Transthoracic aspiration biopsy. Occurrence of non-neoplastic cells in biopsies from malignant and non-malignant lesions.

In the search for a possible distinction between false and true negative biopsies, an analysis of the distribution of the non-neoplastic cells has been performed in 100 randomized transthoracic aspiration biopsies in so many histologically verified cases. Aspiration biopsies from malignant lesions often contain many giant cells, alveolar epithelial cells, and mast cells, but a significant differences was only found for the mast cells. A negative biopsy containing many inflammatory cells and mast cells and showing a pronounced degree of necrosis may represent a chronic obstructive pneumonitis, and such a finding should always prompt further investigation.

Nephrostomy in patients with ureteral obstruction secondary to nonurologic malignancy. Obstructive uropathy in malignant disease.

A retrospective study was undertaken to determine the effects of nephrostomy on renal function in 11 patients with either bilateral ureteral obstruction (nine) or obstruction of a solitary kidney (two) due to advanced metastatic malignancy of nonurologic cause. Nephrostomy resulted in substantial improvement in renal function and lowering of blood pressure. The BUN and serum creatinine levels decreased from 80 +/- 32 and 12.1 +/- 6.3 mg/dL to 34 +/- 24 and 2.4 +/- 2.0 mg/dL, respectively. Mean arterial blood pressure decreased from 112 +/- 22 to 88 +/- 6 mm Hg and correlated significantly with weight loss. Mean patient survival after nephrostomy was 6.7 months. The major complications of nephrostomy included hemorrhage and infection. Nephrostomy resulted in significant improvement in renal function and blood pressure in this group of patients and might add to length of patient survival.

Malignant melanoma of the foot. A clinicopathological study of 125 primary cutaneous malignant melanomas.

In a retrospective study of 125 primary cutaneous malignant melanomas of the foot treated from 1949 to 1977 the prognostic importance of various histological and clinical factors was observed. Clinical stage, location of tumour on the foot, level of invasion, tumour thickness, mitotic rate, and presence of ulceration correlated well with survival, while sex and histological type did not. A histological transition was found between the superifical spreading type and the acral lentiginous type, indicating a close biological relationship between the two types of melanoma.

[Malignant lymphomatous polyposis--a rare disease entity of malignant centrocytic lymphoma with characteristic endoscopic appearance].

It is considered as special gastrointestinal manifestation on the nodal centrocytic lymphoma of intermediate malignancy. We demonstrate a 77 years old male patient, who underwent colonoscopy because of rectal bleeding. We found the colonic mucosa dotted with small, flat, lentiform polyps. In the terminal ileum these polyps were much larger. No changes were seen in the esophagus. In the antrum we found flat elevations and in the duodenum again lentiform nodules. Presenting in stage IV of non-Hodgkin's lymphoma the patient was treated with five cycles of combined chemotherapy with prednimustin and mitroxanthrone without side effects. Restaging showed distinct remission of the disease with significant reduction of the mucosal alterations.

Factor VIII can be positive in a special type of malignant lymphoma, intravascular malignant lymphomatosis: an immunohistochemical investigation.

This report presents seven cases of intravascular malignant lymphomatosis (IML). The patients exhibited clinical signs and symptoms of multiple cerebral infarctions and polyradiculopathy, but in each case the diagnosis of IML was only established by postmortem examination. The autopsies revealed that almost every organ was involved and that there were atypical cells which had proliferated intravascularly. This was especially remarkable in the central nervous system. Extravascular extension of the tumor cells was rarely observed. It was documented by immunohistochemical studies that the abnormal cells were B cell lymphoma cells with a high proliferative capacity. Factor VIII, an endothelial cell-related antigen, was detected in the cytoplasm of the tumor cells of two cases (28%), but this finding was considered to represent a non-specific absorption of factor VIII from the thrombi.

Relation between cardiovascular pathologic conditions and malignant tumours as seen by pathologist. II. Myocardial infarct and coronary atherosclerosis in the population suffering from malignant neoplasm as seen by pathologist.

In the autopsy material of 2932 men and 3051 women the frequency of fresh myocardial infarct and the severity of coronary atherosclerosis were highly significantly lower in the population suffering from malignancies than in that free of cancer. Lower frequency of myocardial infarct seems to result from lower severity of coronary atherosclerosis. The incidence rate of myocardial infarct was also significantly lower in the cancer--bearing group when both compared populations showed identical, severe (grade 4) coronary atherosclerosis. This observation seems to indicate lesser contribution of coronary spasm in cancerous population and justifies the discussion of possible significance of EDRF (endothelium-derived relaxing factor) and catecholamines.

[Intravascular malignant lymphoma (ex-malignant angioendotheliosis). 3 cases].

Malignant angioendotheliomatosis is an intravascular proliferation of tumour cells in the small arteries, veins and capillaries. Its lymphomatous origin has been proved recently by immunohistochemistry. It is a rare, generally rapidly fatal condition. Less than 150 cases have been reported in the literature. We report three cases. Long course fever with delayed neurologic disturbances were observed in the first case and long course fever, hyponatraemia, corticotropic and gonadotropic insufficiency in the second. The third case began with febrile meningo-encephalitis. Diagnosis was made at autopsy in the two first cases and on evidence from kidney needle-biopsy in the third. Clinical manifestations are polymorphic and frequently simulate vasculitis. Neurologic signs are the most common. Histologically, kidneys, skin, endocrine glands, lungs, muscles and most of the organs may be involved. Surprisingly lymph nodes, bone marrow and spleen are often free from disease. Current treatment is based on polychemotherapy. Earlier diagnosis of the disease should improve the prognosis as observed in our third patient.

[DNA cytometry of solitary and aneurysmal bone cysts and low malignancy and high malignancy central osteosarcomas. Current significance within the scope of morphologic diagnosis of intraosseous cystic and osteoblastic lesions].

DNA cytometric investigations of intraosseous cystic lesions and osteoblastic tumors may be helpful in morphological diagnosis. The detection of a DNA-aneuploid stemline supports the diagnosis of a high-grade malignant bone neoplasm, even in small biopsies. Solitary and aneurysmal bone cysts have DNA-diploid cell populations. Low-grade central osteosarcomas may show single-cell aneuploidies of varying extent as well as a DNA-diploid stemline. Their presence may be a sign of genetic instability within the population of neoplastic cells before a DNA-aneuploid stemline is established and should prompt careful postoperative observation.

Immunohistochemical study of alpha, mu and pi class glutathione S transferase expression in malignant melanoma. MMM Group. Multidisciplinary Malignant Melanoma Group.

Human pi, mu and alpha class glutathione S transferases (GST) have been localized immunohistologically in normal skin, naevi and melanoma. Pi GSTs were found principally in the stratum basalis and, to a lesser extent, in the superficial layers. Normal melanocytes showed strong nuclear and cytoplasmatic staining. Distribution of GST mu in the epidermis showed that only the stratum basale, where melanocytes are located, stained well but with weak nuclear staining. Normal melanocytes were also well stained. The alpha GSTs were relatively abundant in the upper strata and to a lesser extent, in the basal layers. The absence of nuclear staining gives these cells a target appearance. Normal melanocytes showed strong cytoplasmatic staining. The pi GSTs seem to be most persistently and strongly expressed in malignant melanoma (MM), but mu GSTs are also found, whereas the alpha GSTs were only occasionally present. The finding of the GST mu in the melanocytes of the basal layer raises new questions regarding the role of GST mu in these cells because of the inherent risk of MM in individuals with a congenital deficiency of this isoenzyme. The role of GSTs in the resistance of cells to chemotherapy is also discussed.

Relationship of Helicobacter pylori infection to several malignant and non-malignant gastrointestinal diseases.

The importance of the Helicobacter pylori infection was investigated as a risk factor for several gastrointestinal diseases. In this study 203 patients with gastric cancer, 61 with peptic ulcus, 60 with gastritis and 100 asymptomatic control subjects were investigated. Serum samples were examined for IgC antibodies to H. pylori by enzyme linked immunoassay - tissue samples were stained for H. pylori by Wartin-Stary technique and by Giemsa for routine histopathology. H. pylori seropositivity was 58.1% in gastric cancer, 54% in peptic ulcus, 63.3% in gastritis and 27% in asymptomatic control group. There was a 10.1% discordance between the serum and tumor determinants in the seropositive group and 11.3% of discordance in the seronegative group. The frequency of H. pylori seropositivity was lowest in cardia tumors (22.7%) and highest in antral tumors (65.5%, p=0.00002). H. pylori seropositivity was 29% in diffuse type of histology, 35% in mixed type and 79% in the intestinal type (p=0.00000). In the gastric cancer patients the frequent use of salty food (p=0.00001, OR=6.4), excessive salt, pickled food (p=0.0000, OR=24.92) and proteins (p=0.003, OR=0.45) were more significant than asymptomatic volunteers. In gastric cancer patients the frequent use of salty and pickled food were relevantly associated with H. pylori infection (p=0.001). It was concluded that H. pylori infection could play a role in the pathogenesis of non-malignant gastrointestinal diseases which may be the precursor of carcinoma. However, other contributing factors to carcinogenesis must be investigated.

[Reproducibility and prognostic value of histopathological classifications of malignant lymphomas. Prolegomena for the 1st international classification proposed by WHO. Group of the non-Hodgkin's Malignant Lymphoma Classification Project].

Different histopathologic classifications have been used in the study of malignant lymphomas. The clinical relevance (reproducibility, prognostic value) has not been precisely studied. The "non-Hodgkin's lymphoma classification project" has been organized to study a cohort of 1,403 cases in 9 sites around the world consisting of consecutive patients seen between 1988 and 1990 in order to have a good follow-up. The reproducibility of the up-dated Kiel and the ILSG (REAL) classifications between the 5 visiting expert hematopathologists was pretty good, at least 85% for the majority of the entities. According to survival curves, the lymphomas can be stratified in 4 different groups. Comparison with the international prognostic index demonstrate that for therapeutic strategy both histopathology and index should be used. The results bring a good support to the project of the WHO to propose the first international classification of lymphomas based on both forme classifications.
