Tazarotene cream (0.1%) in combination with betamethasone valerate foam (0.12%) for plaque-type psoriasis.

A combination of multiple agents is often required to achieve treatment success for plaque-type psoriasis. We report a case series of 10 patients that were treated with betamethasone valerate foam (0.12%) in the morning and topical tazarotene cream (0.1%) in the evening for a total of 12 weeks or until plaques cleared. Erythema, scale, and thickness along with an aggregate severity score were determined at weeks 4, 8, and 12. One patient was lost to follow-up. Eight of the other 9 patients experienced improvement in their disease by week 12. Two patients were clear of their psoriasis at week 4 and 4 were clear at week 8. No adverse events, including irritation were reported; the use of the corticosteroid foam may protect against potential local irritation reported with tazarotene. The combination of tazarotene cream and betamethasone valerate foam is an effective combination approach to treating localized plaque-type psoriasis.

Treatment of guttate psoriasis with a new two-compound product containing calcipotriol and betamethasone dipropionate.

We report a case of guttate psoriasis in a 28-year-old woman who had been suffering from scalp psoriasis from the age of 14. After an upper respiratory infection she presented with the typical exanthema of guttate psoriasis. Despite initial treatment with antibiotics and emollients, the rash continued to spread. Two months after the flare of psoriasis we decided to treat the patient with calcipotriol and betamethasone dipropionate ointment applied once daily, using an intermittent treatment modality. The lesions disappeared and despite two small recurrences on the legs we managed to maintain clearance after a 2-year follow-up. Although the application of calcipotriol and betamethasone dipropionate ointment was cumbersome, due to the extent of the eruption, the result was highly effective and safe without any lesional/perilesional adverse reaction during application and with normal blood examinations.

[Comparative effectiveness of 0.1% metilprednisolone aceponate and 0.05% betamethasone dipropionate among children with nonretractable prepuce].

OBJECTIVE: To compare clinical improvement between treatment with metilprednisolone aceponate vs. betamethasone dipropionate among children with nonretractable prepuce. MATERIAL AND METHODS: Between August 2001 and November 2002, we carried out a double blind and controlled clinical trial in 34 children with a diagnosis of nonretracable prepuce. Children were randomly assigned to one of the following groups and topical treatment was administered: Group A; metilprednisolone aceponate 0. I 1% and Group B; betamethasone dipropionate 0.05%. RESULTS: Improvement was noted in 88.2% of our sample studied; (n= 15) children from group A and 76.4% (n= 13) childrenfrom group B; however, we did not observe a significant difference when comparing percentages between the two groups (chi2 = 0.2; p = 0.6). CONCLUSIONS: The percentage of clinical improvement was similar between the two groups of topical steroid treatment administered.

A controlled clinical trial of a new formulation of betamethasone dipropionate cream in once-daily treatment of psoriasis.

In a multicenter, evaluator-blind, parallel group study of 244 patients with moderate to severe psoriasis, a once-daily application of a new formulation of beta-methasone dipropionate 0.05% cream, augmented formulation (AF), and a twice-daily application of fluocinonide 0.05% cream were compared, with respect to safety and efficacy. Results significantly favored betamethasone dipropionate AF over fluocinonide, as indicated by improvements in signs of erythema, induration, and scaling, as well as the physicians' and patients' global evaluations of response after 14 days of treatment. As a result of adverse experiences, treatment had to be discontinued in three patients on fluocinonide. No patient on betamethasone dipropionate AF had to discontinue treatment.

[Apropos of complications following the use of methylmethacrylate. Possible prevention by betamethasone].

The aim of this study was to determine whether the prior injection of betamethasone was capable of preventing blood pressure changes and the fall in arterial pO2 when following the fixation of total hip protheses with methyl metacrylate. It involved 54 patients in whom systolic, diastolic and mean blood pressure (BP), arterial pO2, pCO2 and heart rate were noted at different times: T1, 15 minutes after the beginning of the operation; T2 and T3, one minute and five minutes after application of the cement; T4, at the end of the operation. There was no significant difference between the various measurements at times T1, T2, and T3 as far as blood pressure and heart rate were concerned. By contrast, there was a fall in pO2 at times T2 and T2 in relation to times T1 and T4. This fall was significant (p is less than 0.01) but never theless remained moderate. The possible influence of betamethasone is discussed.

[Controlled study on the prevention of respiratory distress syndrome with betamethasone in pregnancy and ACTH and cortisol levels in the premature newborn infant].

The influence of maternal corticosteroid administration was studied on the ACTH and cortisol concentrations in neonatal blood of 24 premature infants whose mothers received betamethasone for prevention of RDS, compared with 11 untreated subjects. Cord blood was taken at birth and from venous sample in 5th day. All samples were analyzed for ACTH and Cortisol by radioimmunoassay. No statistically significant differences between these groups were noted. Additional analysis of ACTH and Cortisol levels in 9 RDS premature infants versus 26 control ones failed to demonstrate any deficiency of corticosteroids in newborn infants with RDS. The findings provide a justification for the prepartum treatment of respiratory distress syndrome with glucocorticoids because this dose of betamethasone does not expose the newborn to potentially harmful effects.

Reduced incidence of hyaline membrane disease in extremely premature infants following delay of delivery in mother with preterm labor: use of ritodrine and betamethasone.

Data from two groups of infants (24 to 28 weeks' gestational age) excluded from a controlled trial of the use of calf lung surfactant extract for the prevention of hyaline membrane disease are reported. The two groups were excluded from the trial because the mothers had received betamethasone for greater than 24 hours prior to delivery or because, on admission to the hospital, labor was too far advanced for proper informed consent to enter the trial. Attempts were made to delay delivery of threatened premature labor by the use of ritodrine in all mothers without evidence of infection, heavy vaginal bleeding, or severe preeclampsia and to induce surfactant production by maternal injection of betamethasone. A prospective scoring system and respiratory support variables were used to compare the groups. Infants born to mothers who successfully completed this regimen had a 28% incidence of hyaline membrane disease v a 68% incidence in infants in whose mothers it was unsuccessful due to inability to stop advanced labor (P = .001). Inspired oxygen, mean airway pressure, and ventilator rate were lower and the ventilator efficiency index was higher in the treated group during the first 48 hours of life. An aggressive approach to postpone premature delivery and to induce surfactant production by using tocolysis and a regimen of glucocorticoids reduces the incidence of hyaline membrane disease in very premature infants, 24 to 28 weeks' gestation.

Fusidic acid plus betamethasone in infected or potentially infected eczema.

Eighty-one patients with atopic dermatitis and contact dermatitis received a 1-week treatment with 0.1% betamethasone alone on one side and 0.1% betamethasone plus 2% fusidic acid on the other side. Clinically, the combination was marginally superior and in patient preference a significant difference in favour of the combination was recorded. Bacteriologically, the combination eliminated 67% of bacteria originally present in the skin lesions compared with 51% with steroid alone.

[The anti-inflammatory effect of a combination of dimethindene maleate and betamethasone in topical formulations].

The antiinflammatory activity of three preparations (cream, ointment, jelly) of Fenistil Plus (betamethasone + dimethindene maleate) was shown by means of reflection densitometry, a new pharmacodynamic model. The three preparations led to restitution of the Tesafilm-stripped skin within 4 h. There was no significant difference in the efficacy of the preparations. After application of 10 g of the ointment the plasma levels of betamethasone remained within the limits of identification of the analytical method. An influence of the drug on the endogenous plasma hydrocortisone (cortisol) levels could not be found.

[Comparative clinical trial of a new trihalogenated dermatocorticoid (halometasone) versus betamethasone dipropionate].

In a multicenter controlled study carried out in Austria and Switzerland by 8 dermatologists in 208 patients with acute eczematous dermatoses, 0.05% halometasone cream proved to have significantly superior clinical efficacy than 0.05% betamethasone dipropionate cream (P much less than 0.05) showing an equal tolerability. Good to very good results were reported in 93% of the patients treated with halometasone and in 84% of those treated with betamethasone dipropionate creams (Diprosone, Diproderm). General tolerability of both treatments was good. Neither skin atrophy nor any systemic adverse effect, due to the transcutaneous absorption of the corticoids, were observed in this study.

Intermittent topical treatment of psoriasis with betamethasone dipropionate 0.05 percent cream.

Seventy-three patients with psoriasis were treated in an open multicenter study with betamethasone dipropionate 0.05 percent cream (twice daily application for four days) alternated with the cream base (twice daily application for three days). The therapeutic response at the end of the treatment period of three weeks was very satisfactory: 39 patients had marked improvement or recovery, 16 patients had moderate improvement, and 14 patients had slight improvement. These results did not differ from those of a study on continuous treatment with betamethasone dipropionate cream in the treatment of psoriasis. Side effects (folliculitis) were only noted in one patient.

Cord blood prolactin and thyroid hormone levels after antenatal administration of betamethasone or ambroxol for prevention of respiratory distress syndrome (RDS).

Several studies showed that ambroxol is efficacious in enhancing fetal lung maturation, provided large doses are given. We investigated cord blood levels of PRL and thyroid hormones in 31 infants born after antenatal administration of betamethasone, in 31 infants born to mothers treated with ambroxol for prevention of RDS and in 27 infants whose mothers had not received any drug enhancing fetal lung maturity. The only significant difference (p less than 0.05) between the groups was noted in T3 mean levels that were higher in the ambroxol group than in the betamethasone group between the 33th to 36th week of gestation. Hormone levels did not appear significantly different in infants who developed RDS rather than in infants who did not.

Clinical comparison of triamcinolonehexacetonide and betamethasone in the treatment of osteoarthrosis of the knee-joint.

42 patients with osteoarthrosis with inflammation were included in the study. After randomization and with single-blind technique, they were given intra-articularly in the knee joint either 20 mg of triamcinolonehexacetonide (TH) or 6 mg of the combination betamethasone acetate and betamethasone disodiumphosphate (BM). The patients were studied up to six months after the injection. Initially, week one, two, four and at the time for a possible new injection; pain, local tenderness and joint range were evaluated. At the control in week one, two and four, a general evaluation of effect was also performed. Duration of effect was defined as patient's need of a new injection or other therapy during a six-month period. Results. In the evaluation of effect in local tenderness and pain, TH gave a significant better result already after one week. The improvement in joint range was more pronounced with TH. Also in the general evaluation significantly better results were shown with TH. In general evaluation all patients with TH were considered to improve with two weeks treatment. The duration of effect was significantly longer with TH. After three months five patients in the TH group respectively 12 patients in the BM group needed new therapy. The reported side-effects were not considered to be serious. The investigation has shown that TH gives a pronounced effect and long duration which has resulted in that TH is a preferable drug when long effect is needed in the treatment of osteoarthrosis with concomitant inflammation.

[The use of betamethasone in polychemotherapy of lymphoproliferative diseases].

In recent years very encouraging results have been obtained in the treatment of lymphoproliferative disorders with the use of combination chemotherapy regimens. We have treated with protocols including betamethasone 29 patients with Hodgkin's disease, 33 affected by non-Hodgkin's lymphomas, 29 with chronic lymphocytic leukemia, 7 with acute lymphoblastic leukemia and 9 patients affected by multiple myeloma. Results have been very satisfactory in patients with advanced stage Hodgkin's disease (67% complete remission), in high malignancy non Hodgkin's lymphomas (47% complete remission) and in acute lymphoblastic leukemia (100% of patients in complete remission). Our results suggest that the use of betamethasone as a part of combination chemotherapy regimens may be very useful in the treatment of lymphoproliferative disorders.

[Estriol serum levels following betamethasone treatment for induction of fetal lung maturity (author's transl)].

23 pregnant women with 28 to 37 weeks of gestation, threatened by premature labor, received Betamethasone 8 mg at admission and 4 mg 24 hours later to prevent newborn-RDS. Four women experienced this treatment twice. Blood was sampled for determination of estriol before each administration of Bethametasone and 24 hours after. Group I, patients with no signs of placental insufficiency, showed a drop in estriol levels at the second sample, but these recovered at the third one. There was no recovery seen at the third sample with group III, representing patients with proven placental insufficiency. Patients demonstrating pathologic CTG or delivering babies with low Apgar scores (Group II) showed no consistent course with their estriol levels. The course of estriol after administration of Betamethasone may be used as a test to determine the quality of the feto placental unit.

The comparative efficacy of betamethasone dipropionate cream and ointment formulations in the treatment of psoriasis.

A double-blind, multicentered, cooperative study performed by twelve investigators with 195 patients with chronic psoriasis was undertaken to compare the relative efficacy of betamethasone dipropionate in ointment and cream formulations. After one week of the two week study, those patients treated with the ointment preparation experienced a significantly greater decrease in the severity of psoriatic signs and symptoms and a greater frequency of complete remission. This study verifies vasoconstriction assay data and more rigorously supports the medical opinion that betamethasone dipropionate and possibly other corticoids in ointment formulation are superior to cream formulations in the early treatment of moderate to severe psoriasis.

Experimental modulation of the reactivity of pleural milky spots (Kampmeier's foci) by Freund's adjuvants, betamethasone and mycobacterial infection.

We studied the response of milky spots in the parietal pleura of the rat and mouse to intrapleural instillation of immunomodulatory agents such as complete or incomplete Freund's adjuvants and betamethasone, and also to infection by mycobacteria (M. avium). Both incomplete (mineral oil) and complete (mineral oil plus dead mycobacteria) adjuvants, as well as M. avium infection, induced a striking increase in the size and cellularity of the pleural milky spots whereas betamethasone caused a slight atrophy. The extensive inflammatory infiltrates observed after adjuvant injection differed between milky spots reactive to complete and incomplete Freund's adjuvants. Fifteen days after adjuvant administration, the pleural milky spots of rats were still enlarged and hypercellular but differences were noted in the size of milky spots of the pleura between the 2 adjuvant treatments: animals submitted to injection of complete Freund's adjuvant showed an increase in the size of milky spots from d 3 to d 15, while the size of milky spots of the incomplete Freund's adjuvant treated group showed a decrease in size from d 3 to d 15. The milky spots at d 15 were well organised: reticulin fibres permeated the whole area of the milky spot and the different cell types were evenly distributed. Histiocytes, which were previously confined to the inner layer, were now the main cell type in all areas of milky spots. A moderate number of mast cells and a few eosinophils were also seen. Complete Freund's adjuvant caused the formation of granulomas in the milky spots, a change that was not detected in animals treated with incomplete adjuvant.(ABSTRACT TRUNCATED AT 250 WORDS)
