptoms during the spring season did not differ between groups, and variations in blood measures of immunity were unrelated to URTI.

HIV and aerobic exercise. Current recommendations.

Aerobic exercise training is an important therapy to offer individuals who are HIV positive (HIV+). Six to 12 weeks of moderate exercise sessions (3 times per week for 1 hour) significantly improves aerobic capacity (VO2max and lactic acidosis threshold), apparently without detrimental effects on the immune system. In addition, small but significant beneficial effects on the immune system (skin test reactivity) and quality of life are obtained. The effects on lean body mass with aerobic exercise training have been less marked, but are clearly less costly than androgenic hormone administration in this population. Aerobic exercise training can be recommended with confidence in HIV+ patients.

Plasma glutamine responses to high-intensity exercise before and after endurance training.

Glutamine responses to strenuous interval exercise were examined before and after 6 weeks of endurance training. Glutamine measures were obtained before and after the interval exercise sessions and training in untrained males assigned to training (T; n = 10) or control (C; n = 10) groups. Before training, C and T group glutamine progressively decreased (p < 0.05) by 18% and 16%, respectively, by 150-min postinterval exercise. Over the training period C group glutamine did not change, while T group values increased (p < 0.05) by 14%. After training, glutamine again decreased (p < 0.05) by similar percentages (C = 16% and T = 15%) by 150-min postinterval exercise, but the T group recorded higher (p < 0.05) resting and postexercise glutamine concentrations than the C group. Training induced increases in glutamine may prevent the decline in glutamine levels following strenuous exercise falling below a threshold where immune function might be acutely compromised.

[Effect of immunization with neurospecific proteins and tubulin on learning in the rat].

The paper deals with the effect of anticerebral antibodies on rats learning in T-shaped maze with food reinforcement. The rats of August line were immunized by neurospecific proteins 10-40-4 from the human and rat's brain, 14-3-2 and cerebral tubuline. Rats immunized by bovine serum albumine (BSA) served as a control of the influence of antibodies to noncerebral protein. Rats injected with Freund adjuvant and saline, served as control of Freund adjuvant action. At the time of antibodies formation (from the 7-th day after the last injection) the rats were trained in a T-shaped maze during 4 days. The acquisition of conditioned reactions was inhibited in rats immunized with neurospecific proteins 10-40-4 from the rats brain, 14-3-2 and cerebral tubuline. Antibodies to neurospecific protein 10-40-4 from the rats brain produced the greatest effect on the process of learning. They elicited a significant decrease of conditioning and an increase of the number of errors. Antibodies to the neurospecific protein 10-40-4 from the human brain and to the BSA, i.e. to proteins which are not present in the nervous tissue of the rats, did not affect the learning. The obtained results indirectly confirm the permeability for the antibodies of the blood-brain barrier in immunized animals.

The acute immune response to exhaustive resistance exercise.

Ten young male adults (mean age 46.9 +/- 1.2 yrs) with 9.2 +/- 1.4 years of weight training experience and the ability to parallel squat at least 1.5 times their body mass were selected as subjects. The exercise session consisted of sets of 10 repetitions at 65% 1-RM of the parallel leg squat, with a cadence of one rep every 6 sec and 3 min rest between sets, to muscular failure. The average subject lifted a total of 9711 +/- 1576 kg during 98 +/- 14 reps for a total work output of 72.5 +/- 10.5 kJ before muscular failure occurred. Mean oxygen consumption during exercise was 1.58 +/- 0.06 l/min at 42.5 +/- 2.0% peak VO2. A strong leukocytosis, lymphocytosis, and lymphocytopenia, similar to what has been reported following high-intensity cardiorespiratory exercise, were measured following leg squat exercise. Con A-stimulated lymphocyte proliferation (unadjusted) rose 50% above preexercise levels (p = 0.07), but when these data were adjusted on a per T cell (CD3+) basis, no change from rest was observed. Natural killer cell cytotoxic activity (NKCA), when adjusted on a per NK cell (CD56+) basis, was decreased about 40% below preexercise levels for at least 2 h post-exercise. No significant increase in cortisol was seen after exercise, although norepinephrine and epinephrine increased moderately (465% and 133%, respectively), immediately following exercise. The data demonstrate that leg squat exercise to muscular failure results in a very similar immune response to that associated with intense endurance exercise, despite a lower mean oxygen consumption and only a moderate hormonal response.

[Modulation of behavior of mice of different genotypes by serotonin antibodies].

The effect of active immunization with conjugated serotonin-protein on the "open-field" behaviour and passive avoidance conditioning was studied in genetically different mice strains (C57BL/6 and BALB/c). The obtained immunophysiological effects of serotonin antibodies depended on genetically determined characteristics of animal behaviour. Serotonin antibodies altered rearing and crossings of the "open-field" center and retention of passive avoidance learning in C57BL/6 mice. The active immunization with conjugated serotonin-protein of BALB/c mice resulted in modulation of the "open-field" latency and both training and testing of passive avoidance behaviour.

Simple approach to acute respiratory infection in rural under five children.

The feasibility of acute respiratory infection (ARI) control in 5,535 rural preschool children was studied. The Primary Health Centre (PHC) staff and local practitioners (drug distribution centres) were identified and trained in recognition of moderate/severe ARI, referral, drug administration and in the education of the community. Functional ARI classification as envisaged in ARI control programme was followed. There was significant reduction in moderate (42% reduction) and severe (89% reduction) ARI episodes from year 1985 to 1987. Both ARI (27.8%) and non-ARI (18.3%) deaths showed reduction. Majority of children who died due to ARI were also unimmunized. The moderate and severe ARI related morbidity and mortality was significantly reduced in immunized children compared to unimmunized children. Although, strategies of National ARI control programme by health education, standard case management and strengthening of immunization is a good thought but it is clear that proper implementation of immunizations is going to pay more dividends. It is also evident that the local medical practitioners should be trained and involved in this control programme to have community faith as well as to avoid opposition.

[Influence of information campaigns on the vaccination immunity among the population of a small town area - seroepidemiological results of the 'Wittlich Vaccination Study'].

In the course of the three-phase 'Wittlich Vaccination Study' the population immunity against tetanus, diphtheria and poliomyelitis was controlled in all age groups of the regional population. Antibody measuring was done in specific validated enzyme immunoassays and neutralisation tests. After performing several campaigns for training and informing medical staff and the population about vaccinations the effect of these campaigns was checked in an additional control of the population immunity. The part of not or only partially protected participants decreased for tetanus from 36.7 % to 2.8 % and for diphtheria from 61 % to 51 %. The gap in population immunity was reduced for polio virus type 1 from 6.7 % to 3.1 %, for polio virus type 2 from 6.2 % to 2.5 % and for polio virus type 3 from 13.2 % to 3.6 %. The study design can be transferred to regions with comparable structure of their population.

Active immunization against nicotine prevents reinstatement of nicotine-seeking behavior in rats.

BACKGROUND: The presently available pharmaceutical aids in smoking cessation possess a rather limited effectiveness. Therefore, we have synthesized a series of immunoconjugates that stimulate the induction of antibodies which may bind nicotine in the blood, thereby preventing it from passing the blood-brain barrier. Thus, the reinforcing action of nicotine in the brain, which is the driving force in tobacco smoking, should be abolished. OBJECTIVE: The present study was undertaken to test this notion in a long-term relapse model in rats, measuring the reinstatement of nicotine-seeking behavior, following active immunization with IP18-KLH, one of our immunoconjugates. METHODS: Male Wistar rats were immunized with a nicotine-KLH conjugate (nicotine immunogen) and Freund's adjuvant after having been trained to meet the criteria of stable nicotine self-administration on a fixed ratio (FR3) schedule. The rats were subsequently extinguished from nicotine self-administration behavior and finally, as extinction was completed, they were exposed to small, priming doses of nicotine, which previously have been shown to reinstate the nicotine-seeking behavior. The antibody titers were measured by ELISA. RESULTS: It was found that rats with high titers (>1:10,000) of antibodies against nicotine, in contrast to those with low/no nicotine selective antibodies, do not reinstate nicotine self-administration behavior when they are exposed to nicotine. CONCLUSIONS: Our findings indicate that active immunization against nicotine may effectively abolish the reinforcing action of nicotine in brain, an effect which is critical for relapse in nicotine dependence. These data suggest the potential utility of active immunization in smoking cessation programs.

Mothers as active partners in the prevention of childhood diseases: maternal factors related to immunization status of preschool children in Italy.

BACKGROUND: We examined how maternal socio-demographic factors, together with mother's education, knowledge, and perception of immunizations, can affect the uptake of optional vaccinations of preschool children in Italy. METHODS: Interviews of Italian mothers were performed using a structured questionnaire administered by trained interviewers with no specific medical competence. RESULTS: A convenience sample of 1,035 mothers were interviewed. Fifty-nine percent of the respondents reported to have had their child immunized with the MMR vaccine and 54% reported to have had their child immunized against pertussis. In logistic regression analysis, three variables were significantly associated with both the immunization outcomes: mother's positive attitude toward immunization (OR = 1.69; IC 1.13-2.52 for pertussis; OR = 1.86, IC 1. 17-2.96 for MMR); mothers' residency in the North of the country (OR = 1.74; IC 1.32-2.30 for pertussis; OR = 1.63, IC 1.18-2.24 for MMR); and mother's receipt of satisfactory information on immunization (OR = 1.67; IC 1.15-2.21 for pertussis; OR = 2.25, IC 1. 47-3.43 for MMR). An immunization performed in recent years (after 1994), probably following the widespread use of acellular vaccine, was the most significant predictor for pertussis immunization (OR = 3.21; IC 2.43-4.24). CONCLUSIONS: The findings suggest that mothers' attitudes, educational level, and socio-demographic characteristics, as well as socio-economic factors and local health policies, can influence children's immunization uptake. Health promotion, based on a partnership between parents and health professionals, should become a priority in Italian vaccination policies.

Hepatitis in day care centers: epidemiology and prevention.

Hepatitis A is a significant health problem in day care centers, causing outbreaks that average 12 cases in size and three months in duration. These outbreaks have three characteristic features: children have mild or asymptomatic infections; adults (primarily parents) are the major group with clinical hepatitis; and persons having contact with one- or two-year-old children run the highest risk of infection. Outbreaks are commonest in centers that are large, have long operating hours, and enroll children younger than the age of two years (i.e., those in diapers). The presence of such children is necessary for the rapid spread of the disease. Nationally, outbreaks occur primarily in areas with many infant/toddler centers, which often form the focus for epidemics. Prevention relies on hygiene, especially hand washing. Disease control depends on early detection of outbreaks and aggressive use of immunoglobulin. The spread of hepatitis B has not been documented in day care centers; however, when a child carrying hepatitis B virus enrolls in a center, a low risk of transmission may exist and precautions are recommended, with a focus on personal hygiene.

Anaerobic exercise induces moderate acute phase response.

PURPOSE: It was intended to compare the immune reaction after single and repeated short bouts of anaerobic exercise. METHODS: Twelve unspecifically trained male subjects (27 +/- 2 yr, 75 +/- 2 kg, VO(2peak) 52 +/- 2 mL x min(-1) x kg(-1)) performed one 60-s all-out test (SMT) on a cycling ergometer and the same test followed by eight 10-s all-out tests every 5 min (AN-TS). These tests and one control day (Co-Day) were applied in randomized order. At rest and 15 min, 2 h, and 24 h after cessation of exercise the following venous blood parameters were determined: concentration of neutrophils and (CD16(+ -)) premacrophages (both flow-cytometrically), interleukin 6 and 8 (IL-6, IL-8), C-reactive protein (CRP) and cortisol. RESULTS: Two hours after cessation of exercise the neutrophils increased stronger after AN-TS than after SMT (P < 0.01). The peak in the number of premacrophages occurred earlier after SMT (15 min post; P < 0.01 to Co-Day) than after AN-TS (2 h post; P < 0.05 to Co-Day). IL-6 was elevated at 15 min and 2 h after AN-TS (P < 0.01 to SMT and Co-Day) but only slightly 2 h after SMT (P < 0.01 to Co-Day). There were no significant changes in IL-8. CRP was the only elevated parameter 24 h postexercise exclusively after AN-TS (P < 0.05 to Co-Day). CONCLUSIONS Repeated short anaerobic bouts of cycling lead to an acute phase response, which is more pronounced than after a single bout. Athletes should take care in performing such training sessions several times a week because signs of inflammation are detectable even 24 h after cessation of exercise.

Effects of exercise on the physical condition of college rugby players during summer training camp.

OBJECTIVES: To determine the effects of exercise training on physical condition in 25 college rugby players during a summer training camp, and to compare these variables by the different players' positions. METHODS: Changes in body composition parameters and blood biochemistry were examined before and after a summer training camp. RESULTS: Body weight and percentage body fat did not change significantly during the camp. There were significant decreases in levels of serum total cholesterol, triglycerides, phosphate, uric acid, and immunoglobulin G and M. In contrast, there were significant increases in levels of serum potassium, markers of renal, hepatic, and muscular damage (BUN, GOT, GPT, LDH, CK), and complement C4. Comparison of the changes in biochemical parameters between rugby players playing in different positions showed a significant increase in serum albumin level in the forwards, and significant decreases in serum triglyceride and sodium levels in the backs. The magnitude of change in serum LDH during the camp was significantly greater (p<0.05) for the forwards than for the backs. CONCLUSIONS: These data suggest that, in rugby players attending a 20 day camp, exercise training resulted in muscular damage, loss of electrolytes due to sweating, and changes in immune function. Backs exhibited a higher rate of fat metabolism and loss of electrolytes than forwards, possibly because they did more running during the camp. In contrast, forwards experienced more physical contact, performed more physically strenuous exercise, and exhibited higher levels of muscular damage and tissue protein degradation.

Markers of excessive exercise.

Overtraining is of serious concern to long-distance runners and will affect 65% of them at some time in their competitive career. The clinical presentation is nonspecific but the classical symptoms include fatigue, mood disturbances, frequent upper respiratory infections and injury, and a decrease in performance. Dysfunction of the hypothalamic pituitary axis from repeated stress, of a physical or nonphysical nature, represents the most likely pathogenesis of this condition. There is no single biological marker that is diagnostic of an overtrained state; however, several parameters deserve further study. The time to volitional fatigue on a cycle ergometer at an intensity of 110% of the individual anaerobic threshold represents a possible laboratory test. Salivary IgA holds promise as a useful immunoligical marker of the overtrained state and further research is needed to determine the validity of plasma glutamine as a blood marker. The most promising tool at present is a measure of the athlete's mood state, and several psychological tools can be used for this purpose.

Laparoscopic gastrectomy for cancer.

There are three procedures for the management of early gastric cancer (EGC): laparoscopic wedge resection (LWR), intragastric mucosal resection (IGMR), and laparoscopic gastrectomy. LWR or IGMR can be applied to treat EGC without the risk of lymph node metastasis. However, owing to the recent technical advances in endoscopic mucosal resection for EGC, the use of laparoscopic local resection for these lesions has gradually decreased. On the other hand, laparoscopic gastrectomy with lymph node dissection, such as laparoscopy-assisted distal gastrectomy, is widely accepted for the treatment of EGC with the risk of lymph node metastasis. To establish the acceptability of laparoscopic gastrectomy with D2 lymph node dissection against advanced gastric cancers, safe techniques and new instruments must be developed. The following advantages of laparoscopic surgery for the treatment of gastric cancer have been well demonstrated: clinical course after operation, pulmonary function, immune response. In the future, laparoscopic surgeons have to design and implement education and training systems for standard laparoscopic procedures, evaluate clinical outcomes by multicentric randomized control trial studies, and clarify the oncological aspects of laparoscopic surgery in basic studies.

Integrative analysis identifies a glycosylation-related lncRNA signature associated with prognosis in kidney renal clear cell carcinoma.

BACKGROUND: Glycosylation and long non-coding RNAs (lncRNAs) play critical roles in tumor progression. However, the prognostic significance of glycosylation-related lncRNAs (GRLncs) in kidney renal clear cell carcinoma (KIRC) remains largely unclear. This study aimed to identify prognostic GRLncs and construct a predictive model for KIRC prognosis. METHODS: Transcriptomic and clinical data of KIRC patients were analyzed to identify GRLncs associated with overall survival (OS). A prognostic model was constructed based on selected GRLncs, and its predictive performance was evaluated using Kaplan-Meier (KM) survival analysis, receiver operating characteristic (ROC) curves, and univariate and multivariate Cox regression analyses. Patients were stratified into high- and low-risk groups according to the median risk score, and internal validation was performed using training and testing cohorts to assess the stability of the model. Tumor microenvironment characteristics, immune checkpoint expression, immunotherapy response, and drug sensitivity were further analyzed. In addition, the expression of three signature lncRNAs was validated by real-time quantitative polymerase chain reaction (RT-qPCR) in 10 paired KIRC tumor and adjacent normal tissues. Functional roles of selected lncRNAs were investigated using antisense oligonucleotides (ASOs)-mediated knockdown in KIRC cell lines, followed by Cell Counting Kit 8 (CCK-8), 5-ethynyl-2'-deoxyuridine (EdU) incorporation, colony formation, and migration assays. RESULTS: Five GRLncs (AC093278.2, EPB41L4A-DT, DLGAP1-AS2, AC084876.1, and AC005261.3) were identified and used to construct a prognostic model. AC093278.2 and EPB41L4A-DT were protective factors, whereas DLGAP1-AS2, AC084876.1, and AC005261.3 were risk factors. KM analysis on GRLncs-based risk score stratification revealed patients in the high-risk group had significantly poorer OS than those in the low-risk group. ROC analysis and Cox regression demonstrated that the GRLnc-based risk score served as an independent predictor of KIRC prognosis and exhibited favorable predictive performance compared with conventional clinical variables. High- and low-risk groups also exhibited distinct immune microenvironment characteristics, immune checkpoint expression patterns, and predicted drug sensitivities. RT-qPCR detected significant downregulation of protective factor-EPB41L4A-DT in KIRC tissues, while risk factors-DLGAP1-AS2 and AC084876.1 showed expression trends consistent with their predicted risk attributes. Functional experiments further revealed that knockdown of DLGAP1-AS2 and AC084876.1 suppressed proliferation and migration of KIRC cells, whereas knockdown of EPB41L4A-DT promoted these processes, supporting the biological relevance of these three signature lncRNAs. CONCLUSIONS: This study establishes a novel prognostic model based on five GRLncs that showed promising performance in The Cancer Genome Atlas (TCGA)-based analyses of KIRC. The combined clinical expression analysis and functional validation of three constituent GRLncs (DLGAP1-AS2, EPB41L4A-DT, and AC084876.1) supports the biological plausibility of the model and suggest that GRLncs may serve as potential prognostic biomarkers and therapeutic targets for KIRC.
