[The effect of pulp extirpation on the function of the hard dental tissues].

The authors analyze the results of clinical and experimental studies of the status of hard dental tissues before and after pulp removal. The findings evidence that Ca and P levels reduce and O2 and oxide levels increase in the hard tissues of pulpless teeth, this probably resulting in changed color and friability of pulpless teeth. These data may be useful in dental surgery when forming carious cavities in pulpless teeth and in orthodontics to make pulp insertions with due consideration for the friability of pulpless teeth.

Calcium↗

[Use of calcium hydroxide in the treatment of pulpitis by extirpation under analgesia].

The study included 48 cases of pulpitis grade II of one-root teeth in 42 patients of either sex aged 21-35 years treated by pulp extirpation under analgesia with immediate filling of canals. In 28 teeth Biopulp was used as an intercalated substance between the periapical tissues and the material filling the canal. The control group comprised 20 teeth filled only with Endomethasone paste. The observations made immediately after filling and clinical-radiological control examination after from 6 months to 3 years showed a high usefulness of calcium hydroxide. Good results were achieved in 100% of cases while in the control group in 70% of cases.

[Introduction of a new technique of partial pulp amputation in permanent teeth in children after mechanical injuries. Preliminary communication].

The study material comprised 56 permanent teeth with traumatic pulp exposure complicated in 15 cases by partial tooth luxation in 52 children aged from 7 to 15 years. In each case routine clinical and radiological examinations were done. The time between trauma and procedure ranged from several hours to 70 days. In all teeth partial pulp amputation was done cutting off by the gentle technique a part of the coronal pulp and covering of the remaining pulp with calcium hydroxide, zinc oxide with eugenol or with Dycal, carboxyl cement and permanent filling. The analysis of the studied material showed that the greatest group comprised children aged 8-11 years (40 cases), and fractures of one-half of the crown prevailed (32 cases), point pulp exposure (32 cases), developmental stage Rc and A 1/2 of root (40 cases). The short-lasting follow-up made possible clinical and radiological control of only 39 teeth after 6 months, and 7 teeth after 12 months. After 6 months good results were obtained in 84.6% of cases.

[Evaluation of postoperative pain comparing manual and ultrasonic endodontic instrumentation in patients with cleft lip and palate].

Our purpose was to compare incidence of post-surgical pain associated to the endodontic therapy where the instrumentation on the root canal was performed by the method of Marshall & Pappin and the method of Marshall & Pappin complemented by the ultrasonic. Seventy patients with only one tooth needing endodontic treatment were treated by one of the methods and, posteriorly, evaluated. The endodontic treatment was performed at one time and from the seventy teeth, thirty have been instrumented by the manual method complemented by the ultrasonic and forty by the manual instrumentation. The patients were clinically controlled after the endodontic treatment was finished during periods of 24, 48 and 72 hours to evaluate their post-surgical condition. The results suggest that were no statistically significant differences (p less than 0.05) in the incidence of pain between the employed methods or according to the pulpar semiologic condition in any of the observed periods. However, we have realized that there was a tendency for a smaller percentage of a postoperative pain in those cases of necropulpectamy treated by the endosonic ultrasonic synergistic system. In those cases of biopulpectomy this has been not observed.

Intraalveolar transplantation of teeth. IV. Endodontic considerations.

Twenty-six teeth with crown-root fractures have been subjected to intraalveolar transplantation. In 13 teeth the endodontic treatment was instituted before transplantation and in another 13 teeth 3-4 weeks after transplantation. Bacteriological tests showed that endodontic treatment after transplantation was preferable with statistically fewer treatments and more reliable results. The results thus points in favour of a late endodontic treatment with controlled aseptic conditions.

[Changes in the microhardness of the enamel and dentin in pulp inflammation].

Clinical and experimental studies of the dental enamel and dentin microhardness were carried out. A comparative analysis has made it possible to detect parallels in the changes of dental tissue microhardness in humans and dogs. The studies have revealed a focal pattern of demineralization, with the foci located mainly in the neck of the tooth and the pericervical portion of the root, indicating a considerable reduction of the functional resistance of pulpless teeth.

[Bleaching of devitalized teeth with ultrasonic assistance].

A new bleaching technique for pulpless teeth is demonstrated by a clinical case. The principle of the method is to clean the dentinal tubules by an 1% sodium hypochlorite solution activated by a 40 endosonofile. A mixture of sodium perborate and a 3% hydrogen peroxide is left in the pulp chamber between the treatment sessions.

[Treatment of a canal with a "C" configuration].

The prevalence of "C" shaped root canals in the second mandibular molars is relatively low, but diagnosis and treatment of these canals are sometimes difficult. Through clinical cases, the authors emphasize on the importance of preoperative X-rays and describe convenient preparation techniques. Some "C" shaped canals are difficult to interpretate because of the thickness of bone trabeculae, and a "C" shaped canal must be suspected when the roots are fuse and/or very close each other.

[Ultrasonics in endodontics. Light microscope study].

The purpose of this study was to study the efficacy of an endodontic cleansing procedure to remove the pulp and the dentin from the root canal system with a new endodontic instrument proposed by J.M. Laurichesse, the Shaper.

Evaluation Studies as Topic↗

An evaluation of clinical endosonics.

Reports of the success rate in endodontics range from 70 to 97 per cent. Success rates for ultrasonic endodontics have not been documented. In this survey, endosonically treated cases were reviewed after eight to 33 months and assessed clinically and radiographically. This revealed that endosonic treatment was successful in 93 per cent of the cases.

[Studies on the surgical approach in vital pulp excision in relation to the root canal cross section].

The authors examined in 38 teeth the condition of the pulp stump after vital extirpation of the pulp in relation to the cross-section of the root canal. They found that the wound conditions could not be evaluated in 17 teeth as there was no pulp stump in the apical region. Relatively favourable conditions were seen in 11 teeth, which was achieved only in case of circular or oval cross-section of the root canal. These findings permit the conclusion that the state of the pulp stump after vital extirpation depends upon the anatomical conditions and the accuracy of the mechanical manipulation.

Cephalometry↗

["Endo-prosthetic" preparation of curved roots].

During the tooth preparation for root posts, all must be attempted to avoid weakening of the root. This "must" is rather easy for straight roots, but the same is not true of curved roots. The authors have used the method described by BRAMANTE et al. to study the effects of various techniques of instrumental sequences of endodontic preparation, and of two instrumental sequences of root post preparation. From this studies, it results that: --the respect of the canal path during the endodontic phase conditions the respect of the internal wall of the curvature, during drilling. --among the four endodontic techniques studied: MM 3000, Step-Back, Anti Curvature Filling and Canal Finder System, the last two seem to allow more respect of the canal path. --the instrumental sequence using Largo drills to prepare a canal, presents two types of drawbacks: * Excessive abrasion of the convex (internal) wall of the canal system. It seems that the blunt point rests against the concave portion of the canal and pushes the drill toward the internal wall, therefore weakening it. * Inadequate match between the size of the drill and that of the prepared canal. --the Parapost drills, on the contrary, respect the endodontic path, and may even be used beyond the curvature (2 mm). Besides, the diameter of the prepared canal is similar to that of the instrument used. These two factors permit to obtain a better retention of the post as well as a lesser weakening of the root.

Dental Pulp Cavity↗

[De- and remineralization processes of the surface layer of the enamel in intact and pulp-extirpated teeth].

Clinico-laboratory investigation of 61 intact teeth and 61 teeth after pulp extraction was performed, including in vivo acid enamel biopsy and intravital staining with 2% methylene blue solution. The regularities were established governing Ca and P efflux from superficial enamel layer of intact and pulp-extracted teeth into the acid solution with respect to age, group of the tooth and pulp extraction terms. The pulp extraction factor was investigated with special reference to the tooth acid resistance and its remineralization rate.
