Distribution and density of subepithelial collagen in the endolymphatic sac in patients with Meniere's disease.

Our understanding of the histology and histopathology of the endolymphatic sac (ELS) is based on both temporal bone preparations and biopsy material. The current study will provide the basis for an ultrastructural analysis of the spectrum of pathologic changes observed in biopsy specimens from patients with endolymphatic hydrops in Meniere's disease. Emphasis is placed on the distribution and density of subepithelial collagen in the ELS of normal and hydropic patients. This study will be the light microscopic basis for an ultrastructural clinicopathologic correlation of inner ear tissue from the ELS with the clinical stage of the disease.

Short-lasting drop attacks in Meniere's disease.

OBJECTIVES: To study the vertigo attacks known as Tumarkin attacks or drop attacks (DA). DA are characterized by sudden loss of balance with or without falls but with preserved consciousness, and they are supposedly triggered by changes in the otolith function of Meniere's disease (MD). STUDY DESIGN AND SETTING: Data from 243 consecutive MD patients were collected into a database of an otoneurologic expert system. RESULTS: DA was experienced by 72% (n = 173) of the patients with MD. It correlated with visually provoked vertigo. Gait difficulties, tinnitus, and anxiety were more common in the DA group. Long lasting vertigo attacks were more frequently provoked in the DA subjects by physical strain (58% vs 14%), head movements or changes in head position (71% vs 48%), pressure changes (54% vs 12%), or rapid movements in visual surroundings (65% vs 35%) than in the non-DA subjects. CONCLUSION: DA are common in MD patients. The abrupt occurrence of DA make them poorly tolerated. SIGNIFICANCE: DA are more common in advanced MD.

Meniere's disease and fibrositis syndrome (psychogenic rheumatism). Relationship in audiometric and nystagmographic results.

A prospective neuro-otological study concerning 30 cases of fibrositis syndrome (psychogenic rheumatism, PR) and 30 age-matched normal controls was made and a retrospective study concerning 33 cases of Meniere's disease (MD) diagnosed and followed-up, examined between 1965 and 1982. Results showed: sensorineural hearing loss at low frequencies in all early stages of MD and in 10/30 of PR; hyperacusis (pain threshold below 100 dBHL bilaterally for all frequencies) without other sign of recruitment in 73.3% of PR and in 3/4 cases of MD where it was measured (discomfort or vertigo due to noise was noted retrospectively in 16/33 of early stages of MD); hyperreactivity of per-rotatory nystagmus in 53.3% of PR without neurological or peripheral vestibular lesions and, with or without vestibular unilateral lesions, in 39.9% of MD. None of the controls showed hyperacusis, hyperreactivity of per-rotatory nystagmus or deafness at low frequencies.

Blood levels of glucose and insulin in Meniere's disease.

It has been reported that a very commonly overlooked cause of vertigo is disorder of glucose metabolism. This may not be reflected in the glucose tolerance test alone, but becomes obvious when the insulin levels in blood are evaluated simultaneously. Thirty-one patients with Meniere's disease underwent a 5-hour glucose tolerance test with radioimmune assay of insulin. The results were compared with the normal and abnormal patterns suggested by Kraft. On this basis, it was found that 67.7% of our patients showed some abnormality in the relationship between the blood levels of glucose and insulin. These patients were put on a special diet with a limited carbohydrated content prescribed by a qualified dietitian. The effect of this diet was highly variable and the possible reasons for this are also discussed.
