An interdisciplinary approach to the management of complex medical and dental conditions.

Dental and medical practice often requires an interdisciplinary approach integrating the knowledge, skills and experience of all the disciplines of dentistry, medicine and its associated fields into comprehensive treatment to maximize results. Rapid and comprehensive scientific and technological advances have made it difficult for dental and medical practitioners to keep up to date in their fields; thus, to decrease practitioner frustration and increase patient benefits, an interdisciplinary approach has become essential. This report illustrates how therapy was coordinated for a young medically compromised patient with dental phobia and significant dental problems. The latest advances in dental techniques and materials, medical technology and pharmacology are highlighted. The communication and cooperation of team members with each other and with the patient and guardians is illustrated. The astute observation by a medical practitioner in an unrelated discipline led to the improvement in the quality of life for a patient.

A restorative-orthodontic treatment approach in the older patient.

This report presents a case requiring a combination of restorative dentistry and orthodontic treatment in a mature adult patient. Occlusal splint and periodontal therapies were used initially. Orthodontic treatment combined the use of the occlusal splint and fixed appliance in the maxillary arch. Sectional fixed appliances were used in the mandibular arch. The final restorations were fixed--movable bridges in the mandibular arch and a removable tooth and mucosally borne prosthesis in the maxilla. Retention of the orthodontic result was provided by the fixed prostheses in the lower arch and the continued wear of a full coverage maxillary occlusal splint at night served to prevent relapse of the upper teeth.

Sequencing and timing of treatment in the correction of dentofacial deformities in adult patients with clefts.

During the past six years, 107 patients with various dentofacial deformities secondary to primary clefts were treated in the cleft palate clinic. An overall treatment plan was formulated before any definitive secondary treatment was started. The sequencing and timing of the various aspects of secondary treatment were established to provide optimum esthetic, functional, and psychologic results.

Planning the treatment of the complex prosthodontic case.

An approach to planning the treatment of a complete maxillary denture opposing an imperfect mandibular dentition has been described. Maxillary supporting tissues under complete dentures that oppose natural dentitions are often overstressed during function. Generally, as the damage to dentition increases, so do the forces causing thrusting and rotation of the denture. Every effort should be made to develop an occlusal table at the optimal level and in harmony with the patient's condylar guidance and a minimal incisal guidance in the attempt to enhance the stability and retention of the denture and the preservation of the dentition and supporting tissues that remain.

Communication in patient management as seen in a patient with multiple sclerosis.

A case report has been presented showing the possible psychological impact of a debilitating disease, such as multiple sclerosis, on a patient. Medical and dental evaluations had determined that it was necessary to delay making dentures until oral conditions improved. As this did not satisfy the patient's immediate objectives of treatment, he chose not to follow the proposed treatment plan. Instead he searched to find someone who would give him what he wanted. Dentists must evaluate the patient's objectives in relation to the physical criteria for treatment and should not compromise standards of care to accommodate the patient's demands.

Indications for orthodontic-prosthodontic collaboration in dental treatment.

In dental practice, combined plans of orthodontic and prosthodontic treatment are underused. Indications for a collaborative effort include preservation of integrity of the dental arch, biomechanical factors, limitations in design of prostheses, esthetic appearance, periodontal considerations, prevention of malocclusion, and better retention of position of teeth.

Importance of combined periodontal and acid-etch composite treatment in restoration of anterior teeth and periodontal health.

Severely fractured, hypoplastic, and carious mandibular anterior teeth in a 17-year-old patient precluded the ability to use adequate methods of control of plaque. With simple, inexpensive techniques, periodontal health was restored and a functional and esthetic occlusion was provided. This treatment plan was accomplished with minimal risk of injuring the pulps of the teeth.

The mutilated dentition: an alternative approach.

To provide esthetic yet inexpensive restoration of severely carious anterior teeth, stainless steel posts with composite resin cores for crowns were used as interim substitutes for more costly crowns.

Phased-in comprehensive care: a concept and case report.

A complicated case often requires a multidisciplinary approach. But such an approach makes it difficult for the practitioner to offer the patient a definitive treatment plan. This article outlines the concept of a phased-in comprehensive treatment plan and reports on a case in which such a plan was used successfully.
