The aqueous humor of rabbit contains high concentrations of pyrroline-5-carboxylate.

Accumulating evidence suggests that the interconversion of proline and pyrroline-5-carboxylate (P5C) transfers oxidizing or reducing potential within or between cells. We report here that rabbit aqueous humor contains P5C at a concentration at least 20 times that found in rabbit plasma or in biologic fluids from other animals, and, in isolated lens, P5C stimulates the activity of the pentose phosphate shunt. However, aqueous humor in other species contains P5C in the same range of concentrations as their respective plasma. The high P5C concentration in rabbit aqueous humor may be due to comparatively low levels of P5C reductase activity in ocular tissues, especially the cornea. The levels in lens epithelium, though lower in the rabbit, nevertheless could mediate the transfer of oxidizing potential from P5C and stimulate the pentose-phosphate shunt. These results suggest that the eye may serve as a convenient model for the study of proline and P5C-related transfer of reducing or oxidizing potential between tissues.

Cathepsin B and collagenolytic cathepsin in the aqueous humor of patients with Behçet's disease.

Cathepsin B and collagenolytic cathepsin activities were studied biochemically in human aqueous humor. The enzyme activities were found to be significantly high in the aqueous humor of the patients with Behçet's disease, but not detectable in those with senile cataract, retinoblastoma, and maxillar carcinoma. These data suggest that cathepsin B and collagenolytic cathepsin have a certain role in the inflammatory collagenolytic process of Behçet's disease.

Determination of mitomycin C in human aqueous humor and serum by high-performance liquid chromatography.

Mitomycin C (MMC) is used in the treatment of disseminated adenocarcinoma of the stomach and pancreas and is used in ophthalmology as adjunctive therapy in trabeculectomy. Since only small volumes of aqueous humor are available for analysis, a sensitive method requiring limited sample preparation was developed. An internal standard, 4-aminoacetophenone, was added to aqueous humor specimens, and the solution was directly injected into the high-performance liquid chromatographic (HPLC) column. The use of a short 50-mm C18 reversed-phase column gave adequate resolution of peaks with improved sensitivity. The method was applicable for determination of MMC in serum, although solid-phase extraction for sample clean-up was required prior to injection into the HPLC column, and analytical columns of 150-250 mm were necessary for adequate resolution of peaks. The method has been validated and is linear from 6.25 to 50 ng/ml in aqueous humor and from 10 to 500 ng/ml in serum.

A fluorophotometric study of the effect of argon laser trabeculoplasty on aqueous humor dynamics.

Fluorophotometry and tonography were used before and after argon laser trabeculoplasty to measure aqueous humor dynamics in 15 eyes of 14 patients. A new fluorophotometric method used to measure true outflow facility showed that it increased from a mean pretreatment value of 0.016 microliter/min/mm Hg to a mean posttreatment value of 0.075 microliter/min/mm Hg. Similarly, total outflow facility, measured tonographically, increased from 0.112 microliter/min/mm Hg to 0.151 microliter/min/mm Hg. Uveoscleral flow decreased after laser therapy; however, this was secondary to the decrease in intraocular pressure. No effect was found on the rate of aqueous humor production. We concluded that the primary effect of argon laser trabeculoplasty was to increase true outflow facility, thereby decreasing intraocular pressure.

[Studies of the penetrating ability of fosfomycin into the aqueous humor and vitreous body of the eye].

Fosfomycin is an antibiotic with a new simple chemical structure and a broad spectrum of antibacterial activity, especially against staphylococci. In the study reported here, the penetration of fosfomycin into the aqueous humor and vitreous body was investigated after intravenous infusion of 4 and 8 g. The highest average concentration approximately 2 hours after administration was 28.3 micrograms/ml after the 4 g dose and 52 micrograms/ml after the 8 g dose. Both the 4 g and 8 g doses thus produced aqueous humor levels exceeding the minimum inhibitory concentrations required for most gram-positive and gram-negative organisms commonly responsible for bacterial endophthalmitis, including strains of pseudomonas aeruginosa. Extremely high aqueous humor levels of fosfomycin were observed in two patients with inflamed eyes because of a change in the blood-aqueous barrier.

Studies on lamoxactam penetration into the aqueous humor of the human eye.

Eighteen patients scheduled for surgical lens removal were assigned to six groups of three patients each. These patients received 25 mg/kg lamoxactam i. v. 30 to 180 min prior to surgery. In the course of the operation, aqueous humor was obtained and a serum sample taken to determine the antibiotic concentration. The mean peak serum level was 154.5 mg/l after 30 min; it was still 46.4 mg/l after 3 h. The lamoxactam concentrations in the aqueous humor increased slowly. Peak levels after 2 h were 4.2 mg/l. The ratio of aqueous humor level: serum level after 1 h was 4.7%, after 1.5 h 5.3% and after 2 h 6.9%. The ratio then remained constant up to the third hour. Aside from some gram-positive bacterial species and Pseudomonas species, all relevant microorganisms which cause eye infections are included in lamoxactam's spectrum of susceptibility.

Cytomegalovirus (CMV) retinitis activity is accurately reflected by the presence and level of CMV DNA in aqueous humor and vitreous.

To evaluate the potential of ocular and systemic specimens to provide markers of active cytomegalovirus (CMV) retinitis, we examined the relationship between virologic and clinical aspects of CMV infections in AIDS patients with CMV retinitis. CMV polymerase chain reaction (PCR) analysis of 74 aqueous humor and vitreous specimens indicated that ocular specimens can provide accurate markers to differentiate active and inactive CMV retinitis (aqueous or vitreous PCR, P<.001). Moreover, these markers were superior to extraocular measures, including plasma PCR (P=.08) and blood and urine CMV cultures (P=.05). A direct correlation was identified between the quantity of CMV DNA in aqueous humor or vitreous specimens and the corresponding surface area of active CMV retinitis (r2=.69 and.44, respectively). Thus, qualitative and quantitative PCR-based analyses of aqueous humor can provide valuable markers of CMV retinitis activity. Such assays could provide rapid and reliable tools to assist in management of patients with CMV retinitis in whom the view of the retina is obscured.

Ophthalmic beta-blockers: determination of plasma and aqueous humor levels by a radioreceptor assay following multiple doses.

We determined the binding affinities of multiple doses of four ophthalmic beta-blockers, timolol, betaxolol, levobunolol and carteolol, to the beta-1 and beta-2 receptors. With a Ki value of 0.39 nM and 0.36 nM for the beta-1 and beta-2 receptors, respectively, levobunolol shows the highest binding affinity to both beta receptors. The Ki values of timolol (1.97 nM for the beta-1 receptor and 2.0 nM for the beta-2 receptor) and of carteolol (0.83 nM and 0.85 nM for the beta-1 and beta-2 receptors, respectively) are characteristic of a nonspecific beta-blocker. On the contrary, betaxolol is a beta-1 specific antagonist (Ki of 23.33 nM) and has a very low binding affinity to the beta-2 receptor (Ki of 200.00 nM). With a radioreceptor assay, levels of beta-antagonist were measured in the plasma and aqueous humor 1 hour and 12 hours after instillation of 50 microliters of 0.5% or 2% each of the four beta-blockers into the rabbit eye. At 1 hour after administration, the plasma levels of timolol, levobunolol, and carteolol are 9.89 ng/ml, 1.60 ng/ml and 8.00 ng/ml, respectively; such levels of 11.82 to 29.22 times the respective Ki values cause a virtually total blockade of both beta-1 and beta-2 receptors and suggest significant systemic absorption. Betaxolol has a peak 1 hour plasma level of 22.28 ng/ml, which is equivalent to only 3.08 times its Ki for the beta-1 receptor and 0.36 times its Ki for the beta-2 receptor; it has less systemic beta-1 blocking activity than the other three drugs and very minimal systemic beta-2 blocking activity. Peak aqueous humor concentrations of all 4 beta-blockers are extremely elevated at 1 hour after administration (timolol 1613.58 ng/ml; betaxolol 866.06 ng/ml; levobunolol 750.89 ng/ml; and carteolol 859.18 ng/ml). Such levels of 14 to 7192 times the respective Ki values should cause a virtually complete blockade of both beta-1 and beta-2 receptors in the iris-ciliary body complex. At 12 hours after administration, plasma levels of all four beta blockers remain moderately elevated (timolol 0.94 ng/ml; betaxolol 9.43 ng/ml; levobunolol 0.66 ng/ml; and carteolol 1.61 ng/ml). Trough aqueous humor levels of levobunolol (43.38 ng/ml) and carteolol (92.81 ng/ml) remain elevated at least 300 times their Ki value. On the contrary, 12-hour trough aqueous humor levels of timolol (33.67 ng/ml) and betaxolol (94.86 ng/ml) have decreased to 2 to 40 times their Ki value, which may explain their requirement for twice daily administration clinically.

Carcinoembryonic antigen in aqueous humor of patients with primary malignant choroidal melanoma.

Carcinoembryonic antigen levels were simultaneously determined in the aqueous humor and in the plasma of the 3 patients with primary choroidal malignant melanoma undergoing enucleation. An elevated carcinoembryonic antigen level was detected in the aqueous humor of 1 patient with no increase in plasma CEA level. In the other 2 patients no such elevation was found. A local increase of CEA levels produced by the tumor is likely to occur. Further studies are required of CEA levels of patients with primary malignant choroidal melanoma as an aid in diagnosis and treatment of this condition.

Effect of MK-507 on aqueous humor dynamics in normal human eyes.

The effects of the topical carbonic anhydrase inhibitor, MK-507, on aqueous humor dynamics were studied in 6 normal volunteers. As a baseline study, intraocular pressure (IOP) was determined and aqueous protein concentration was checked hourly by laser flare-cell photometry from 0900 to 1800 hours. One week later, IOP and aqueous protein concentration were determined in the same subjects on the same time schedule with instillation of 1% MK-507 at 1100 hours. After one more week, the same experiment was carried out with oral administration of acetazolamide at a dose of 250 mg at 1100 hours. Topical MK-507 showed the maximum IOP reduction at 3 hours and increased aqueous protein concentration reaching its maximum at 4 hours. Oral acetazolamide showed the maximum IOP reduction at 2 hours and the maximum increase in aqueous protein concentration was at 3 hours. Although the peak effect on IOP or aqueous protein concentration did not significantly differ between topical MK-507 and oral acetazolamide, only oral acetazolamide showed a significant effect at 1 hour. These results suggested that topical MK-507 reduced the aqueous humor production, but it took more time for the ocular effect to become manifest than after administration of oral acetazolamide.

[Quantitative determination of aqueous humor proteins by electroimmodiffusion in an antiserum containing agarosegel (author's transl)].

An exact quantitation of aqueous humor proteins within a few hours can be attained by an modification of the quantitative estimation of the proteins by electrophoresis in agarose gel established by Laurell. The aqueous humor proteins form long immunoprecipitations in agargel. The length of the precipitats is proportional to the concentration. By determination of albumin, coeruloplasmin and transferrin the application can be pointed out.

Use of polymerase chain reaction assays of aqueous humor in the differential diagnosis of retinitis in patients infected with human immunodeficiency virus.

We performed polymerase chain reaction (PCR) for detection of cytomegalovirus (CMV), varicella-zoster virus (VZV), herpes simplex virus (HSV), and Toxoplasma gondii DNA in aqueous humor from 15 patients who were infected with human immunodeficiency virus (HIV) and who had retinitis of unclear origin; these patients were selected from among 820 patients evaluated by ophthalmoscopic examination. On the basis of the final response to treatment, CMV, VZV, and T. gondii retinitis was diagnosed in 5, 2, and 4 of the 15 patients, respectively. No final etiologic diagnosis was reached for four patients. All 5 patients with CMV retinitis were CMV DNA-positive. 1 of 2 patients with VZV retinopathy were VZV DNA-positive, and 3 of 4 patients with T. gondii retinitis were T. gondii DNA-positive. All PCR assays of aqueous humor from the four patients without infectious retinitis were negative. PCR assay of aqueous humor is helpful in the etiologic diagnosis of retinitis of unclear origin in HIV-infected patients.

Effects of topical PGF2 alpha on aqueous humor dynamics in cynomolgus monkeys.

Single topical applications of prostaglandin F2 alpha (PGF2 alpha) tromethamine salt to living cynomolgus monkey eyes reduced intraocular pressure (IOP). Twice daily topical application was far more effective, so that after the 7th 50 micrograms or 100 micrograms dose on day 4, IOP fell 40-50%, to 8-10 mm Hg. Following twice daily application of 50 or 100 micrograms for greater than 3 days: (1) no increase in total outflow facility could be demonstrated by 2-level constant pressure perfusion or Schiotz tonography; (2) no decrease in aqueous humor formation rate could be demonstrated by fluorophotometry--rather, aqueous flow may have increased; (3) anterior chamber aqueous humor protein concentration was unaltered, but entry of intravenously injected fluorescein into the cornea and anterior chamber tended to increase; (4) there was a weak but sometimes statistically significant miosis of up to approximately 0.5 mm. We conclude that in the cynomolgus monkey: (1) PGF2 alpha is a potent ocular hypotensive agent with only very weak miotic and blood-aqueous barrier-disrupting effects; (2) the ocular hypotensive action of PGF2 alpha is definitely not due to increased conventional outflow facility or decreased aqueous production, but probably to increased uveoscleral drainage of aqueous humor.

Effects of alpha-chymotrypsin on the outflow routes for aqueous humor.

The anterior chamber was perfused with alpha-chymotrypsin, 50 micrograms ml-1, in mock aqueous humor in cynomolgus monkeys. The enzyme caused a marked rise in outflow facility and had clear effects on the structure of the outflow routes for aqueous humor. The intertrabecular spaces in the iridocorneal chamber angle were wider than normal, the openings in the uveal and corneoscleral meshwork appeared enlarged, there was ballooning of the juxtacanalicular region and the inner wall of Schlemm's canal into the canal and also splitting of the inner wall of the canal. The trabecular cells appeared relaxed with most of the cell processes retracted, some broken. There were discontinuities in the cell membrane and blebs. Parts of the trabecular beams were denuded. Two days after the enzyme treatment the facility was in the normal range, no splits in the inner wall of Schlemm's canal we observed but many of the other changes in structure remained. The number of invaginations in the inner wall of Schlemm's canal was markedly reduced after 2-18 days. A second and third perfusion with alpha-chymotrypsin caused a rise in outflow facility similar to the first. Some morphological changes remained 163 days after enzyme treatment. Pilocarpine a few days after alpha-chymotrypsin treatment caused a marked increase in outflow facility. The results suggest that alpha-chymotrypsin affects the glycoproteins of the cell membrane and as a result causes disorganization of the cytoskeleton, loss of loose adhesions and breaks of cell processes. The rapid recovery of the resistance to outflow was probably due to healing of the splits in the inner wall of Schlemm's canal; a marked reduction in the number of cells with invaginations suggests that in addition there may have been a reduction in the number of transcellular pores in the inner wall of Schlemm's canal.

Ciprofloxacin and ofloxacin aqueous humor concentrations after topical administration in dogs undergoing cataract surgery.

OBJECTIVE: To determine if preoperative topical administration of ciprofloxacin or ofloxacin results in aqueous humor drug concentrations that exceed the MIC(90) of common ocular contaminants in dogs undergoing cataract surgery. PROCEDURES: Twelve dogs were treated with topical 0.3% ciprofloxacin and 13 dogs with topical 0.3% ofloxacin once the night before surgery, and then with 1 drop of ciprofloxacin or ofloxacin every 15 min for 2 h immediately before surgery. Aqueous humor samples were taken from each eye immediately before the incision was made and frozen at -70 degrees C. First eye samples (S1) were taken closer to the time of topical treatments than second eye samples (S2). Samples were analyzed by high performance liquid chromatography (HPLC) at the North Carolina State University (NCSU) Clinical Pharmacology laboratory. RESULTS: In ciprofloxacin patients, S1 concentrations were 0.03-0.69 (0.17 median) microg/mL, and S2 concentrations were 0.09-0.95 (0.36 median) microg/mL. Aqueous humor concentrations did not exceed the MIC90 of Streptococcus sp. Few eyes (1/12 OU) exceeded the MIC90 for Staphylococcus sp. or Corynebacterium sp.; moderate numbers (5/12 S1; 8/12 S2) exceeded the MIC90 for E. coli. In ofloxacin patients, S1 concentrations were 0.48-2.81 (1.05 median) microg/mL, and S2 concentrations were 0.45-3.63 (1.30 median) microg/mL. Although few eyes (<or=2/13 OU) exceeded the MIC90 for Streptococcus sp. or Corynebacterium sp., moderate numbers (7/13 S1, 9/13 S2) exceeded the MIC90 for Staphylococcus sp. and most eyes (12/13 OU) exceeded the MIC90 for Bacillus sp. All eyes exceeded the MIC90 for E. coli. CONCLUSIONS: Ofloxacin has higher corneal penetration and ability to exceed MIC90 of common ocular contaminants than ciprofloxacin, and would be a more appropriate prophylactic choice for canine cataract patients.

Reduced fibrinolytic activity in aqueous humor of chronic simple glaucoma.

Fibrinolytic activity of aqueous humor was tested by the Astrup fibrin plate lysis technique in 7 patients who had chronic simple glaucoma and was compared with a control group of 7 patients with senile cataract. The plasminogen activator content of aqueous humour in glaucoma patients was remarkably reduced; it was not demonstrable in as many as six out of seven patients. The lysis zones in the control group ranged from 110 to 900 mm2 with an average of 423 mm2. The want of aqueous fibrinolytic activity may have a pathogenetic implication for glaucoma since deposition of fibrin in the angle of the eye to depressed fibrinolytic activity could increase resistance to aqueous flow.

[Analysis of aqueous humor in intraocular toxoplasmosis].

BACKGROUND: Despite many clinical examinations and laboratory tests, the cause of uveitis is very often not discovered. The examination of local intraocular antibody production is a valuable additional diagnostic means. PATIENTS AND METHODS: Aqueous humor was obtained from 46 eyes with severe uveitis: retinochorioiditis (n = 27), intermediate uveitis (n = 11), iridocyclitis (n = 5), and panuveitis (3). With the enzyme-linked immunosorbent assay (ELISA) the antibodies against Toxoplasma gondii were measured in aqueous humor and serum and the intraocular antibody production was calculated by the formation of an antibody index (AI). As the AI values are corrected for possible polyspecific immune response, the AI index is more sensitive than the Goldmann-Witmer index. RESULTS: An intraocular antibody synthesis against T. gondii could be confirmed in 20 eyes. The serum antibody concentration against T. gondii was in the reference range in 45 patients and only one patient had a very high increase of antibodies. In nine of 20 eyes, intraocular synthesis of antibodies against T. gondii was unexpected due to the lack of clinical evidence. CONCLUSIONS: The aqueous humor analysis is more reliable than the proof of serum antibodies, and it helps to increase the number of eyes with uveitis in which a specific therapy can be established.

Aqueous humor dynamics associated with the phorbol ester-induced decrease in intraocular pressure in the rabbit.

PURPOSE: To determine the effects of injection of the protein kinase C (PKC) activator phorbol 12-myristate 13-acetate (PMA) into the anterior chamber of the eye on intraocular pressure (IOP) and aqueous humor dynamics. METHODS: IOP was measured for 24 h after intracameral injection of PMA (3 to 50 pmol) in unanesthetized rabbits. Aqueous humor dynamics (aqueous flow, total outflow facility, and uveoscleral outflow) were determined approximately 6 h after injection of 50 pmol of PMA in animals pretreated with indomethacin. RESULTS: Intracameral injection of 50 pmol of PMA induced a biphasic effect on IOP, consisting of a transient increase apparent at 0.5 and 1 h and a sustained decrease apparent after 2 h. This effect of PMA was dose dependent. Whereas pretreatment with indomethacin attenuated the PMA-induced increase in IOP, the sustained decrease in IOP remained apparent in the pretreated rabbits. Intracameral injection of 4alpha-PMA, an inactive PMA analog, had no effect on IOP. PMA also significantly increased uveoscleral outflow, but it had no effect on aqueous flow or total outflow facility. CONCLUSION: Intracameral injection of PMA reduced IOP in the rabbits by increasing the rate of uveoscleral outflow. This IOP-lowering effect of PMA may be mediated by activation of PKC.
