The value of tear film breakup and Schirmer's tests in preoperative blepharoplasty evaluation.

The authors published their article on the value of tear film breakup and Schirmer's tests in preoperative blepharoplasty evaluation in 1989. The purpose of this update is to expand the original article in light of refinements and experiences in subsequent years. It was concluded in the 1989 article that the abnormal tear film breakup and Schirmer's tests were not good predictors of possible postblepharoplasty dry eye complications. It was concluded in the review that the anatomy and the history including scleral show, lagophtholmus, negative vector, snap test, previous surgery, increased blinking, dryness, grittiness and pain were more important predictors of postoperative dry eye problems than the ocular tests. The authors' opinion remains so, and they do less preoperative tear film testing and place more emphasis on the history and the anatomy as predictors of potential problems. In this follow-up to the 1989 article, the authors include a survey from several surgeons as to how they evaluate their patients for elective aesthetic blepharoplasty, which in large measure confirms the authors' practice.

Mucoepidermoid carcinoma of the lacrimal gland fossa: confirmation by ultrastructural study.

We have described a patient with a biopsy-proven mucoepidermoid carcinoma of the orbit. Although it may have represented a metastasis from a malignancy of the upper respiratory tract, clinically it appeared to be a primary neoplasm of the lacrimal gland. This case emphasizes the value of transmission electron microscopy in diagnosing poorly differentiated orbital tumors.

Lacrimal disease.

The lacrimal gland has been investigated by a number of authors with specific interest in the relationship between keratitis sicca and systemic disease. Neoplastic involvement of the lacrimal gland and lacrimal sac have drawn the attention of others. Imaging of the lacrimal drainage system by ultrasonography, computerized axial tomography, and magnetic resonance imaging has been reviewed. Finally, surgical approaches to dacryocystorhinostomy and attendant complications have been studied.

Downward extension of the lacrimal gland under the bulbar conjunctiva after trabeculectomy.

PURPOSE: To describe two cases in which the lacrimal gland was considered to extend downward under the bulbar conjuctiva following trabeculectomy. PATIENTS AND METHODS: We report two patients who complained of epiphora after trabeculectomy. A biopsy was performed on the tissue under the bulbar conjunctiva on the periphery of the bleb of case 1. The anatomical relationship between the holes and the avascular filtering bleb in case 2 was investigated using ultrasound biomicroscopy. RESULTS: In case 1, a biopsy showed the lacrimal gland tissue filtrated with mononuclear cells. In case 2, ultrasound biomicroscopy revealed no connection between the multiple leaking holes and localized filtering bleb. CONCLUSION: Following trabeculectomy at the superior temporal quadrant, we should consider downward extension of the lacrimal gland as a possible source of a patient's complaint of epiphora.

Anatomy and histopathology of the human lacrimal gland.

PURPOSE: To review the anatomy and histopathologic changes of the human main lacrimal gland. METHODS: Samples of lacrimal gland including palpebral lobes and orbital lobes were taken in autopsies, and the relationship between histopathologic changes and age and sex, as well as histopathologic differences between palpebral and orbital lobes of the lacrimal gland, were studied using light microscopy. RESULTS: Various histopathologic changes were observed in the human main lacrimal gland as follows: acinar atrophy; periacinar fibrosis; periductal fibrosis; interlobular ductal dilatation; interlobular ductal proliferation; lymphocytic infiltration; and fatty infiltration. Several histopathologic differences exist between the palpebral and orbital lobes. There were statistically significant correlations between age and diffuse fibrosis, diffuse atrophy, and periductal fibrosis in the orbital lobes of women. Diffuse fibrosis and diffuse atrophy in orbital lobes were more frequently observed in women than in men. CONCLUSION: It is speculated that periductal fibrosis is related to a decrease of tear flow with age and that interlobular ductal dilatation in palpebral lobes may be caused by stenosis of the excretory duct in conjunctival fornix. However, the mechanisms of these histopathologic changes in the human main lacrimal gland are not yet clear.

Recurrent acute lymphoblastic leukemia presenting in the lacrimal gland.

A 14-year-old girl with a history of acute lymphoblastic leukemia presented with periorbital swelling of the right eyelid associated with enlargement of the lacrimal gland and cervical lymphadenopathy. Radiographic imaging showed diffuse enlargement and enhancement of the right lacrimal gland. The patient underwent an incisional biopsy of the lacrimal gland that showed atypical lymphocytes consistent with recurrent acute lymphoblastic leukemia. The lacrimal gland is a rare extramedullary site for recurrence of this disease.

Atypical presentation of Rosai-Dorfman disease in the lacrimal gland mimicking malignancy.

A 29-year-old man presented with a painful, rapidly growing mass in the right upper eyelid. Ophthalmic and systemic examinations and results of laboratory investigations were all normal. CT revealed a mass in the right lacrimal region with surrounding bone destruction, which was suggestive of a malignant lacrimal gland tumor. Histologic analysis of the biopsy specimen supported a diagnosis of Rosai-Dorfman disease with no lymphadenopathy.

Duration of botulinum toxin effect in the treatment of crocodile tears.

PURPOSE: To provide clinical evidence of the duration of botulinum toxin type A (BTX-A) effect when applied in the palpebral lobe of the lacrimal gland in patients with gustatory epiphora. METHODS: Prospective, nonrandomized, nonblinded study. Patients with history of gustatory epiphora were included. A Schirmer test was performed to quantify tearing induced by chewing. Clinical examination included visual acuity, tear-duct syringing, slit lamp examination, corneal staining, and eyelid malpositions. A questionnaire was completed by each patient to asses the severity of hyperlacrimation. A single dose of 2.5 units of BTX-A was injected directly into the lacrimal gland palpebral lobe. Patients were evaluated before and at 1, 4, 12, and 24 weeks after injection. The same person performed the examination and the BTX-A injection. Descriptive statistics, using repeated measures and a paired t test, were used for statistical analysis. RESULTS: Fifteen patients were included. Mean age was 63 years. Before BTX-A injection, mean Schirmer test values were 5.47 mm in the unaffected eyes (NAE) and 12.07 mm in the affected eyes (AE). When comparing Schirmer test values in the AE before and after BTX-A injection, there were statistically significant differences (p < 0.05). Only 2 patients developed mild transitory ptosis. No other complications were noted. CONCLUSIONS: The effect of 2.5 units of BTX-A injected into the lacrimal gland lasted 6 months, a duration similar to that reported for other application sites.

Lacrimal caruncle primary basal cell carcinoma: case report and review.

BACKGROUND: Basal cell carcinoma (BCC) is a common malignant skin neoplasm. The surface of the caruncle contains sebaceous glands, hair follicles, and lacrimal and sweat gland elements. Consequently, the caruncle may spawn any neoplasm that occurs in the conjunctiva, skin, or lacrimal gland. We report a patient with a primary BCC located on the lacrimal caruncle. METHODS: An 80-year-old man with a 5-month history of a gradually enlarging left caruncular neoplasm was seen. The lesion was nodular and irregularly brown-black colored, with no involvement of adjacent conjunctiva or skin. He had a history of sun exposure, but no personal or family history of other malignant neoplasms. RESULTS: The mass was excised completely. Histopathologic examination revealed a BCC originating in the basal layer of the conjunctival epithelium. No tumor recurrence has been noted after 7 years of follow-up. Primary BCC of the caruncle is unusual, and only four cases have been described in the PubMed. BCC should be considered in the differential diagnosis of caruncle and adjacent skin lesions.

Microsurgical reconstruction of destroyed canaliculi lacrimales.

The authors describe the microsurgical technique applied for the primary and secondary reconstruction of destroyed lacrimal canaliculi. In the event of fresh injuries, they recommend the immediate repair of the mucosal tissue and the reconstruction of the muscle tissue after direct or retrograde intubation of the lumina canaliculi. For secondary reopening of a stenosis, depending on the localization and the extent of the injury, various methods can be used, such as retrograde probing, the opening of the canaliculus in the area of the punctum papilla as described by Arlt,, retrograde opening from the tear sac in the manner used by Sattler or from the canaliculus communis following the method of Busse and colleagues. In the event that a substitute is necessary, then the authors recommend the implantation of a hydrophilic tear duct prosthesis in combination with a conjunctivodacryocystostomy in the manner descibed by Stallard.

Hemicrania with massive autonomic manifestations and circumscribed eyelid erythema.

OBJECTIVES: To describe a unilateral headache that in addition to the typical shortlasting unilateral neuralgiform headache attacks with conjunctival injection, tearing, sweating and rhinorrhea (SUNCT) syndrome - traits with excessive and ipsilateral autonomic phenomena - had circumscribed eyelid erythema and adjacent ocular redness. OBSERVATIONS: A 60-year-old female had excessive, right-sided lacrimation and local pain at 15 years of age. Due to steadily increasing discomfort, with lacrimation and swelling over the outer part of the upper eyelid, the right lacrimal gland was removed at 20 years of age, with a suspicion of lacrimal gland adenoma. Preoperatively, symptomatic side mild-degree eyelid erythema/rhinorrhea were integral parts of the attack. After years with minor complaints, she, in the mid-twenties, experienced more long-lasting pain attacks, and pain soon became the main problem. A marked, distinct erythema on the lateral part of the right-sided eyelids and marked, localized 'eye redness' in the adjacent area were main ingredients of the attacks together with eyelid edema and viscous rhinorrhea. There were visible vessels below the eye, and telangiectasia of the upper eyelid. CONCLUSIONS: This headache has many similarities with SUNCT - but has several, grossly deviating traits: the temporal aspects, excruciatingly intense pain attacks, and above all marked, lateral eye-lid erythema, and adjacent, massive ocular reddening. This constellation probably alienates it from SUNCT.

Quantitative study of the effect of dacryocystorhinostomy on lacrimal drainage.

Using scintigraphy, we have studied the lacrimal drainage from the conjunctival sac of normal subjects and patients who have undergone dacryocystorhinostomy. A mathematical model of drainage was constructed that accurately described the complete activity curves for both groups. The initial rate of drainage was shown to be a relevant parameter to characterize drainage, and it was found to be different from normals to patients, indicating that the lacrimal pump mechanism is affected by dacryocystorhinostomy.
