[Unsaturated fatty acids intake and breast cancer risk: epidemiological data review].

The relationship between fatty acids and breast cancer has been debated for long, because of the high frequency of breast cancer and the contradictory results from the numerous studies devoted to this issue. The present review includes case-control and prospective studies, according to specified methodological criteria, which estimated the exposure to monounsaturated fatty acids (MUFA) and n-6 and n-3 polyunsaturated fatty acids (PUFA) using dietary questionnaires or markers (plasma, erythrocytes, adipose tissue). The relationship between MUFA intake and breast cancer risk seems to depend on the contributing food : neutral or beneficial for vegetable oil, rather deleterious for animal products. Contrary to data from animal experiments, human studies do not show an increase of breast cancer risk with n-6 PUFA intake. Estimating the risk associated with alpha-linolenic acid appears difficult due to the incompleteness of food composition tables and studies on biomarkers remain few. The same applies to long-chain n-3 PUFA despite the suggestion of a decrease in risk, in agreement with animal studies. However, it is difficult in human to disentangle the effect of nutrient intake from that of contributing foods or even nutritional profile.

[Current state and future perspectives of oncology care in Hungary based on epidemiologic data].

The cancer mortality and morbidity data over the period of 1999-2003 in Hungary has been analyzed. The attempts for the harmonization of cancer care's organization between European Cancer Centers and Hungarian Cancer Centers will also be reviewed. Total cancer mortality of Hungary was found to be 33 530 persons in 2003. According to the cancer mortality data of 36 European countries the highest mortality rate were in Hungary for male in 1999. Hungary had the second place in the female cancer mortality in Europe. It has also been found that the Hungarian men had the highest rate tobacco related cancers (lung, oral cancers) in Europe. The trends of the cancer mortality for women is changed during the period 1999-2003. In 1999 breast cancer was the leading cause of cancer's death for women followed by colorectal and lung cancers. The lung cancer rates have been rising in Hungarian women and became the main cause of cancer death in 2002. The unfavorable cancer mortality trends in Hungary might be attributed to the high consumption of tobacco and alcohol, moreover the unsatisfactory care of cancer patients. The cancer mortality in Hungary could be reduced by the improvement of prevention, diagnosis and treatment of cancer patients. The European accreditation of the cancer control activities should be based on the network of Cancer Centers in Hungary. This accreditation of the Hungarian Cancer Centers is indispensable for the high quality of the care of cancer patients.

[Epidemiologic data on obesity and overweight in the elderly].

The incidence and the prevalence of obesity are progressively increasing in all the more industrialized Western nations: in the USA, approximately 280,000 deaths/year are attributable to obesity. Moreover the general structure of the population to world-wide level is enduring a strong change, since it is increasing towards the bands of more advanced ages: the strong aging of population makes it necessary to deepen the behavior and the consequences of obesity as risk factor also in the third age. In Italy, overweight subjects, amounting to more than 15 million over the age of eighteen, have not increased since 1994, but the obese ones in the general population are approximately 8%. The distribution of the prevalence of obesity in the range of age between 45 and 75 does not seem to be much age correlated in the male population, while it is more age related in the women: the prevalence decreases in the last class of age. We can found a similar trend for the overweight conditions. Differences in the distribution of this important risk factor, even if often neglected, also are tied to the professional skills and education.

[What are the epidemiologic data in regard to episiotomy?].

OBJECTIVES: Description of episiotomy rates and current trends in France. Description of episiotomy variation according to medical situations and inter or intra country in obstetrical variation practices. MATERIALS AND METHODS: We analyzed the AUDIPOG Perinatal network. The medical literature review used Pubmed and the Cochrane Library databases. RESULTS: The episiotomy rate has decreased, in France, from 1996-97 to 2002-03. The percentage of episiotomies among nulliparae is 68% and 31% among multiparae. The episiotomy rate increases with maternal age. The risk of perineal traumatism is variable according to women's ethnic background. The risk of episiotomy is linked to obstetrical situations. It remains an international variation of the global episiotomy rate. However, national rates include regional and hospital variations. French data do not find important variations according to hospital administrative status. We found inter professional variations in the literature. CONCLUSION: Episiotomy is a frequent obstetrical intervention in the world and in France. This intervention is linked with variations in obstetrical practice and with other medical factors at the time of delivery. We note a reduction of episiotomy rate in western countries but this reduction is not the same in these countries.

[Epidemiologic data of rupture of the fibular ligament of the upper ankle joint].

Epidemical studies of fibular ligament lesions of the ankle joint lateral ligamentous lesions at the ankle joint are one of the most accidental injuries. It is necessary to bestow great care on prevention and therapy to avoid after effects. We report about 150 cases of acute injuries of the fibular ligament lesions of the ankle joint in 1987 treated by surgery and followed up to explore the cause of injury, its dimensions, the groups of patients and time of accident. Corresponding to literature sports activities were mostly the reason of accidents followed by casional accidents and industrial accidents. Mainly young people suffered from this injury preferring men. The part of reruptures was 14%. In 61.3% both Lig. fibulotal. ant. and Lig. fibulocalc. were ruptured followed by isolated rupture of the Lig. fibulotal.ant. in 32.7% of the cases.

[Epidemiological data on precancerous conditions and neoplasms among workers in the petroleum-refining industry].

A prospective cohort research revealed an increased morbidity level among oil-processing workers with chronic pretumour diseases (chronic bronchitis, anacid gastritis, ailments of the larynx and skin, diseases of cervix uteri, benign tumour of the uterus and pretumour mammary gland diseases). Statistically reliable data have been received with regard to lung cancer mortality of males in the sample cohort, and lung and cervix uteri cancer mortality of females, as well as the risk factors of lung, larynx and skin cancer in males, and lung and large intestine cancer in females,--all in comprison with workers who have no contact with oil-processing products.
