Activity of hyaluronidase in placental tissues from cows with and without retained fetal membranes.

Hyaluronidase, a proteoglycan-degrading enzyme, may have an influence on collagenolysis in bovine placenta and take part in the separation processes of the placenta in cows. There was, however, no evidence concerning the activity of this enzyme in those tissues. The experiment was performed on cows divided into two groups as follows: A, placenta not retained (n = 16); and B, placenta retained (n = 9). The activity of the enzyme was measured using a spectrophotometric method and zymography. The results showed statistically significant higher activity of hyaluronidase in the maternal and fetal parts of the placenta in cows with a retention of fetal membranes. Further experiments require substrate specificity studies and the presence of regulatory factors of this enzyme in bovine placental tissues.

Peripartal changes in serum alkaline phosphatase activity and lactate dehydrogenase activity in dairy cows.

Peripartal serum alkaline phosphatase activity and lactate dehydrogenase activity were measured in 30 dairy cows in order to examine the association between retained fetal membranes and enzyme activity. Daily blood samples were obtained from pregnant cows, starting 15 days before the expected day of calving until eight days after parturition. Sera from 15 cows which retained fetal membranes longer than 24 hours and 15 cows which shed fetal membranes within six hours after parturition were analyzed for alkaline phosphatase and lactate dehydrogenase enzyme activities. Mean alkaline phosphatase enzyme activities ranged from 15.93 to 32.6 U/L in retained and nonretained placenta cows. There was a trend towards higher serum alkaline phosphatase activities in retained placenta cows but the differences were not significant among the groups (P greater than 0.05). Mean lactate dehydrogenase activities ranged from 307.2 to 438.86 U/L in nonretained and retained placenta cows. Lactate dehydrogenase enzyme activities in nonretained and retained placenta cows were similar (P greater than 0.05). The alkaline phosphatase and lactate dehydrogenase enzyme activities peaked at the time of parturition in both groups. However, the differences in alkaline phosphatase and lactate dehydrogenase activities on different days within non-retained and retained placenta cows were significant (P less than 0.05). Results indicate that prepartal changes in alkaline phosphatase and lactate dehydrogenase enzyme activities are not predictive of placental retention postpartum.

Activity of 72-kDa and 92-kDa matrix metalloproteinases in placental tissues of cows with and without retained fetal membranes.

This study compared the activity of matrix metalloproteinases (MMP-2 and MMP-9) in retained and non-retained bovine placenta. The activities of MMPs and their zymogens were measured in fetal and maternal placental tissues from control cows (group B) and animals affected with retention of fetal membranes (group A) using a zymography technique on 10 per cent SDS polyacrylamide gels. The activity of proMMP-9 detected only in the maternal part of the placenta was lower in group A than in group B. ProMMP-2 activity was higher in group A than in group B in both tissues. The active forms of MMP-2 were observed in the maternal and fetal part of placenta in group B, but only the 68-kDa form was detected in the placental tissues of group A. The differences in enzyme activity between the groups and the lack of 64- and 60-kDa active forms of MMP-2 in the maternal and fetal parts of the retained placenta may have influenced the hydrolysis of collagen and the proper release of fetal membranes.

Retained products of conception masquerading as acquired arteriovenous malformation.

This case documents a seldom-described event of retained products of conception masquerading as an acquired arteriovenous malformation (AVM) of the uterus. The patient presented with sudden onset of heavy vaginal bleeding 6 weeks after artificial abortion. Ultrasonography and magnetic resonance imaging revealed a diffuse intramural lesion predominantly consisting of prominent vessels, which raised suspicion of an AVM. Hysterectomy revealed retained products of placenta that were necrotic and encroached into thin myometrium.

Most of diaplacentally transferred allergen is retained in the placenta.

BACKGROUND: Transplacental transfer of nutritive and inhalant allergens has been described being potentially responsible for a series of events leading to antigen-specific immune responses in the fetus. As such, cord blood T cell responses appear ubiquitously. However, studies failed to reveal a consistent dose-response relationship between antenatal allergen exposure and allergen-specific cellular reactivity in cord blood. OBJECTIVE: To examine the transfer process of allergens (ovalbumin (OVA), beta-lactoglobulin (BLG), birch pollen allergen Bet v1) in placental tissue (BeWo cell line, ex vivo placenta model). METHODS: The choriocarcinoma cell line BeWo was used to study the allergen uptake and transfer experiments in vitro. In the ex vivo placenta model the contribution of different placental compartments was evaluated. For this, immuno-histochemistry, immuno-electronmicroscopy and ELISA techniques were applied using monoclonal antibodies to Bet v1, OVA and -BLG. RESULTS: In vitro transfer studies on a BeWo cell-layer revealed an intracellular allergen uptake and a trans-trophoblastic allergen transfer, which was temperature- and concentration dependent, pH sensitive and asymmetric. Allergen-specific staining of placental tissue after allergen perfusion (BLG) demonstrated bulk of the allergen in the syncytio-trophoblastic cell layer and minor staining in the villous stroma and in the endothelium of fetal vessels. Immunogold staining revealed an accumulation of the perfused allergen in the trophoblastic basement membrane. CONCLUSION: In vitro/ex vivo trans-trophoblastic and trans-placental allergen transfer is shown with an accumulation of most of the allergen in placental tissues, potentially explaining the missing direct dose-response relationship between prenatal (maternal) allergen exposure and allergen-specific cellular reactivity in cord blood.

Secondary postpartum hemorrhage: treatment with selective arterial embolization.

PURPOSE: To evaluate the efficacy and safety of selective arterial embolization of the uterine arteries in the management of intractable delayed postpartum hemorrhage. MATERIALS AND METHODS: Fourteen consecutive women with secondary postpartum hemorrhage were treated with selective embolization of the uterine arteries. In all cases, hemostatic embolization was performed because of intractable hemorrhage that could not be controlled with the administration of uterotonic drugs or with uterine curettage. RESULTS: The causes of bleeding included genital tract tears in four women and endometritis in eight women; the endometritis was associated with proved, retained portions of placenta in four women. In two women, no evident cause of bleeding was found before angiography. Angiography revealed extravasation in three women. A false aneurysm of the uterine artery was found in two women. In one patient, an arteriovenous fistula was observed. Immediate resolution of external bleeding was observed in all women. No complication related to embolization was found. Normal menstruation resumed in all women. CONCLUSION: Selective arterial embolization of the uterine arteries is a safe and effective means of controlling secondary postpartum hemorrhage.

Effects of retention of fetal membranes on subsequent reproductive performance of dairy ewes.

Seventy-one pairs of ewes with or without retention of fetal membranes in the previous lambing season, were included in a cohort study of 25 flocks in southern Greece; in 27 pairs regulation of the breeding cycle was applied, whilst in the other 44 pairs natural mating took place. The following measures of reproductive performance were calculated: cycling rate, mating rate, return-to-estrus rate, abortion rate, lambing rate, total lambs born per ewe, liveborn lambs per ewe, stillbirth rate, lamb-bodyweight per ewe; furthermore, the incidence risk of retention of fetal membranes during the lambing examined in this study was also calculated. No statistically significant differences were observed in the reproductive performance among ewes that had or had not retained their fetal membranes during the previous lambing, whether the breeding cycle was regulated or not. The incidence risk of retention of fetal membranes among ewes that had not retained its placenta in the previous lambing was 1.6%, whilst that among ewes that had retained its placenta in the previous lactation was 0%. It is concluded that retention of fetal membranes did not appear to adversely affect subsequent reproductive performance of ewes.

Monoclonal antibodies specific for rat relaxin. II. Passive immunization with monoclonal antibodies throughout the second half of pregnancy disrupts birth in intact rats.

The purpose of this investigation was to use an approach targeted specifically on endogenous relaxin to determine the influence of relaxin on birth in the rat. To that end, a monoclonal antibody specific for rat relaxin, designated MCA1, was used to passively neutralize endogenous relaxin in intact pregnant rats. MCA1 was administered iv to intact rats daily from days 12-22 of pregnancy. Animals were observed for birth continuously from 2100 h on day 22 until 1200 h on day 24. MCA1-treated rats exhibited significantly prolonged durations of straining and litter delivery as well as reduced incidence of live pups compared with controls. Whereas approximately 20-25% of fetuses and placentae were retained in utero at 1200 h on day 24 in MCA1-treated rats, neither fetuses nor placentae were retained in control rats. Passive immunization with MCA1 throughout the second half of pregnancy had no apparent influence on normal ovarian function. The time of occurrence of the antepartum decline in serum progesterone levels (functional luteolysis) as well as the time interval between the attainment of basal progesterone levels and the onset of litter delivery in MCA1-treated and control rats that delivered on day 23 were in close agreement with previous reports. In conclusion, the prolonged durations of straining and litter delivery, reduced incidence of live pups, and increased incidence of retained fetuses and placentae after passive immunization with MCA1 establish the physiological need for endogenous relaxin to attain normal delivery of the young in the rat.

Spontaneous oxidative DNA damage in bovine retained and nonretained placental membranes.

Retention of fetal membranes (RFM) is believed to be associated with conditions of oxidative stress. In this study, 8-hydroxy-2'-deoxyguanosine (8-OH-dG) was used for the determination of spontaneous oxidative DNA lesions in maternal and fetal parts of bovine retained and nonretained placentas. Placental specimens were collected directly after spontaneous delivery or during cesarean section from cows divided into 6 groups: (A) cesarean section before term without RFM, (B) with RFM, (C) cesarean section at term without RFM, (D) with RFM, (E) spontaneous delivery at term without RFM and (F) with RFM. Isolated DNA was hydrolyzed and analyzed by HPLC; native nucleosides were monitored at 254 nm and 8-OH-dG by electrochemical detection. No significant differences in 8-OH-dG levels between retained and nonretained placental tissues were found in all samples from preterm groups (mean concentrations between 13 and 42 micromol/mol deoxyguanosine (dG)). In the term cesarean section group with RFM a significant increase in 8-OH-dG concentration in DNA from maternal (8-fold) and fetal (18-fold) membranes were detected when compared to the respective nonretained tissues. Also, in the term spontaneous delivery groups maternal nonretained placental tissues showed increased levels of 8-OH-dG in comparison to the respective tissues of the retained placenta group. In placental tissues oxidative DNA lesions appear to be controlled by responsive mechanisms which, possibly following exhaustion, give rise to increased 8-OH-dG levels.

The analysis of fatty acid content and phospholipase A2 activity in placenta of cows with and without retained fetal membranes.

The aim of the following study was the comparison of phospholipase A2 (PLA2) activity and fatty acids content in cows with and without retained fetal membranes. The activity of the enzyme and levels of fatty acids were measured in maternal and fetal placental tissues obtained from cows immediately after spontaneous parturition without (group A) and with (group B) retained fetal membranes. PLA2 activity was statistically significantly (P < 0.05) higher in group A than in group B, and was significantly lower in the fetal part than in the maternal part of placenta in both groups. The content of the examined fatty acids-myristic (14:0), palmitic (16:0), palmitoleic (16:1), stearic (18:0), oleic (18:1), linolenic (18:3), arachidic (20:0), arachidonic (20:4, AA)-was higher in group A than in B. Concentration of linoleic acid (18:2) showed the opposite relationship. There was a positive correlation between enzyme activity and content of det2ermined fatty acids. The results seem to confirm the bypothesis that the arachidonic acid pathway is altered in cases of retained placenta in cows.

Timing of carbetocin administration in vaginal deliveries: a double-blind, randomized controlled trial.

OBJECTIVES: This study aimed to compare the efficacy of administering carbetocin before vs. after placental delivery in preventing postpartum hemorrhage (PPH) in low-risk vaginal deliveries. METHODS: The randomized controlled trial was conducted at Kartal City Hospital, Istanbul, Turkey. A total of 160 primiparous women with uncomplicated pregnancies who underwent vaginal delivery were enrolled. Participants were randomly assigned to receive 100 μg of carbetocin either before or after placental delivery. The primary outcome was the incidence of PPH. Secondary outcomes included the need for additional uterotonics, manual removal of the placenta with consequent antibiotic administration, blood transfusions, maternal adverse events, and changes in hemoglobin levels at baseline and 24 h postpartum. RESULTS: The incidence of PPH was significantly lower in the carbetocin-before group than in the carbetocin-after group (p=0.015). The carbetocin-before group had a significantly lower mean hemoglobin drop compared to the carbetocin-after group (p<0.001). The need for additional uterotonics was significantly higher in the carbetocin-after group (p<0.001). Manual placenta removal and the need for antibiotics were more frequent in the carbetocin-before group (p=0.017). No significant differences in adverse maternal events were observed between the groups. CONCLUSIONS: Administering carbetocin before placental delivery significantly reduces the incidence of PPH, blood loss, and the need for additional uterotonics. However, the increased rate of manual placenta removal necessitates individualized risk-benefit assessment; pre-placental administration may be most advantageous in women at elevated risk for PPH, in whom the hemorrhagic benefit outweighs the risks associated with manual extraction.

Association of parturient hypocalcemia with eight periparturient disorders in Holstein cows.

The association between parturient hypocalcemia and 8 periparturient disorders (dystocia, retained fetal placenta, metritis, left displaced abomasum, foot problems, ketosis, mastitis, and coliform mastitis) was evaluated in 33 Holstein dairy herds (2,190 cows) that calved from March 1981 through February 1982. There were highly significant (P less than 0.0001) associations between parturient hypocalcemia and dystocia, retained fetal placenta, ketosis, and mastitis. The odds ratios (multiplicative increases in occurrence) for these associations were 6.5, 3.2, 8.9, and 8.1, respectively. There was a significantly (P less than 0.03) association between parturient paresis and coliform mastitis (odds ratio, 9.0). There was a tendency for association with left displaced abomasum (odds ratio, 3.4; P less than 0.06).

Caesarean section and other methods for assisted delivery: comparison of effects on mare mortality and complications.

Data from 116 mares that had caesarean section or vaginal delivery at 2 university hospitals were analysed in 5 groups, as follows: dystocia corrected by caesarean section, Group DCS (n = 48); elective caesarean section, Group ECS (n = 10); caesarean section concurrently with colic surgery, Group CCS (n = 8); assisted vaginal delivery, Group AVD (n = 22); and controlled vaginal delivery under general anaesthesia, Group CVD (n = 28). Survival rate in all mares that had caesarean section, excluding Group CCS, was 88% (51/58). All mares in Group ECS survived and Group CCS had the lowest survival rate (38%). In 98 mares with dystocia, Groups DCS (15%) and AVD (14%) had significantly lower (P<0.05) mortality rates than Group CVD (29%). There were no differences between groups for duration of dystocia. The placenta was retained in 75 (65%) of 116 mares, and for a longer period following elective caesarean section than following assisted vaginal delivery. Multiple complications (> or = 3) were recorded in 6 mares in Group CVD but not in the other groups. Of the 102 foals delivered from 98 mares with dystocia, 11 (11%) were alive at delivery and 5 (5%) survived to discharge. Survival rate for foals was 38% in Group CCS, and 90% in Group ECS. Under conditions similar to those in this study, it is calculated that caesarean section is preferable to CVD if dystocia is protracted and great difficulty and trauma is involved, even if CVD allows delivery of the foal.
