Redefining sexuality from women's own experiences.
Traditionally women's sexuality has been defined and described using narrow and socially prescribed concepts. In the past 20 years new ideas about women's sexuality have emerged. At the same time, sexuality has become a legitimate concern for health professionals. Nurses and their clients can benefit from learning about the emerging woman-centered concepts of sexuality. Women's sexuality is not primarily a response to men's sexuality but has its own characteristics. A woman's sexuality is an expression of mutuality, involving not only physical gratification but also a sense of herself as a woman in the context of her life, her relationships, and emotions. Several groups of women whose sexual needs have traditionally been unmet by nurses and other health care professionals include adolescents, physically disabled women, lesbians, and older women. A self-care frame-work differentiates socially constructed definitions of sexuality from lived experience and suggests the importance of attending to individual women's experiences throughout life.

Primary care delivery in the United States and four northwest European countries: comparing the "corporatized" with the "socialized".
Primary care--often considered the weak link in the United States health care delivery system--is the focal point of several European systems. Thoughtful analysis of structural and service differences and similarities among these approaches--whether "corporatized" or "socialized"--reveals areas for helpful innovation in the United States. Matters of access to and continuity of services, preventive care for children, long-term care, and accountability are especially relevant for primary care in an American context.
Towards a national health service for South Africa. A personal view.
Circumstances are propitious for a new look at the Gluckman Report of 1942-1944 with a view to introducing a national health service for South Africa, including the homelands and national states. The circumstances relate to the ethical question of the patient's medical care, without economic disadvantage, being paramount in a health care system. Other issues concern doctors in a salaried service; a burgeoning population; the high cost of health care to the individual patient and to the State as a result of a proliferation of health bureaucracies. The issues are compounded by economic decline, unemployment and adverse socio-economic factors, which militate against the privatization of health care.
Who is the odd man out?: the experience of Western Europe in containing the costs of health care.
Economic, demographic, and technological pressures have led at the same time to increased demands upon health services and the need to contain spending on health care. As the United States has focused singularly on an array of market mechanisms, Western European countries have each adopted a broader set of strategies along with limited de-insurance: innovative regulatory approaches and a move from compulsory health insurance models to flexible national health service models of providing health care. Pursuit of cost-containment in Western Europe has proceeded along with pursuit of quality and, above all else, equity.
