[Effect of ethinyl estradiol sulfonate/norethisterone acetate on PRI secretion in pubertal girls].

In 13 healthy tall girls the influence of the hormonal treatment with depotestrogen ethinylestradiolsulfonate and norethisterone acetate was investigated on the basal and TRH stimulated PRL secretion. The investigations were performed before treatment, during the 8th to 11th cycles of treatment, and in the 4th month after finishing the therapy. PRL levels were determined by RIA. Under treatment significant higher basal PRL levels were found than before (p less than 0.05), but never hyperprolactinemic values. The responsibility of the lactotropic cells to TRH stimulation was increased, too.

[Effect of ethinyl estradiol sulfonate/norethisterone acetate on HGH secretion in pubertal girls].

In 13 healthy tall girls the influence of the hormonal treatment with depotestrogen ethinylestradiolsulfonate and norethisterone acetate was investigated on basal and arginine stimulated HGH secretion. The investigations were performed before, during and after finishing the therapy. HGH serum levels were determined by RIA. During the hormonal therapy the mean basal HGH levels were significant higher (p less than 0.05) than before or after therapy. Independent from hormonal therapy there was no stimulation of the HGH secretion by arginine in about 2/3 of the girls.

[Clinical value of ethinyl estradiol sulfonate (Turisteron)--long-term results of a randomized compound study].

In a randomized compound study the therapy effect of ethinylestradiol sulfonate (Turisteron) is tested. As comparative groups serve patients with chemotherapy and Turisteron in combination with chemotherapy. The cumulative 5-year survival rate under the therapy of Turisteron was 61.2%. The comparative groups do not achieve this high survival rate. In connection with the slight stress of the patients (application form, side effects) ethinylestradiol sulfonate (Turisteron) is an effective estrogen in the long-term treatment of the advanced carcinoma of the prostate gland.

[Testing synergism between ethinyl estradiol and norethisterone on the histamine-induced deciduoma and vaginal mucification of castrated rats].

Through the histaminic deciduoma test and a vaginal mucification test, we have endeavoured to show that, in spayed female rats there exists a possibility of synergism between ethynylestradiol and pure norethisterone per os. No synergism could be shown in the former test, owing to, or so it seems, a lack of deciduogenic power proper to norethisterone. The latter test showed, indeed, that ethynylestradiol can, in low quantities, bring about a synergism on the mucifying properties of norethisterone, whereas, in large quantities, it creates an antagonism on the aforesaid properties.

[Ethinyl estradiol and hepatocarcinogenesis. An experimental study].

The influence of ethinylostradiol on hepatocarcinogenesis was investigated in the female Dawley-rat. After 14 weekly gastric instillations of 50 mg diethylnitrosamine/kg and simultaneous administration of diet supplemented with ethinylostradiol 10 mg/l kg diet, 77.8% of the rats developed a hepatocellular carcinoma. In contrast, only 31.8% had carcinoma of the liver under similar experimental conditions. The relation of preneoplastic lesions correlated with that of carcinoma induction in both groups. If the results were to be valued on clinical relevance without difficulties a new orientation on surgical indication and transformation of the dignity of benign focal nodular hyperplasia would have to be considered.

[Effect of ethinyl estradiol sulfonate/norethisterone acetate on gonadotropin secretion in pubertal girls].

In 13 healthy tall girls the influence of the hormonal treatment with depotestrogen ethinylestradiolsulfonate and norethisterone acetate on basal and GnRH stimulated gonadotropin secretion was investigated. The investigation were performed before treatment, during the 8th to 11th cycles of treatment, and in the 4th month after finishing the therapy. LH and FSH serum levels were determined by RIA. Under treatment a suppression both of basal gonadotropin serum levels and of the stimulated LH secretion results. After treatment the mean basal gonadotropin levels are higher than before or during the therapy. The LH release after GnRH stimulation shows the same trend.

[Behavior of LH, FSH, total testosterone, free testosterone and SHBG serum levels in the therapy of prostatic cancer with Turisteron (ethinyl estradiol sulfonate)].

45 patients with prostatic cancer were treated conservatively with Turisteron at a dosage of 2 mg per week. After 2 months the serum levels of total testosterone were reduced to castration values and thereafter furthermore to 6% of the pretreatment values. The free testosterone (FT) serum level did show a decrease to 2%, while the binding capacity of the sexual hormone binding globulin (SHBG) was increased from 4,1% to 97,9%. The serum levels of the two gonadotropic hormones luteinizing hormone (LH) and follicle-stimulating hormone (FSH) were decreased only to nearly 20% of the pretreatment value. The investigations confirm the definite "antiandrogenic" effect of Turisteron, which is 10-times higher than the antigonadotropic effect.

[Effect of ethinyl estradiol sulfonate on the urethral epithelium after bilateral oophorectomy].

The influence of ethinylestradiolsulfonate on the cytological image of the urethra was examined on 11 women who underwent the salpingo-oophorectomia bilateralis. The criterion of the proliferative effect was the Dynamic-Estrogenicity-Index (DEI). After a 6 to 8 weeks treatment at a dosage of one milligram weekly a DEI of 7,27 (+/- 0,74) was attainable. From this point of view we can also summarize a very good urethrotrophic effectiveness. Ethinylestradiolsulfonate is recommended for the treatment of affection on the urethra, especially of those in which an estrogen deficit is the genuine reason or a favoured factor by women after hysterectomia and salpingo-oophorectomia bilateralis for substitution with hormones.

[Endocrinologic and pharmacologic-toxicologic findings with ethinyl-estradiol sulfonate. 8th communication. Complement and lysozyme in the serum of beagles after 6 months application of ethinylestradiol sulfonate].

In course of studies for the chronic toxicity of estrogens after oral application two parameters of the unspecific resistance - complement (CH50U and CB50U) and lysozyme - in sera of 30 beagle-dogs were studied. The doses of the daily (over 6 months) applicated hormones were 1,0 mg/kg ethinylöstradiol and 1,0, 0,1 and 0,01 mg/kg ethinylöstradiolsulfonate (depotform). In the majority of all measurements no significant variations of serum complement and -lysozyme could be detected, however after application of high doses of the hormone ethinylöstradiol a little reduction of complement and significant elevations of lysozyme were observed.

[Effect of ethinyl estradiol sulfonate/norethisterone acetate on TSH, T4, TBC and the FT4 index in pubertal girls].

In 13 healthy tall girls the influence of the hormonal treatment with depotestrogen ethinylestradiolsulfonate and norethisterone acetate on basal and TRH stimulated TSH secretion, total thyroxine (T4), thyroxine binding capacity (TBC) and free thyroxine index (FT4 index) was investigated. The investigations were performed before treatment, during the 8th to 11th cycles of treatment, and in the 4th month after finishing the therapy. TSH and T4 serum levels were determined by RIA, TBC by radio agent assay, the FT4 index was calculated. The mean basal TSH levels before, during and after therapy showed no significant differences. During hormonal therapy T4, TBC and FT4 were significant higher (p less than 0.05) than before and after therapy.

Carbohydrate metabolism studies after one year of using an oral contraceptive containing gestodene and ethinyl estradiol.

UNLABELLED: The objective of the study is to evaluate the effects of a gestodene-containing oral contraceptive on carbohydrate metabolism. The design of the study is prospective. The setting is at University of South Florida Outpatient Unit. The patients consisted of twenty-three normal women desiring contraception. Serum glucose and insulin levels were measured during a three-hour glucose tolerance test at control time and after one year of drug use. RESULTS: All of the one-year glucose values were significantly elevated as well as the fasting and three-hour insulin values. These changes were mostly confined to women over 26 years of age and not in the younger 18 to 23 year olds. An oral contraceptive containing 75 micrograms of gestodene and 30 micrograms of ethinylestradiol can significantly alter carbohydrate metabolism in older women.

[Paradoxic improvement of glucose intolerance in 8 females using estrogen-progestagen contraceptives containing norgestrel and ethinyl-estradiol].

Sixty females, aged 20 to 35, received an oral contraceptive pill (Norgestrel 0.5 mg and Ethinyloestradiol 50 micrograms/day) during one year. Group I was formed by 41 normal patients before and after 12 months. Group II was formed by 11 patients, normal before and with impaired oral glucose tolerance after 12 months. Group III was formed by 8 patients with impaired glucose tolerance before treatment and normal oral glucose tolerance after 12 months. All patients were submitted to 3 oral glucose tolerance tests, the first one at the 16th day of the control cycle, the second and the third at the 16th days of the 6th and 12th cycles during treatment. No significative modification of glycemia, insulinemia or weight was noted in group I. In group II the impaired oral glucose tolerance was associated with an increase of weight. In group III the improvement of glucose tolerance was marked by hyperinsulinemia.

[Effect of levonorgestrel and ethinyl estradiol and their combination on biotransformation reactions].

Caffeine is mainly metabolized by 3-methylcholanthreneinducible cytochrome P-450, whereas metamizol (Analgin) is probably mainly metabolized by the phenobarbital inducible cytochrome P-450 family. Therefore the elimination of caffeine from serum and the elimination of the main metabolites of metamizol in urine reflect the activity of these two cytochrome P-450 families. Sex hormones can influence the activity of cytochrome P-450. Intake of levonorgestrel (0.125 mg) daily for 14 days reduced the metabolism of caffeine slightly, but the elimination of metamizol-metabolites is not influenced. Longterm administration of levonorgestrel (0.250 mg daily) for three months did not change the metabolisms of both model substances tested. In contrast, ethynylestradiol (0.050 mg) alone and also the combination with levonorgestrel markedly retarded the elimination rate of caffeine and metamizol-metabolites. This fact should be taken into consideration in drug therapy.

Serum folate and Vitamin B12 levels in women using modern oral contraceptives (OC) containing 20 microg ethinyl estradiol.

OBJECTIVE: The effects of modern oral contraceptives (OC) on serum concentrations of folate and cobalamin are controversial. STUDY DESIGN: Case-control study on the cobalamin and folate status of 71 healthy female nulligravidae using "low dose" OC for >or=3 months and 170 controls. Factors interfering with vitamin metabolism were thoroughly controlled. Serum concentrations were measured by commercial assays. The results were evaluated using Mann-Whitney's U-test and chi(2) analysis. RESULTS: OC-users showed significantly lower concentrations of cobalamin than controls. The rates of women with reduced, normal, and elevated levels differed significantly. Nine users but no control had frank cobalamin deficiency without clinical symptoms. Folate levels did not differ between the groups. Vegetarian diet, smoking or obesity did not have a significant influence. CONCLUSIONS: Routine measurement of cobalamin or folate in women using "low dose" OC is not warranted. Vitamin supplementation or different contraceptive methods should be considered in women with pre-existing cobalamin deficiency or restrictive dietary habits.

Blood coagulation with a combination pill containing gestodene and ethinyl estradiol.

The coagulation effects of oral contraceptives are determined by the interaction of the estrogen and the progestogen components. Serial studies on the coagulation system were carried out in 120 randomly allocated healthy women to compare the effects of (1) triphasic gestodene/ethinyl estradiol and triphasic levonorgestrel/ethinyl estradiol and (2) monophasic gestodene/ethinyl estradiol and monophasic desogestrel/ethinyl estradiol. The triphasic study showed significant increases in factor VII, factor X, fibrinogen, plasminogen, and fibrinolytic activity. No significant differences were noted between levonorgestrel and gestodene except for factor VII, which was higher with gestodene. Antithrombin III and anti-Xa were unchanged, and platelet aggregation was slightly accelerated with gestodene. The monophasic study is still in progress: Significant increases of factor VII, factor X, plasminogen, and fibrinolytic activity and no changes in antithrombin III, anti-Xa, and platelet aggregation have been found with both monophasic gestodene and desogestrel. The results of the triphasic study indicate that the effect of gestodene on the blood coagulation system is similar to that of levonorgestrel and that both formulations increase fibrinolytic activity. The increased coagulation activity would appear to be counteracted by enhanced fibrinolysis, so protecting the dynamic balance between coagulation and fibrinolysis. Further data are required in the monophasic gestodene and desogestrel study before any conclusions can be drawn.

[Effect of hormonal contraceptives containing 0.05 mg ethinyl estradiol and 0.125 mg desogestrel in normophasic regimen (Oviol)].

This evaluation is based on data obtained from a multicentre trial in which to date 953 women have used the tested oral contraceptive for a total of 15,737 cycles (average 16.5 cycles, maximum 40). The contraceptive is a combination of an estrogen (ethinylestradiol) and a progestogen (desogestrel). It is designed to be used in 28-day cycles: 22 days of tablet-taking followed by 6 tablet-free days during which a withdrawal bleeding may normally be expected to occur. On days 1-7 the dose in 50 micrograms ethinylestradiol, and on days 8-22 it is 50 micrograms ethinylestradiol + 125 micrograms desogestrel. This trial shows the described oral contraceptive to be highly effective. No pregnancies occurred when the tablets were taken as directed: one pregnancy occurred in a cycle in which two tablets were missed and in which there was also an acute bout of gastroenteritis. No serious side effects occurred, and the incidence of minor complaints which arose was lower than during a non-treated control cycle. Cycle regularity was exceptionally good. Withdrawal bleedings occurred in 97.6% of the treated cycles. Spottings occurred in 3%, and breakthrough bleedings in 2.1%. These facts combine to suggest that the tested oral contraceptive is highly effective, safe, and very acceptable.

Promoting effects of ethinyl estradiol on development of renal proliferative lesions induced by N-nitrosobis (2-oxopropyl)amine in female Syrian golden hamsters.

The modulating effects of female sex hormones, ethinylestradiol and levonorgestrel, on the development of renal proliferative lesions after initiation with N-nitrosobis(2-oxopropyl)amine (BOP) were investigated. Three groups of female Syrian golden hamsters, each comprising 30 animals, were given four weekly s.c. injections of BOP (10 mg/kg body weight) and then fed diet containing 1 ppm ethinylestradiol (group 1: BOP/EE), diet containing 10 ppm levonorgestrel (group 2) or basal diet (group 3) for the next 27 weeks. As hormone controls, two groups of 20 female hamsters each received diet containing 1 ppm ethynylestradiol (group 4) and 10 ppm levonorgestrel (group 5) from week 3 for 27 weeks without the prior initiation treatment. The severity of diffuse anisokarya, characterized by varied nuclear size and the incidence of dysplasias of the proximal tubular epithelia induced by BOP, were significantly increased in the BOP/EE group, indicating the promoting effects of the hormone. In the renal dysplastic lesions (small-cluster, cystic, clear-cell and acidophilic cell types), adenomas and nephroblastomas, increases in the numbers of argyrophilic proteins associated with nucleolar organizer regions (NOR) in the nucleus, suggesting a high proliferative activity, were seen in dysplasia of acidophilic cell types and adenomas. In addition, the number of bizarre NOR per nucleus was significantly higher in adenomas than in dysplasias and highest in nephroblastomas. This morphological change in NOR should therefore be a useful parameter for the diagnosis of malignancy of renal tumors.

[Estrogenic effects after 14-days administration of 0.06 mg ethinyl estradiol in postmenopausal women].

The effect of a 26 day oestrogen-gestogene sequence therapy on double endometrium thickness, uterus size, blood flow in the uterine blood vessels (pulsatility and resistance index), serum concentration of FSH, LH and SHBG and on vaginal cytology of postmenopausal women was tested double-blind in a placebo-controlled trial. Thirteen postmenopausal women received 0.06 mg ethinyloestradiol (EE2) over 14 days and then a 12 day combination treatment with 0.04 mg EE2 and 0.125 mg levonorgestrel. Eight women received placebo treatment over 26 days. In each group, half the women were less, and half were more than 10 years postmenopausal. The above parameters were unchanged in the placebo group. However, in the verum group, the double endometrium thickness increased from 2 mm to about 6 mm after a 7 day treatment with EE2 and after a further treatment week with EE2, increased another millimeter. There was no obvious difference in the treatment groups between women less than ten years after menopause or more than ten years after menopause. Blood flow in the uterine artery increased significantly (decrease in pulsatility and resistance index by about 50%). Again, there did not seem to be any obvious connection with the menopausal interval. FSH decreased after the first treatment week by about 50% and 65% after the second treatment week. There was no significant decrease in LH. SHBG increased by about a factor of 5. An oestrogen effect could be demonstrated in the vaginal cytology of all cases in the verum group.
