[Treatment of relapsing-remitting multiple sclerosis with recombinant interferon-beta preparations].

Three recombinant interferon-beta preparations, Betaferon (IFN beta-1b) and Avonex and Rebif (INF beta-1a) have been approved for the therapy of relapsing-remitting multiple sclerosis (MS). The results from pharmacokinetic and pharmacodynamic studies with different interferon-beta preparations reflect only in part the mechanisms of action of this substance, because standard assays primarily measure antiviral bioactivity. These studies are often overinterpreted in relation to the therapy of MS. In this article, we critically evaluate the results of pharmacologic studies with respect to the dose-dependent efficacy of different interferon-beta preparations.

How to analyze an article.

In clinical research investigators generalize from study samples to populations, and in evidence-based medicine practitioners apply population-level evidence to individual patients. The validity of these processes is assessed through critical appraisal of published articles. Critical appraisal is therefore a core component of evidence-based medicine (EBM). The purpose of critical appraisal is not one of criticizing for criticism's sake. Instead, it is an exercise in assigning a value to an article. A checklist approach to article appraisal is outlined, and common pitfalls of analysis are highlighted. Relevant questions are posed for each section of an article (introduction, methods, results, discussion). The approach is applicable to most clinical surgical research articles, even those of a nonrandomized nature. Issues specific to evidence-based surgical practice, in contrast to evidence-based medicine, are introduced.

Development of clinical practice guidelines: surgical perspective.

Clinical practice guidelines (CPGs) are systematically developed statements to assist practitioner and patient decisions about appropriate health care for specific clinical circumstances. The implications of this definition are that: the methodologic perspective and operations for systematic development must be specified, a strategy is needed to account for the patient's perspective in the CPG development process and the clinical decision, and a mechanism is required to determine how appropriateness ought to be conceptualized and defined. Addressing these issues, we review models of CPG development, outline challenges to evidence-based approaches to CPG development, address unique factors relevant to the development of guidelines for the surgical community, introduce an Ontario practice guidelines strategy that uses complementary methods of CPG development, and summarize the feedback provided by the surgical community regarding the practice guidelines produced in Ontario's cancer system.
