Patterns of mechanical obstruction of the small bowel in Nigeria.

One hundred sixty-one patients with mechanical small bowel obstruction were treated within 2 years at the University of Nigeria Teaching Hospital, Enugu, Nigeria. The various causes of obstruction fairly typify the patterns of mechanical small bowel obstruction in Nigeria, except for slight differences in some parts of the country. The fairly high mortality rate reflects the difficulties encountered in management of the patients. Measures to reduce mortality are suggested.

Review of colorectal cancer in patients under age 40 years.

Brooke Army Medical Center Tumor Registry records from 1947 through 1980 were reviewed. One hundred forty of 819 patients with colorectal cancer were aged 40 or less. The 5 year survival rate was 35 percent and the 10 year survival rate 32 percent. The predominant presenting symptoms were bleeding, pain and change of bowel habits. The median duration of symptoms was 3 months. There were no Dukes'A patients. The 5 and 10 year survival rates in 19 Dukes' B patients were 67 percent. Thirty-three Dukes' C patients had 5 and 10 year survival rates of 37 and 30 percent, respectively. In 42 Dukes' D patients, however, there were no 5 year survivors, and the mean length of survival was only 10 months. Our findings support previous surveys which have concluded that stage at the time of diagnosis, rather than symptoms, duration or patient age, is the most accurate prognostic factor.

Method for intraoperative assessment of organ perfusion and viability using a miniature oxygen sensor.

A specially designed miniaturized Clark polarographic electrode was used to measure organ surface oxygen tension during surgical operations in 10 patients as a means of determining tissue perfusion and viability. When applied to organ surfaces, the sensor noninvasively provides real-time assessment of tissue PO2. Values obtained are dependent on both arterial PO2 and local blood flow and therefore may be used to quantitatively assess local oxygen delivery (perfusion). Measured values may be compared with arterial PO2 or to areas of certain normal perfusion on the same organ to better define the adequacy of local oxygen transport. Tissue temperature is independently measured and may be used to assess the metabolic activity of the tissue monitored. Organ surface oximetry using the miniature PO2 sensor is a practical and repeatable method for the intraoperative assessment of organ perfusion and viability.

Postoperative adjuvant chemotherapy and radiotherapy for cancer of the large bowel and rectum.

Forty patients with invasive regional-stage adenocarcinoma of the large bowel and rectum received adjuvant postoperative chemotherapy combined with doses of radiation below the maximal tissue tolerance level. This treatment was reserved for patients with stage B2, C1, and C2 lesions, with only two exceptions. The treatment was well tolerated. It appeared to result in a longer disease-free interval when compared with population-based results for patients with sigmoid cancer who had surgery alone. Our results paralleled those of the Gastrointestinal Tumor Study Group (GITSG) for combined adjuvant therapy of rectal cancer, who also indicated an advantage for long-term survival. Patients who received additional extended chemotherapy had at least the same percentage of favorable outcomes. Tumors above the peritoneal reflection also appeared to share the same improved results. We believe a multicenter randomized study should be performed to evaluate this group of patients.

Is routine appendectomy beneficial in the management of ovarian cancer?

A retrospective analysis of 136 patients with ovarian carcinoma subjected to appendectomy as a part of surgical procedure was carried out to assess the importance of appendectomy in patients with ovarian cancer. Of the 136 patients studied, 94 had epithelial and 38 had non-epithelial type of primary ovarian carcinoma. In the remaining 4 patients, the primary operation was performed with an intraoperative diagnosis of ovarian carcinoma but the final pathological examination revealed appendiceal carcinoma metastatic to ovaries. The overall appendiceal involvement in the study group with primary ovarian cancer was found to be 32.5% (43/132). This figure was 15.7% and 39.3% for non-epithelial and epithelial tumors, respectively. Involvement of the appendix ranged from 8.8% for patients with stage I disease to 46.0% for patients with stage III-IV disease. Four of the appendices found to be inflamed during the surgical explorations were later reported as acute appendicitis. The frequent occurrence of metastatic disease in the appendix in patients with ovarian cancer suggests routine appendectomy is appropriate for staging early cases and for contributing to maximal cytoreduction in advanced cases.

Unusual appendiceal pathology presenting as urologic disease.

We report on 3 cases of unusual appendiceal pathology presenting as urologic disease: 2 cases were benign mucoceles and 1 a malignant mucocele or cystadenocarcinoma of the appendix. Two cases presented as pelvic masses causing urinary frequency and the third with fever and hydronephrosis. The appendix must remain in the differential diagnosis for both acute and chronic disease processes.

Recurrent lymphoma resulting in Kock pouch incontinence.

Incontinence following the Kock pouch continent cutaneous urinary diversion currently occurs in < 20 percent of patients undergoing the operation. Various causes of cutaneous incontinence have been identified, including efferent nipple prolapse, incompetent valve, pinhole fistula, parastomal herniation, and Marlex mesh erosion into the efferent nipple mechanism. We report the first case of a patient with recurrent lymphoma involving the small bowel utilized for the efferent continence nipple valve mechanism resulting in incontinence.

Diagnosis of large bowel tumours by colonic sonography.

To investigate whether the retrograde instillation of fluid into the colon would improve the diagnostic value of ultrasonography in assessing neoplastic diseases of the large intestine 220 patients were examined by colonic sonography before colonoscopy. Polyps larger than 7 mm diameter could be identified in 91% of cases, whereas polyps smaller than 7 mm could not always be visualised. The sensitivity of the technique in the detection of colonic carcinomas was 94% and the specificity was 100%. The results suggest that colonic sonography is an effective diagnostic procedure that may find application as a screening procedure for neoplastic diseases of the colon.

Direct chromosome analysis of seven primary colorectal carcinomas.

Chromosome studies were performed on direct preparations of seven cases of primary colorectal carcinomas. Two cases had relatively simple chromosome changes: 48,XY,+8,+21/51, XY,+8,+9,+10,+i(17q),+21, and 47,der(X)t(X;14)(q11;q11)-Y,t(6;18)(p22;q24)+7,+8,der(19)t (19;?)(q13;?). The five others had complicated deletions and translocations; 1p- was noted in five cases, and i(17q) was noted in three cases.
