Effect of using warfarin plasma concentrations in Bayesian forecasting of prothrombin-time response.

The predictive performance of a Bayesian computer program using prothrombin-time (PT) response data with and without warfarin plasma concentrations to forecast patients' PTs at the time of hospital discharge was evaluated. A log-linear pharmacokinetic-pharmacodynamic model was used to describe and predict warfarin dose response in patients recently started on warfarin sodium. Individual patients' pharmacodynamic variables relating warfarin concentration to clotting-factor synthesis were obtained by Bayesian nonlinear regression analysis. Pharmacokinetic values for warfarin clearance and volume of distribution were either calculated using nonlinear regression from measured plasma warfarin concentrations or estimated based on literature-derived population regression equations. Percent mean absolute prediction error (precision) and prediction error (bias) for PT predictions were compared among and between analysis methods that used only literature data to estimate PT response and methods that used zero to five PTs with or without warfarin plasma concentrations. Eleven women and eight men completed the study. Predictions after four days of warfarin therapy using PT measurements beginning after either the first or third warfarin dose were clinically useful regardless of whether warfarin concentrations were used in the predictions. Predictions using fewer than four PT measurements were imprecise and biased. The Bayesian method in this study provided good predictions of PTs immediately before hospital discharge based on warfarin dosing and PT response after either four or five days of therapy. The use of warfarin plasma concentrations in the pharmacokinetic-pharmacodynamic model used here appears unwarranted.

Improving the accuracy of migration age detail in multiple-area population forecasts.

Population projections are often required for many geographical areas, and must be prepared with maximal computer and minimal analytical effort. At the same time, realistic age detail forecasts require a flexible means of treating age-specific net migration. This report presents a migration projection technique compatible with these constraints. A simplified version of Pittenger's model is used, where future migration patterns are automatically assigned from characteristics of historical patterns. A comparative test of age pattern accuracy for 1970-1980 indicates that this technique is superior to the commonly used plus-minus adjustment to historical rates.

Psychophysiological forecasting of efficiency.

The method of mathematical forecasting of the changes in efficiency throughout an increase in emotional stress is suggested on the basis of experimental data from experiments in which paratroopers discerned visual patterns. The comparison of human errors with the experimental results obtained in animals permitted formulation of several suppositions on the role of different cerebral structures in the genesis of emotional stress and the mechanisms of its effect on perceptive activity.

[Ovine dicrocoeliasis: incidence of climatic factors and share in the setting of a forecasting method (author's transl)].

Study of annual activity cycle of ants parasited with Dicrocoelium lanceolatum. Effect of some climatic factors (temperature, rainfall) on seasonal variations of the number of clinching parasited ants. A close analysis of the data displays a double correlation: the increase of the number of clinching ants indirectly brought about by precipitations and this with some delay; a strong decrease by high temperatures in the height of summer. Setting of a calendar including the different stages of the developing cycle of Dicrocoelium lanceolatum in Limousin and proposition of a forecasting technic established from land observations.

Mutation of an aspartate residue highly conserved among G-protein-coupled receptors results in nonreciprocal disruption of alpha 2-adrenergic receptor-G-protein interactions. A negative charge at amino acid residue 79 forecasts alpha 2A-adrenergic receptor sensitivity to allosteric modulation by monovalent cations and fully effective receptor/G-protein coupling.

We have mutated the aspartate residue in the putative second transmembrane spanning domain of the alpha 2A-adrenergic receptor (alpha 2AAR) to the non-negatively charged asparagine (D79N) and glutamine (D79Q) and the negatively charged glutamate (D79E) residue in an effort to better characterize the role of this residue, highly conserved among G-protein-coupled receptors, in Na+ regulation of ligand binding and in receptor G-protein coupling. Allosteric modulation of receptor-ligand interactions by Na+ is retained by the D79E alpha 2AAR but lost upon mutation to the uncharged D79N and D79Q residues. Loss of allosteric effects of Na+ is paralleled by a complete loss of retrograde information transfer from G-proteins to alpha 2AAR in AtT20 cells, measured via the sensitivity of radiolabeled agonist binding to Gpp(NH)p. In contrast to the complete elimination of retrograde signaling via the D79N and D79Q alpha 2AAR, anterograde information transfer from receptor to G-protein is modified in a more subtle quantitative way, since agonist-stimulated GTPase activity via D79N and D79Q alpha 2AAR, although apparently attenuated compared to wild type and D79E alpha 2AAR, is no less than the GTPase activity elicited by endogenous somatostatin receptors in AtT20 cells. These data indicate that a negative charge at amino acid residue 79 forecasts sensitivity to allosteric regulation by monovalent cations and its mutation to non-negatively charged residues elicits a nonparallel modulation of receptor-->G-protein versus G-protein-->receptor communication between alpha 2AAR and pertussis toxin-sensitive GTP-binding proteins.

[Application of the mathematical model to forecast the incidence rates of seasonal infectious diseases].

The incidence rates of infectious diseases were selected and analysed according to data from the Beijing Railway Area during 1981-1991. We put forward the mathematical model to forecast the incidence rate of dysentery each quarter in 1992. The best mathematical model was selected from analysis of precision, and very useful in the prevention and treatment of seasonal infectious diseases.

[A simple procedure to forecast the course of diseases (author's transl)].

A procedure is presented which enables the physician to forecast the course a disease will take by looking through a list of items with attached numbers, i.e. weights, and simply adding the weights of those items which hold true for a specific patient. The application of this procedure to three different diseases, namely schizophrenia, facial paresis, and aphasia is presented and proves its accuracy. Furthermore, a Monte Carlo Analysis of the procedure underlying this method--multiple linear regression of dichotomous items--leads to recommendations concerning the optimum definitions of items and the structuring of data. The procedure is compared with other methods used in computer diagnosis and its main advantage, apart from its high reliability, i.e. independence of computers in its application, is stressed. Its application to other diseases is encouraged and the presentation of data for processing by this Department is invited.

Validity of Bayesian forecasting programme in therapeutic drug monitoring of vancomycin in a surgical intensive care unit: a prospective evaluation.

The objectives of this study were: (a) to assess the predictive performance of a 2-compartment Bayesian vancomycin forecasting programme in critically ill patients in the surgical intensive care unit, and (b) to show the applicability of the programme, which is based on parameters derived from Western population, in our local Asian population. Twenty critically ill patients were enrolled into the study programme. All patients received multiple-dose vancomycin for infections due to methicillin-resistant staphylococcus aureus (MRSA). The patients dosage regimen were optimised by entering a set of peak and trough vancomycin serum levels into a clinical computer program (MB; USC*PACK PC collection; University of Southern California, USA) by utilising a 2-compartment Bayesian population model. The correlation between observed and predicted serum peak and trough concentrations were evaluated for both the non-fitted and fitted models by linear regression analysis. There was a significant correlation between observed and predicted concentrations using the fitted model (r = 0.97, P < 0.05). There was no significant correlation of these concentrations in the non-fitted model (r = 0.8). This study shows that the Bayesian programme is able to accurately predict future vancomycin concentrations in our local Asian population. It is possible to optimise patients dosage regimens with the knowledge of two concentrations of vancomycin in order to achieve targeted therapeutic goals.

Specific immunotherapy in pollinosis: II. Forecasting changes in certain cytoimmunological indicators after four years of immunotherapy in pollinosis.

The determination of an appropriate period of duration for specific immunotherapy presents an important problem in the treatment of allergic diseases. The aim of this paper was to conduct a prognostic study of certain immunological indicators after 4 years of desensitization, and to determine which of the tested indicators are characteristic for an evaluation of the clinical condition. Individuals with pollinosis were tested after the first, second, third and fourth year of desensitization using the pollen allergen. The B and T-lymphocyte counts, the levels of immunoglobulins A, M, G, and total and specific E, and the volume of basal, total and specifically released histamine were all measured. Furthermore, the clinical condition of the patient was defined using a score method (0-3 points), taking into account the results of testing and the basic symptoms of the disease. Following this, a prediction of results after the fifth year of desensitization was made, using Brown's method of exponential smoothing. The majority of the data obtained from the forecasting study indicate that in the subsequent fifth year of desensitization, the indicators studied should undergo further changes in values, offering a strong argument for the necessity of extending the period of immunotherapy in pollinosis beyond three years. In the results of the regression function estimates it was determined that, among the indicators studied, those which are helpful in monitoring treatment are: T-RFC, T-FcG-BG, T-FcM-EN and B-EAC lymphocyte counts, levels of IgA, IgM and specific IgE, and the percentage of specifically released histamine.

The household-composition matrix as a notion in multiregional forecasting of population and households.

"The household-composition matrix has been used in the past for single-region projection of population and households; here the application is expanded to multiregional projection of population and households. Geographic regions of residence are considered as household categories, and a household-composition matrix is constructed to indicate average numbers of persons by age per household, for each of the regions considered." The method is illustrated using data for the city of Edmonton, Canada. Changes over time due to population aging and to observed patterns of household mobility are considered. "These patterns are listed as six possible household mobility types, which in turn, are viewed as constraints within which population growth takes place. With the assumption that external net-migration to the regions, as a whole, is known for a projected period, an iterative procedure is offered to estimate future population by age and region, as well as future number and average size of households by region."

Forecasting state and local population growth with limited data: the use of employment-migration relationships and trends in vital rates.

In this study, the rate of growth of state and local populations in the United States is related to the growth rate of national employment, and to demographic and economic features of the regions concerned, in order to make more realistic long-term population projections. Both natural increase and migration are taken into account in the model developed. "The model is estimated for two regions, the El Paso, TX, standard metropolitan statistical area (SMSA) and the State of New Mexico. Several statistical problems arise because of the limited number of available time-series observations. Fully dynamic historical simulations track the population quite closely, in one case never differing from the actual value by more than 1.17% and in the other never differing by more than 1.36%."
