[The diagnostic and therapeutic possibilities of endolaryngeal microsurgery (author's transl)].

The technique and types of anesthesia for endolaryngeal microsurgery are described, utilizing a personal series of 1283 procedures which were performed over a preceding ten-year period. The great diagnostic value of this method of examination for the early diagnosis of glottic carcinoma is shown. 54.4% of all endolaryngeal microsurgical operations performed were used for the diagnosis, therapy and control of tumors. The remaining 45.6% of cases were undertaken to eliminate benign voice=disturbing processes. The indications for surgery are discussed, with note given to the low complication rate associated with such surgery.

Prevalence of ovarian cancer found at the time of infertility microsurgery.

We found the prevalence of ovarian cancer in infertile women less than 40 years at the time of planned microsurgery for infertility to be 1 in 95. This may represent a greater prevalence of ovarian cancer than that of all women less than 40 years. Importantly, laparoscopy did not find the ovarian cancer in these women. In conclusion, laparoscopists and microsurgeons need to be aware of the possibility of finding ovarian cancer in these young, infertile women, and this knowledge should be used to counsel and manage the patient appropriately.

[Microsurgery in the radiation treatment of laryngeal cancer].

Endolaryngeal microsurgery was used for early diagnosis of laryngeal cancer and raising the efficacy of radiation therapy. Of 141 laryngeal cancer patients early stages were revealed in 115 (81.56%). Total and subtotal biopsy made it possible to reduce tumor volume and to create conditions for more effective radiation therapy. A primary curative effect was obtained in 88.17% of the patients. Endolaryngeal resection of stenosing tumors of III-IV stages was conductive to combined and palliative radiation therapy without performing tracheostomy.

[Value of propofol in microsurgery of the middle ear].

In 21 anaesthetics on patients ASA I-II undergoing middle ear microsurgery, the induction was done by 2 mg/kg propofol and 2 mcg/kg fentanyl. Anaesthesia was maintained by a continuous infusion of propofol 100 mcg/kg/min and bolus injections of fentanyl 1 mcg/kg every 30 min, and controlled ventilation with 60% Nitrous Oxide. The side effects during induction and recovery were minor. After a slight fall in blood pressure at induction, the haemodynamic values were steady during the whole surgery. Haemostasis was always good: the operating field was bloodless in 13 patients (62%), relatively bloody in 8 (38%), but never inconvenient for the surgeon. While recovery happened later than in other studies, the time from eyes opening to complete consciousness was always short, less than 10 minutes. The adjunction of fentanyl could be responsible for the long recovery time. No correlation is found between the infusion rate of propofol and recovery time. All patients but one are satisfied with their anaesthesia, especially with their smooth recovery.

[Artificial circulation in microsurgery of the coronary arteries].

Results of 69 aortocoronary bypass operations with the use of microsurgery equipment were analysed. Perfusion was performed under conditions of constant flow, which made it possible to avoid transmissive pulsation, making difficulties for creating microanastomoses. One venous double opening canule was used with no drainage of the left ventricule, which made cannulation easier and created optimal technical conditions for the surgeon. Duration of extracorporeal blood circulation and frequency of intraoperational complications (cardiac failure, myocardial infarction) depended on the operational methods: initial creation of proximal anastomoses resulted in decreased duration of the perfusion, improved protection of myocardium and lower number of complications.

Implications of pattern of tubal disease for microsurgery or in vitro fertilization in Lagos.

Tubal disease was found in 78 of 100 consecutive Nigerian patients who had laparoscopy for infertility, while 22 patients had patent tubes. Forty-two patients had bilateral tubal occlusion; 18 had unilateral occlusion, while 18 had gross pelvic adhesions. Forty-one patients had cornual block, and 10 had hydrosalpinges. Multiparas were more prone to unilateral tubal occlusion or gross pelvic adhesions compared with nulliparas. In 27 (35 percent) patients with tubal disease in vitro fertilization (IVF) would be the only suitable treatment, while in another 10 (12 percent) patients with hydrosalpinges, IVF probably offers a better prognosis than microsurgery.

The rat groin flap as an experimental model in microsurgery.

The value of the rat groin flap in microsurgical experiments is discussed. It provides the practising microsurgeon, whatever his specialty, the opportunity to work on vessels ranging from 0.4 mm to 2.0 mm in diameter as well as on small nerves. The single, long, microvascular pedicle of this flap enables numerous experimental procedures to be performed. Most important of all, long term patency of the vessels after anastomosis can be assessed by observing changes in the flap. Rat groin flaps have been used in microsurgery for evaluating techniques of end-to-end and end-to-side anastomoses, testing microvascular auto-and allografts, studying the delay phenomenon in flap surgery, evaluating and comparing the effects of various vasodilator drugs on microvessels and for assessing the effects of avulsion and tension forces on blood vessels and nerves. These aspects are described and discussed.

[Experimental microsurgery in rats (author's transl)].

Experimental microsurgical techniques using a magnifying glass and surgical microscope enable nearly all abdominal operations and organ transplantations to be performed in rats. Vascular and prosthetic segment implantations, portocaval anastomoses, heterotopic heart-, kidney-, spleen-, pancreas-, and liver transplantations some with superselective organangiography, are excellently demonstrated. Continuing research in microsurgery has considerably expanded experimental surgery and given new impulse to clinical surgery.

Microsurgery--looking ahead.

The evolving areas of microsurgery pertinent to hand surgery are briefly described and outlined. The spectacular developments in the recent past are merely glimpses of what may yet be ahead for hand surgery. This is illustrated by a science fiction story of the management of an amputation injury in outer space in which many of the currently evolving technologies are employed.

[Effect of the etiology of sterility on the results of tubal microsurgery].

The authors have carried out a prospective study from the 1 January 1977 to the 31 December 1981 on 168 women, 89.3% of whom (150/168) had pure distal tubal microsurgery and 10.7% (18/168) had mixed surgery. The results were studied in relationship to the aetiological reason for tubal sterility, which was: salpingitis in 60 cases = 35.7%; appendicitis in 23 cases = 13.7%, endometriosis in 29 cases = 17.3%, termination of pregnancy in 22 cases = 13.1% and unknown causes in 34 cases = 20.2%. The figures for intra-uterine pregnancies were 32.74% after 24 months using a classical method of determining it and 31% using an actuarial method. There was a significant difference between endometriosis and the four other aetiologies. These latter could not be differentiated between one another. The level of ectopic pregnancies was 5.3% at 24 months, or 6.3% using an actuarial method. It was higher after endometriosis. It would therefore appear from the results of this study that endometriosis seems to be poor from a prognostic point of view when compared with the other aetiological causes and the results shown in the literature. Finally the authors discuss in cases of endometriosis whether it is correct to carry out a second operation after the first or to resort to the other possibility, namely in vitro fertilisation.

Microsurgery of the middle ear.

Microsurgery of the middle ear has evolved over the past three decades through the art of the technically possible to the art of the clinically advisable. Stapedectomy has been refined to achieve an acceptably high level of success. Tympanoplasty in its simpler forms (myringoplasty +/- incus transposition) is very successful in the restoration of a dry hearing ear. Reconstruction in the absence of a mobile stapes remains an unresolved problem. The safe avoidance of an open cavity in cholesteatomatous disease continues to be an elusive goal.

[Use of fibrin glue in microsurgery of the ear. An analysis of operations at the Pisa ENT Clinic].

The authors report the indications for and methods of application of fibrin glue in reconstructive microsurgery of the middle ear. On the basis of 180 interventions, they describe in detail the use of fibrin glue in myringo- and tympanoplasty for correct fixing of grafts. Furthermore, they illustrate the particular use of fibrin glue in reconstruction of the incudostapedial joint, in covering eroded areas of the lateral semicircular canal and in one case of perilymph gusher.

[Use of microsurgery in the operative treatment of primary hydronephrosis].

Ureteropelvic junction obstruction is a congenital anomaly commonly afflicting the pediatric population, however, it is also more frequently than generally appreciated in the adult population. The method of postoperative urinary diversion - to divert by nephrostomy tube and/or splint or not to divert - was a matter of debate during the last twenty years. Through the application of microsurgery in pyeloplasty an external urinary diversion under commonly circumstances is not necessary. The Culp vertical flap pyeloplasty was modified by means of microsurgical instruments, ophthalmologic loupes (X 2, 6) and fine absorbable polyglactine sutures and used in 7 cases in 1984. The top of the flap is fixed in the distal ureteral incision by a U-shaped suture of 5-0 or 6-0 Vicryl followed by 2 continuous watertight sutures of the flap and the ureter and pyelon respectively. Only an extrarenal drainage for 4-8 days postoperatively was used. The mean duration of postoperative hospitalization was 10 days. The short-term radiographic, functional and clinical results after microsurgical pyeloplasty are excellent in 4 patients and good in 1 patient. The mean follow-up was only 4 months. Further experience, long-term follow-up and comparison with conventionally operated patients are necessary to evaluate the advantage of our microsurgical technique.

Gas exchange during microsurgery of the larynx using Foley catheter insufflation technique--preliminary results.

The technique of insufflating anesthetic gases via oro-tracheal Rusch type Foley catheter with spontaneous ventilation for microsurgery of the larynx is described. Fifteen unselected patients were used to assess the adequacy of gas exchange with this technique. Arterial blood gas analysis was carried out 5 minutes and 25 minutes after induction of anesthesia. The results at 25 minutes indicate adequate gas exchange. There was a significant increase in the PaO2 at 25 minutes but the decrease in PaCO2 at 25 minutes was insignificant. The technique also provided very satisfactory conditions for surgery with only minimal risk of gross tracheal soiling. No dysrhythmias were encountered and there was hemodynamic stability in all cases.

[Pathology of the isthmo-interstitial segment of the oviduct, tubal pregnancy and microsurgery].

With regard to the retrospective and prospective analysis of two series of patients, a first series concerning 180 patients presenting pathology of the isthmo-interstitial segment of the oviduct, and a second series concerning 149 patients operated on for extrauterine pregnancy, the authors have established the responsibility of the pathology of the proximal segment in a very large number of cases of sterility regarded as inexplicable, and in a certain number of cases of extrauterine pregnancy. Medical treatment with danazol gives remarkable results. In cases of failure of medical treatment, microsurgery involves isthmo-ostial reimplantation.
