Use of a balloon-expandable intravascular graft in the management of type B aortic dissection in an animal model.

PURPOSE: To evaluate obliteration of an experimental aortic dissection with a balloon-expandable intravascular stent. MATERIALS AND METHODS: Fourteen adult dogs were divided into two groups. In group 1 (n = 6), a thoracoabdominal aortic dissection was surgically created to observe the natural course of this lesion. In group 2 (n = 8), a balloon-expandable intraluminal vascular graft was introduced via the femoral artery in a dissected aorta to try to obliterate the dissection. Angiography was performed postoperatively and again 6 weeks later before the aortae were explanted for pathologic evaluation. RESULTS: In group 1, postoperative aortography depicted evidence of aortic dissection in all animals. Autopsy revealed persisting dissection with reentry tear near the celiac axis in five animals. In group 2, placement of a stent at only entry and reentry sites resulted in partial obliteration of the dissection (n = 3). When the entire length of dissected aorta was treated, the dissection was completely obliterated (n = 4). CONCLUSION: An aortic dissection can be obliterated with a balloon-expandable stent if the entire dissected aorta is treated.

Is acute type A aortic dissection a true surgical emergency?

Untreated acute aortic dissection involving the ascending aorta (type A) is associated with a high early mortality owing to rupture. Despite advancements in surgical technique and critical care, early mortality remains high. Operative mortality may be related to the technical challenges associated with intervening on the acutely dissected aorta as well as the multiorgan insult it induces. In this article, we review our approach to acute type A aortic dissection with regard to diagnosis, initial medical management, surgical repair, and timing of repair.

Crack-cocaine-associated aortic dissection in early pregnancy--a case report.

Even though uncommon in pregnancy, aortic dissection is a potentially catastrophic vascular complication, occurring mainly in the late stages of pregnancy. Vascular events, including aortic dissection are recognized complications of crack-cocaine use. The authors report a case of aortic dissection in early pregnancy related to crack-cocaine use. They believe that the combined effects of pregnancy and crack cocaine on the vasculature create the requisite milieu potentiating such catastrophic events as aortic dissection. This paper reviews the possible underlying pathophysiologic mechanisms and the available diagnostic, therapeutic, and management options.

Ascites after rupture of dissecting aortic aneurysm into the right atrium.

We report successful repair of an aneurysmal aorta-right atrial fistula causing intractable ascites. The clamped "ascending aorta" was drained for mixed return after perfusion through the femoral vessels and opened during hypothermic arrest. Return cannulation through the fistula permitted definitive repair.
