Synchronous cancer of the biliary tract and pancreas associated with anomalous arrangement of the pancreaticobiliary ductal system.

A 58-year-old man on abdominal ultrasonography and CT had an irregularly elevated lesion at the neck of the gallbladder and a cyst of approximately 6.5 cm in diameter at the pancreatic tail. Percutaneous transhepatic cholangiography revealed a 2-cm shadow defect at the neck of the gallbladder and an irregular, translucent 30 x 12 mm lesion in the intrapancreatic bile duct. Total pancreatectomy and extended cholecystectomy with regional lymph node dissection was performed. An anomalous arrangement of the pancreaticobiliary ductal system (AAPBD) was demonstrated by postoperative contrast radiography of resected specimen. The lesions of the gallbladder and common bile duct were papillary adenocarcinoma. In addition, papillary adenocarcinoma was limited almost entirely to the mucosal layer of the main pancreatic duct and its branches, from the junction of the common bile duct and pancreatic duct to the pancreatic tail. The three tumors were not continuous. The cyst at the pancreatic tail was a pseudocyst. This case represents synchronous cancer of the gallbladder, common bile duct, and pancreas associated with AAPBD.

[Case of simultaneous triple cancers of the duodenal papilla, gallbladder and stomach].

The patient was a 76-year-old man with simultaneous triple cancer arising in the duodenal papilla, gallbladder and stomach, whose complaint was epigastralgia. Endoscopic and radiologic studies revealed early gastric cancer (IIc) at the posterior wall of the antrum; ERCP disclosed obstruction with "apple core" appearance in the distal bile duct. Pancreaticoduodenectomy, cholecystectomy and distal gastrectomy with lymph node dissection were performed. Histopathological examination of the resected specimens revealed papillo-tubular adenocarcinoma in the duodenal papilla, well differetiated tubular adenocarcinoma in the gallbladder, and moderately differentiated tubular adenocarcinoma in the gastric mucosa.

Mutation and altered expression of beta-catenin during gallbladder carcinogenesis.

Gallbladder carcinoma has two main morphologic developmental pathways: a dysplasia-carcinoma sequence and an adenoma-carcinoma sequence. beta-Catenin is a key regulator of the cadherin-mediated cell adhesion system, and altered expression and mutation of beta-catenin have been identified in many human malignancies. To clarify its role in gallbladder carcinogenesis, we investigated mutation and immunohistochemical expression of beta-catenin in adenomas, dysplasias, and carcinomas. We classified adenomas according to the expression of apomucins and cytokeratin and compared the mutational and expression pattern among each type. beta-Catenin mutations were identified in 58% (14 of 24) of the adenomas, and they were absent from all carcinomas (37 cases) and dysplasias (13 cases). Altered expression of beta-catenin, such as nuclear or cytoplasmic expression and loss of membranous expression, was also significantly higher in adenomas than in dysplasias or carcinomas (p <0.001). Of the adenomas, papillary adenomas and tubular adenomas of intestinal type showed infrequent beta-catenin abnormality, which was similar to the carcinomas. The cytoplasmic and nuclear expression of beta-catenin in carcinomas was correlated with less aggressive tumor behavior; in particular, cytoplasmic expression was associated with improved patient outcome (p = 0.028). Gallbladder adenoma may be a heterogeneous entity, and the majority of adenomas are not responsible for carcinoma progression.

Minute carcinoid tumor of the gallbladder.

A minute carcinoid tumor of the gallbladder is reported. The tumor was incidentally identified in a 77-year-old Japanese man with cholecystolithiasis, hepatocellular carcinoma and sigmoid colon carcinoma. The tumor formed a 5-mm-sized sessile polyp at the neck of the gallbladder. The tumor cells, which were argyrophilic and non-argentaffinic, belonged to the foregut-type. Immunohistochemically, they were positive for neuron-specific enolase (NSE) and somatostatin.

[Carcinoma of the biliary tract].

Six problems were pointed out in General Rules for Surgical Studies on Cancer of Biliary Tract; division of extrahepatic bile duct and papilla of Vater, definition of carcinoma of the bile duct, gallbladder and papilla of Vater, classification of the lymphnodes, macroscopic classification of cancer of the biliary tract, tumor size and staging, pathological rules. Furthermore, in 15 cases of the gallbladder carcinoma and 21 cases of the bile duct carcinoma, the correlation between the prognosis and the histological findings of cancer invasion to the wall or of lymphnode metastasis was evaluated.

Polyposis coli associated with adenocarcinoma of the gallbladder. Report of a case.

A case of malignant polyposis coli, occurring in a 71-year-old man in association with a primary invasive adenocarcinoma of the gallbladder is presented. The eventuality that the carcinoma of the gallbladder could be a metastasis from the colonic neoplasia was excluded due to the observation in the mucosa of the gallbladder of changes corresponding to carcinoma in situ. This is the fourth case reported of polyposis coli coexisting with a carcinoma of the biliary tract.

Primary biliary tract malignant melanoma: US, CT, and MR findings.

We report a case of primary biliary tract malignant melanoma occurring in a 47-year-old male. Ultrasonography and computed tomography showed multiple masses in the gallbladder and distal common bile duct that caused biliary tract dilatation. Magnetic resonance imaging showed that the polypoid masses in the gallbladder and common bile duct were of low signal intensity on T2-weighted images and of high signal intensity on unenhanced T1-weighted images.

[Synchronous metastasis of renal adenocarcinoma to gallbladder and ipsilateral adrenal gland].

Herein we describe a patient in whom a renal mass had been detected. He had no previous urological symptoms or signs, but had undergone laryngectomy in 1989 for well-differentiated squamous cell carcinoma of the glottic region. The radiological and ultrasound evaluations disclosed a right renal mass, a mass in the ipsilateral adrenal gland and a polypoid mass in the gallbladder. The patient underwent radical nephrectomy, adrenalectomy and cholecystectomy. The surgical specimens of the gallbladder and adrenal gland showed histological evidence of clear cell carcinoma with the same characteristics as those of the primary renal tumor.

Hepatopancreatoduodenectomy for squamous and adenosquamous carcinoma of the gallbladder.

The characteristics of squamous or adenosquamous cell carcinoma of the gallbladder differ quite markedly from those of adenocarcinoma, although the incidence is extremely low. Recently, we encountered both of the former types of gallbladder carcinoma: a 77-year-old man with squamous cell carcinoma and a 70-year-old man with adenosquamous cell carcinoma of the gallbladder. Both had a large mass in the gallbladder fossa region with infiltration to the liver and invasion of the duodenum. Hepatopancreatoduodenectomy was performed on both of these patients. The TNM stage of the former was IV (T4N0M0) and of the latter IV (T4N0M0) and of the latter IV (T4N1bM0). The former has remained well without recurrence for about 1 year and 4 months after the operation, while the latter died of recurrent disease 6 months after operation. The true reason for the difference in the prognosis of these two patients was not known. However, hepatopancreatoduodenectomy is considered to be a most adaptable operative procedure for squamous or adenosquamous cell carcinoma of the gallbladder in view of their mode of spread, and the presence of lymph node metastasis might be a factor of poor prognosis.

[A case of triple simultaneous cancer of the esophagus, gallbladder and colon].

The patient was a 60-year-old man with the chief complaints of bloody stools, and their abnormal motion. The diagnosis was sigmoid colon cancer. Esophageal cancer and cholelithiasis were also found by several preoperative examinations. Subtotal esophagectomy, cholecystectomy and sigmoidectomy were performed. Resected Specimen: There was an early cancer measuring 2.5 X 2.0 cm at the abdominal esophagus. It was a poorly differentiated squamous cell carcinoma. There were two polyps at the gallbladder, in which carcinoma in situ was found. A well-differentiated papillotubular adenocarcinoma of type Borr 2 was found at the sigmoid colon.

Malignant melanoma of the gallbladder. Case report and review of the literature.

The authors are presenting a new case of malignant melanoma of the gallbladder to be added to those described in the literature. The tumour appeared as a polypoid mass of the gallbladder and had metastasized to the jejunum and the brain. Review of the literature and analysis of this case require a thorough discussion of the real existence of tumours in this unusual primary site.

Indications for surgical resection of metastatic ocular melanoma. A case report and review of the literature.

CONCLUSIONS: Ocular melanoma can metastasize to the gallbladder and porta hepatic nodes and mimic pancreatic carcinoma. If one suspects metastatic disease, a complete metastatic work-up must be done prior to surgery to prevent unnecessary surgery. If no distant disease is present or the patient is symptomatic, metastatic disease should be resected. PURPOSE: To review the literature pertaining to the spread of ocular melanoma and to determine if distant disease should be resected. PATIENTS AND METHODS: A 44-yr-old Egyptian male presented to an outside institution with mid-epigastric and right upper quadrant abdominal pain. His past medical history was significant for a left orbital enucleation for uveal melanoma in 1982. On physical examination, there was no supraclavicular adenopathy and no skin lesions were noted. There was a mass in the right upper quadrant. The total bilirubin was 4.8 mg/dL. A computed tomography showed a mass in the head of the pancreas and portal vein involvement could not be determined. RESULTS: The patient was taken to the operating room and a pancreatico-duodenectomy was performed for a cystic mass in the head of the pancreas. Final pathology revealed metastatic melanoma in the gallbladder and an enlarged, cystic lymph node growing into the head of the pancreas replaced with metastatic melanoma. The patient did well post-operatively and was discharged home on the eighth post-operative day.

[Risk groups for pancreatic and bile duct carcinomas].

Biliopancreatic carcinoma has a poor prognosis since the diagnosis of the tumor occurs late when advanced disease is present. The identification of potential causes and earlier diagnosis are needed to prevent the disease or identify it early enough to improve survival. The main risk factors for pancreatic cancer include advanced age, cigarette smoking, high-fat diet, diabetes mellitus, chronic pancreatitis (especially hereditary pancreatitis) and a positive family history of pancreatic cancer. The most important etiologic factor for the development of gallbladder cancer is gallstone disease. Patients with anatomic abnormalities and chronic inflammatory conditions (primary sclerosing cholangitis, infections with parasites) have an increased incidence of bile duct cancers. Several new and promising imaging techniques have recently become available and our understanding of the mechanisms of carcinogenesis are growing rapidly. However, there is currently no effective screening strategy applicable and it is unknown when to begin screening. For pancreatic cancer, reduction of risk is likely to occur with avoidance of smoking and promotion of healthful diets. Cholecystectomy rates have increased since the introduction of new laparoscopic techniques and will eventually reduce the incidence of gallbladder cancer. Improved imaging techniques, the identification of new genes and a better definition of genetic alterations that characterize preinvasive lesions will hopefully allow to develop sensitive and specific technologies to screen and to detect early biliopancreatic cancer for even premalignant lesions to improve the mostly fatal prognosis if this tumor.

Glycocalyceal bodies and microvillous core rootlets: their value in tumor typing.

An ultrastructural study of the cell surface of lumen-forming tumors was carried out to determine the distribution of two morphologic markers seen in relation to the microvilli. These are membrane-bound glycocalyceal bodies and microvillous filament cores that penetrate the underlying cytoplasm as rootlets. They were found (especially when in combination) to be valuable in identifying tumors of what is referred to as intestinal-type epithelium, and could be seen in cases in which brush borders were absent. They have been demonstrated in intestinal-type carcinomas of the stomach and gallbladder, in adenocarcinomas of the small and large intestines and pancreatic ducts, in mucin-forming bronchiolar carcinomas, and in certain mucinous ovarian and endocervical tumors. Other tumors, whether mucin-producing or not, have been found to consistently lack these structures.

[Nonvisualization of the gallbladder lumen by sonography].

Out of 12 000 sonographic examinations of the abdomen in 211 cases the gallbladder was not visualized. The retrospective analysis of 41 patients revealed in 75% gallstones, in 15% space occupying lesions (4 carcinomas, 1 abscess, 1 sludge). In 10% the sonography gave a false positive result. The reasons are discussed.
