Machine learning for development of an expert system to predict premature birth.

Normal pregnancy involves a term of 40 weeks gestation. Problems associated with low birthweight and prematurity continue to plague childbearing families and the healthcare system because 8-12% of all newborns in the United States deliver prior to 37 weeks gestation. The high cost of caring for premature babies increasingly treats all pregnant women as if they are 'high risk' for preterm birth. Artificial intelligence techniques used a machine learning program named LERS1 with large datasets (n = 18,890; 214 variables), statistical analysis, expert verification techniques, and a prototype expert system2 that yielded improved accuracy (53-90%) over existing manual techniques (17-38%) for predicting preterm birth.

Can chaos theory be used to increase preventive maintenance effectiveness?

Clinical engineering programs typically establish the content and frequency of a device's inspection and preventive maintenance procedures at the time of implementation. In some programs, these are not altered throughout the device's useful life. In others, history data and traditional statistical methods are used to adapt procedures to change in risk measures. Such methods are essentially reactive in that they are based upon past trends and do not readily consider potentialities for future change in the performance and utilization environments. Chaos theoretical concepts and related measures, when implemented in artificial intelligence programs such as neural networks and genetic algorithms, and used as an adjunct with computerized technology management programs, can assist in asking and answering the more dynamic, proactive questions necessary for effective inspection and preventive maintenance optimization. Today's healthcare environment is ideal for exploring their utilization.

Preliminary results of neural networks and zernike polynomials for classification of videokeratography maps.

PURPOSE: Our main goal in this work was to develop an artificial neural network (NN) that could classify specific types of corneal shapes using Zernike coefficients as input. Other authors have implemented successful NN systems in the past and have demonstrated their efficiency using different parameters. Our claim is that, given the increasing popularity of Zernike polynomials among the eye care community, this may be an interesting choice to add complementing value and precision to existing methods. By using a simple and well-documented corneal surface representation scheme, which relies on corneal elevation information, one can generate simple NN input parameters that are independent of curvature definition and that are also efficient. METHODS: We have used the Matlab Neural Network Toolbox (MathWorks, Natick, MA) to implement a three-layer feed-forward NN with 15 inputs and 5 outputs. A database from an EyeSys System 2000 (EyeSys Vision, Houston, TX) videokeratograph installed at the Escola Paulista de Medicina-Sao Paulo was used. This database contained an unknown number of corneal types. From this database, two specialists selected 80 corneas that could be clearly classified into five distinct categories: (1) normal, (2) with-the-rule astigmatism, (3) against-the-rule astigmatism, (4) keratoconus, and (5) post-laser-assisted in situ keratomileusis. The corneal height (SAG) information of the 80 data files was fit with the first 15 Vision Science and it Applications (VSIA) standard Zernike coefficients, which were individually used to feed the 15 neurons of the input layer. The five output neurons were associated with the five typical corneal shapes. A group of 40 cases was randomly selected from the larger group of 80 corneas and used as the training set. RESULTS: The NN responses were statistically analyzed in terms of sensitivity [true positive/(true positive + false negative)], specificity [true negative/(true negative + false positive)], and precision [(true positive + true negative)/total number of cases]. The mean values for these parameters were, respectively, 78.75, 97.81, and 94%. CONCLUSION: Although we have used a relatively small training and testing set, results presented here should be considered promising. They are certainly an indication of the potential of Zernike polynomials as reliable parameters, at least in the cases presented here, as input data for artificial intelligence automation of the diagnosis process of videokeratography examinations. This technique should facilitate the implementation and add value to the classification methods already available. We also discuss briefly certain special properties of Zernike polynomials that are what we think make them suitable as NN inputs for this type of application.

Individual prognosis of diabetes long-term risks: a CBR approach.

We present DIRAS, an application to support physicians in determining the risk of complications for individual diabetic patients. The risk pattern of each diabetic patient is obtained using a Case-based Reasoning method called LID. Case-based Reasoning is an Artificial Intelligence technique based on solving new situations according to past experiences. For each patient, the LID method determines the risk of each diabetic complication according to the risk of already diagnosed patients. In addition, LID builds a description that can be viewed as an explanation of the obtained risk.

Decision-theoretic refinement planning: a new method for clinical decision analysis.

Clinical decision analysis seeks to identify the optimal management strategy by modelling the uncertainty and risks entailed in the diagnosis, natural history, and treatment of a particular problem or disorder. Decision trees are the most frequently used model in clinical decision analysis, but can be tedious to construct, cumbersome to use, and computationally prohibitive, especially with large, complex decision problems. We present a new method for clinical decision analysis that combines the techniques of decision theory and artificial intelligence. Our model uses a modular representation of knowledge that simplifies model building and enables more fully automated decision making. Moreover, the model exploits problem structures to yield better computational efficiency. As an example we apply our techniques to the problem of management of acute deep venous thrombosis.

The interpretation of time-varying data with DIAMON-1.

Applying the methods of Artificial Intelligence to clinical monitoring requires some kind of signal-to-symbol conversion as a prior step. Subsequent processing of the derived symbolic information must also be sensitive to history and development, as the failure to address temporal relationships between findings invariably leads to inferior results. DIAMON-1, a framework for the design of diagnostic monitors, provides two methods for the interpretation of time-varying data: one for the detection of trends based on classes of courses, and one for the tracking of disease histories modelled through deterministic automata. Both methods make use of fuzzy set theory taking account of the elasticity of medical categories and allowing discrete disease models to mirror the patient's continuous progression through the stages of illness.

Strategies in engineering sustainable biochemical synthesis through microbial systems.

Growing environmental concerns and the urgency to address climate change have increased demand for the development of sustainable alternatives to fossil-derived fuels and chemicals. Microbial systems, possessing inherent biosynthetic capabilities, present a promising approach for achieving this goal. This review discusses the coupling of systems and synthetic biology to enable the elucidation and manipulation of microbial phenotypes for the production of chemicals that can substitute for petroleum-derived counterparts and contribute to advancing green biotechnology. The integration of artificial intelligence with metabolic engineering to facilitate precise and data-driven design of biosynthetic pathways is also discussed, along with the identification of current limitations and proposition of strategies for optimizing biosystems, thereby propelling the field of chemical biology towards sustainable chemical production.

Assessment of atypical glandular cell interpretation in Pap tests using the Hologic Genius Digital Diagnostics System.

Atypical glandular cells (AGC) are a diagnostic challenge. The aim of this study was to evaluate the efficacy and diagnostic performance of AGC detection on the Hologic Genius Digital Diagnostics System (HGDDS). A retrospective analysis of 451 ThinPrep Pap cases was conducted, including 207 cases of AGC, 27 cases of high-grade squamous intraepithelial lesion (HSIL), 25 cases of low-grade squamous intraepithelial lesion (LSIL), and 192 benign cases. All AGC cases had follow-up histologic diagnoses, with 66 cases subsequently diagnosed as adenocarcinoma. The slides were randomized, scanned, and analyzed by the HGDDS. Patient age and HPV test results were provided to reviewers, an experienced cytologist, who screened the cases, followed by two cytopathologists who independently examined the cases on the HGDDS. Diagnostic concordance between the two cytopathologists indicated strong agreement (κ = 0.829). Sensitivity of AGC on Papanicolaou (Pap) tests for adenocarcinoma detection on HGDDS was 98.5% and 95.5%, respectively, comparable to the original ThinPrep interpretation (OTPI). Specificity for adenocarcinoma detection was significantly higher (84.6% and 85.6%) with the HGDDS than 27.7% with OTPI. Overall, the diagnostic performance for AGC/HSIL interpretation to detect CIN2/3/adenocarcinoma appeared to have improved with HGDDS compared with OTPI, particularly for specificity and positive predictive value (PPV). This is the first study evaluating AGC diagnosis using the HGDDS. The findings demonstrate that the sensitivity of adenocarcinoma detection as AGC on HGDDS is comparable to the ThinPrep Imaging System, but the specificity and PPV are improved. This suggests the potential of artificial intelligence to augment the performance of cervical cancer screening.

Algorithms for inferring qualitative models of biological networks.

Modeling genetic networks and metabolic networks is an important topic in bioinformatics. We propose a qualitative network model which is a combination of the Boolean network and qualitative reasoning, where qualitative reasoning is a kind of reasoning method well-studied in Artificial Intelligence. We also present algorithms for inferring qualitative networks from time series data and an algorithm for inferring S-systems (synergistic and saturable systems) from time series data, where S-systems are based on a particular kind of nonlinear differential equation and have been applied to the analysis of various biological systems.

[Virtual surgical planning in liver surgery].

UNLABELLED: The operability of a liver tumour depends on its three-dimensional relation to the intrahepatic vascular trees which define autonomously functioning liver (sub-)segments. The aim of our study was to establish a computer-based three-dimensional volumetric operation planning system for the liver. METHODS: Using data from routine helical CT scans the three tissue subclasses of liver parenchyma, liver vessels and liver tumour were segmented semiautomatically. A dedicated segmenting tool was established using region growing algorithms in combination with an "intelligent" border finder. Visualisation is performed by the "Heidelberg Raytracer". The vascular trees are visualised as 3D graphs. Pseudoconnections between portal and hepatic venous trees are separated automatically. Security margins are calculated and the system presents a virtual resection proposal. RESULTS: The 3D anatomy of the liver can be visualised in high quality resulting in good depth perception. Security margins are demonstrated. Dependent liver parenchyma can be recognized automatically on the basis of the vascular trees. The system offers a individualised resection proposal including the tumour, security margin and dependent liver parenchyma. CONCLUSION: Three-dimensional presentation of the individual liver anatomy of a given patient facilitates the perception of the pathology. Virtual reality combined with artificial intelligence allows calculation of complete resection protocols, which can be quantified and modified interactively. This will make operation planning more objective; patient selection may be improved, and in cases of difficult tumour localisation different resection strategies may be tested. Thus virtual reality in liver surgery will improve teaching, surgical training and planning. It may lead to improved surgical care.

A connectionist computational model for epistemic and temporal reasoning.

The importance of the efforts to bridge the gap between the connectionist and symbolic paradigms of artificial intelligence has been widely recognized. The merging of theory (background knowledge) and data learning (learning from examples) into neural-symbolic systems has indicated that such a learning system is more effective than purely symbolic or purely connectionist systems. Until recently, however, neural-symbolic systems were not able to fully represent, reason, and learn expressive languages other than classical propositional and fragments of first-order logic. In this article, we show that nonclassical logics, in particular propositional temporal logic and combinations of temporal and epistemic (modal) reasoning, can be effectively computed by artificial neural networks. We present the language of a connectionist temporal logic of knowledge (CTLK). We then present a temporal algorithm that translates CTLK theories into ensembles of neural networks and prove that the translation is correct. Finally, we apply CTLK to the muddy children puzzle, which has been widely used as a test-bed for distributed knowledge representation. We provide a complete solution to the puzzle with the use of simple neural networks, capable of reasoning about knowledge evolution in time and of knowledge acquisition through learning.
