[Idiopathic periaortic fibrosis: a difficult diagnosis in internal medicine].

Peri-aortitis retroperitoneal fibrosis is characterized by a reaction of a variable inflammatory nature while constricting the organs crossing the retroperitoneal space, notably the ureters and the blood vessels. It is difficult to diagnose such a rare disease. We bring about here six cases of periaortic retroperitoneal fibrosis diagnosed from systemic, digestive, urinary or vascular signs. Early diagnosis is often difficult and is shown to be established after an average of three months investigation. The average age of these patients, all of the male sex, is 58 years old (54 to 90). In the six cases this disease appears to be idiopathic even though in two cases it is associated to giant temporal arteritis and polymyalgia rheumatica. TDM remains the best diagnostic tool to point out the existence of this fibrosis, to observe its evolution and to investigate for any extension of the disease. The etiology of this fibrosis remains a mysterious one; however an immunologic origin has been suggested. Medical treatment by corticosteroids is often successful but the long term evolution of the disease is still uncertain.

[The phenotype Hp O in several African and Central American populations].

This work shows the higher probability for some individuals of having Hp O phenotype: that is, children under twelve, and individual carriers of haemoglobin S (haemoglobin D carriers do not present this characteristic). This observation appears as a confirmation of the hypotheses explaining Hp O phenotype as a consequence of haemolytic anaemia. Besides, the test of two genetic models taking into account both Hp O and Hp21 M phenotypes leads to strong difficulties due to a certain irregularity of "haptoglobin" genetic system.

[Quantitative study of the genetics of haptoglobin levels].

A quantitative study, based on several African and Pyrenean populations led to the estimate of the effect of some factors on haptoglobin rate: it shows an influence of age and electrophoretic phenotype, but no apparent effect of sex. Moreover, this study led to the conclusion that there is heritability of haptoglobin rate.

[A new PiN mutation at the Pi locus in human populations].

By a bidimensional immuno-isofocalisation electrophoresis method a new PiN allel was revealed in 365 samples collected in a Caucasoïd population from a Central Pyrenees Valley (Les Baronnies). Its high frequency (0,151) contributes to the decrease of the PiM one. The different NS, MN and NN phenotypes were formerly classed respectively as MS and MM phenotypes. PiN allel seems to be widely distributed in Mediterranean populations.

[Pathology of injuries due to snow surfing].

The amount of injured snowboarders is increasing each year. The reason is due to the success of snowboarding for teenagers. Accidents occur more during the first hours of practice. Upper limb is the first location: fore-arm breaks and wrist fractures are very common. Elbow and shoulder dislocations also occur often for snowboarders. The location of most injuries for lower leg is the ankle; we found many ankle sprains and astragalus's breaks. The knee sprain is rare unlike alpine ski. Prevention has to be developed to give a better image of this sport, in fact the attitude of the participants is the main reason for the accidents.

Plasma thrombomodulin: new approach of endothelium damage.

Endothelium damage is associated with thrombotic risk in a variety of diseases including atherosclerosis, gram negative sepsis, viral infections and neoplastic disease. Therefore, it appears necessary to find a mean for the clinical investigation for such a damage. Among the markers of these cells, thrombomodulin which is a membrane glycoprotein, seems to be of great interest for this purpose. Actually, thrombomodulin is also found in plasma, following an endothelial lesion. Plasma levels of thrombomodulin are increased in a certain number of pathologies associated with endothelium lesion: atheromatous arterial disease, disseminated intravascular coagulation syndrome and also in systemic lupus erythematosus where the levels of plasma thrombomodulin are related to the severity of the pathology. Moreover, previous in vitro studies confirm the fact that the release of thrombomodulin from the endothelial cell membrane occurs during the course of injury by activated leukocytes or hydrogen peroxide. So, one can suppose a prospective interest in the measurement of plasma thrombomodulin as a diagnostic tool for the approach of endothelium damage.

[Arterial thrombosis of the arm and hemorrhagic rectocolitis. Report of a case].

Several cases of arterial thrombosis associated with inflammatory enterocolitis affections have been reported. In all but one case reported in the literature the outcome was favorable. A 40 year old woman developed thrombosis of the radial and ulnar arteries during an acute episode of ulcerative colitis. The course of the thrombotic lesions was irreversible and all fingers of the right hand had to be amputated. The corticotherapy as a possible predisposing factor for these thromboses is discussed, but their onset during an inflammatory episode suggests a role for the inflammatory enterocolitis. A thrombocytosis (807,000 platelets/mm3) could have predisposed to their onset, and in this case the hemostatic disturbances and fibrinolysis associated with the ulcerative colitis could have played a major role.

[Evidence of elevated soluble plasma thrombomodulin in atherosclerosis].

Thrombomodulin (TM) is a very efficient natural anti-thrombin glycoprotein expressed on the endothelial cell surface. Circulating soluble thrombomodulin is also detected by enzyme immunoassay in plasma and represents some fragments of membrane TM with various molecular weight. Plasma TM (TMp) levels are elevated in diseases associated with endothelium damage. We have explored TMp in patients with atheromatous disease and compared its level with others endothelial cell markers, particularly those who indicate cell activation, as tissue-type plasminogen activator (t-PA), inhibitor of plasminogen activator (PAI-1) and prostacyclin (PG12). Thirty seven patients with documented atheromatous artery disease were included in this study. They were not diabetics and their hepatic and renal functions were normal. Mean age was 71 +/- years. Routine serum parameters were checked out as well as others more specific for endothelium activation (TMp, PG12, PAI-1, t-PA) measured by enzyme immunoassay. Patients were classified according to three localizations of atheromatous involvement: - 15 patients with peripheral occlusive arteriopathy disease (POAD) - 6 with coronary artery disease (CAD); and 16 with polyvascular involvement (POLY). They were compared with 21 controls without any vascular lesions (mean age: 43 +/- 13 years). In controls TMp was 36 +/- 8 ng/ml without significant change according with age and sex. In patients whatever the localization of atheroma, TMp was found significantly higher: POAD = 51.3 +/- 19.7 ng/ml (p = 0.003); CAD = 49.2 +/- 15.4 ng/ml (p = 0.008); POLY = 49.6 +/- 17.2 ng/ml (p = 0.003). A positive correlation was pointed out in all patients between TMp and t-PA (p = 0.047), TMp and PG12 (p = 0.008). A positive correlation between TMp and t-PA (p = 0.034) was found only in the subgroup with POAD. In this study, there was no correlation between TMp and the following parameters: leucocytes, haemoglobin, cholesterol, HDL, LDL-cholesterol, Lp(a), fibrinogen.(ABSTRACT TRUNCATED AT 250 WORDS)

[Study of the Gc protein in several French population samples : genetic polymorphism by isoelectrofocusing and quantitative results].

The authors describe the results obtained in an examination by isoelectrofocusing of more than 1,000 serum samples from various regions in France. The autochthonous populations of the north side of the Pyrénées are characterized by a high frequency of the gene Gc2 and a very low frequency of the Gc1F gene. This distinguishes them from the other groups studied (southwestern, northwestern and eastern France). The quantitative analysis of the data emphasizes the difference. These results can only be explained by the interdependance of the genetic and metabolic factors.
