It is reported on three female patients who fell ill with a second and third tumour, respectively, within 4 years. The cases in question were spontaneous neoformations, since risk factors, such as radiotherapy and applications of cytostatics were not used causally.
Specificity of cellular immune reactivity was tested in 115 cancer patients. The electrophoretic mobility test was used as in vitro test for cellular immunity, KCl extracts from various human carcinomas were used as antigens. For each tumor a typical lymphocytic reaction was noted: the most pronounced migration inhibition in cancer patients was caused by corresponding tumor extracts. Lymphocytic reactions to extracts from tumors of different histogenesis were weaker or negative. A total of 158 tests in 54 healthy control persons with the same antigenic substances showed negative results in 139 cases. Demonstrated specificity of sensitized lymphocytes could prove to be a valuable aid in immunological tumor recognition.
Skeletal muscle is one of the most unusual sites of metastasis from any malignancy. We report on a case of skeletal muscle metastasis from colonic carcinoma, and review seven previously reported cases. The patient was a 66-year-old man who exhibited a painful metastatic mass in the right major teres muscle six months after surgery for colonic carcinoma. He underwent three operations and one course of arterial infusion chemotherapy, but died of carcinoma after two years and seven months. In seven previous cases and in the present case (3 males and 5 females, aged between 30 and 75 years), the sites of the primary lesion and metastatic lesion in skeletal muscle were diverse. The interval between primary carcinoma and relapse ranged from synchronous to 5 years. Spontaneous pain in the recurrent region was a characteristic symptom. Surgical removal of the metastatic lesions relieved pain. Prognosis was considered poor. However, complete resection of the metastatic lesion at a time when no other recurrent lesions were present might have cured the patient.

In some human tumors, reduced or defective MHC class I surface expression has been attributed to functional deficiencies of the genes of the antigen-processing machinery, the proteasome subunits low molecular weight (LMP)-2 and LMP-7, as well as the peptide transporters associated with antigen processing (TAP)-1 and TAP-2. Using normal epithelial kidney cells (MZ1851NN) and renal cell carcinoma cell lines established from the primary tumor (MZ1851RC) and a lymph node metastasis (MZ1851LN) of the same patient, we investigated whether the modulation of MHC class I antigens, TAP and LMP molecules, occurs during transformation and subsequent progression. The mRNA and protein expression of MHC class I heavy and light chain TAP and LMP was strongly reduced in MZ1851RC when compared to the corresponding normal kidney cells MZ1851NN, and this suppression was even more pronounced in the metastatic cell line MZ1851LN. In addition, the activity of the TAP molecules, as measured by peptide translocation assays, was also markedly diminished in MZ1851RC compared to MZ1851NN cells and was further down-regulated in cells of the metastatic lesion. MHC class I surface expression was enhanced by either culturing MZ1851RC and MZ1851LN cells at 26 degrees C instead of 37 degrees C or by incubation of both cell lines with class I-specific binding peptides, whereas MHC class I surface expression of MZ1851NN cells was not affected under these culture conditions. IFN-alpha and in particular IFN-gamma treatment enhances the steady-state mRNA and/or protein levels of TAP, LMP, and MHC class I genes of MZ1851 cell lines but had no additional effect on the stability of MCH class I surface expression. These data indicate that malignant transformation and subsequent in vivo selection of renal tubular cells can lead to the recovery of carcinoma cells that show stable expression of an immune escape phenotype. Deficiencies associated with this phenotype involve all levels of the MHC class I-restricted antigen presentation machinery, are at least partially reversible by IFN treatment, and are even more pronounced in cells that had acquired metastatic potential.
An irradiation dose of 5000 rads in 5 weeks appears to be highly efficient for eradication of subclinical disease in the oropharynx or nasopharnyx. Irradiation alone in the range of 4500 to 5000 rads, five treatments per week, 4 1/2 to 5 weeks, appears to be greater than 90% efficient for eradicating subclinical disease for both squamous-cell carcinoma and adenocarcinoma in regional lymph nodes. The site of origin of the primary squamous-cell carcinoma or adenocarcinoma seems to be independent of the response in the regional lymph nodes. It is unknown whether concomitant elective chemotherapy will modify the dose for control required for subclinical disease in squamous-cell carcinoma or adenocarcinoma.

Second malignant neoplasms (SMN) in late childhood or young adulthood in individuals who have been successfully treated for an initial malignancy have emerged as a late effect of therapy in survivors of childhood cancer. Although radiation therapy is frequently implicated, chemotherapy with alkylating agents and antimetabolites has also been associated with SMN. Soft tissue sarcomas are among the most frequent primary malignancies complicated by a SMN and account for a majority of nonhematolymphoid SMN. We present the clinical and pathologic findings in a patient who had a peripheral neuroepithelioma (primitive neuroectodermal tumor, PNET) of the soft tissues diagnosed at 17 years of age, was treated with high-dose irradiation and multidrug chemotherapy, and developed an angiosarcoma 14 years later. This case represents an uncommon combination of mesenchymal malignancies in a young patient with an unusually favorable clinical course following the diagnosis of PNET.
A method for identifying lymph nodes in tissue specimens from patients with carcinoma of the colon and rectum by soft X-rays is described and assessed. The method seems superior to more time-consuming and possibly tissue-damaging procedures, and it affords a possibility of identifying lymph nodes down to a size 1--2 mm in formalin-fixed as well as in non-fixed tissue specimens.
