Biofilm formation on hydrophilic intraocular lens material.

PURPOSE: To estimate bacterial biofilm formation on the hydrophilic acrylic (hydrogel) intraocular lens (IOL) Meridian (HP60M, Baush & Lomb) and to investigate a preventive effect against biofilm formation of hydrogel IOLs presoaked in antibiotics. METHODS: Two Staphylococcus epidermidis strains, ATCC 12228 and ATCC 35984 (biofilm-producer), and an Enterococcus faecalis strain (KOS1, clinical isolate from an endophthalmitis patient) were used. Biofilms were cultivated on disks of different IOL materials: hydrogel, PMMA (polymethylmethacrylate), and acrylic. Biofilms were stained with crystal violet (CV), which served as an index of biofilm formation. The bacterial population was enumerated after biofilm homogenization. Biofilms were also examined by scanning electron microscopy (SEM). IOLs were presoaked in two antibiotics, levofloxacin (LVFX) and gatifloxacin (GFLX), and then the bacterial population was enumerated. As in vivo experiment, antibiotics-treated and nontreated Meridian IOLs were implanted in rabbit eyes, which served as an endophthalmitis model, and the bacterial population was enumerated. RESULTS: The amount of biofilm formed was the least on hydrogel from among the three materials tested after 48- and 72-hr incubation (p < 0.05 to 0.01). The bacterial population was the least on hydrogel from among the three materials with ATCC 12228 (p < 0.05 to 0.01), and the bacterial population was significantly different between hydrogel and acrylic after 72-hr incubation with ATCC 35984 (p < 0.05). Biofilm by the two S. epidermidis strains were recognized after 24-hr incubation. Rates of biofilm-positive SEM fields, which were defined as being occupied by biofilm over at least half of the area, were increased through 72 hr with ATCC 35984. While the E. faecalis strain showed no bacterial adherence on the antibiotics-treated hydrogel IOLs, adherence of the S. epidermidis strain, ATCC 35984 was recognized on the LVFX-treated IOLs after 48-hr incubation (103 to 104 CFU/ml). In the rabbit in vivo model, the bacterial populations in eyes with an antibiotics-treated Meridian IOL were significantly smaller than in eyes with a nontreated IOL for 72 hr after surgery (p < 0.05 to 0.01). CONCLUSIONS: The biofilm formation was less on hydrogel than on other two materials tested. Hydrogel presoaked in antibiotics exhibited a preventive effect against biofilm formation at least for 24 hr in vitro and against bacterial proliferation in the rabbit in vivo endophthalmitis model.

Biocompatibility of a physiological pressure sensor.

A newly developed fiber optic micropressure sensor was evaluated for biocompatibility using the International Organization for Standardization (ISO) test standard 10993-6. The test material and an inert control (fused silica glass) were tested in New Zealand white rabbits. Four test specimens were implanted in the paravertebral muscles on one side of the spine about 2-5 cm from the mid-line and parallel to the spinal column. Similarly, four control specimens were implanted on the opposite side. The implantation periods were 1, 4, and 12 weeks to ensure a steady state biological tissue response. Four animals were tested at each time period. Macroscopic and microscopic observations were performed to compare the biological reactions between the test and control materials. There was an inflammatory reaction at 1 week which subsided at 4 weeks. There was fibrous tissue growth near the implant that also decreased over time. Most importantly, there was no significant difference in the biological response between the test and control materials. Therefore, we conclude that the pressure microsensor is biocompatible.

Dichotic speech tests.

When central auditory dysfunction is present, ability to understand speech in difficult listening situations can be affected. To study this phenomenon, dichotic speech tests were performed with test material in the Swedish language. Digits, spondees, sentences and consonant-vowel syllables were used as stimuli and the reporting was free or directed. The test material was recorded on CD. The study includes a normal group of 30 people in three different age categories; 11 years, 23-27 years and 67-70 years. It also includes two different groups of subjects with suspected central auditory lesions; 11 children with reading and writing difficulties and 4 adults earlier exposed to organic solvents. The results from the normal group do not show any differences in performance due to age. The children with reading and writing difficulties show a significant deviation for one test with digits and one test with syllables. Three of the four adults exposed to solvents show a significant deviation from the normal group.

Bone mineral content and fixation strength of femoral neck fractures. A cadaver study.

We measured the bone mineral content (BMC) of 21 human femoral autopsy specimens using Radiographic Attenuation Technique (RAT). The specimens were then tested for stiffness in a material-testing machine. Osteosynthesis was performed after medial femoral neck osteotomy, using 1 of 3 different pinning methods: 2 hook pins (LIH), 2 cannulated screws (Uppsala), and 3 cannulated screws (CHP). The specimens were allocated to each method by grouping them in triplets with similar BMC. Cyclic loading at 500-1000 N and 1 Hz was performed during 50 minutes. The amount of axial compression during loading was measured. Finally, loading to failure was done and post-fixation stiffness and ultimate compression strength recorded. With one exception, all cases failed where the BMC was less than 0.4 g/cm2. No differences between the methods of osteosynthesis were found. The fixation strength was mostly dependent on the strength of the bone, i.e., the degree of osteoporosis.

Biomechanical investigations of different meniscal repair implants in comparison with horizontal sutures on human meniscus.

PURPOSE: The use of biodegradable implants for arthroscopic repair of meniscal lesions is becoming increasingly popular. The aim of this study was to test the biomechanical stability and the mode of failure of these implants. TYPE OF STUDY: Biomechanical testing study. METHODS: Biomechanical investigations were performed on human menisci using 6 commonly used biodegradable implants for meniscal repair to compare them with horizontal mattress suture using 2/0 Ethibond (Ethicon, Norderstedt, Germany). Included in this study were the Meniscus Arrow (Bionx, Tampere, Finland), Dart (Arthrex, Naples, FL), Stinger (Linvatec, Largo, FL), Meniscal Screw (Innovasive, Marlborough, MA), T-Fix (Acufex, Mansfield, MA), and the Fastener (Mitek, Westwood, MA). The tests were carried out using a material testing machine at a loading rate of 10 N/second. The ultimate tension load (UTL), elongation, and stiffness were evaluated for each implant technique. RESULTS: The highest UTL was shown by Ethibond (62 +/- 7.91 N) and the T-Fix (51.35 +/- 16.31 N), followed by the Fastener (32.67 +/- 2.97 N). All other implants had a significantly lower UTL (P =.001). Less elongation under a load of 5 N was noted for Ethibond (0.64 +/- 0.25 mm) and for T-Fix (0.43 +/- 0.32 mm) compared with the other implants. The greatest elongation was found for the Fastener (2.239 +/- 0.581 mm). The stiffness of the fixation was similar in all implants, except for the Dart and Fastener, which were significantly inferior (P <.05). CONCLUSIONS: All of the biodegradable implants had lower UTL than the suture techniques. Therefore, when using the implants, they should be inserted close together to provide sufficient stability. In cases of an extended lesion, there might even be an option to combine the implant and suturing techniques.

Tissue response to an experimental calcium hydroxide-based endodontic sealer: a quantitative study in subcutaneous connective tissue of the rat.

Silicone tubes containing a freshly mixed experimental Sealapex, in which titanium dioxide was removed from the original formula, were implanted in the dorsal subcutaneous connective tissue of the rat. Solid silicone rods of the same size as the tubes were also implanted and used as inert controls. The tissue reaction to both these materials was histometrically and quantitatively studied under light microscopy and by elemental electron microprobe analysis. After 7, 30 and 90 d of implantation, different grades of tissue reaction to the tested materials were recorded. A granulomatous tissue containing numerous foreign-body giant cells and macrophages with engulfed particles in their cytoplasms was initially detected in contact with the test material. In addition, many fibroblasts and newly formed vessels were also observed at these areas. The results of this study revealed that these reactions increased progressively at the 30- and 90-d observations. The electron microprobe analysis of the granulomatous tissues showed the presence of heavy components of the experimental material.

Effects of subretinal implant materials on the viability, apoptosis and barrier function of cultured RPE cells.

BACKGROUND: Subretinal microphotodiode array (MPDA) is a type of visual prosthesis used for the implantation in the subretinal space of patients with progressive photoreceptor cell loss. The present study aimed to evaluate the effect of materials for MPDA on the viability, apoptosis and barrier function of cultured pig retinal pigment epithelium (RPE) cells. METHODS: Primary culture of pig RPE cells was performed and 24 pig eyes were used to start RPE culture. The third passage of the cultures was plated on different materials for MPDA and MPDAs. The tetrazolium dye-reduction assay (MTT) was used to determine RPE cell viability. Flow cytometry was measured to indicate the apoptosis rates of RPE cells on different materials. RPE cells were also cultured on microporous filters, and the transepithelial resistance and permeability of the experimental molecule were measured to determine the barrier function. RESULTS: The data from all the methods indicated no significant difference between the materials groups and the control group, and the materials tested showed good biocompatibility. CONCLUSIONS: The materials for MPDA used in the present study had no direct toxicity to the RPE cells and did not release harmful soluble factors that affected the barrier function of RPE in vitro.

Influence of 0.05% sodium fluoride solutions on microhardness of resin-modified glass ionomer cements.

This study aimed to evaluate the influence of fluoride-containing mouthrinse solutions (Fluorgard and Oral B) on the superficial microhardness of two resin-modified glass ionomer cements (Vitremer and Fuji II LC). Fifteen discs-shaped specimens of each glass ionomer cement (Ø10 mm; 2 mm thick) were prepared, thereby forming two groups. After 24-hour storage in artificial saliva, the microhardness was measure and the data were recorded. Next, each group was divided into three subgroups (n = 5), according to the solution to be immersed in. Control specimens were kept in artificial saliva along the whole experiment. The test specimens were kept in mouthrinse solution for 30 days. Vickers surface microhardness was analyzed at predetermined evaluation periods: 24 h, 48 h, 7, 14, 21 and 30 days after specimens' preparation. Data were subjected to three-way ANOVA and to Tukey test (p<0.05). A better behavior of Fuji II LC was observed and Fluorgard affected most the characteristics of the tested materials. It may be concluded that fluoride-containing solutions influenced the tested characteristics of materials, mainly of Vitremer.

Changes in gametogenesis and fecundity of acroporid corals that were exposed to elevated nitrogen and phosphorus during the ENCORE experiment.

Colonies of two scleractinian reef coral species, Acropora longicyathus and Acropora aspera were transplanted into patch reefs at One Tree Reef, Great Barrier Reef, Australia as part of the ENCORE experiment. These corals and colonies of A. aspera which were naturally present in the patch reefs were exposed to four treatments over two years: controls with normal seawater, elevated levels of nitrogen only, phosphorus only, or nitrogen plus phosphorus. These corals were sampled and used to determine whether gametogenic cycles and fecundity were affected by nutrient enrichment. Acropora longicyathus had a single annual gametogenic cycle. Corals exposed to elevated nitrogen produced significantly smaller and fewer eggs and contained less testes material than those which were not exposed to nitrogen. Exposure to elevated phosphorus only resulted in corals producing more but smaller eggs, and more testes material. Egg numbers of colonies from other treatments decreased as the gametogenic cycles continued, but those of the phosphorus colonies showed almost no reduction in egg numbers between the early and late stages of the gametogenic cycles. These results have important management implications for coral reefs as they demonstrate that small increases in concentrations of nitrogen and phosphorus can have severe effects on reproductive activity in these species of scleractinian corals.

An in vitro comparison of the motion behavior of different bipolar endoprostheses.

The relative motion of 3 different bipolar endoprostheses was evaluated in vitro. A paired fresh acetabulum was frozen at 0 degrees C and defrosted 12 hours before the experiment. Three bipolar endoprostheses were evaluated: UNIQHIP system (United Orthopedics), UHR system (Osteonics), and AML system (Depuy). The surface roughness and spherical roundness of outer shells and inner heads of the bipolar prostheses were measured before the experiments. The acetabulum and outer shell of the bipolar prostheses were fixed on a Bionix 858 material testing system axially by separate fixation tools. The axial load of 1,400 N and 2,800 N was than applied on the specimen. The axis was rotated from 0 degrees to 90 degrees at the speed of 1 degree/s. All 3 outer shells were tested to this paired acetabulum randomly and separately. The frictional torque on the outer bearing surface of the different prostheses was recorded by the material testing system. The frictional torque on the inner bearing surface was also measured by the same procedure as was done for the outer bearing. The final results were statistically compared by the 1-way analysis of variance test method. Bipolar prostheses of the UHR system showed the largest frictional torque on outer bearing when it was loaded with 1,400 N and 2,800 N. The final results showed that all the bipolar prostheses had ideal motion behavior when functioning under the loading of 1,400 N. The frictional torque on the inner bearing was found to be larger than the frictional torque on the outer bearing in some prostheses when the loading was increased to 2,800 N. Thus, the bipolar endoprostheses functioned as unipolar prostheses. The only relative motion remained between the outer bearing surface and the acetabulum. This effect causes complications, such as implant protrusion in the acetabulum.

Shock absorbability of various restorative materials used on implants.

The purpose of this study was to compare, using an in vitro model, the stress absorbing ability of a microfilled composite resin and of a new low fusion ceramic (Duceram LFC) to that of gold alloy and conventional ceramic, when used as restorative materials in implant-supported prosthesis. Test crowns made of the tested materials were rigidly connected to a Brånemark implant clone. The maximum amplitude of the force transmitted to the bone-implant interface, and the time to reach this amplitude were measured after applying a 100 N impact load on the occlusal surface. The gold alloy restorations transmitted the highest impact force in the shortest delay at the bone-implant interface. Microfilled composite resin Dentacolor, and low fusion ceramic Duceram LFC did not reduce the amplitude of the impact-force when compared to conventional ceramic. Nevertheless, the time to reach the maximum amplitude of this force was longer when using composite resin than when using ceramic, while Duceram LFC had no influence on this criteria.

Wettability, imbibition, and mass change of disinfected low-viscosity impression materials.

STATEMENT OF PROBLEM: There is an ongoing effort by dental manufacturers to create impression materials with improved wetting properties. Disinfection solutions may alter the surface characteristics of these newer materials. PURPOSE: This study compared wettability, imbibition, and mass change of various recently introduced automixed low-viscosity addition silicone and polyether materials before and after immersion disinfection. MATERIAL AND METHODS: The Wilhelmy technique was used for deriving wetting properties of 5 addition silicone materials (Clinician's Choice Affinity, Clinician's Choice Superhydrophilic [experimental], Kerr's Take One, 3M's Imprint II, and Dentsply's Aquasil LV) and 2 polyether materials (ESPE's Permadyne Garant and Impregum Garant). Conditions included a control with no disinfection (0 hours), as well as (1/2) hour of immersion disinfection in a full-strength solution of 2% acid glutaraldehyde disinfectant (Banicide). Weight changes before and after disinfection and weight loss in air were measured over an 18-hour period to detect imbibition and mass change over time. The data were analyzed with a 1-way analysis of variance at alpha=0.05, with n = 3 for advancing (ACA) and receding (RCA) contact angles and n = 2 for imbibition and mass change. RESULTS: Statistical significant differences in wettability (P<.001) were found among nondisinfection control groups, as well as among (1/2)-hour disinfection groups. Polyethers were the most wettable materials overall. Impregum Garant polyether demonstrated significantly lower RCA for the control (48.4 degrees) and at (1/2) hour of disinfection (51.8 degrees). The 2 polyethers and Take One lost mass, whereas Aquasil LV gained mass in air; however, all materials exhibited some degree of imbibition during disinfection. CONCLUSION: Within the limitations of this study, the 2 polyether materials tested exhibited significantly lower ACA's and RCA's compared with the 5 addition silicones tested. Imbibition for the 2 polyether materials was significantly higher (P<.001). Polyether materials lost significantly more (0.6% to 0.8%) and Aquasil LV gained significantly more (0.6%) mass in air.

Accelerated aging affects color stability of provisional restorative materials.

PURPOSE: The color stability of two shades each of five acrylic resin and seven resin composite provisional restorative materials was evaluated by reflection spectrophotometry following in vitro accelerated aging. MATERIALS AND METHODS: Specimens of provisional restorative materials were polymerized according to manufacturers' instructions and aged in an artificial aging chamber with exposure to a total ultraviolet irradiation of 60 kJ/m2. Color was measured by CIE L*a*b* on a reflection spectrophotometer before and after aging. Color change (delta E*) was calculated and analyzed statistically. RESULTS: Statistically significant changes in color were observed after accelerated aging. Nine of the 12 provisional materials tested showed perceptible color change of at least one of the shades tested. The most color-stable materials were the acrylic, Alike, and the resin composites, Luxatemp and Protemp Garant, which had no perceptible color changes under these conditions. CONCLUSIONS: Some acrylic resin and composite provisional materials change color significantly and perceptibly when exposed to in vitro accelerated aging conditions.

Bonding fresh amalgam to existing amalgam: a shear and flexural strength study.

The purpose of this study was to compare the shear and flexural strength obtained when various adhesives or liners were used to bond freshly placed amalgam to existing amalgam. The adhesives used in this study were the following: cavity varnish, 4-META adhesive (Amalgambond), and a dentin bonding agent (Clearfil New Bond). After samples were fabricated, shear bond strengths were determined by using an Instron Testing Machine, and flexural strengths determined using a Material Test System (MTS) testing machine. All test groups produced shear and flexural strengths less than one half of the control group. The dentin bonding agent and the 4-META adhesive produced significantly higher shear and flexural strengths than the test group in which no lining material was used.

[A 4-week intravenous toxicity study of the active metabolite (NM394) of prulifloxacin (NM441) in rats followed by a 4-week recovery test].

A repeated dose toxicity study of ( +/- )-6- fluoro-1-methyl-4-oxo-7-(1-piperazinyl)-4H- [1,3]thiazeto[3,2-a]quinoline-3-carboxylic acid (NM394), the active metabolite of a new antibacterial agent, prulifloxacin, was conducted in Sprague-Dawley rats. Male and female rats were given the test material intravenously for 4 weeks at doses of 0 (control), 3, 10 and 30 mg/kg. After discontinuation of the treatment, a 4-week recovery test was also conducted. There were no treatment-related effects on survival, clinical signs, body weight and food consumption. Ophthalmoscopic and hematologic examinations failed to show any abnormalities related to the treatment. Increased water consumption was observed in the 10 and 30 mg/kg groups. In these dose groups, increased urine volume and lowered urine specific gravity, and crystalline substance and small epithelial cells in urinary sediments were seen. Cloudy urine was also seen in the 30 mg/kg group. Blood chemical examination showed decreased gamma-globulin in the 10 and 30 mg/kg groups and increased BUN and creatinine in the 30 mg/kg group. Pathological changes caused by the treatment were as follows. In kidney, tubular nephrosis with crystalline substance was observed in the 10 and 30 mg/kg groups and its organ weight was increased in the 30 mg/kg group. Cecal weight was increased in the 30 mg/kg group. The above-mentioned changes were reversible except for decreased gamma-globulin. Plasma levels and urinary concentrations of the test material were increased in all dose groups with dose-related manner, whereby no sex differences were observed. No effects caused by the repeated dosing were seen in the plasma concentrations. Toxicological findings were not observed in the 3 mg/kg group. The results show that the NOAEL of NM394 is 3 mg/kg for 4-week repeated dose toxicity in rats.

[A 13-week oral toxicity study of prulifloxacin (NM441) in rats followed by a 5-week recovery test].

A repeated dose toxicity study of prulifloxacin, a new antibacterial agent, was conducted in Sprague-Dawley rats. Male and female rats were given the test material orally for 13 weeks at doses of 0 (control), 30, 170 and 1000 mg/kg. After discontinuation of the treatment, a 5-week recovery test was also conducted. Salivation, soft feces, reduced body weight gain and increased water consumption were seen in the 1000 mg/kg group. There were no treatment-related effects on survival and food consumption. Ophthalmoscopic and hematologic examinations failed to show any abnormalities attributable to the treatment. Urinalysis revealed increased urine volume and decreased K+ excretion in the 1000 mg/kg group. Blood chemical examination showed increased BUN, decreased triglyceride, K+, Cl- and total protein in the 1000 mg/kg group. Pathological changes caused by the treatment were as follows. Renal tubular nephrosis with crystalline substance was observed in the 170 and 1000 mg/kg groups. Renal weight was increased and crystalline substance was noted in the lumen of the urinary bladder in the 1000 mg/kg group. Cecal distention with increased its organ weight was observed in all dose groups and swelling of its absorptive cells was seen in the 170 and 1000 mg/kg groups. Swelling of jejunal goblet cells was observed in the 1000 mg/kg group. In femoral articular cartilage, focal accumulation of chondrocytes, small cavities and proliferation of fibrous tissue were seen in the 170 and 1000 mg/kg groups. The above-mentioned changes were reversible except for renal tubular nephrosis and cecal distention with its increased organ weight, of which the degree and frequency, however, were lowered. The cecal distention in the 30 mg/kg group was considered to be attributable to the pharmacological effect of the test material. The results show that the NOAEL of prulifloxacin is 30 mg/kg for 13-week repeated dose toxicity in rats.

Technical comments on the bioassay of erythropoietin.

Quantitative erythropoietin determinations can only be obtained after the examination of both Standard and Test materials in a multiple dose parallel line bioassay with appropriate statistical control. The expression of results in erythropoietin units (implying quantitation) in the absence of similar dose-response relationships for Standard and Test materials can lead to errors in the estimated potency which differ markedly from the true potency. Results obtained from one-dose test systems cannot therefore be considered quantitative. Criteria which allow valid quantitation of bioassay results are discussed. Where these cannot be fulfilled, it is suggested that the results should be expressed only in terms of the measured parameter, e.g. 59Fe uptake, per constant volume of test solution.

Effect of Brahmi Rasayan on the central nervous system.

Brahmi Rasayan, an Ayurvedic preparation, was studied in mice and rats for its effects on the central nervous system at oral doses ranging between 1 and 30 g/kg. Observational screening in mice was carried out following a multiparametric check list. The test material was studied for its effect on pentobarbitone hypnosis, motor co-ordination, tail-withdrawal reaction time, electroshock, chemoconvulsions, haloperidol-induced catalepsy and conditioned avoidance response. The test material exhibited a sedative effect and significantly prolonged the hypnotic action of pentobarbitone. It produced a variable blockade of conditioned avoidance response. The presence of a significant antinociceptive effect, coupled with the ability of the test substance to offer protection against electroshock seizures and chemoconvulsions plus the ability to antagonize the haloperidol-induced catalepsy, suggests an involvement of the GABA-ergic system in the mediation of the central nervous system effects of Brahmi Rasayan.
