Periodontal regeneration with enamel matrix derivative--case reports.

Recent studies have shown that enamel matrix proteins may induce regeneration in periodontal intrabony defects. This paper presents preliminary results (6 to 12 months observation period) of the clinical application of enamel matrix protein (Emdogain) as an adjunct to periodontal surgical therapy of intrabony defects. The clinical results support reports that enamel matrix derivative may induce regeneration in intrabony defects, irrespective of the type of periodontal disease and/or type of intrabony defect involved.

Microbiologic and radiographic analysis of ligature-induced peri-implantitis with different dental implant surfaces.

PURPOSE: The goal of this study was to evaluate microbiota and radiographic peri-implant bone loss associated with ligature-induced peri-implantitis. MATERIALS AND METHODS: Thirty-six dental implants with 4 different surfaces (9 commercially pure titanium, 9 titanium plasma-sprayed, 9 hydroxyapatite, and 9 acid-etched) were placed in the edentulous mandibles of 6 dogs. After 3 months with optimal plaque control, abutment connection was performed. On days 0, 20, 40, and 60 after placement of cotton ligatures, both microbiologic samples and periapical radiographs were obtained. The presence of Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis, Prevotella intermedia/nigrescens, Campylobacter spp, Capnocytophaga spp, Fusobacterium spp, beta-hemolytic Streptococcus, and Candida spp were evaluated culturally. RESULTS: P intermedia/nigrescens was detected in 13.89% of implants at baseline and 100% of implants at other periods. P gingivalis was not detected at baseline, but after 20 and 40 days it was detected in 33.34% of implants and at 60 days it was detected in 29.03% of dental implants. Fusobacterium spp was detected in all periods. Streptococci were detected in 16.67% of implants at baseline and in 83.34%, 72.22%, and 77.42% of implants at 20, 40, and 60 days, respectively. Campylobacter spp and Candida spp were detected in low proportions. The total viable count analysis showed no significant differences among surfaces (P = .831), although a significant difference was observed after ligature placement (P < .0014). However, there was no significant qualitative difference, in spite of the difference among the periods. The peri-implant bone loss was not significantly different between all the dental implant surfaces (P = .908). DISCUSSION AND CONCLUSIONS: These data suggest that with ligature-induced peri-implantitis, both time and periodontal pathogens affect all surfaces equally after 60 days.

Effectiveness of preventive treatment on approximal caries progression in posterior primary and permanent teeth: a review.

AIM: A literature review to assess the effectiveness of preventive treatment on approximal caries progression in posterior teeth is presented. METHODS: Retrospective and prospective studies conducted on primary and permanent teeth published in the literature were identified and reviewed. RESULTS: The assessment of the reviewed studies showed that they were inconclusive regarding the primary teeth. The studies reviewed showed that different preventive measures, especially fluoride varnish, fluoride solution and fluoridated toothpaste, had a significant effect on the retardation of the progression of approximal lesions in permanent teeth. However, the survival time of approximal caries at the non-restorative stage after application of a preventive treatment, the frequency of application of a preventive measure, the most effective preventive measure, the suitable interval of taking radiographs to evaluate the progression of a lesion and the health and economic benefits cannot be determined from the reviewed studies. CONCLUSION: It was concluded that further studies are needed to evaluate the effectiveness of various preventive measures on approximal caries progression.

Restoration of partially edentulous patients using dental implants with a microtextured surface: a prospective comparison of delayed and immediate full occlusal loading.

PURPOSE: The aim of this study was to determine the clinical effectiveness of placing dental implants with microtextured surfaces into full occlusal loading at the time of placement in partially edentulous patients. MATERIALS AND METHODS: Two demographically similar groups of 14 patients each were treated with a total of 92 Spline Twist Implants (Centerpulse Dental, Carlsbad, CA). Test implants were placed into immediate full occlusal loading, and control implants were restored using a conventional delayed loading procedure. Otherwise, both groups of patients received similar therapy from the same treatment team. Radiographs, periodontal indices, and Periotest values were recorded every 6 months during routine clinical follow-up appointments. The mean loading time for all prostheses was 24 months at the time of this report. RESULTS: No implants failed in the test group, and 1 implant failed before loading in the control group. Cumulative implant success was 98.9% for all implants placed (test group = 100%; control group = 92.9%). Periodontal measurements indicated no significant clinical differences between implants placed into immediate full occlusal loading and those loaded via a conventional delayed protocol. DISCUSSION: Immediate full occlusal loading of partial prostheses supported by microtextured implants in partially edentulous patients demonstrated excellent clinical results, with no adverse periodontal effects after 24 months of function. Additional follow-up will provide invaluable information on the long-term effects of this technique. CONCLUSION: Immediate full occlusal loading of partial prostheses supported by microtextured implants can be successfully achieved for 24 months in highly motivated patients with excellent oral hygiene.

Preliminary 3-dimensional surface texture measurement and early loading results with a microtextured implant surface.

PURPOSE: This investigation was conducted to obtain preliminary roughness data on a microtextured implant surface and to determine its ability to sustain a 1-stage surgical procedure and early full occlusal loading of single-tooth restorations in humans. MATERIALS AND METHODS: Three-dimensional (3D) vertical scanning interferometry was conducted on samples of the test surface (MTX) and 2 control surfaces (Osseotite and sandblasted/acid-etched [SLA]). Test implants were also placed in vivo, restored with fully occluding single-tooth restorations (n = 27) after 2 months of nonsubmerged healing, and clinically monitored for 48 months of follow-up. RESULTS: Microtexture was relatively uniform on the test surface and more random and irregular on the control surfaces. MTX and Osseotite were similar in some roughness parameters, but the MTX surface had a greater number of micropits that were spaced closer together (Stylus Y lambda q) and with higher slope values (Stylus Y delta q). Cumulative life table results were 100% for all MTX implants placed in maxillary and mandibular jaw locations, and no discernible marginal bone changes were observed. Overall implant success was 100% after 4 years of clinical functioning. DISCUSSION: The findings of this study appear promising but should be considered preliminary, because of the limitations in the number of locations measured on each product sample and the small number of implants clinically studied. CONCLUSION: Within the scope of the present study, MTX implants exhibited a uniform micropitted surface, as well as 100% survival and 100% clinical success after nonsubmerged placement, early loading with single-tooth restorations at 2 months, and 48 months of clinical functioning. Dose response efficacy of sodium fluoride dentifrice at 9 and 21 months with supervised brushing.

PURPOSE: To assess whether a fluoride dose response could be detected with a relatively small sample size in a short duration using a novel randomized, double-blind study design. METHODS: Subjects (N = 644, with approximately 215 per group) with a mean age of 10.4 years old (9-12 years old) used a placebo dentifrice, an 1100 ppm F dentifrice or a 2800 ppm F dentifrice for the first 9 months of the study. Subjects in the placebo group were then switched to either 1100 ppm or 2800 ppm F dentifrice for the remainder of the study, while subjects in the fluoride groups continued with their original treatment assignments. Three calibrated examiners measured visual-tactile caries as DMFS that was supplemented with a radiographic examination at baseline, 9 months and 21 months for each subject. RESULTS: The results of this study are consistent with the previous results reported for sodium fluoride dentifrices. For all examiners, the 1100 ppm and 2800 ppm fluoride dentifrices delivered statistically significantly (P < 0.05) lower DMFS scores than the placebo control dentifrice at 9 months, while at 21 months the 1100 ppm and 2800 ppm fluoride dentifrices delivered statistically significantly lower DMFS scores compared to the both the placebo/1100 ppm and the placebo/2800 ppm dentifrice groups. In addition, one of the three examiners observed a directional (P = 0.11) dose response (2800 ppm F < 1100 ppm F) at 9 months, while at 21 months all three examiners observed evidence of a dose response, with one examiner observing a statistically significant difference between 1100 ppm and 2800 ppm F. Caries scores on occlusal surfaces provided the strongest evidence of an 1100 ppm F vs. 2800 ppm F difference.

[Caries diagnosis: the lesions are central].

In this paper two new visual scoring systems are described, aimed at estimating both depth and activity of occlusal lesions. Their validity is acceptable for lesion depth estimation. However, the validity of the activity estimate is to be questioned. Both for approximal enamel and dentine lesions as for deep dentinal occlusal lesions, bite-wing radiographs are still useful for evaluation of new lesions and lesion progression. Many quantitative caries diagnostic methods are being developed, but to this date none of them has been shown to be reliably applicable in lesion monitoring. The evaluation of caries risk factors is primarily useful for selecting preventive treatment options. Other caries prediction-tests than those based on the present caries lesions are of little value. The main disadvantage of such prediction models is that they are usually limited to application in children, where caries is a disease that affects all ages.

Caries experience in teenagers related to changes in a dental health programme.

Comparisons were made of the number of approximal DF-surfaces radiographically recorded at the ages of 13 and 16 years between two groups of children, born in 1964 and 1971, respectively. The aim of the study was to examine if the differences in caries prevalence could be related to the effects of the changes in the dental health programme with time in Göteborg, Sweden. The caries prevalence at 16 years of age was significantly lower in the 1971 group. This was mainly due to a decrease in the number of fillings and caries lesions in girls. However, no statistically significant differences were found between the groups with regard to the mesial surfaces of the first permanent molars. In comparisons between the 20 per cent of children of both sexes with most caries, the 1971 group presented significantly lower DF-values at 16 years of age. The children in the 1971 group had a lower age at the start of organized dental care, had had more topical fluoride applications and, especially the girls, had probably used fluoride dentifrices more frequently. These factors, taken singly as well as in combination, are presumed to have contributed to the lower caries prevalence in the 1971 group.

Clinical evaluation of a compomer and an amalgam primary teeth class II restorations: a 2-year comparative study.

PURPOSE: This study was performed to compare the clinical performance between the compomer F2000 and amalgam Dispersalloy in Class II restorations in primary molars over a 2-year period. METHODS: Seventy-five amalgam and 75 compomer restorations were placed in 75 children based on a split-mouth design. The restorations were evaluated after 1 week and after 6, 12, 18, and 24 months of oral function. The evaluation consisted of a clinical assessment according to modified Ryge criteria, a radiographic examination using bite-wing radiographs, and an observation of epoxy casts under scanning electron microscopy. RESULTS: The results showed statistically significant differences in the marginal adaptation and anatomic form between amalgam and compomer restorations. A higher number of compomer restorations were rated as Bravo, while a higher number of amalgam restorations were rated as Alpha at 24 months. Significant differences in the failure of the restoration and development of secondary caries were not found between the materials. CONCLUSIONS: The use of compomer F2000 in Class II resorations in primary molars, although it presents a significantly higher number of restorations rated as Bravo regarding the marginal adaptation and anatomic form vs the amalgam, does not increase the risks of developing secondary caries and failure of the restoration over a period of 2 years.

Changes in caries experience among 6-year-olds in Blekinge, Sweden between 1994 and 2000.

For 6-year-old children in the County of Blekinge, the goal was that > 65 percent should have dmft = 0 in the year 2000. To formulate this goal a study on prevalence and distribution of caries in 6-year-old children was performed in 1994. The aim of the present study was to evaluate whether the dental health goal for the year 2000 was achieved and compare clinical data regarding oral status in children in 1994 and in 2000. The material comprised a random sample of 225 children. The studyfrom year 1994, which included 213 children, was repeated. A clinical investigation was performed and completed with bite-wing radiographs on individual indication, whenever the proximal surfaces could not be inspected clinically due to close and broad contact. In addition, the parents were asked questions about toothbrushing and use of fluoride tablets. 53 percent of the children showed healthy teeth with no initial caries lesions or restorations compared to 35 percent in 1994. The pattern of dental health was still very skewed among the children. The children with nine or more decayed surfaces increased from 2 percent in 1994 to almost 4 percent in 2000. The number of fissure sealings performed had increased between 1994 and 2000 by almost 50 percent in both primary molars and first permanent molars. However, the use of fluoride tablets had decreased from 20 percent in 1994 to 7 percent of the children in 2000. The data showed a decline in prevalence of dmf teeth and an increase in the percent of caries-free children but the difference was not significant on the 5% level. However, as 47% of the children were in need of restorative treatment, our dental health goal was not achieved. We also recommend that our preventive program with focus on primary prevention and individual risk assessment should also be targeted at the whole population of pre-school children and their parents.
