[Disorders of bladder emptying after labor. Study of 511 puerperal patients regarding the incidence and risk of residual urine post partum].

In a Study of 500 unselected women in childbed interested the incidence and risk factors of residual urine over 50 ml (RH), sonographically proven in the first days after birth. The incidence after vaginal birth was 17% and after C-section 13%, 7% and 1% suffered from a urinary tract infection respectively. The RH-phenomenon lasted approximately 3 days. Significant risk factors have been: vaginal-operative deliveries (forceps, Vacuum-Extraction) protracted deliveries (over 12 hours) protracted expulsion period (over 1 hour) longer expulsive periods (over 15 minutes) newborn head-circumference over 36 cm I-Parae with episiotomy -PDA had only an additive effect. The shown quantitative aspects make it easier to evaluate the risk and the exact sonographic search for residual urine in the days pp.

The role of estrogen therapy in postpartum psychiatric disorders: an update.

OBJECTIVES: The aim of this review is to assess and summarize the existing literature data about the efficacy of estrogen replacement therapy (ERT) in preventing and treating postpartum psychiatric disorders. METHODS: An extensive and unrestrictive computerized search (from 1970 to June 2005) on MEDLINE/PubMed, TOXNET, EMBASE, and Cochrane Databases was conducted with the following search terms: estradiol, estrogen-treatment, hormonal treatment, therapy, postpartum depression, postnatal depression, puerperal depression, postpartum blues, postpartum psychosis, lactation, and breastfeeding. RESULTS: In assessing available literature information about the role of ERT in preventing and treating puerperal psychiatric diseases, all reviewed studies were found to suffer from severe methodological limitations. CONCLUSION: Well-designed and strictly focused multicenter trials are warranted in order to firmly establish the effectiveness of ERT in puerperal psychiatric disorders.

The genetics of puerperal psychosis.

This paper examines the role of familial factors in the aetiology of puerperal psychosis. It reviews diagnostic issues associated with puerperal psychosis and the aetiology of the disorder in general before considering genetic studies in more detail. Owing to methodological difficulties, uncertainty over the importance of genetic factors persists.

An investigation into puerperal psychoses in black women admitted to Baragwanath Hospital.

OBJECTIVES: Puerperal psychosis was studied in black African women at Baragwanath Hospital in Johannesburg. DESIGN: A retrospective study analysed the clinical notes of 314 cases of puerperal psychosis seen over previous years. A prospective study researched 67 cases of puerperal psychosis referred during a full calendar year. A control group of 98 patients was matched with the prospective study patients for age, marital status, parity and month of delivery. RESULTS: The incidence (2-3 cases per 1,000 births), onset and pattern of illness are all remarkably similar to that described in the international literature. Confirmed risk factors were a primiparous patient; a family history of psychiatric illness; and a personal psychiatric history, particularly a history of mania. Additional risk factors found in this study were substance dependence; a medical illness; the season of the year; a male child; and psychosocial stress including need for intensive medical care for the baby or death of the baby. CONCLUSION: The conclusion reached is that the puerperal psychoses are undifferentiated psychoses, usually mood disorders, showing some special symptomatology, and are precipitated in constitutionally predisposed patients by the physiological factors of the involutionary period.
